RESUMO

The Solitaire stent is a self-expanding nitinol, fully retrievable stent that was originally designed for applications in cerebral circulation. Expanded indications for its use in mechanical embolectomy during acute ischemic stroke have proven safe and effective. Herein, we describe a novel use of this stent to perform a mechanical thrombectomy of a large left main coronary artery thrombus. © 2016 Wiley Periodicals, Inc.

BACKGROUND: Febrile neutropenia (FN) is a significant adverse effect post-chemotherapy due to its high morbidity and mortality. There are few studies in our country with these kind of patients. OBJECTIVE: To describe the characteristics and mortality in patients with hematologic neoplasms who developed FN post-chemotherapy. METHODOLOGY: A descriptive case series in patients with hematologic neoplasms who developed FN post-chemotherapy in Hospital Pablo Tobón Uribe. RESULTS: 101 episodes of FN in 43 patients. The median age was 44 years. 63.5% of patients had no apparent clinical focus of infection at admission, 11.8% had soft tissue compromise and 8.9% urinary tract infection. Bacteremia was documented in 41.5% and catheter-associated bacteremia in 3.9%. The most common organisms were Escherichia coli 43.4%, Klebsiella pneumoniae 17.3% and Staphylococcus aureus 8.6%. Of those isolated in blood 84.7% were Gram negative rods and 15.2% were Gram positive bacteria. Piperacillin/tazobactam was the most common empirically prescribed antibiotic (81.1%). Mortality of FN episodes occurred in 8 (7.92%) patients, 5 (62.5%) attributable to infection and 3 (37.5%) due to progression of hematologic malignancy with a resolution of FN. CONCLUSIONS: In our case series of FN the microbiological characteristics differed significantly from developed countries, but a similar mortality rate per episode was observed.

RESUMO

We report the case of a 61 year-old male who underwent heart transplantation eight months before developing a systemic condition with central nervous system, lung, kidney, colonic, cutaneous, and hematologic involvement, found to be secondary to a systemic toxoplasmosis despite co-trimoxazole prophylaxis in a previous-to-transplant seronegative patient receiving a heart from a seropositive donor. A review of prophylactic options in our environment is discussed.

Resumen Se reporta el caso de un paciente de sexo masculino, de 61 años de edad, quien ocho meses después de someterse a un trasplante de corazón presentó una enfermedad sistémica con compromiso del sistema nervioso central y del sistema inmunológico, así como de pulmón, riñón, colon y piel, y a quien finalmente se le diagnosticó toxoplasmosis diseminada, a pesar de haber recibido profilaxis con trimetoprim-sulfametoxazol, debido a que el órgano provenía de un donante positivo para toxoplasmosis siendo él un receptor negativo. Se discuten las opciones de profilaxis en nuestro medio.

