RESUMO

This article introduces a novel method named "Parallel Imaging and Noquist in Tandem" (PINOT) for accelerated image acquisition of cine cardiac magnetic resonance imaging. This method combines two prior information formalisms, the SPACE-RIP implementation of parallel imaging and the Noquist method for reduced-data image reconstruction with prior knowledge of static and dynamic regions in the field of view. The general theory is presented, and supported by results from experiments using time-resolved two-dimensional simulation data and retrospectively sub-sampled magnetic resonance imaging data with acceleration factors around 4. A signal-to-noise ratio analysis and a comparison study with TSENSE and k-t SENSE show that PINOT performs favorably in preserving edge detail, at a cost in signal-to-noise ratio and computational complexity.

RESUMO

A bathymetric lidar survey is the most cost efficient method of producing bathymetric maps in near shore areas where the ocean bottom is both highly variable and of greatest importance for shipping and recreation. So far, not much attention has been paid to the influence of bottom materials on the lidar signals. This study addresses this issue using a Monte Carlo modeling technique. The Monte Carlo simulation includes a plane parallel water body and a flat bottom with or without seagrass. The seagrass canopy structure is adopted from Zimmerman (2003). Both the surface of the seagrass leaves and the bottom are assumed to be Lambertian. Convolution with the lidar pulse function followed by the median operator is used to reduce the variance of the resultant lidar waveform. Two seagrass orientation arrangements are modeled: seagrass in still water with random leaf orientation and seagrass with a uniform orientation as would be expected when under the influence of a water current. For each case, two maximum canopy heights, 0.5 m and 1 m, three shoot densities, 100, 500, and 1000, and three bending angles, 5, 25, and 45 degrees, are considered. The seagrass is found to induce a depth bias that is proportional to an effective leaf area index (eLAI) and the contrast in reflectance between the seagrass and the bottom material.

RESUMO

BACKGROUND: Vaspin was a recently identified adipokine, playing a protective role in many metabolic diseases. The present study aimed to investigate the association between vaspin plasma level and stable angina pectoris (SAP) and unstable angina pectoris (UAP). METHODS: A total of 88 patients with angiographically-proved coronary artery disease (CAD) (SAP 47, UAP 41) and 103 control subjects without cardiovascular diseases were enrolled in this study. Circulating vaspin, mRNA expression of vaspin in peripheral blood mononuclear cells (PBMC), clinical parameters, lipid profile and high-sensitivity C-reactive protein (hsCRP) were assayed. The severity of CAD was also assessed according to the number of vessels diseased. RESULTS: There are significant differences in circulating vaspin levels and mRNA levels of PBMC between SAP and UAP groups (SAP 0.91±0.95 ng/mL and UAP 0.43±0.38 ng/mL, p<0.01 in circulating vaspin level; SAP 1.19±0.85 and UAP 0.82±0.56, p<0.05 in mRNA level of PBMC). An inverse correlation between the number of diseased vessels and plasma vaspin concentration was observed (r=-0.350, p<0.01) in the CAD group. Construction of receiver operating characteristic curves confirmed that vaspin plasma concentrations significantly differentiated CAD patients (area under the curve=0.684, p<0.001), as well as UAP (area under the curve=0.640, p<0.05). CONCLUSION: Decreased vaspin plasma levels and mRNA levels in PBMC were observed in patients with UAP. Low vaspin concentrations correlate with CAD severity. The findings suggested that vaspin could serve as a novel biomarker of CAD as well as UAP.

