RESUMO

Acute toxicity data of six heavy metals [Cu, Hg, Cd, Cr(VI), Pb, Zn] to aquatic organisms were collected and screened. Species sensitivity distributions (SSD) curves of vertebrate and invertebrate were constructed by log-logistic model separately. The comprehensive comparisons of the sensitivities of different trophic species to six typical heavy metals were performed. The results indicated invertebrate taxa to each heavy metal exhibited higher sensitivity than vertebrates. However, with respect to the same taxa species, Cu had the most adverse effect on vertebrate, followed by Hg, Cd, Zn and Cr. When datasets from all species were included, Cu and Hg were still more toxic than the others. In particular, the toxicities of Pb to vertebrate and fish were complicated as the SSD curves of Pb intersected with those of other heavy metals, while the SSD curves of Pb constructed by total species no longer crossed with others. The hazardous concentrations for 5 % of the species (HC5) affected were derived to determine the concentration protecting 95 % of species. The HC5 values of the six heavy metals were in the descending order: Zn > Pb > Cr > Cd > Hg > Cu, indicating toxicities in opposite order. Moreover, potential affected fractions were calculated to assess the ecological risks of different heavy metals at certain concentrations of the selected heavy metals. Evaluations of sensitivities of the species at various trophic levels and toxicity analysis of heavy metals are necessary prior to derivation of water quality criteria and the further environmental protection.

RESUMO

Objective:To compare the left ventricular (LV) reverse remodeling after transcatheter aortic valve replacement (TAVR) between patients with bicuspid aortic valve (BAV) stenosis and tricuspid aortic valve (TAV) stenosis. Methods:The data of patients who underwent TAVR procedure from March 2013 to December 2018 in the Second Affiliated Hospital of Zhejiang University were retrospectively reviewed. The patients were divided into BAV group and TAV group according to cardiac computed tomography. Echocardiographic parameters, including aortic valve peak velocity (Vmax), mean gradient (PGmean), effective orifice area(EOA), interventricular septum diastolic thickness (IVSd), left ventricular posterior wall diastolic thickness (LVPWd), left ventricular end diastolic diameter( LVEDd), LV mass index (LVMI), ΔLVMI%, left ventricular ejection fraction( LVEF) of the two groups at baseline, 1 week, 1 month and 1 year post TAVR procedure were obtained and compared.Results:①Compared with preoperative measurements, both groups showed decreases in Vmax, PGmean and increase in EOA at 1 week, 1 month, 1 year follow-ups(all P<0.05). No significant differences were found in Vmax, PGmean, EOA, moderate/sever perivalvular leakage(PVL), moderate/sever prosthetic-patient mismatch(PPM) between BAV group and TAV group at 1 year. ②Both groups showed decreases in IVSd, LVPWd, LVEDd at 1 month, 1 year post TAVR compared with those before the procedure (all P<0.05), as well as increases in LVEF at 1 week, 1 month, 1 year (all P<0.05). Downward trends of LVMI were detected in both groups within 1 year follow-up( P<0.05). ③Compared to TAV group, BAV group showed smaller baseline LVMI( P<0.05), while there were no significant differences in ΔLVMI% post TAVR for all follow-up times of the two groups(all P>0.05). Repeated measures analysis of variance also showed no significant differences in downward trend of LVMI between the two groups after TAVR within 1 year( P>0.05). Conclusions:Left ventricular reverse remodeling can be detected in both BAV and TAV patients after TAVR, which starts from 1 week and can be lasted for 1 year post procedure. Patients with bicuspid morphology might experience similar reverse LV remodeling post TAVR versus patients with tricuspid morphology.

