RESUMO

The persistent water treatment and separation challenge necessitates innovative and sustainable advances to tackle conventional and emerging contaminants in the aquatic environment effectively. Therefore, a unique three-dimensional (3D) network composite film (BNC-KC) comprised of bacterial nanocellulose (BNC) incorporated nano-kaolinite clay particles (KC) was successfully synthesized via an in-situ approach. The microscopic characterization of BNC-KC revealed an effective integration of KC within the 3D matrix of BNC. The investigated mechanical properties of BNC-KC demonstrated a better performance compared to BNC. Thereafter, the sorption performance of BNC-KC films towards basic blue 9 dye (Bb9) and norfloxacin (NFX) antibiotic from water was investigated. The maximum sorption capacities of BNC-KC for Bb9 and NFX were 127.64 and 101.68 mg/g, respectively. Mechanistic studies showed that electrostatic interactions, multi-layered sorption, and 3D structure are pivotal in the NFX/Bb9 sorption process. The intricate architecture of BNC-KC effectively traps molecules within the interlayer spaces, significantly increasing sorption efficiency. The distinctive structural configuration of BNC-KC films effectively addressed the challenges of post-water treatment separation while concurrently mitigating waste generation. The environmental evaluation, engineering, and economic feasibility of BNC-KC are also discussed. The cost estimation assessment of BNC-KC revealed the potential to remove NFX and Bb9 from water at an economically viable cost.

OBJECTIVES: This study aimed to evaluate patient-reported quality of life and incidence of decision regret in patients undergoing radial (RFFF) and ulnar forearm-free flaps (UFFF) reconstruction. MATERIALS AND METHODS: Patients undergoing either RFFF or UFFF were assessed with the University of Washington Quality of Life (UW-QOL) and Oral Health Impact Profile (OHIP-14) questionnaires, and the Decision Regret Scale (DRS), both before and at least 12 months post-reconstruction. RESULTS: In total, 40 RFFF and 40 UFFF were included. Harvesting time was longer in RFFF (p = 0.043), and the donor-site defect was significantly larger in RFFF than in UFFF (p = 0.044). Patients with UFFF scored better UW-QOL in the appearance, pain, activity, mood, and social functioning domains (p < 0.05). However, the RFFF group excelled in swallowing and chewing domains. The DRS score revealed a significant difference between RFFF and UFFF, with scores of 36.26 versus 27.36, respectively. Moreover, the mean DRS score reduced at 12 months compared with 6 months, significantly superior for UFFF. CONCLUSION: Oral cancer patients reconstructed with UFFF exhibited a better appearance, social domain, and mild decision regret compared with RFFF, indicating that the UFFF may contribute to improving postoperative quality of life in oral cancer patients.

The continuous increase of urbanization and industrialization brought various climatic changes, leading to global warming. The unavailability of meteorological data makes remotely sensed data important for understanding climate change. Therefore, the land surface temperature (LST) is critical in understanding global climate changes and related hydrological processes. The main objective of this work is to explore the dominant drivers of land use and hydrologic indices for LST in drainage and non-drainage areas. Specifically, the relationship between LST changes, land use, and hydrologic indices in Northeast Qena, Egypt, was investigated. The Landsat 5 and 8 imagery, Geographic Information System (GIS), and R-package were applied to identify the change detection during 2000-2021. The normalized difference between vegetation index (NDVI), bare soil index (BSI), normalized difference built-up, built-up index (BUI), modified normalized difference water index (MNDWI), and soil-adjusted vegetation index (SAVI) were employed. The non-drainage or mountain areas were found to be more susceptible to high LST values. The comprehensive analysis and assessment of the spatiotemporal changes of LST indicated that land use and hydrologic indices were driving factors for LST changes. Considerably, LST retrieved from the Landsat imaginary showed significant variation between the maximum LST during 2000 (44.82°C) and 2021 (50.74°C). However, NDBI has got less spread during the past (2000) with 10-13%. A high negative correlation was observed between the LST and NDVI, while the SAVI and LST positively correlated. The results of this study provide relevant information for environmental planning to local management authorities.