RESUMO

ObjectiveTo investigate the clinical efficacy and safety of iguratimod combined with the Chinese medicine Runzaoling in the treatment of primary Sjögren's syndrome (pSS). MethodSeventy-two patients treated in the Department of Rheumatology and Immunology of the Second Affiliated Hospital of Guizhou University of Traditional Chinese Medicine(TCM) from January 2021 to June 2022 who met the Western medical diagnosis of pSS and had the TCM syndrome of Yin deficiency and heat toxin syndrome were randomly assigned into an observation group and a control group, with 36 patients in each group. The observation group was treated with iguratimod combined with Runzaoling, and the control group was treated with iguratimod. The treatment in both groups lasted for 12 weeks. The clinical symptoms, EULAR Sjogren's syndrome patient reported index (ESSPRI), EULAR Sjögren's syndrome disease activity index (ESSDAI), erythrocyte sedimentation Rate (ESR), C-reactive protein (CRP), immunoglobulin (IgG), Schirmer score, and saliva flow of the two groups were determined before and after treatment. Furthermore, the incidence of adverse reactions was compared between the two groups. ResultThe total response rate in the observation group was 75.0% (27 patients with response and 9 patients with no response), which was higher than that (61.11%, 22 patients with response and 14 patients without response) in the control group (P<0.05). After treatment, the ESSPRI, ESSDAI, and TCM syndrome scores in both groups decreased and the decreases were more obvious in the observation group than in the control group (P<0.05). The treatment in both groups recovered the ESR, CRP, IgG, Schirmer score, and saliva flow (P<0.05). Moreover, the observation outperformed the control group in terms of the ESR, CRP, IgG, and saliva flow (P<0.05) and had no significant difference in the Schirmer score compared with the control group. During the treatment period, 2 patients in the observation group had nausea, and 1 patient had an abnormal liver function, which were relieved after symptomatic treatment and did not affect the treatment. In the control group, 1 patient withdrew from the study due to rashes and showed no special discomfort in the follow-up 4 weeks, and 1 patient had nausea, which was relieved after symptomatic treatment. ConclusionIguratimod combined with Runzaoling has good clinical efficacy and safety in the treatment of pSS.

RESUMO

Background and Objectives@#This study aims to clarify the systems underlying regulation and regulatory roles of hydrogen combined with 5-Aza in the myogenic differentiation of adipose mesenchymal stem cells (ADSCs). @*Methods@#and Results: In this study, ADSCs acted as an in vitro myogenic differentiating mode. First, the Alamar blue Staining and mitochondrial tracer technique were used to verify whether hydrogen combined with 5-Aza could promote cell proliferation. In addition, this study assessed myogenic differentiating markers (e.g., Myogenin, Mhc and Myod protein expressions) based on the Western blotting assay, analysis on cellular morphological characteristics (e.g., Myotube number, length, diameter and maturation index), RT-PCR (Myod, Myogenin and Mhc mRNA expression) and Immunofluorescence analysis (Desmin, Myosin and β-actin protein expression). Finally, to verify the mechanism of myogenic differentiation of hydrogen-bound 5-Aza, we performed bioinformatics analysis and Western blot to detect the expression of p-P38 protein. Hydrogen combined with 5-Aza significantly enhanced the proliferation and myogenic differentiation of ADSCs in vitro by increasing the number of single-cell mitochondria and upregulating the expression of myogenic biomarkers such as Myod, Mhc and myotube formation. The expressions of p-P38 was up-regulated by hydrogen combined with 5-Aza. The differentiating ability was suppressed when the cells were cultivated in combination with SB203580 (p38 MAPK signal pathway inhibitor). @*Conclusions@#Hydrogen alleviates the cytotoxicity of 5-Aza and synergistically promotes the myogenic differentiation capacity of adipose stem cells via the p38 MAPK pathway. Thus, the mentioned results present insights into myogenic differentiation and are likely to generate one potential alternative strategy for skeletal muscle related diseases.

RESUMO

Objective:To compare the clinical efficacy between reservation and sacrifice of remnants in the footprint area in arthroscopic repair of rotator cuff tear. Methods:A retrospective study was conducted to analyze the clinical data of 32 patients with rotator cuff tear plus remnants in the footprint area (2 cm < tear size <5 cm) who had been admitted to Department of Sports Medicine, The People's Hospital of Northern Jiangsu from May 2020 to July 2021. The patients were divided into 2 groups according to reservation or sacrifice of remnants in the footprint area in arthroscopic repair of rotator cuff tear. In the remnant-reservation group (16 cases): 5 males and 11 females with an age of (61.8±9.9) years, 9 left and 7 right shoulders affected, and (3.7±1.1) cm in size of rotator cuff tear; in the remnant-sacrifice group (16 cases): 4 males and 12 females with an age of (61.3±8.8) years, 8 left and 8 right shoulders affected, and (3.9±0.9) cm in size of rotator cuff tear. The 2 groups were compared in terms of visual analogue scale (VAS), American Shoulder and Elbow Surgeons (ASES) score, Constant-Murley shoulder function score (Constant score), and range of motion of the affected shoulder before surgery, 3 months after surgery and at the last follow-up. The ratio of bilateral abductor muscle strengths (affected side/healthy side) was analyzed and compared between the 2 groups, and the healing of the rotator cuff was evaluated by MRI at the last follow-up.Results:The 2 groups were comparable because there were no significant differences in all their preoperative demographic data ( P>0.05). The 32 patients were followed up for (14.3±3.5) months after surgery. At 3 months after surgery, the VAS score in the remnant-reservation group [1.0 (0.0,1.0) point] was significantly lower than that in the remnant-sacrifice group [1.0 (1.0,1.0) point] ( P<0.05), but there was no significant difference between the 2 groups in ASES score, Constant score or range of motion of the affected shoulder ( P>0.05). At the last follow-up, the ASES score, forward flexion, abduction and ratio of bilateral abductor muscle strengths (affected side/healthy side) in the remnant-reservation group [(96.1±4.8) points, 170.0 (170.0,170.0)°, 160.0 (160.0,170.0)°, and 85.5%±13.8%]were significantly better than those in the remnant-sacrifice group [(91.4±5.9) points, 160.0 (160.0,170.0)°, 150.0 (140.0,155.0)°, and 72.6%±16.9%] ( P < 0.05), but there were no statistically significant differences between the 2 groups in VAS score, Constant score, neutral external rotation angle, or body-side internal rotation ( P>0.05). The Sugaya grading for MRI rotator cuff healing was significantly different between the 2 groups at the last follow-up ( P<0.05). Conclusion:In arthroscopic repair of rotator cuff tear, reservation of remnants in the footprint area can significantly relieve postoperative shoulder pain, and has obvious advantages in restoration of shoulder forward flexion, abduction and abductor muscle strength, leading to better healing of the rotator cuff and the large nodule than the remnant-sacrifice technique.