RESUMO

OBJECTIVE@#To compare laboratory and clinical outcomes of fertilization-embryo transfer (IVF-ET) in patients with endometriosis using antagonist protocol, long agonist protocol or prolonged agonist protocol.@*METHODS@#Totally 313 patients with endometriosis were recruited in Reproductive Centers of the Second Affiliated Hospital of Zhejiang University School of Medicine,Jiaxing Women and Children's Hospital, and Ningbo Women and Children's Hospital from April 2017 to October 2018, including 81 patients treated with antagonist protocol (antagonist group), 148 treated with long agonist protocol (long agonist group) and 84 treated with prolonged agonist protocol (prolonged agonist group). The clinical and laboratory data of the patients were retrospectively analyzed to investigate the effect of ovarian stimulation protocols on the IVF-ET outcomes of patients with endometriosis.@*RESULTS@#The average age in the antagonist group patients was significantly higher than those in the other two groups (all 0.05). Fertilization rate and available embryo rate were comparable among the three groups (all >0.05). Considering analysis per cycle with embryo transfer, the human chorionic gonadotrophin (HCG) positive rate, clinical pregnancy rate and total implantation rate showed no significant difference among the three groups (all >0.05). The implantation rate after fresh embryo transfer in the antagonist group was lower than that in the long agonist group (0.05). While the implantation rate of freeze-thaw embryo transfer showed a higher trend in the antagonist group, but there was no significant difference (>0.05). The patients were further divided into diminished and normal ovarian reserve subgroups, the per cycle with embryo transfer, the HCG positive rate, clinical pregnancy rate and total implantation rate still showed no significant difference between two subgroups (all >0.05), no matter in which ovarian stimulation protocol groups. Besides, in women with diminished ovarian reserve, the available embryo rate in antagonist group was significantly higher than that in the long agonist group (<0.05). The amount and duration of Gn application in antagonist group were significantly lower than those in long and prolonged agonist groups (all <0.05).@*CONCLUSIONS@#Patients with endometriosis who used the antagonist protocol in IVF procedure could reduce the cost and time of Gn treatment, when combined with frozen-embryo transfer strategy the antagonist protocol has comparable clinical pregnancy outcome with long or prolonged agonist protocol, especially in those with diminished ovarian reserve, the higher available embryo rate can be achieved.

RESUMO

Objective@#To compare aortic annular diameter measured by transthoracic echocardiography (TTE), transesophageal echocardiography (TEE), and multislice computed tomography (MSCT) in patients with severe aortic stenosis, and to evaluate the impact on selection of prosthetic valve type in transcatheter aortic valve implantation (TAVI).@*Methods@#Clinical data of 138 patients with severe aortic stenosis referred for TAVI between January 2014 and June 2016 in our hospital were retrospectively analyzed. The difference of aortic annular diameter measured by TTE, TEE, and MSCT were compared.TTE was performed after TAVI to evaluate the accuracy of measurement before TAVI.@*Results@#(1) Aortic annular diameter was (23.37±2.22) mm by TTE and (23.52±1.70) mm by TEE (P=0.12). Pearson correlation analysis showed that aortic annular diameter measured by TTE was correlated to that measured by TEE (r=0.87, P<0.05). (2)The long-axis diameter and the short-axis diameter measured by MSCT multiplanar reconstruction were significantly different ((27.86±2.87) mm vs. (21.91±2.53) mm, P<0.05). There was no significant difference between the mean of the long- and short-axis diameters and the diameter derived from cross-sectional area by MSCT ((24.92±2.38) mm vs. (24.84±2.25) mm, P=0.08). However, the diameter derived from the circumference ((25.35±2.34) mm) was significantly larger than the mean of the long- and short-axis diameters and the diameter derived from cross-sectional area by MSCT, and the difference were (0.43±0.62) mm and (0.51±0.62) mm respectively(both P<0.05). (3) Measurements of the aortic annulus diameter by MSCT including the mean of the long- and short-axis diameters, diameter derived from cross-sectional area, and diameter derived from the circumference were larger than the measurement by TTE and TTE (all P<0.05). (4) Implantation was successful in all patients.Moderate to severe paravalvular leakage was detected in 3 patients at 7 days post TAVI, and 1 patient developed severe prosthetic valve restenosis at 6 months post TAVI and received valve-in-valve implantation.@*Conclusions@#In severe aortic stenosis patients referred for TAVI, the aortic annular diameter derived from TTE and TEE measurements are smaller than that from MSCT.In the absence of a gold standard, selection of prosthetic valve type in TAVI procedure should rely on comprehensive considerations, which is of importance to get good clinical results for severe aortic stenosis patients underwent TAVI.