Oral submucous fibrosis (OSF) is a chronic, progressive condition affecting the oral mucosa associated with areca nut consumption. It leads to restricted tongue movement, loss of papillae, blanching and stiffening of the mucosa, difficulty in opening the mouth, and challenges in eating due to inflammation and fibrosis. This report presents a rare case of oropharyngeal stenosis secondary to OSF in a 43-year-old male with a history of chewing betel nut. A surgical procedure similar to Uvulopalatopharyngoplasty was performed to excise the submucous oropharyngeal stenosis and to reconstruct the uvula, palatoglossal arch, and palatopharyngeal arch. At 8 years postoperatively, the patient exhibited a normal mouth opening and oropharyngeal aperture.

OBJECTIVE: This study aimed to evaluate enamel surface integrity and time consumed during residual cement removal after bracket debonding using different adhesive removal burs with and without a dental loupe. MATERIAL AND METHODS: Sixty human-extracted premolars were collected, cleaned, mounted, and prepared for orthodontic bracket bonding. Teeth were randomly divided into three main groups (n = 20) based on the adhesive removal method: tungsten carbide system (TC), sof-lex discs system (SD), and diamond system (DB) groups. Then, each group was subdivided into two subgroups (naked eye and magnifying loupe subgroups). The brackets were bonded and then debonded after 24 h, and the Adhesive Remnant Index (ARI) was assessed. The adhesive remnants were removed by different systems, and the final polishing was performed by Silicone OneGloss. The enamel surface roughness was evaluated before bracketing (T0), after residual cement removal (T1), and finally after polishing (T2) using surface Mitutoyo SJ-210 profilometry and Scanning Electron Microscopy (SEM) to determine the Enamel Damage Index (EDI) score. The time consumed for adhesive removal was recorded in seconds. RESULTS: The Kruskal Wallis test showed a statistically significant difference in roughness values at T1 compared to T2 between subgroups (p < 0.001). When comparing EDI at T1 and T2, the Kruskal-Wallis H-test showed statistically significant differences in all subgroups. The pairwise comparisons revealed that EDI scores showed a statistically significant difference at T1 and T2 between DB vs. TC and SD (p = 0.015) but not between TC vs. SD (p = 1.000), indicating the highest roughness value observed in the DB group. The time for cement removal was significantly shorter in the magnifying loupe group than in the naked eye group and was shortest with the TC group, whereas the time was the longest with the DB group (p < 0.05). CONCLUSION: All three systems were clinically satisfactory for residual orthodontic adhesive removal. However, TC system produced the lowest enamel roughness, while the DB system created the greatest. The polishing step created smoother surfaces regardless of the systems used for resin removal.

Global water security and energy demands associated with uncontrollable population growth and rapid industrial progress are one of the utmost serious needs dangerously confronting humanity. On account of waste as a wealth strategy; a multifunctional eco-friendly sorbent (MGAP) from green alga was prepared successfully for remediation of cationic/anionic organic dyes and biohydrogen production. The structural and morphological properties of sorbent were systematically scrutinized by a variety of spectral analyses. The loading capacity of MGAP towards rhodamine B (RhB) and methyl orange (MO) dyes was inclusivity inspected under variable experimental conditions. The adsorption kinetics of both dyes onto MGAP was in good agreement with pseudo-second-order theory, whereas adsorption isotherms could fit well with the Langmuir model, with satisfactory loading capacities of 144.92 and 196.04 mg g-1 for RhB and MO molecules, respectively. Moreover, ultra-sonication treatment admirably decreased the sorption equilibrium time from 180.0 min to 30.0 min. Furthermore, spent sorbent was managed particularly for biohydrogen production with a measured yield of 112.89, 116.59, and 128.17 mL-H2/gVS for MGAP, MGAP-MO, and MGAP-RhB, respectively. Overall, the produced MGAP can potentially be offered up as a promising dye scavenger for wastewater remediation and biohydrogen production, thereby fulfilling waste management and circular economy.