Objective:To investigate the role of depth of anesthesia monitoring in predicting postoperative cognitive impairment in elderly cancer patients. Methods:The clinical data of 70 elderly cancer patients in Shanxi Province Cancer Hospital from February 2020 to February 2021 were retrospectively analyzed. According to the postoperative cognitive function, they were divided into the postoperative cognitive impairment group (20 cases) and the normal postoperative cognitive function group (50 cases). Statistical analyses of the mini mental state examination (MMSE) score of cognitive function, mean arterial pressure (MAP) and bispectral index score (BIS) before anesthesia (T 0), before skin incision (T 1), 1 h after skin incision (T 2), 2 h after skin incision (T 3), at the completion of surgery (T 4), and at the time point of extubation (T 5), as well as the levels of interleukin-6 (IL-6) and central nervous specific protein (S100-β) at 1 d before operation, immediately after surgery and 1 d after surgery between the two groups were performed. Results:The MMSE score of patients in the postoperative cognitive impairment group was lower than that in the normal postoperative cognitive function group [(21±3) points compared with (25±5) points], and the difference between the two groups was statistically significant ( t = 2.98, P < 0.05). The MAP of the two groups at T 1, T 2, T 3, T 4, and T 5 gradually increased (both P < 0.05), the MAP at T 1, T 2, T 3, T 4 was lower than that at T 0 (all P < 0.05), and the MAP at T 5 was higher than that at T 0 (both P < 0.05). There were no statistical differences in MAP between the two groups at T 0, T 1, T 2, T 3, T 4, and T 5 (all P > 0.05). BIS values of the two groups at T 0, T 1, T 2, and T 3 gradually decreased (both P < 0.05), BIS values at T 4 and T 5 gradually increased (both P < 0.05). At T 2 and T 3, BIS value of the postoperative cognitive impairment group was lower than that of the normal postoperative cognitive function group (both P < 0.05). The duration of BIS < 45 in the postoperative cognitive impairment group was longer than that in the normal postoperative cognitive function group [(44.1±7.7) min than (12.6±2.2) min], and the difference between the two groups was statistically significant ( t = 26.68, P < 0.001). The levels of IL-6 and S100-β in the two groups were gradually increased 1 d before surgery, immediately after surgery and 1 d after surgery (both P < 0.05). On the 1st day after surgery, the S100-β level in the postoperative cognitive impairment group was higher than that in the normal postoperative cognitive function group ( P < 0.05). Conclusions:The application of depth of anesthesia monitoring is of high value in predicting postoperative cognitive impairment in elderly cancer patients.

