OBJECTIVES: Risk factors for loss to follow-up (LTFU) were assessed for people living with HIV (PLHIV) at various reference out-patient clinics (expertise level II) and hospitals (expertise level III) in Mali. METHODS: HIV-1-positive adults starting antiretroviral therapy (ART) in 2006-2013 were eligible for inclusion. Risk factors for LTFU, defined as no visit in the 6 months preceding the last database update, were assessed with the Cox model, taking into account the competing risks of transfer and death. Potential risk factors at the start of ART were demographic and socioeconomic variables, World Health Organization (WHO) stage, CD4 count, period of ART initiation, type of ART, region of care, expertise level and distance from home. RESULTS: We included 9821 PLHIV, 33% of whom were male, starting ART at nine out-patient clinics and seven hospitals [five and two in the capital Bamako and four and five in the 'regions' (i.e. districts outside the capital), respectively] with a median (interquartile range) CD4 count of 153 (56-270) cells/µL. Five-year cumulative incidences of LTFU, transfer and death were 35.2, 9.7 and 6.7%, respectively. People followed at Bamako hospitals > 5 km from home, at regional hospitals or at regional out-patient clinics < 5 km from home were at higher risk of LTFU than people followed at Bamako out-patient clinics, whereas people followed at regional out-patient clinics 5-50 km away from home were at lower risk for LTFU. Deaths were less frequent at hospitals, whether in Bamako or in the regions, than at Bamako out-patient clinics, and more frequent at regional out-patient clinics. CONCLUSIONS: Expertise level and distance to care were associated with LTFU. Stigmatization may play a role for PLHIV living close to the centres in the regions.

OBJECTIVES: In resource-limited settings, few data are available on virological failure after long-term first-line antiretroviral therapy. This study characterized the genotypic resistance patterns at the time of failure after at least 36 months of a first-line regimen in Mali, West Africa. METHODS: Plasma samples from 84 patients who were receiving first-line antiretroviral treatment and with an HIV-1 RNA viral load (VL) >1000 copies/mL were analysed. Genotypic resistance testing was performed and HIV-1 drug resistance was interpreted according to the latest version of the National Agency for HIV and Hepatitis Research algorithm. RESULTS: At the time of resistance testing, patients had been treated for a median of 60 months (IQR 36-132 months) and had a median CD4 cell count of 292 cells/mm(3) (IQR 6-1319 cells/mm(3)), a median HIV-1 RNA level of 28266 copies/mL (IQR 1000-2 93 495 copies/mL) and a median genotypic susceptibility score of 1 (IQR 1-4). The prevalence of nucleoside reverse transcriptase inhibitor (NRTI) and non-nucleoside reverse transcriptase inhibitor (NNRTI) resistance mutations was 78% and 82%, respectively. Viruses were resistant to at least one drug in 92% of cases. Although etravirine and rilpivirine were not used in the first-line regimens, viruses were resistant to etravirine in 34% of cases and to rilpivirine in 49% of cases. The treatment duration, median number of NRTI and NNRTI mutations and some reverse transcriptase mutations (T215Y/F/N, L210W, L74I, M41L and H221Y) were associated with the VL at virological failure. CONCLUSIONS: This study demonstrated a high level of resistance to NRTIs and NNRTIs, compromising second-generation NNRTIs, for patients who stayed on long-term first-line regimens. It is crucial to expand the accessibility of virological testing in resource-limited settings to limit the expansion of resistance and preserve second-line treatment efficacy.

This study aimed to compare the inactivation rate and the mechanisms of pathogenic bacteria in three matrixes (sawdust, rice husk and charcoal) during the composting process. The inactivation rate was evaluated with Escherichia coli strain and the damaged parts and/or functions were evaluated with three different media. Normalized inactivation rate constant in three media and from three matrixes had no significant difference in each process (pure, 1 month and 2 months). The value in rice husk was relatively increased during 2 months but there was no significant difference. The inactivation rate constants of Tryptic Soy Agar (TSA) and Compact Dry E. coli/Coliform in pure sawdust and rice husk were relatively lower than that of Desoxycholate Agar, but increased in 2 months. This indicated that damaging part was changed from outer membrane to enzymes and metabolisms during the 2-month composting process. In the case of charcoal, only the TSA value in apure matrix was relatively lower than that of others, but it increased in 2 months. This indicated that damaging part was changed from outer membrane and enzyme to metabolisms during the composting process. Composting matrix and composting process did not significantly affect inactivation rate of pathogenic bacteria during the process but affected the damaging part of the bacteria.

To solve the unpleasant disposal of greywater in rural area and allow its collection for reuse in gardening, a slanted soil treatment system (SSTS) was designed and installed in two households. Granitic gravel of 1-9 mm size was used as the filter medium. The aim of this study was to design a SSTS and assess its suitability as a treatment system allowing greywater reuse in gardening. The efficiency of the SSTS was assessed based on organic matter and bacterial pollution removal. The developed SSTS allowed the collection of greywater from three main sources (shower, dishwashing and laundry) in rural area. The SSTS is efficient in removing at least 50% of suspended solids, chemical oxygen demand and biological oxygen demand. The study highlighted that, contrary to the common perception, greywater streams in rural area are heavily polluted with faecal indicators. The removal efficiency of faecal indicators was lower than 2 log units, and the bacteriological quality of the effluents is generally higher than the WHO reuse guidelines for restricted irrigation. Longer retention time is required to increase the efficiency. The possibility of reusing the treated greywater as irrigation water is discussed on the basis of various qualitative parameters. The SSTS is a promising greywater treatment system for small communities in the rural area in the Sahelian region. To increase the treatment efficiency, future research will focus on the characteristics of the SSTS, the grain size and the establishment of a pretreatment step.