Engineering research has been expanded by the advent of material fusion, which has led to the development of composites that are more reliable and cost-effective. This investigation aims to utilise this concept to promote a circular economy by maximizing the adsorption of silver nanoparticles and silver nitrate onto recycled chicken eggshell membranes, resulting in optimized antimicrobial silver/eggshell membrane composites. The pH, time, concentration, and adsorption temperatures were optimized. It was confirmed that these composites were excellent candidates for use in antimicrobial applications. The silver nanoparticles were produced through chemical synthesis using sodium borohydride as a reducing agent and through adsorption/surface reduction of silver nitrate on eggshell membranes. The composites were thoroughly characterized by various techniques, including spectrophotometry, atomic absorption spectrometry, scanning electron microscopy, transmission electron microscopy, Fourier transform infrared spectroscopy, and X-ray photoelectron spectroscopy, as well as agar well diffusion and MTT assay. The results indicate that silver/eggshell membrane composites with excellent antimicrobial properties were produced using both silver nanoparticles and silver nitrate at a pH of 6, 25 °C, and after 48 h of agitation. These materials exhibited remarkable antimicrobial activity against Pseudomonas aeruginosa and Bacillus subtilis, resulting in 27.77% and 15.34% cell death, respectively.

OBJECTIVE: This study aimed to investigate the effect of inflammatory states following impacted lower third molar (ILTM) surgery regarding postoperative bleeding and wound healing. METHODS: The study included patients who underwent extraction of ILTMs associated with or without inflammatory conditions. Post-extraction bleeding and wound healing were assessed. In addition, mean grey values (MGVs) of alveolar bone and bone height using an orthopantomography radiograph were analyzed. RESULTS: A total of 376 patients were enrolled; 171 pericoronitis, 51 pulpitis, 44 chronic periapical periodontitis, 36 chronic periodontitis, and 74 control. The bleeding score in the control group was significantly lower than in the periapical periodontitis and periodontitis groups. Excellent wound healing for control, pericoronitis, pulpitis, periapical periodontitis, and periodontitis groups was (78.38%, 35.67%, 70.59%, 70.45%, and 33.33%, respectively). Patients with pericoronitis and periodontitis had significantly poorer wound healing (P < 0.01). The MGV in periapical periodontitis and periodontitis was considerably lower than in the control group. CONCLUSIONS: The inflammatory conditions associated with ILTMs increase the risk of bleeding. So suturing with the placement of local hemostatic agents over a pressure pack alone is recommended. The poorest wound healing was in localized gingival inflammation. Furthermore, MGV was affected by age and was lower with periapical periodontitis.

OBJECTIVE: This study aimed to assess the alveolar bone changes following maxillary and mandibular molars' intrusion and extrusion movements using Clear Aligners using Cone-Beam Computed Tomography (CBCT). MATERIALS AND METHODS: This is a retrospective clinical study in which 24 adult patients with pre-designed selection criteria and a mean age of 31.1 + 9.9 years were enrolled. The alveolar bone changes around one hundred thirty-three maxillary and mandibular molars intruded or extruded by Clear Aligners therapy were analyzed from CBCT using Invivo 6.0 software. Intra- and inter-examiner reliability analysis was performed using the intra-class correlation coefficient (ICC) and Cronbach's Alpha statistics. The paired t-test was used to analyze significant differences before and after treatment (T0-T1). The significance level was considered at P < 0.05. RESULT: The patients were divided into two groups: extrusion (48.9%, n = 65 molars' root) and intrusion (51.1%, n = 68 molars' root) group. There was a significant decrease in the alveolar bone changes in the buccal surface of the mandibular right and left 1st molars in the extrusion group (-1.05 ± 0.97, -0.76 ± 1.12 mm, respectively) and the maxillary left 2nd molars in intrusion group (-0.42 ± 0.77 mm), and the lingual surface of intrusion of the mandibular left 1st molar (-0.64 ± 0.76 mm). Comparing the mean maxillary and mandibular changes (T0-T1) of both studied groups showed that the buccal alveolar bone changes for the left 1st and right 2nd molars showed a significant difference in extrusion and intrusion groups, respectively. CONCLUSIONS: The buccal alveolar bone changes is considered the most affected surface following maxillary and mandibular molars' intrusion and extrusion movements using clear aligners, with mandibular molars being more affected than the maxillary ones.