RESUMO

Pregnancies complicated by pre-gestational diabetes (PGD) are associated with a higher rate of adverse outcomes, including an increased rage of preterm delivery, pregnancy-induced hypertension, pre-eclampsia, caesarean section, perinatal mortality, stillbirth, shoulder dystocia, macrosomia, small for gestational age, large for gestational age, low birth weight, neonatal hypoglycemia, neonatal death, low Apgar score, NICU admission, jaundice and respiratory distress. In the past two decades, numerous reports have been published regarding associations between PGD and risk of adverse outcomes. However, study results are inconsistent. To provide a synopsis of the current understanding of PGD for risk of adverse pregnancy outcomes, a random-effects meta-analysis over 40 million subjects from 100 studies was performed to calculate the pooled ORs. Potential sources of heterogeneity were systematically explored by multiple strata analyses and meta-regression. Overall, PGD were significantly associated with increased risk of preterm delivery (OR=3.48), LGA (OR=3.90), perinatal mortality (OR=3.39), stillbirth (OR=3.52), pre-eclampsia (OR=3.48), caesarean section (OR=3.52), NICU admission (OR=3.92), and neonatal hypoglycemia (OR=26.62). Significant results were also observed for 7 adverse outcomes with OR range from 1.54 to 2.82, while no association was found for SGA and respiratory distress after Bonferroni correction. We found that women with T1DM had higher risks for most of adverse pregnancy outcomes compared with women with T2DM. When stratified by study design, sample size, type of diabetes, geographic region, and study quality, significant associations remains. Our findings demonstrated that PGD is a strong risk-conferring factor for adverse maternal, perinatal and neonatal outcomes.

RESUMO

Objective:To evaluate the safety and effectiveness of Tiandan Tongluo capsule in the treatment of cerebral infarction (CI) in convalescence (stoke involving meridians and collaterals due to wind-phlegm-static blood obstructing vessels), with Naoshuantong capsule as a control. Method:A total of 352 convalescent patients with CI differentiated into stoke involving meridians and collaterals due to wind-phlegm-static blood obstructing vessels in traditional Chinese medicine (TCM) were included in this multi-center, randomized, double-blind, single-simulated, Naoshuantong capsule-controlled clinical trial, which lasted from 28 December, 2016 to 12 April, 2019. After being randomized into an experimental group and a control group at a ratio of 3∶1, patients in the experimental group were provided with oral Tiandan Tongluo capsule, five capsules per time, three times per day, whereas those in the control group received both Naoshuantong capsule simulator, two capsules per time, three times per day, and Naoshuantong capsule, three capsules per time, three times per day, for 12 successive weeks. The patients were followed up until 180 days after onset. The Barthel activities of daily living (ADL) index (BI) score was used as the primary outcome, and the secondary outcomes included neurological deficit score [assessed with National Institutes of Health Stroke Scale (NIHSS)], modified Rankin scale (mRS), TCM syndrome score, and proportion of patients with new vascular events. The changes in laboratory indexes and the incidence of adverse reactions during treatment were observed. Result:Among the 389 cases enrolled, 30 dropped out, with the drop-out rate being 7.71%. There were 374 cases included in the full analysis set and 377 in the safety set. The comparison with the control group revealed that the total BI score and the percentage of BI score ≥ 75 in the experimental group were increased, but the difference was not statistically significant. The percentage of mRS score ≤ 2 within 180 days after onset in the experimental group obviously elevated in contrast to that of the control group (P<0.05). As demonstrated by TCM syndrome score analysis, the markedly effective rate in the experimental group was significantly higher than that in the control group (P<0.05). During the trial, the incidence rates of new vascular events in the experimental group and the control group were 0.00% and 1.09% (one case), respectively, exhibiting no statistically significant difference between the two groups. Conclusion:Tiandan Tongluo capsule and Naoshuantong capsule both produce definite therapeutic effects in the treatment of CI in convalescence (stoke involving meridians and collaterals due to wind-phlegm-static blood obstructing vessels). Compared with Naoshuantong capsule, Tiandan Tongluo capsule better alleviates neurological deficit, promotes neural functional recovery, and improves TCM syndrome score, without inducing severe adverse reactions.

RESUMO

Purpose@#We aimed to investigate whether obtaining a higher level of education was causally associated with lower breast cancer risk and to identify the causal mechanism linking them. @*Methods@#The main data analysis used publicly available summary-level data from 2 large genome-wide association study consortia. Mendelian randomization (MR) analysis used 65 genetic variants derived from the Social Science Genetic Association Consortium as instrumental variables for years of schooling. The outcomes from the Breast Cancer Association Consortium (BCAC) were the overall breast cancer risk (122,977 cases/105,974 controls in women) and the two subtypes: estrogen receptor (ER)-positive breast cancer and ER-negative breast cancer. Fixed and random effects inverse variance weighted methods were used to estimate the causal effects, along with other additional MR methods for sensitivity analyses. @*Results@#Results showed that each additional standard deviation of 4.2 years of education was causally associated with a 27% lower risk of ER-negative breast cancer (odds ratio, 0.73; 95% confidence interval, 0.64–0.84; p-value 10 minutes explain a small part of the causal effect of education on the risk of developing breast cancer, and their mediation proportion is approximately 1%. @*Conclusion@#A low level of education is a causal risk factor in the development of breast cancer as it is associated with poor lipid profile, obesity, smoking, and types of physical activity.