OBJECTIVES: It has been demonstrated for some drugs that the genetic barrier, defined as the number of genetic transitions and/or transversions needed to produce a resistance mutation, can differ between HIV-1 subtypes. We aimed to assess differences in the genetic barrier for the evolution of resistance to the second-generation non-nucleoside reverse transcriptase inhibitors etravirine and rilpivirine in subtypes B and CRF02_AG in antiretroviral-naive patients. METHODS: An analysis was undertaken of 25 substitutions associated with etravirine and rilpivirine resistance at 12 amino acid positions in 267 nucleotide sequences (136 HIV-1 B and 131 HIV-1 CRF02_AG subtypes) of the reverse transcriptase gene. RESULTS: The majority (7/12) of amino acid positions studied were conserved between the two HIV-1 subtypes, leading to a similar genetic barrier. Different predominant codons between the subtypes were observed in 5/12 positions (90, 98, 179, 181 and 227), with an effect on the calculated genetic barrier only at the V179D and V179F codons (2.5 versus 3.5 for V179D, and 2.5 versus 5 for V179F, respectively, for subtype B versus subtype CRF02_AG). CONCLUSIONS: The majority of amino acids involved in etravirine and rilpivirine resistance showed a high degree of conservation of the predominant codon between the B and CRF02_AG subtypes. For rilpivirine, the genetic barrier was the same between the two subtypes. Nevertheless, subtype CRF02_AG showed a higher genetic barrier to acquiring mutations V179D and V179F (mutations associated with resistance to etravirine) compared with subtype B, suggesting that it would be more difficult to produce resistance to etravirine in the CRF02_AG subtype than the B subtype.

the goal of this study was to investigate the internal hemorroid disease in our endoscopic center. METHOD: This transversal study went from December 2011 to December 2012. RESULTS: Sixty cases of internal hemorroid desease were enroled with36 men and 24 women and a middle age of 47 ±14.4 years. Clinical signs were frequently: bleeding (66.7%), constipation (58.4%), anal pain (58.3%) and hemorrhoid prolapsed (18.3%). The anal and rectum scopy found only internal hemorrhoid in 78.4% of cases and the grade I was about 81.7% of cases. All patients were beneficied of medical treatement. CONCLUSION: hemorrhoid disease is a frequent pathology in general population mainly around third decade.


le but de ce travail était d'étudier la maladie hémorroïdaire interne. MÉTHODE: Il s'agissait d'une étude transversale déroulée au centre d'endoscopie digestive du CHU Gabriel Touré de Décembre 2011 à décembre 2012. RÉSULTATS: Nous avons colligé 60 cas de maladie hémorroïdaire interne dont 36 hommes et 24 femmes avec un âge moyen de 47 ±14,4 ans. Le tableau clinique était dominé par le saignement (66,7 %), la constipation (58,4 %), la douleur anale (58,3%) et le prolapsus hémorroïdaire (18,3%). L'anorectoscopie réalisée objectivait des hémorroïdes internes isolées dans 78,4% des cas et le stade I de la maladie était observé dans 81,7% des cas. Tous les patients ont bénéficié d'un traitement médical. CONCLUSION: La maladie hémorroïdaire est une pathologie relativement fréquente dans la population générale surtout vers la troisième décade.

INTRODUCTION: The pure digestive form of COVID-19 is possible and initially considered rare. Our objectives were to determine the frequency of COVID-19 in patients treated in general surgery, identify the circumstances of COVID-19 discoveries in surgery, describe the post-operative complications in patients operated on COVID-1919 and describe the reorganization of post-operative COVID-19 diagnosis management. MATERIALS AND METHODS: We conducted a prospective descriptive study from March 2020 to August 2021 (18 months). All patients who have been operated on, hospitalized or consulted in the department, the Emergency Department (ERS) or other CHU-GT departments for surgical pathology with COVID-19 or infected with COVID-19 while hospitalized were included in the study. RESULTS: 23 cases collected, representing 8.91% (23/258) of COVID-19 cases identified, a frequency of 1.27/month. The age group 66-75 was the most affected, the average age was 49.13 18.75 years, the predominance was male (sex ratio of 1.3). More than 34.78% were recruited at the SAU, 16 patients (69.56%) consulted in emergency, and digestive signs of COVID-19 were: Abdominal pain 20 cases (89.96%), anorexia 19 cases (82.61%), vomiting 8 cases (34.78%) and diarrhea 3 cases (13.04%). The pulmonary signs of COVID-19 were: Cough 18 cases (78.26%), chest pain 15 cases (65.22%), and dyspnea 9 cases (39.13%). Manifestations of COVID-19 were: pulmonary 9 cases, digestive 9 cases, associated 3 cases, incidental discovery 2 cases. The diagnostic mean was Thoracic CT (100%), Test-PCR 14 cases (60.86%) with a positive PCR test in 50% of cases. Surgical pathologies were surgical emergencies in 7 cases (30.43%), cancers in 6 cases (26.09%), COVID-19 digestive event (30.43%) and other 3 cases (13.04%). More than half of patients were operated on 12 cases (52.17%). The overall mortality was 60.87% and the mortality of surgical patients was 41.67%. CONCLUSION: The infectious risk of COVID-19 during hospitalization, during or after digestive surgery is a real and potentially serious risk for the patient and caregivers.