RESUMO

Objective To evaluate the accuracy of a novel echocardiographic three-dimensional automated software for the assessment of the aortic root in candidates for transcatheter aortic valve replacement(TAVR).Methods Four patterns(Lax,Lax-Res,Sax,Sax-Res) of 3D-TEE imaging of aortic root were gotten preoperatively in 18 patients with severe tricuspid aortic stenosis who were referred to our center for TAVR.The specialized 3D-TEE reconstruction software,eSie Valves, then automatically configured a geometric model of the aortic root from the images obtained by 3D-TEE and performed a quantitative analysis of these structures:the minimal diameter(Dmin),maximal diameter(Dmax),area and perimeter(Peri) of aortic annulus, height of the ostia of the left/right coronary artery above the aortic annulus(LOH/ROH).The echo dimensions were compared with the MDCT measurements. Results No statistically significant difference were found in above parameters between the ZOOM pattern (Sax-Res and Lax-Res) and CT measurements(all P>0.05).Lax-Res pattern measurements had good correlation with MDCT,with r valves of 0.81,0.77,0.89,0.84 for Lax-Res-Dmin,Lax-Res-Dmax,Lax-Res-Area,Lax-Res-Peri,respectively(all P<0.05).3D-TEE LOH/ROH had poor correlation with MDCT LOH/ROH (all r<0.7).Conclusions The new automatic 3D-TEE software allows modelling and quantifying the aortic annulus dimensions from 3D-TEE data in patients with tricuspid aortic valves, and Lax-Res pattern is recommended.Quantified assessment of LOH/ROH is not ideal and needs to be improved.

RESUMO

Lung cancer is ranked the first among all malignant neoplastic diseases because of its high morbidity and mortality rate. Thus, lung cancer seriously threatens human health. More than 80%of lung cancer cases are non-small cell lung cancer (NSCLC). Unfortunately, the majority of NSCLC patients are already at the terminal stage of the disease when they seek medical attention. Such patients miss the optimal opportunity of surgery;thus, medical treatment becomes the main choice for them. Recently, molecular-target-ed therapy has made tremendous progress in clinical practice and provides a new potential treatment for patients at the terminal stage. In this review, the authors introduced some molecular-targeted therapeutic drugs to provide benefits for the individualized treatment of patients.

RESUMO

Objective To investigate the inhibitory effects of oridonin combined with gemcitabine on pancreatic cancer SW1990 cells in vitro, and the potential mechanisms thereof. Methods The pancreatic cancer SW1990 cells were treated with vehicle alone and various concentrations (10,20,40,80 and160μmol/L) of oridonin, followed by 24, 48 and 72 h cell culture. Effects of oridonin on cell proliferation were determined by using a CCK-8 kit. SW1990 cells were treated with oridonin (40μmol/L) and gemcitabine (20μmol/L) alone or together for 48 h, and the untreated cells were used as the con-trol. The cell survival rate was detected by CCK-8 assay. Apoptosis induction was assessed by using Annexin V-FITC kit. Semi-quantitative RT-PCR was used to examine the changes of NF-κB mRNA and XIAP mRNA expressions. Results Oridonin inhibited the growth of pancreatic cancer SW1990 cells in a dose-and time-dependent manner. Compared with the other groups, the cell survival rate was significantly lower in the combination group (P<0.05). Oridonin combined with gemcitabine induced a higher percentage of apoptosis in pancreatic cancer cells than that of oridonin or gemcitabine alone (P<0.05). Moreover, the expressions of NF-κB and XIAP mRNA in pancreatic carcinoma cells were obviously down-regu-lated in combination group (P<0.05). Conclusion Oridonin can enhance the antitumor effect of gemcitabine on pancreatic cancer in vitro, which may be related to through the down-regulation of NF-κB and its downstream of XIAP, and then induc-ing cell apoptosis in pancreatic cancer.