In the present era of significant industrial development, the presence and dispersal of countless water contaminants in water bodies worldwide have rendered them unsuitable for various forms of life. Recently, the awareness of environmental sustainability for wastewater treatment has increased rapidly in quest of meeting the global water demand. Despite numerous conventional adsorbents on deck, exploring low-cost and efficient adsorbents is interesting. Clays and clays-based geopolymers are intensively used as natural, alternative, and promising adsorbents to meet the goals for combating climate change and providing low carbon, heat, and power. In this narrative work, the present review highlights the persistence of some inorganic/organic water pollutants in aquatic bodies. Moreover, it comprehensively summarizes the advancement in the strategies associated with synthesizing clays and their based geopolymers, characterization techniques, and applications in water treatment. Furthermore, the critical challenges, opportunities, and future prospective regarding the circular economy are additionally outlined. This review expounded on the ongoing research studies for leveraging these eco-friendly materials to address water decontamination. The adsorption mechanisms of clays-based geopolymers are successfully presented. Therefore, the present review is believed to deepen insights into wastewater treatment using clays and clays-based geopolymers as a groundbreaking aspect in accord with the waste-to-wealth concept toward broader sustainable development goals.

The presence of emerging pollutants especially hazardous chemicals and pharmaceuticals in aquatic environments is a matter of grave concern to human health and the environment. In this study, coffee bean waste (CBW) was utilized to synthesize pristine (CBW550) and activated (CBW550HPO) biochars for the elimination of diclofenac (DF) and levofloxacin (LEV) from water. A facile two-step approach was used to synthesize CBW550HPO using chemical pretreatment and pyrolysis under N2 purging. BET results of CBW550HPO revealed that chemical pretreatment increased surface area by approximately 160 times compared to CBW550. The calculated ID/IG ratio from Raman spectra confirmed that CBW550HPO had a high functionalized surface. Different operational parameters such as contact time, pH, adsorbent dose, ionic strength, and adsorbate concentration were studied and optimized. Maximum Langmuir adsorption capacity of CBW550HPO was found to be 61.17 and 110.70 mg/g for DF and LVX, respectively. Experimental results demonstrated that presence of NaCl in solution enhanced DF removal efficiency due to the salting-out effect. Electrostatic attraction, π-π bonding, and hydrophobic interaction were prominently responsible mechanisms for the adsorption of DF and LVX. Furthermore, continuous-flow mode studies confirmed that CBW550HPO can be successfully utilized in large-scale treatment applications.