MATÉRIELS ET MÉTHODES: L'étude était descriptive prospective de Mars 2020 à Aout 2021 (18 mois). Tous les patients qui ont été opérés, hospitalisés ou consultés dans le service, au service d'accueil des urgences (SAU) ou dans d'autres services du CHU. GT pour pathologie chirurgicale avec COVID-19 ou infecté au COVID-19 en cours d'hospitalisation étaient inclus dans l'étude. RÉSULTATS: 23 cas colligés ; représentant 8,91 % (23/258) des cas de COVID-19 recensés, une fréquence de 1,27/mois. La tranche d'âge 66-75 ans était la plus atteinte, la moyenne d'âge était de 49,13 ± 18,75ans, la prédominance était masculine (sex-ratio de 1,3). Plus de 34,78% ont été recrutés au SAU, 16 malades (69,56%) ont consultés en urgence, les signes digestifs du COVID-19étaient : Douleur abdominale 20 cas (89,96%), anorexie 19 cas (82,61%), Vomissements 8 cas (34,78%), diarrhée 3 cas (13,04%). Les signes pulmonaires du COVID-19 étaient : Toux 18 cas (78,26%), douleur thoracique 15 cas (65,22%), dyspnée 9 cas (39,13%). Les manifestations du COVID-19 étaient : pulmonaire 9 cas, digestives 9 cas, associées 3 cas, découverte fortuite 2 cas. Le moyen diagnostic a été la TDM Thoracique (100%), le Test-PCR 14 cas (60,86%) avec un test-PCR positif dans 50% des cas. Les pathologies chirurgicales étaient des urgences chirurgicales dans 7 cas (30,43%), des cancers dans 6 cas (26,09%), manifestation digestive COVID-19(30,43%), autres 3 cas (13,04%).Plus de la moitié des malades étaient opérés 12 cas (52,17%). La mortalité globale était de 60,87% et la mortalité des malades opérés était de 41,67%. CONCLUSION: Les pathologies chirurgicales et COVID-19 n'étaient pas fréquentes. La plupart des malades avaient plus de 50 ans avec une présence moyenne de comorbidité. Les signes pulmonaires étaient les plus marquants, cependant les signes digestifs étaient inaugurales dans la moitié des cas de notre étude. La TDM Thoracique associée ou non au Test-PCR oro-pharyngé permet de faire le diagnostic. Le traitement peut-être médico-chirurgical ou médical. Le risque infectieux de la COVID-19 au cours de l'hospitalisation, pendant ou après la chirurgie est réel et potentiellement grave pour le malade ainsi que les soignants.

Background: Progressive Myoclonic Epilepsy (PME) is a heterogeneous group of pathologies associating epileptic seizures and other neurological and non-neurological disorders. Objectives: We aim to characterize patients with symptoms of PME and identify the underlying genetic disorder. Methods: After informed consent, the patients seen in the protocol for hereditary neurological diseases and presenting signs of epilepsy without a secondary cause were clinically evaluated over a three-year period in the Department of Neurology of the CHU Point "G". EEG, brain imaging and laboratory tests were performed to consolidate our diagnosis. DNA was extracted for genetic analysis. Results: 141 families including five families with PME totaling eight cases were enrolled. The predominant symptoms in our patients were myoclonus in 87.5% (N = 8), followed by GTCS and cognitive impairment in 50%, each. A notion of parental consanguinity was found in 60% and autosomal recessive transmission evoked in 80% (N = 5). The EEG was pathological in 62.5% and imaging showed ponto-cerebellar atrophy in 25% (N = 8). The combination of sodium valproate and clonazepam was the main treatment. One case of death was recorded. Conclusion: We report cases of PME in Mali with a possibility of discovering new genes.

Atrial fibrillation is the most frequent permanent rhythm disorder.Theaim of this work was to determine the epidemiological and clinical aspects of atrial fibrillation cases hospitalized in the cardiology department of the University Hospital of Kati. MATERIALS AND METHODS: We carried out a descriptive retrospective study, from January 2018 to December 2019.Patients of all ages and both sexes with clinical and EKG atrial fibrillation admitted to the department during the study period were included in the study.The variables studiedwere: socio-demographic characteristics, clinical signs, classification of atrial fibrillation, and comorbidities associated. RESULTS: A total of 52 patients were included in the studyof 203 cases hospitalized in the department, with a hospital prevalence of 25.61%, the female sex was predominant 69.23%. The main reason for consultation was heart failure syndrome (46.15%).Arterial hypertension (55.76%) was the mostassociatedcomorbidity. Permanent atrial fibrillation accounted for 63.46% of cases . CONCLUSION: Atrial fibrillation is the most frequent arrhythmia in our environment, preventive measures must be put in place for early detection and better management of comorbiditiesassociatedwith atrial fibrillation.