RESUMO

The aim of the present study is to construct the recombinant eukaryotic expression and short hairpin RNA (shRNA) expression vectors of microRNA-21 and phosphatase and tensin homolog deleted on chromosome ten (PTEN). According to the gene sequence of microRNA-21 and PTEN, we designed and synthesized two pairs of single-stranded siRNA oligonucleotides and PCR primers. After annealing, the double-stranded DNA oligonucleotides were cloned into vector Psilencer4. 1-CMV. In addition, the gene sequences encoding pre-miR-21 and PTEN were amplified from colorectal cancer cell HCT-116 by RT-PCR. Then the PCR products were digested with restrictive endonuclease enzyme and cloned into vector pEGFP-N1. The constructed recombinant vectors were identified by restrictive digestion and DNA sequence analysis. The positive clone was confirmed by double enzyme digestion, and the enzyme fragments were consistent with the vector and purpose gene sequence. DNA sequencing confirmed that the purpose oligonucleotide fragments were correctly inserted in to the eukaryotic expression plasmids. It could be concluded that the microRNA-21 and PTEN eukaryotic expression and shRNA expression vectors have been successfully constructed, providing a foundation for further study on the effect of miR-21 on human colorectal cancer.

RESUMO

<p><b>OBJECTIVE</b>To examine the protective effects of Astragalus injection (AI) on cisplatin (DDP)-induced nephrotoxicity and to explore the mechanism of its nephroprotection.</p><p><b>METHOD</b>The S180-bearing model was established, and fifty mice were randomly divided into five groups: control mice, DDP-injected mice (3.5 mg x kg(-1)) and AI-pretreated mice (12, 8, 4 g x kg(-1)). Tumor inhibition rate, renal functions, histological findings were investigated. The apoptotic cells were counted by TUNEL and the presence of Bax, Bcl-2 in renal tissues was determined by immunohistochemistry.</p><p><b>RESULTS</b>Administration of DDP or DDP plus AI showed promising anti-tumor activities as compared with control group. Interestingly, a combination of DDP and AI (12, 8 g x kg(-1)) led to enhanced tumor growth inhibition as compared with DDP alone (all P < 0.05). Compared with that from control, the number of TUNEL-positive cells and the Bax/Bcl-2 ratio was increased significantly in the kidneys of DDP treated mice (P < 0.05). High-dose AI pretreatment significantly reduced the elevated number of TUNEL-positive cells and the Bax/Bcl-2 ratio (P < 0.05), and it showed a superior nephroprotective effect than any other dose. AI significantly also decreased both the damage to renal function and histological pathology.</p><p><b>CONCLUSION</b>Pre-treatment with AI attenuates cisplatin-induced nephrotoxicity without compromising the anti-tumor efficacy of DDP, which might be involved in regulating the Bax and Bcl-2 expression.</p>

RESUMO

A new algorithm, the coordinates transform iterative optimizing method based on the least square curve fitting model, is presented. This arithmetic is used for extracting the bio-impedance model parameters. It is superior to other methods, for example, its speed of the convergence is quicker, and its calculating precision is higher. The objective to extract the model parameters, such as Ri, Re, Cm and alpha, has been realized rapidly and accurately. With the aim at lowering the power consumption, decreasing the price and improving the price-to-performance ratio, a practical bio-impedance measure system with double CPUs has been built. It can be drawn from the preliminary results that the intracellular resistance Ri increased largely with an increase in working load during sitting, which reflects the ischemic change of lower limbs.