PURPOSE: To evaluate: 1) the efficacy of acellular dermal matrix (ADM) to prevent Frey's syndrome (FS) after superficial parotidectomy of benign tumors. 2) the effect of different follow-up intervals on the incidence of FS. METHODS: Fifty-five patients with benign parotid gland tumors underwent superficial parotidectomy were divided into two groups: Non-ADM group (n = 31, 56.3%) and ADM group (n = 24, 43.6%). The primary outcomes measure was the incidence of FS. The secondary outcomes were surgical site depression, infection, salivary sialocele, and salivary fistula. Subjective FS was evaluated using a clinical questionnaire submitted via WeChat at 3, 6, 12, 18, 24, and 32 months postoperatively. Objective FS was evaluated using Minor starch-iodine test at 6 and 12 months postoperatively. RESULTS: There was a statistically significant difference in the incidence of subjective and objective FS when ADM compared with Non-ADM groups (P<0.05). The respective incidence of subjective FS at 3, 6, 12, 18, 24, and 32 months was 4.2%, 8.3%,20.8%, 20.8%, 20.8%, 20.8% in ADM group and 3.2%, 9.7%, 29%, 38.7%, 45.2% in Non-ADM group. The incidence of objective FS after 6 and 12 months was 4.2%, 8.3% in ADM group and 8.3% (2/24), 38.7% (12/31) in Non-ADM group respectively. Five patients in ADM group and 1 patient in Non-ADM group developed sialocele, and all patients resolved with conservative management. Surgical site depression was significantly higher in the Non-ADM group (15/31) compared with (5/24) in ADM (P=0.049). No cases of immune rejection, infection, hematoma, or salivary fistula were observed in ADM group. CONCLUSIONS: The current study demonstrated that 1) ADM could reduce the incidence of FS in the patients undergoing superficial parotidectomy. 2) the peak onset of about 18 months appear to be acceptable follow-up for the development of FS.

Harnessing coastal biowaste for dual valorization in water treatment and biofuel production holds paramount importance for sustainability and resource challenges. This study investigated the potential of engineered composite (CABC) derived from coastal biowaste-based materials for tetracycline (TC) removal and biomethane production. High-yield calcium carbonate (CaCO3; 95.65%; bivalve shells) and biochar (GA-BC; 41.50%; green macroalgae) were produced and used as precursors for CABC. The characterization results revealed presence of ß-CaCO3 and ν2-CO3 aragonite in CaCO3, and composite homogeneity was achieved. The CABC exhibited a maximum TC sorption capacity of 342.26 mg/g via synergistic sorption mechanisms (i.e., surface/pore filling, electrostatic attraction, calcium ion exchange, and chelation). Supplementation of anaerobic digestion process with GA-BC, CaCO3, and CABC was investigated via three consecutive cycles. Biochemical methane potential of glucose as a sole substrate was increased from 157.50 to 217.00, 187.00, and 259.00 mL-CH4, while dual substrate (glucose+TC) treatment was increased from 94.5 to 146.5, 129.0, and 153.00 mL-CH4 for GA-BC, CaCO3, and CABC, respectively. Moreover, system stability and TC removal were increased with the addition of GA-BC (40.90%), CaCO3 (16.30%), and CABC (53.70%). Therefore, this study exemplifies the circular bioeconomy approach, demonstrating the sustainable use of biowaste-derived composite for water treatment and biofuel production.

The gas flaring network is an inseparable constituent commonly present in most of the oil and gas refineries and petrochemical facilities conferring reliable operational parameters. The improper disposal of burn-off gases improperly results in environmental problems and loss of economic resources. In this regard, waste to energy transforming nexus, in accord with the "carbon neutrality" term, has potentially emerged as a reasonable pathway to preserve our planet. In a transdisciplinary manner, the present review article deeply outlines the different up-to-date strategies developed to recover the emitted gases (flaring minimization) into different value-added products. To analyze the recovery potential of flare gases, different technologies, and decision-making factors have been critically reviewed to find the best recovery methods. We recommend more straightforward recovery methods despite lower profits. In this regard, electricity generation seems to be an appropriate option for application in small amounts of flaring. However, several flare gas utilization processes such as syngas manufacturing, reinjection of gas into petroleum reservoirs, and production of natural gas liquid (NGL) are also recommended as options because of their economic significance, technological viability (both onshore and offshore), and environmental benefits. Moreover, the adopted computational multi-scale data assimilation for predictive modeling of flare gas recovery scenarios has been systematically reviewed, summarized, and inspected.