Más de la mitad de los pacientes con infección por virus de inmunodeficiencia humana (VIH) pueden tener compromiso pulmonar en el transcurso de su vida. Este puede ser multicausal y las infecciones oportunistas son las principales, sin embargo, las causas no infecciosas no son menos importantes. Objetivo: Describir las características del compromiso pulmonar en pacientes hospitalizados con infección por VIH. Metodología: Estudio descriptivo observacional. Resultados: Se incluyó a 63 pacientes, el 85,7% fueron hombres. La edad promedio fue 40,6 ± 12 años. El 21,4% tuvieron antecedente de promiscuidad sexual y 28,6% fueron hombres con sexo con hombres. Se identificó tabaquismo en 60,3%, alcoholismo en 52,4% y consumo de drogas en 20,6%. El 23,8% tuvo historia de tuberculosis. Al ingreso, el 79,4% tenía sida, el recuento promedio de CD4 fue 138,5 ± 17,7 células/mm3 y el 49,2% tenía CD4 < 100 células/mm3. Las principales comorbilidades fueron: enfermedad neoplásica 20,6%, diarrea crónica 19%, EPOC 6,3% y diabetes mellitus 3,2%. Los principales síntomas al ingreso fueron: respiratorios 66,7%, gastrointestinales 47,6% y neurológicos 34,9%. Los principales oportunistas documentados fueron Mycobacterium spp, H. capsulatum y P. jirovecii. El sarcoma de Kaposi, la enfermedad lifoproliferativa y la EPOC fueron causas de compromiso no infeccioso. Los principales hallazgos radiográficos fueron: infiltrados intersticiales (42,9%), compromiso pleural (23,8%) y lesiones cavitarias (7,9%). El compromiso multilobar fue evidente en el 15,9%. Las principales complicaciones fueron falla respiratoria, disfunción orgánica múltiple y falla renal aguda. El 19% requirió ingreso a UCI y, de estos, el 83,3% necesitó ventilación mecánica. La estancia hospitalariapromedio fue de 18 ± 20 días y la mortalidad del 17,5%. Discusión: Las infecciones por oportunistas son las principales causas de compromiso pulmonar y, dentro de estas, las causadas por micobacterias. Puede ocurrir más de una infección oportunista simultáneamente, pero en nuestra población fueron infrecuentes. Las causas noinfecciosas también son importantes.


More than a half of patients with HIV infection have pulmonary involvement throug-hout their lives. Opportunistic infections are the main cause; however, many patients havenoninfectious pulmonary involvement. Objective: To describe the characteristics of pulmonary involvement in inpatients with HIVinfection.Methods: Descriptive observational study. Results: A total of 63 patients were included, and 85.7% were men. The average age was40.6 ± 12 years. Some 21.4% had a history of sexual promiscuity and 28.6% were men whohad sex with men. Smoking was found in 60.3%, alcoholism in 52.4% and drug abuse in 20.6%.A tuberculosis history was documented in 23.8% of patients. At admission, 79.4% had AIDS, themean CD4 count was 138.5 ± 17.7 cells/mm3and 49.2% had CD4 counts < 100 cells/mm3. Majorcomorbidities included neoplastic disease in 20.6%, chronic diarrhea in 19%, COPD in 6.3% anddiabetes in 3.2%. Major opportunistic agents were Mycobacterium spp, H. capsulatum and P.jirovecii. Kaposi's sarcoma, lymphoproliferative disease and COPD were causes of noninfectiouspulmonary involvement. The main radiographic findings included interstitial infiltrates in 42.9%,pleural involvement in 23.8%, alveolar infiltrates in 7.9% and cavitary lesions in 7.9%. Multilobarcompromise was evident in 15.9% of the patients. The major complications were respiratoryfailure, multiple organ dysfunction and acute renal failure. Some 19% required care in the ICUand 83.3% of these required mechanical ventilation. The mean stay was 18 ± 20 days and themortality was 17.5%. Discussion: Opportunistic infections are the main causes of lung involvement and mycobacterialdiseases were most frequent. Mycobacterial diseases can occur simultaneously with an oppor-tunistic infection, but in our series this was infrequent. Non-infectious diseases are importantbut are less common.