RESUMO

Objective To investigate whether emodin(1,3,8-trihydroxy-6-methylanthraquinone) can attenuate inflammatory response in rats' lungs with acute necrotic pancreatitis (ANP). Method Acute necrotic pancreatitis model was induced by injection of 3% sodium taurocholic acid into the subcapsular of pancreas and emodin was administered by intestine perfusion. Plasma amylase of 3, 6 and 12 h with acute necrotic pancreatitis was measured together with the detection of IL-1β, IL-6 and IL-10 mRNA expressions in rats' lungs by semi-quantitative RT-PCR. Immunohistochemistry was detected the expression of IL-1β converting enzyme (ICE) in the rats' lungs. Result Plasma amylase of 3, 6 and 12 h with acute necrotic pancreatitis groups are obviously high as compared with normal group(P<0.05). Plasma amylase was (1 611.20±218.72)IU/L in normal group. Plasma amylase of 3, 6 and 12 h with acute necrotic pancreatitis groups were (1 981.40±56.81)IU/L, (3 287.40±612.37)IU/L and (4 914.60±746.82)IU/L. Plasma amylase of 3, 6 and 12 h with acute necrotic pancreatitis after treatment with emodin groups were obviously low as compared with acute necrotic pancreatitis groups. The plasma amylase was(1 617.20±136.80)IU/L,(2 323.40±318.19) IU/L and (2 670.20±390.03)IU/L respectively. The study showed that the mRNA expression of pro-inflammatory cytokin IL-1β and the expression of IL-6, as well as the expression of IL-1β converting enzyme(ICE) were decreased and IL-10 was increased. Conclusion The study demonstrates that emodin plays an important role in reducing plasma amylase level. Emodin exerts anti-inflammatory effects in acute necrotic pancreatitis rats' lungs by downregulating the mRNA expression of IL-1β and IL-6 and upregulating the mRNA expression of IL-10.

It still needs to investigate the promotive effects of electric stimulation in neuranagenesis.OBJECTIVE: To compare the effect and result of percutaneous nerve-muscle electric stimulation therapy in peripheral neuranagenesis of different position.DESIGN: A case analysis based on patients.SETTING: Laboratory of neuroelectrophysiology and department of orthopedics of a medical university affiliated hospital.PARTICIPANTS: Totally 78 patients with peripheral neurologic impairment proved by clinical diagnosis and electromyogram(EMG) were selected from the patients with peripheral nerve injury visited or hospitalized by the Department of Neurology and Orthopedics, Affiliated Hospital of Ningxia Medical University between October 1999 and June 2003.METHODS: Cantana EMG was used. Totally 78 cases received percutaneous nerve-muscle electric stimulation (PNMES) therapy for the observation of the restoration of peripheral nerve function. Therapeutic effectiveness of different course of disease was compared and analyzed by comparing and analyzing the EMG before and after therapy.RESULTS: In 78 patients with brachiplex nerve and sci atic nerve impairment, 53 cases had completely restored motor and sensory functions, 18 cases had significantly improved motor and sensory functions with the effective rate of 91% (71/78) after 1 to 10 courses of therapy. Post-therapeutic denervated potential, motor nerve conduction velocity(MNCV) and sensory nerve conduction velocity(SNCV) were significantly improved compared with that of before therapy.CONCLUSION: PNMES therapy can promote peripheral neuranagenesis, improve the local blood circulation of the impaired nerve and elevate the excitability of nerve and muscle.

RESUMO

Objective To explore the value of high frequency and color doppler ultrasonography on diagnosis of deep venous thrombus in lower extremitiy.Method 32 cases with lower extremitiy deep venous thrombi diagnosed by angiography were further examined by CDFI,36 healthy cases were randomly selected as the normal group (NOR).Results High frequency and CDFI displayed the venous image in normal lower limbs rapidly and clearly as well as the echographic features of different shapes and types of the obstructed vessels. Conclusion High frequency and CDFI as a non-invasive and realtime method is better than angiography in diagnosis of the disease.