OBJECTIVE: To evaluate the impact of molar distalization with clear aligners (CAs) on maxillary sinus parameters and its proximity to the root apices of maxillary molars using Cone Beam Computed Tomography (CBCT). METHODS: The study involved pre- and post-treatment CBCT images of 27 adult patients. MIMICS 21.0 software was used to measure maxillary sinus volume and surface area, while Invivo Dental 6.0 program was used to measure the sinus diameters and proximity of the maxillary molar root apices to the sinus floor. RESULTS: Post-treatment, there was a marked increase in the average volume and surface area of the maxillary sinus on both sides; however, changes in dimensions were not statistically significant. Sinus proximity to the root apices of the maxillary molars showed statistically significant differences, particularly on the left side for all roots. Gender and age impacted the outcomes, with males and younger individuals showing more pronounced changes. No significant differences were observed between the left and right sinuses in all parameters. CONCLUSION: Molar distalization with CAs increases maxillary sinus parameters, more notably in younger and male patients. All molar teeth roots moved toward the sinus, particularly the mesiobuccal root of the maxillary second molar protruded to the sinus.

BACKGROUND: Head and neck cancer is the sixth most common malignancy worldwide, and oral squamous cell carcinoma (OSCC) is the most common head and neck cancer, being one of the leading causes of cancer morbidity and mortality worldwide. CC Chemokine receptor 7(CCR7) is a multifunctional G protein-coupled trans-membrane chemokine that affects immune cell chemotaxis, migration, and cancer progression through its interaction with its ligands C-C motif chemokine ligand 19(CCL19) and C-C motif chemokine ligand 21(CCL21). Numerous studies have demonstrated the involvement of CCR7 in the malignant progression of a variety of cancers, reflecting the pro-cancer properties of CCR7. The Cancer Genome Atlas data suggests CCR7 has elevated expression in oral cancer. Specifically, CCR7 expression in tumor microenvironment (TME) may regulate the ability of some immune cells to engage in anti-tumor immune responses. Since CD8+ T cells have become a key immunotherapeutic target, the role of CCR7 in antitumor immune response of naïve CD8+ T cells in TME has not been thoroughly investigated. METHODS: A CCR7 knockout mouse model was constructed, and the mechanism of ccr7 on the regulation of tumor microenvironment by naïve CD8+ T cells was verified under the guidance of single-cell RNA sequencing combined with in vivo animal experiments and in vitro cell experiments. RESULTS: CCR7 is knocked out with impaired tumor growth and altered CD8+ T cell profiles, revealing the importance of this protein in OSCC. CONCLUSIONS: Inhibition of CCR7 enhances CD8+ T cell activation, proliferation, and anti-tumor function, suggesting its potential as a therapeutic target.

The rapid development and mutations have heightened ceramic industrialization to supply the countries' requirements worldwide. Therefore, the continuous exploration for new reserves of possible ceramic-raw materials is needed to overwhelm the increased demand for ceramic industries. In this study, the suitability assessment of potential applications for Upper Cretaceous (Santonian) clay deposits at Abu Zenima area, as raw materials in ceramic industries, was extensively performed. Remote sensing data were employed to map the Kaolinite-bearing formation as well as determine the additional occurrences of clay reserves in the studied area. In this context, ten representative clayey materials from the Matulla Formation were sampled and examined for their mineralogical, geochemical, morphological, physical, thermal, and plasticity characteristics. The mineralogical and chemical compositions of starting clay materials were examined. The physicochemical surface properties of the studied clay were studied utilizing SEM-EDX and TEM. The particle-size analysis confirmed the adequate characteristics of samples for white ceramic stoneware and ceramic tiles manufacturing. The technological and suitability properties of investigated clay deposits proved the industrial appropriateness of Abu Zenima clay as a potential ceramic raw material for various ceramic products. The existence of high kaolin reserves in the studied area with reasonable quality and quantity has regional significance. It would significantly help reduce the manufacturing cost and overwhelm the high consumption rate. The ceramic manufacturers in the investigated areas are expected to bring steady producers into the industry in the long term to gain the advantage of low-cost raw materials, labor, and factory construction.