La fibrillation auriculaire est le trouble du rythme le plus fréquent. Le but de ce travail était de déterminer les aspects épidémiologiques et cliniques de la fibrillation atriale dans le service de cardiologie du CHU de Kati. MATÉRIELS ET MÉTHODES: Il s'agit d'une étude rétrospective descriptive menée de janvier 2018 à décembre 2019. Ont été inclus dans cette étude les patients de tout âge et des deux sexes ayant présenté une fibrillation atriale sur des critères cliniques et électrocardiographiques, admis dans le service pendant la période d'étude. Les variables étudiées étaient : les caractéristiques sociodémographiques, les signes cliniques et électriques, et les comorbidités associées. RÉSULTATS: Au total 52 patients ont été inclus dans l'étude sur 203 cas hospitalisés dans le service soit une prévalence hospitalière de 25,61%, le sexe féminin a été prédominant 69,23%. Le principal motif de consultation était le syndrome d'insuffisance cardiaque (46,15%). L'hypertension artérielle (55,76%) a été la comorbidité la plus associée. La fibrillation atriale permanente représentait 63,46% des cas. CONCLUSION: La fibrillation atriale est l'arythmie la plus fréquente dans notre milieu, des mesures préventives doivent être mise en place pour une détection précoce et une meilleure prise en charge des comorbidités associées.

Introduction: Autosomal recessive cerebellar ataxias (ARCA) are a group of rare and heterogynous neurodegenerative diseases mainly characterized by unbalance and walking difficulty and movement incoordination. Objectives: To clinically and paraclinically characterize ARCA in the department of Neurology at the Teaching Hospital of Point G and identify the underlying genetic defect. Patients and method: We have conducted a longitudinal and prospective study from January 2018 to December 2020. Patients with ARCA phenotype seen in the Department of Neurology at the Teaching Hospital of Point "G" were enrolled. Results: We have enrolled 7 families totaling 13 patients after giving an informed verbal and written consent. The sex ratio was 2.2 in favor of males, Kayes region and Fulani ethnic group were respectively the most represented region and ethnic group.Walking difficulty represented the major symptom followed by loss of vibration and joint sense, nystagmus, dysarthria and skeletal deformities. Alpha-foetoprotein level was high in one patient. Genetic testing confirmed Friedreich ataxia in one family and was not conclusive in 4 families. Conclusion: This study showed that ARCA are not uncommon in Mali and genetic testing is crucial to confirm the diagnosis.


Introduction: Les ataxies cérébelleuses autosomiques récessives (ACAR) constituent un groupe de maladies neurodégénératives rares et hétérogènes caractérisées essentiellement par un trouble de l'équilibre et de la marche, et un trouble de la coordination des mouvements. Objectifs: Caractériser les signes cliniques, paracliniques et génétiques des ataxies cérébelleuses autosomiques récessives au Service de Neurologie du CHU du Point "G". Patients et méthodes: Nous avons réalisé une étude de cas enrôlé dans le cadre d'une étude longitudinale et prospective allant de Janvier 2018 à Décembre 2020, portant sur des patients présentant des symptômes d'ACAR et ayant donné leur consentement éclairé. Résultats: Nous avons enrôlé sept familles totalisant 13 patients. Le sexe ratio était de 2,2 en faveur des hommes, la région de Kayes était la plus représentée et l'ethnie peulh était majoritaire. Les troubles de la marche ont représenté les signes majeurs suivis de troubles de la sensibilité profonde, de nystagmus, de dysarthrie, et des déformations ostéoarticulaires. L'alpha-foetoprotéine était élevée chez une patiente. Le test génétique a retrouvé l'ataxie de Friedreich dans une famille et n'a pas été concluant dans quatre autres. Conclusion: Cette étude montre que les ACAR ne sont pas rares au Mali et l'exploration génétique constitue un outil indispensable pour leur diagnostic de certitude.

The objective of this work is to raise the issue of contamination of covid 19 disease during endoscopic endonasal surgery for pituitary tumor. This is a 32-year-old patient with a body mass index at 49,38 seen in an outpatient clinic for headache, erectile dysfunction and gynecomania, a CT scan revealed a pituitary macroadenoma. The preoperative workup was unremarkable including the rapid polymerase chain reaction (PCR) test was negative. The endonasal approach was decided for surgery. The tumor was removed by aspiration. The postoperative course was straightforward until the fifth day when the patient presented a dry cough with a fever at 38 ° 5 associated to respiratory discomfort. The thoracic CT-scan showed "ground glass" opacities located at peripheral and bilateral, and essentially posterior suggesting covid 19 disease. A second test was performed. Before the result, the anticoagulant treatment associated with third generation cephalosporin was done. The test results came back after 72 hours and was positive. Azithromycin, hydroxychloroquine, and oral vitamin C have been used for 11 days. The outcome was favorable and the patient was discharged from the hospital on the twelfth day after the negative PCR test.


L'objectif de ce travail est de poser la problématique d'une contamination de la maladie à covid 19 lors L'objectif de ce travail est de poser la problématique d'une contamination de la maladie à covid 19 lors d'une chirurgie endoscopique endonasale pour tumeur hypophysaire. Il s'agissait d'un patient de 32 ans avec un indice de masse corporelle à 49,38 vu en consultation pour céphalées, dysfonctionnement érectile et gynécomanie, la tomodensitométrie avait mis en évidence un macroadénome hypophysaire. Le bilan préopératoire était sans particularité incluant le test rapide « réaction en chaine par Polymérase ¼ (PCR) qui était négatif. La voie endonasale a été décidée pour la chirurgie. La tumeur a été évidée par aspiration. Les suites opératoires ont été simples jusqu'au cinquième jour ou le patient a présenté une toux sèche avec une fièvre à 38°5 associée à une gêne respiratoire. Le scanner thoracique a visualisé des opacités « en verre dépoli ¼ de topographie périphérique et bilatérales et essentiellement postérieure évoquant la maladie à covid 19. Un deuxième test a été effectué.En attendant le résultat le traitement anticoagulant associé à la céphalosporine de troisième génération a été instauré. Les résultats du test sont revenus positifs après 72 heures. L'azithromycine, l'hydroxychloroquine, et la vitamine C par voie orale ont été associées pendent 11 jours. L'évolution a été favorable et le patient est sorti de l'hôpital au douzième jour après la négativité du test PCR de contrôle.