Antecedentes: Las infecciones por micobacterias no tuberculosas (MNT) se describen en los últimos años con mayor frecuencia, especialmente en pacientes con inmunosupresión y en pacientes tratados por procedimientos estéticos. Las MNT incluyen especies del género Mycobacterium , diferentes del complejo Mycobacterium tuberculosis y Mycobacterium leprae . Objetivo: Describir las características demográficas y clínicas de pacientes hospitalizados con infecciones por MNT. Metodología: Estudio descriptivo retrospectivo. Resultados: De 187 pacientes con infección por micobacterias documentadas por cultivo, 17 (9,1%) tuvieron infección por MNT. Edad promedio de 38,4 ± 19,2 años. El 58,82% fueron hombres. Las principales comorbilidades fueron VIH/sida (41,17%), diabetes mellitus (23,53%), enfermedad renal crónica (17,64%), terapia inmunosupresora (17,64%) y neoplasias (17,64%). En los coinfectados con VIH el recuento de CD4 fue <50 en 85,71%. Las especies más frecuentes fueron complejo M. avium (CMA) 35,29%, M. abscessus (17,65%) y M. chelonae (11,76%). Las formas de infección fueron: diseminada (35,29%), pulmonar (23,53%), piel y tejidos blandos (17,64%) y gastrointestinal (11,76%). Estancia promedio de 22,1 días; un 23,53% requirió atención en UCI. La mortalidad general fue 23,53%. Conclusión: Las infecciones por MNT causan una serie de condiciones patológicas, los pacientes inmunocomprometidos son la población de mayor riesgo y las formas diseminada y pulmonar,las más frecuentes. La sospecha temprana así como la toma de muestras adecuadas y el uso de métodos diagnósticos apropiados son indispensables para su diagnóstico oportuno y tratamiento adecuado.


Background: Nontuberculous mycobacteria (NTM) infections has been described more frequently in recent years, especially in immunosuppression conditions and after cosmetic surgical procedures. The NTM include species of the genus Mycobacterium , other than Mycobacterium tuberculosis complex and Mycobacterium leprae. Objective: To describe the demographic and clinical characteristics of Colombian in-patientswith NTM infections. Methodology: A retrospective descriptive study. Results: In 187 patients with culture- confirmed mycobacterial infection, 17 (9,1%) had NTM.The mean age was 38,4 ± 19,2 and 58,82% were men. Major comorbidities were: HIV/AIDS(41,1%), diabetes mellitus (23,5%), chronic renal disease (17,6%), immunosuppressive therapy(17,6%) and neoplasms (17,6%). In patients co-infected with HIV, CD4 count was <50 in 85,7%.The most frequent species were M. avium complex (MAC) in 35,2%, M. abscessus in 17,6% and M. chelonae in 11,7%. Infections were disseminated (35,2%), pulmonary (23,5%), skin and soft tissue (17,6%) and in gastrointestinal system (11,7%). The average hospital stay was 22,1 day sand 23,5% required intensive care unit. Overall mortality was 23,5%. Conclusion: MNT infections cause a number of pathological conditions, being more frequent in immunocompromised patients. The disseminated and pulmonary forms were the most common. Early clinical suspicion and appropriate samples and diagnostic assays, are crucial for early diagnosis and treatment.

Introducción: La neutropenia febril (NF) es un efecto adverso importante post-quimioterapia por su alta morbi-mortalidad. Hay pocos estudios en nuestro entorno con estos pacientes. Objetivo: Describir las características de los pacientes adultos con neoplasia hematológica que desarrollaron NF post-quimioterapia. Metodología: Estudio descriptivo de serie de casos, en pacientes con neoplasia hematológica y NF post-quimioterapia en el Hospital Pablo Tobón Uribe. Resultados: Ciento un episodios de NF en 43 pacientes con una mediana de edad de 44 años. El 63,5% no tenían foco infeccioso clínico aparente al ingreso, 11,8% tenía compromiso de tejidos blandos y 8,9% foco urinario. Se documentó bacteriemia primaria en 42 (41,5%) y bacteriemia asociada al catéter en 4 (3,96%). Los microorganismos más frecuentes fueron Escherichia coli 43,4%, Klebsiella pneumoniae 17,3% y Staphylococcus aureus 8,69%. De los aislados en sangre, 84,7% fueron bacilos gramnegativos y 15,2% cocáceas grampo-sitivas. Piperacilina/tazobactam fue la antibioticoterapia empírica inicial en 81,1% de los episodios. La mortalidad por episodios de NF fue de 7,92%, en 62,5% atribuible a la infección y en el resto a progresión de la neoplasia hematológica con resolución de la NF. Conclusión: Serie de casos de NF con características microbiológicas que difieren significativamente a los países desarrollados, pero con una mortalidad por episodios similar.