RESUMO

Objective: To study the HPLC fingerprint of Bupleurum chinense and the relationship between the spectrum and the toxicity in vitro, construct the network of the liver toxicity induced by B. chinense, and integrate the spectrum and the network toxicology to predict the material basis of the liver toxicity induced by B. chinense. Methods: The fingerprints of 10 batches of decoction of B. chinense were established by HPLC, the contents of ALT and AST in L02 cells were determined by decoction of B. chinense, and the toxic components of liver were preliminarily determined by the method of grey correlation analysis. Combined with network toxicology, the candidate components of hepatotoxicity were predicted. Integrated analysis on the components of liver toxicity induced by decoction of B. chinense. Results: The established fingerprint of B. chinense was calibrated with 29 common peaks. The results of grey correlation analysis showed that common peaks 13, 12, 8, 26, 10, 14, 27 and 11 had a high correlation degree with the ALT content of L02 cells. The common peaks of 12, 13, 26, 8, 10, 14, 27 and 11 had a high correlation degree with the AST content of L02 cells. According to the network toxicology, 17 components of B. chinense, stigmasterol, baicalin, etc were speculated to be the hepatotoxicity components of B. chinense. The integrated analysis initially determined that the hepatotoxicity components of B. chinense were 13, 12, 8, 26, 10, 14, 27 and 11 peaks in total, and stigmasterol, baicalin, saikosaponin D, etc. Conclusion: The hepatotoxicity of B. chinense is the result of the interaction of various components. This study can provide data support for the further research on the material basis of B. chinense hepatotoxicity.

RESUMO

OBJECTIVE@#To investigate the effect of preoperative condylar condition for mandible retrognathism deformities with severe temporomandibular joint osteoarthrosis on the stability of the jaw after orthognathic surgery and on the postoperative condylar volume changes.@*METHODS@#In this retrospective study, from 2014 to 2019, 37 patients including 1 male and 36 female, aged between 21 to 34 years old with an average age of (28.03±6.52) years, were diagnosed with mandible retrognathism deformities with severe temporomandibular joint osteoarthrosis by Peking University School and Hospital of Stomatology and received orthognathic surgery, meeting the inclusion criteria were included. According to the preoperative condylar condition. There were divided into smooth group and non-smooth group, the lateral cephalometric films 1 week (T0), 3 months (T1), 6 months (T2) and 1 year (T3) after surgery were used to establish the coordinate system and cephalometric analysis to determine the stability of the jaw after operation. The three-dimensional model of the condyle was segmented by cone beam computed tomography (CBCT) 1 week (T0), 3 months (T1), 6 months (T2) and 1 year (T3) after surgery and the volume was obtained to evaluate the change of the condyle volume after surgery. CBCT image data was used to evaluate the changes of the condylar condition after surgery, and to clarify the correlation between the postoperative condylar condition and jaw stability. SPSS 20.0 statistical software was used for statistical analysis, Fisher's exact probability methods were used to compare whether there were statistically significant differences in the stability of the mandibular joint at stages T1, T2 and T3 with different preoperative condylar condition.Spearman correlation coefficient analysis and Mann-Whitney test were used to compare whether there were statistically significant differences in the volume changes at stages T1, T2 and T3 after surgery between the two groups.@*RESULTS@#The recurrence rates of the mandible in the condylar smooth group were T1 36.85%, T2 47.37% and T3 42.11%, respectively. The recurrence rates in the non-smooth condylar group were T1 27.78%, T2 44.44% and T3 55.56%, respectively. There was no statistical difference in the recurrence rates between the two groups at different time points. There was no significant difference in the condylar volume change between smooth group and non-smooth group.@*CONCLUSION@#For patients with mandible retrognathism deformities with severe temporomandibular joint osteoarthrosis and no significant changes in the condyle observed for one year before surgery, there is no difference in the influence of the preoperative condylar condition on the stability of jaw after operation, and no definite influence on the volume of the condyle after operation. Condylar resorption 3 months after surgery can cause instability of the jaw after orthognathic surgery.