INTRODUCTION: The use of blotting paper as a support for quantification of viral load could improve the virological monitoring of patients on Human Immunodeficiency Virus treatment in Mali. The urgency is obvious to us that the coverage in charge is only 10% for the achievement of the 3rd 90. OBJECTIVE: To assess the performance of viral load screening (sensitivity, specificity, concordance) of DBS from the one spot DBS protocol according to plasma in people living with the human immunodeficiency virus (PLHIV) in Bamako. METHODS: A total of 130 blotting papers were made from blood samples received from five sites for monitoring live persons with HIV. These blotting papers thus made were analyzed with the Abbott m2000 apparatus for the purpose of quantifying the viral load with plasma as reference sample. The extraction was done with the m2000SP automatic extractor, following the protocol 1.0 mL HIV-RNA DBS Protocol for paper. Furthermore, the extraction of plasma RNA on the m2000SP was done according to the 0.6 mL HIV-1 RNA protocol. RESULTS: With 130 samples with detectable viral loads, we obtained a correlation of r = 0.837 (p <0.001). In addition, the average difference between the viral load on blotting paper and plasma was 0.512 log / virological copies with a kappa coefficient = 0.708. The threshold of 1000 copies / mL defined as virological success in our study allowed us to obtain a sensitivity of 87% and a specificity of 100%. CONCLUSION: Decentralized zones or virological plasma monitoring is not accessible, the new Protocol one spot of Abbott laboratory offers an interesting opportunity for the follow-up of these patients with good performance at the threshold of 1000 copies. The use of DBS as a virological support can contribute effectively to the achievement of the 3rd 90.


INTRODUCTION: L'utilisation du papier buvard comme support de quantification de la charge virale pourrait améliorer le suivi virologique des patients sous traitement antirétroviral (ARV) au Mali. L'urgence s'impose à nous vula couverture en charge faible(10%) pour l'atteinte du 3ème 90. OBJECTIF: Evaluer les performances de dépistage de la charge virale (sensibilité, spécificité, concordance) des DBS issus du protocole one spot DBS en fonction du plasma chez les personnes vivant avec le virus de l'immunodéficience humaine (PVVIH) à Bamako. MÉTHODES: Au total, 130 papiers buvards ont été confectionnés à partir d'échantillons sanguin reçus de cinq sites de suivi des personnes vivant avec le virus de l'immunodéficience humaine. Ces papiers buvards ainsi confectionnés ont été analysés avec l'appareil Abbott m2000 en vue de la quantification de la charge virale avec comme échantillon de référence le plasma. L'extraction a été faite avec l'extracteur automatique m2000SP, suivant le protocole 1.0 mL HIV-RNA DBS Protocol pour le papier buvard. Par ailleurs, l'extraction de l'acideribonucléique (ARN) plasmatique sur le m2000SP a été faite selon le protocole 0.6 mL HIV-1 RNA. RÉSULTATS: Avec les 130 échantillons avec des charges virales détectables, nous avons obtenu une corrélation de r= 0.837(p<0,001). Par ailleurs, la différence moyenne entre la charge virale sur papier buvard et sur plasma était de 0,512 log/copies virologique avec un coefficient kappa= 0,708. Le seuil de 1000 copies/mL défini comme succès virologique dans notre étude nous a permis d'obtenir une sensibilité de 100% et une spécificité de 87%. CONCLUSION: Dans les zones décentralisées où le suivi virologique sur plasma n'est pas accessible, le nouveau Protocol one spot du laboratoire Abbott offre une opportunité intéressante pour le suivi de ces patients avec de bonne performance au seuil des 1000 copies. Aussi, l'utilisation du DBS comme support virologique peut contribuer efficacement à l'atteinte du 3ème 90.

COVID-19 was declared a pandemic in March 2020. For case management, Mali has created several treatment sites including the site of the CHU Gabriel Touré. AIMS: The objective of the study was to analyse drug prescriptions for the COVID-19 treatment at the CHU Gabriel Touré. METHODS: We performed a retrospective and descriptive study from April to September 2020. Drug prescriptions and hospital records were used to collect data. Prescriptions and hospital records were used to collect data. RESULTS: A total of 29 patients were hospitalized. The median age was 44 years, 75.90% of patients had at least one pathology associated with COVID-19. The number of prescriptions was 333 comprising 870 lines of prescriptions including 33.21% for standard treatments, and 66.79% for associated pathologies. with 86.23% available at the Hospital Pharmacy. Chloroquine, dosed at 250mg, was administered at 500mg twice a day. The national guidelines from the treatment of COVID-19 recommends 200mg of chloroquine in two doses. Vitamin C was prescribed for all patients although not included in the national guidelines. The class of drugs for the blood and blood-forming organs was the most prescribed (31.49%). The average cost of treatments was 65,602 ± 106,858 FCFA with a maximum of 567,860 FCFA. An evaluation of prescriptions in other treatment sites is necessary.