This study investigated the successful synthesis of functionalized algal biochar-clay composite (FBKC). Subsequently, the sorption performance of FBKC towards norfloxacin (NFX) antibiotic and crystal violet dye (CVD) from water was extensively assessed in both batch and continuous flow systems. A series of characterization techniques were carried out for FBKC and the utilized precursors, indicating that the surface area of FBKC was increased thirty-fold with a well-developed pore structure compared to the original precursors. FBKC demonstrated a maximum sorption capacity of 192.80 and 281.24 mg/g for NFX and CVD, respectively. The suited fitting of the experimental data to Freundlich and Clark models suggested multi-layer sorption of NFX/CVD molecules. The mechanistic studies of NFX/CVD sorption onto FBKC unveiled multiple mechanisms, including π-π interaction, hydrogen bonding, electrostatic attraction, and surface/pore filling effect. The estimated cost of 5.72 €/kg and superior sorption capacity makes FBKC an efficient low-cost sorbent for emergent water pollutants.

In this study, the functionalized smectitic clay (SC)-based nanoscale hydrated zirconium oxide (ZrO-SC) was successfully synthesized and utilized for the adsorptive removal of levofloxacin (LVN) from an aqueous medium. The synthesized ZrO-SC and its precursors (SC and hydrated zirconium oxide (ZrO(OH)2)) were extensively characterized using various analytical methods to get insight into their physicochemical properties. The results of stability investigation confirmed that ZrO-SC composite is chemically stable in strongly acidic medium. The surface measurements revealed that ZrO impregnation to SC resulted in an increased surface area (six-fold higher than SC). The maximum sorption capacity of ZrO-SC for LVN was 356.98 and 68.87 mg g-1 during batch and continuous flow mode studies, respectively. The mechanistic studies of LVN sorption onto ZrO-SC revealed that various sorption mechanisms, such as interlayer complexation, π-π interaction, electrostatic interaction, and surface complexation were involved. The kinetic studies of ZrO-SC in the continuous-flow mode indicated the better applicability of Thomas model. However, the good fitting of Clark model suggested the multi-layer sorption of LVN. The cost estimation of the studied sorbents was also assessed. The obtained results indicate that ZrO-SC is capable of removing LVN and other emergent pollutants from water at a reasonable cost.

BACKGROUND: A poor evidence basis exists regarding direct comparing objective and subjective donor site morbidity associated with forearm flap. The authors evaluated the postoperative donor-site complications and quality of life outcomes between radial forearm free flap (RFFF) and ulnar forearm free flap (UFFF). METHODS: All patients undergoing RFFF or UFFF harvest were included. Grip strength, pinch strength, wrist range of movement, and testing skin sensitivity were assessed with the appropriate scales at different time intervals. In addition, cosmetic and quality of life were assessed using the Patient and Observer Scar Assessment Scale (POSAS) and Disability of Arm, Shoulder, and Hand (DASH) score. RESULTS: Eighty patients were enrolled (40 RFFF and 40 UFFF). The results showed a short-term reduction in grip strength, fine motor skills (tip pinch, key pinch, palmar pinch), and range of motion was observed for RFFF and improved over time. None of the patients in either group experienced functional disturbance in grip strength, wrist motion, fine motor skills, or sensation to light touch at 1 year. Nine patients experienced partial skin graft loss (RFFF = 6; UFFF = 3). There was a significantly higher incidence of temporary numbness in the RFFF group (p=0.040). Persistent numbness occurred in 3 cases in RFFF. Cold intolerance was significantly lower in UFFF (2.5 %) than in RFFF (22.5%). Moreover, the mean POSAS and DASH score reduced at 12 months compared to 6 months, significantly superior for UFFF. CONCLUSION: Objective function limitations are reversible short-term effects after forearm flap and do not affect daily routines in the long term. Additionally, UFFF appears to be preferred over RFFF for subjective outcomes, which emphasizes the UFFF should be considered an alternative to RFFF for reconstructing soft-tissue defects.
