RESUMO

A technique for intraoperative segmental preparation of the large bowel, using 10% povidone iodine, was evaluated in 25 patients undergoing elective colon resection. Qualitative and quantitative bacteriology was obtained from the normal bowel content and from segments of colon treated with povidone iodine or normal saline. Forty-five of 50 segments treated with povidone iodine demonstrated no growth, whereas the segments injected with normal saline maintained bacterial counts of 3.5 x 10(8) colony forming units per milliliter. There were no septic complications in this group of patients and the levels of triiodothyroninc and thyroxin remained unchanged despite a substantial absorption of iodine, as demonstrated by protein-bound iodine determinations. Intraoperative segmental preparation of the colon with 10% povidone iodine is a simple technique that may be useful in the enhancement of other methods of bowel preparation by further reducing the endogenous bacterial inoculum at the time of transection of the colon.

Hetastarch, 6% hydroxyethyl starch solution, is an artificial colloid proposed for use as a volume expander. There is concern that hetastarch, like dextran, may adversely affect coagulation. To compare the effects of hetastarch with 5% albumin in postoperative patients, 60 consecutive patients who underwent coronary artery bypass were prospectively randomized into two study groups. Both groups were comparable in all respects preoperatively. Group 1 (27 patients) received a mean of 1,241 ml of 5% albumin and Group 2 (33 patients) a mean of 1,210 ml of hetastarch as volume replacement within the first 24 postoperative hours. Indices of postoperative hepatic, renal, and pulmonary function did not differ significantly between the groups immediately postoperatively, at 24 hours postoperatively, or at 7 days postoperatively. Hematocrits, platelet counts, prothrombin times, partial thromboplastin times, and serum fibrinogen levels were comparable between the two groups at all times. The mean volume of chest tube drainage did not differ between the groups (Group 1, 495 +/- 216 ml; Group 2, 637 +/- 402 ml; not significant), and no patient required reexploration for bleeding. Eighteen percent of Group 1 and 15% of Group 2 patients received banded blood during their hospitalization and required similar amounts (Group 1, 0.37 unit per patient; Group 2, 0.36 unit per patient; not significant). The use of hetastarch as a postoperative volume expander after myocardial revascularization is safe and effective, and results in substantial financial savings.

RESUMO

Acquired intracardiac shunt after prosthetic valve replacement is a rare complication, and reoperation for correction of this disorder carries a high mortality rate. We report such a case of left ventricular-right atrial fistula after mitral valve replacement in which the patient died of multiple complications. Awareness, prevention, and early diagnosis are, therefore, extremely important.

"The intricate machinery of our health care system can no longer grasp the threads of experience. The mischief that began long before the health care crisis of the 1970s is progressively disabling the vast machinery of medicine. Too often, payers, physicians, and health care executives do not share common insights into the life of the patient. We acknowledge that our common interest is the patient, but we represent that interest from such divergent, even conflicting, viewpoints that everyone loses perspective. As a result, the health care system has become an organism guided by misguided choices; it is unstable, confused, and desperately in need of a central nervous system that can help it cope with the complexities of modern medicine. The problem is our inability to measure and understand the effect of the choices of patients, payers, and physicians on the patient's aspirations for a better quality of life. The result is that we have uninformed patients, skeptical payers, frustrated physicians, and besieged health care executives."

RESUMO

This report presents the successful use of normothermic cardiopulmonary bypass (CPB) and warm continuous retrograde cardioplegia to facilitate coronary artery bypass grafting (CABG) in a patient with cold agglutinins. As a result of their agglutinating action and resultant haemolysis, cold-reactive autoantibodies have the potential for causing morbidity and mortality when a patient is placed on hypothermic CPB and the heart is arrested with profoundly cold cardioplegia. The crux of the situation is to keep the temperature above the critical temperature of the cold agglutinin; this technique guarantees accomplishing that goal.

RESUMO

We report one survivor and two deaths from cocaine toxicity. Cocaine, packaged in condoms, was deliberately ingested to avoid detection by customs agents. Because cocaine is a toxic drug, we recommend surgical removal of the ingested packages, as alternative methods of removal are unreliable and potentially dangerous.

We performed a prospective study of early post-coronary artery bypass graft (CABG) ventilator management and weaning. Sixty-three patients were studied consecutively; 27 were managed by the standard post-CABG weaning practice at this institution, and 36 were managed by respiratory therapists using a protocol with specific ventilation and weaning guidelines. The mean time to extubation was decreased by 41% using the protocol. Arterial blood gas sampling was reduced 42%. There were no deaths in either group. Physicians were alerted to problems necessitating interruption of the protocol in six of 36 patients studied. This protocol proved easy to follow and safe in the hands of respiratory therapists. It lowered costs and improved patient comfort.

Patients with multivessel coronary atherosclerosis and stable angina pectoris were examined by outpatient testing and admitted to a staging area with their families on the morning of coronary bypass surgery. This experimental group was not hospitalized before surgery. Their hospital charges were compared with those of cohorts of coronary artery surgery patients from 1977 to 1981 who met the same entrance criteria. Length of stay in the hospital was reduced by two days. A 10% savings in hospital charges was realized in the 1981 experimental group as compared with the 1981 control group. A comparison of total hospital charges, adjusted for inflation, shows that 1981 experimental group patients paid 35% less for their hospital room, 45% less for their intensive care period, and 17% less in total charges than the 1977 control group. Interviews indicated that these patients with stable cardiac conditions preferred to stay with their families or friends before surgery.