La COVID-19 est une infection virale qui s'est propagé dans tout le monde. Pour la prise en charge des cas, le Mali a créé des sites de traitement dont celui du CHU Gabriel Touré. OBJECTIF: C'était d'analyser les prescriptions médicamenteuses pour le traitement de la COVID-19 au CHU Gabriel Touré. MÉTHODOLOGIE: L'étude était rétrospective et descriptive et a concerné la période d'avril à septembre 2020. Elle s'est déroulée au Centre de Traitement COVID-19 du CHU Gabriel Touré et à la Pharmacie Hospitalière de cet hôpital. Les ordonnances et les registres d'hospitalisation ont servi à collecter les données. RÉSULTATS: Au total, 29 patients ont été hospitalisés. L'âge médian était de 44ans, 75,90% des patients présentaient au moins une pathologie associée à la COVID-19. Le nombre d'ordonnances était de 333 comportant 870 lignes de prescriptions dont 33,21% de traitements standards et 66,79% pour les pathologies associées avec 86,23% disponibles à la Pharmacie Hospitalière. Le phosphate de chloroquine, dosé à 250mg, était administré à 500mg deux fois quotidiennement. Dans les directives nationales le phosphate de chloroquine était à 100mg pour 200mg trois fois quotidienne. La vitamine C non prévue dans les directives a été prescrite à tous les patients. La classe des médicaments du sang et des organes hématopoïétiques a été les plus prescrits (31,49%). Le coût moyen des traitements était de 65602±106858 FCFA avec maximum de 567860 FCFA. Une évaluation des prescriptions dans les autres sites de traitement est nécessaire.

HIV infection is a chronic infectious disease requiring long-term management and regular follow-up of patients. OBJECTIVES: The aims of this study was to describe the socio-demographic, clinical, biological and therapeutic aspects of adolescents treated and followed for HIV infection at the Hospital Gabriel Toure paediatric center. PATIENTS AND METHODS: From 01/01/2001 to 31/12/2017, the medical records of children followed for HIV infection until adolescence were analyzed. It was a descriptive and analytical retrospective study. RESULTS: One thousand five hundred and fourteen patients received antiretroviral treatment and 587 were still in follow-up on 31 December 2017, including 393 adolescents (sex-ratio = 1.2). The median age was 14.25 years and 55.1% of children had lost at least one parent. HIV serology was positive among mothers in 61.7% of cases (n=342), and 63% of them were on ARVs. Sixty-eight per cent of children were WHO Stage III or IV at the time of ART initiation. The median age at onset of ART was 53 months (26-96 months). The combination of 2 nucleotide reverse transcriptase inhibitors (NRTIs) with a non-nucleotide reverse transcriptase inhibitor (NNRTI) was used in 89% of patients. The median CD4 count before ARV treatment was 438/mm3. The average duration of follow-up under treatment was 9.8 ± 3.4 years. Fifty-one percent of adolescents had undetectable viral load. There was a correlation between the initiation of a second line of treatment and treatment failure (p<0.001). CONCLUSION: The adherence of adolescents to ARV treatment requires the implementation of innovative strategies to improve the therapeutic success rate.


L'infection à VIH est une maladie chronique infectieuse nécessitant une prise en charge longue et un suivi régulier des patients. OBJECTIFS: L'objectif de ce travail était de décrire les aspects socio-démographiques, clinico-biologiques et thérapeutiques du VIH chez l'adolescentau centre d'excellence pédiatrique de prise en charge du CHU Gabriel Toure. PATIENTS ET MÉTHODES: Il s'agissait d'une étude transversale avec recueil rétrospectif de données, qui s'est déroulée du 01/01/2001 au 31/12/2017. C'était une étudetransversale à visée analytique portant sur les dossiers des adolescents d'au moins10 ans. RÉSULTATS: Trois cent quatre-vingt-treize (393) adolescents d'au moins10 ansont été inclus. La sérologie VIH était positive chez les mères dans 61,7% des cas (n=342), et 63% d'entre elles étaient sous ARV. Soixante-huit pour cent des enfants étaient classés stade III ou IV de l'OMS au moment de la mise sous TARV. L'association de 2 inhibiteurs nucléosidiques de la transcriptase inverse (INTI) à un inhibiteur non nucléotidique de la transcriptase inverse (INNTI) a été utilisée chez 89% des patients. Le taux de CD4 médian avant la mise sous traitement ARV était de 438/mm3 La durée moyenne de suivi sous traitement était de 9,8 ± 3,4 ans. Cinquante un pourcent (51%) des adolescents étaient en succès thérapeutique avec une charge virale indétectable (< 1000 copies/ml). Il y avait une corrélation entre l'instauration d'une seconde ligne de traitement et l'échec thérapeutique (p<0,001). CONCLUSION: L'adhésion des adolescents au traitement ARV nécessite la mise en place de stratégies innovantes permettant d'améliorer le taux de succès thérapeutique.

INTRODUCTION: Indications for pulmonary excision are dominated by infectious pulmonary pathologies in developing countries. We conducted this study to describe the indications and results of pulmonary resections in the thoracic surgery department of the Mali hospital. PATIENTS AND METHODS: This is a retrospective and descriptive study from January 2012 to December 2019, carried out in the thoracic surgery department of the Mali hospital. It involved 76 patients who had a pulmonary resection. The variables studied were the epidemiological data, the operating indications, the therapeutic data and the prognosis. RESULTS: The mean age of the patients was 35.5 years. The sex ratio was 1.7. The average consultation time in thoracic surgery was 11.6 months with extremes of 7 days and 96 months. A history of pulmonary tuberculosis was noted in 46.1% of patients. The main indications for pulmonary resection were infectious parenchymal destruction in 64.5%, bullous dystrophy in 14.5%, bronchopulmonary cancer in 11.8% and thoracic trauma in 4% of the cases. The procedures performed were: a lobectomy (39.5%), atypical resection (36.8%), culminectomy (7.9%) and pneumonectomy (15.8%). Morbidity was dominated by thoracic empyema (9.2%) postoperative hemorrhage (5.2%), parietal suppuration (7.8%) and bronchopleural fistula (1.3%). The average length of hospital stay was 14.3 days. Mortality was 10.5%. There was a statistically significant correlation between pneumonectomy and deaths (P=0.01). CONCLUSION: Infectious lung destruction is the main indication for pulmonary resection in Mali. The consultation period is quite long. Morbidity and mortality remains high.

INTRODUCTION: Limb-Girdle Muscular dystrophies (LGMD) is a group of inherited diseases characterized by predominantly proximal and limb muscle weakness. These are rare diseases that have not been well studied in sub-saharan Africa. The aim of our was the clinical and paraclinical characterization of patients with recessive LGMD at the Department of Neurology of the Teaching Hospital of Point G. PATIENTS AND METHODS: We conducted a longitudinal prospective study which took place from March 2014 to May 2019. Patients with recessive LGMD phenotype were enrolled. Sociodemographic, clinical and laboratory data were analyzed. RESULTS: We enrolled 46 families (67 patients), i.e. a frequency of 16.7% among the neurodegenerative diseases seen in the service. Among them, 45.6% came from the Sikasso region. Autosomal recessive inheritance pattern was suspected in 67.4% of the families. Symptoms appeared mainly in the first decade of life. Proximal muscle weakness was found in almost all patients. Cardiac examination showed dilated cardiomyopathy in 4.5% of cases. CONCLUSION: Limb-Girdle muscular dystrophy is a disabling disease that is found in Mali. Further study of these cases could elucidate the underlying genetic defects.

INTRODUCTION: High blood pressure is a major cardiovascular risk factor. Patients with cardiovascular risk factors are at risk of developing COVID-19. The objective of this study was to determine epidemiology of Covid-19 infected in patients with high blood pressure. PATIENTS AND METHOD: Descriptive cross-sectional study from April 2020 to June 2020 about patients hospitalized for Covid 19 by PCR diagnosis at the Hopital du Mali Bamako and having high blood pressure. Admission registry and patient charts were used to collect data. RESULTS: We collected 78 out of 484 in patients which mean hospital frequency of 16.11%. The mean age was 55.21 +/- 14.61 years. Sex ratio M / F was 1.36. Patients were followed for high blood pressure in 59% of cases. Medical history was ischemic heart disease in 2.6% and dilated cardiomyopathy in 2.6%. Main functional signs were cough in 41.02% and lost of taste in 11.53%. High blood pressure on admission was grade 2 in 37.2% and grade 3 in 3.8%. Treatments received were calcium channel blockers 41.02%, inhibitors of the reninangiotensinaldosterone system 16.66% and combinations 15.38%. Hospital mortality was 10.3%. There was no statistically significant difference in mortality between known hypertensive patients and de novo hypertensive patients. There was also no statistically significant difference in mortality by grade of hypertension. CONCLUSION: High blood pressure can be associated to Covid 19. Treatment is based on calcium channel blockers and reninangiotensinaldosterone system inhibitors. It has an impact on the prognosis of the disease with significant mortality.

Dans la littérature elle est fréquemment retrouvée chez les patients atteints de la COVID-19.L'objectif de cette étude est de décrire l'épidémiologie de cette association chez les patients hospitalisés pour Covid-19. PATIENTS ET MÉTHODE: L'Etude est transversale et descriptive ; elle a été réalisée sur la période du 1erAvril 2020 au 30 Juin 2020. Elle a concerné les patients hospitalisés pour Covid 19 avec un test PCR positif à l'hôpital du Mali de Bamako et ayant une HTA. Les registres d'admission et les dossiers des patients ont servi pour la collecte des données. RÉSULTATS: Nous avons colligé 78 sur 484 patients hospitalisés soit une fréquence de 16,11%. L'âge moyen était de 55,21 +/- 14,61 ans. Le sex ratio H/F était de 1,36.Les patients étaient suivis pour HTA dans 59% des cas. Les antécédents médicaux étaient la cardiopathie ischémique chez 2,6% et la cardiomyopathie dilatée chez 2,6%. Les principaux signes fonctionnels étaient la toux chez 41,02% et l'agueusie chez 11,53%. L'HTA à l'admission était de grade 2 dans 37,2% des cas et de grade 3 dans 3,8% des cas. Les traitements reçus étaient les inhibiteurs calciques 41,02%, les inhibiteurs du système rénine angiotensine aldostérone 16,66% et les associations 15,38%. La mortalité hospitalière était de 10,3%. Il n'y avait pas de différence statistiquement significative concernant la mortalité entre les patients connus hypertendus et les patients hypertendus de novo. Il n'y avait pas non plus de différence statistiquement significative concernant la mortalité selon le grade de l'HTA. CONCLUSION: l'HTA peut être associée au Covid 19. Le traitement est basé sur les inhibiteurs calciques et sur les inhibiteurs du système rénine angiotensine aldostérone. Elle a un impact sur le pronostic de la maladie avec une mortalité importante.

Accumulation rates and pathogen concentrations in primary stabilization pond sludges in developing countries are important parameters for adequate sludge management and the safeguarding of public health with sludge reuse in agriculture. An anaerobic pond has been investigated for sludge accumulation rates and helminth egg viability after four years of operation in Burkina Faso. The rate of sludge accumulation was measured at 0.037 m(3)/capita-year or 2.26 kg dry weight/capita-year. An equation describing vertical distribution of total solids in the accumulated sludge was found to be adequately represented by a regression equation. Influent helminth egg concentrations were reduced on average by 90% in the anaerobic pond effluent. Ascaris lumbricoides and Ancylostoma sp. were the most common eggs present in the sludge after four years of operation. The average concentration of helminth eggs in pond sludge was 536 eggs/g TS, and the percentages of viability ranged from 10.8% (47 viable eggs/g TS) to 57.2 (1,772 viable eggs/g TS, with an average rate of 36% (336 viable eggs/g TS). From a sludge depth and section study, egg viability was found to be randomly distributed in the sludge layer.

RESUMO

Cyst scar post circumcision of the stump of the prepuce is pathology of which we have not found any cases in the literature. The circumcision traditional is still practiced in our regions with its attendant complications, in particular, the high section, the whole or part of the gland, infections. The case we report concerns a patient 24 years of age, having a large cyst of 8cm diameter, evolving for the past 17 years. The patient has not informed his parents that when the desire to marry has become very pressing. The consultation was motivated by the psychological trauma, the discomfort and the pain caused by the infection, and the fistulisation. The treatment was surgical with bedroom suites. This observation underscores the importance of the knowledge of good surgical technique in order to minimize complications; it also sheds light on the taboo side of the pathologies the sphere uro-genitale.


Le kyste cicatriciel post circoncision du moignon du prépuce est une pathologie dont nous n'avons pas trouvé de cas dans la littérature. La circoncision traditionnelle est toujours pratiquée dans nos régions, avec son cortège de complications, notamment l'hémorragie, la section partielle ou totale du gland, les infections. Le kyste cicatriciel post circoncision traditionnelle que nous rapportons concerne un patient de 24 ans présentant un volumineux kyste de 08cm de diamètre, évoluant depuis 17ans. Le patient n'a informé ses parents que lorsque le désir de se marier est devenu très pressant. La consultation fut motivée par le traumatisme psychologique, la gêne et la douleur provoquée par l'infection et la fistulisation. Le traitement a été chirurgical avec des suites simples. Cette observation souligne l'importance de la maitrise de la bonne technique opératoire afin de minimiser les complications de la circoncision traditionnelle, elle met aussi la lumière sur le côté tabou des pathologies touchant la sphère uro-génitale.

OBJECTIVES: To study health care-associated infections (HCAI) in teaching hospital Gabriel TOURE. METHODOLOGY: This was a prospective study of 6 months (from April to September 2016) which included patients admitted to the General Surgery Department, operated or not, except those who had undergone a necrosectomy. The criteria used for the diagnosis of the infection were those of the CDC of Atlanta. RESULTS: A total of 200 patients were included in the study. Twenty one patients developed IAS that is a frequency of 10.5%. There were 11 men and 10 women with a mean age of 37.7 years with a standard deviation of 17.6 years. Surgical site infection was the most common HCAI (77.3%) followed by urinary tract infection (13.6%) and burn infection (9.1%). The influencing factors were those related to the patients (nutritional status p = 0.004, anemia RR = 3.1 IC p = 0.003 and diabetes), those related to the surgical intervention (the duration of the intervention ≥ 2H, p = 0,0001, the Altemeier class 3 and 4, RR = 4.24, IC p = 0.005, the number of interveners in the blocks ≥7, p = 0.000, the NNISS score 1 and 2 p = 0.0009), invasive procedures (bladder catheter ≥ 4 days p = 0.0000). Escherichia coli was the most isolated microorganism (31.2%) followed by Klebsiella pneumonia and A baumannii (18.7%). The treatment was local (twice-daily dressing with antiseptics), surgical (necrosectomy 16% and re-intervention 10%) and general (adapted to the antibiogram). The consequences of HCAI were an extension of total hospital stay (greater than 7 days) with p = 0.0000, morbidity 3% and mortality 5%. CONCLUSION: HCAI remains a concern in our country and globally. They prolong the hospital stay. The implementation of a prevention, control and surveillance program will improve the quality of care by significantly reducing HCAI.


Une infection est dite associée aux soins (IAS) si elle survient au cours ou au décours d'une prise en charge. L'OBJECTIF ÉTAIT: d'étudier les infections associées aux soins en chirurgie générale du CHU Gabriel Touré. MÉTHODOLOGIE: Il s'agissait d'une étude prospective de 6mois (d'avril à septembre 2016) intéressant les malades hospitalisés dans le service de chirurgie générale opérés ou non, sauf ceux ayant subi une nécrosectomie. Les critères utilisés pour le diagnostic de l'infection ont été ceux du CDC d'Atlanta. RÉSULTATS: Au total 200 malades ont été inclus dans l'étude. Vingt un patients ont développé des IAS soit une fréquence de 10,5%. Il s'agissait de 11 hommes et 10 femmes ayant un âge moyen de 37,7 ans avec un écart type de 17,6 ans. L'infection du site opératoire a été la plus fréquente des IAS (77,3%) suivie par l'infection urinaire (13,6%) et l'infection des brûlures (9,1%). Les facteurs influençant ont été ceux liés aux malades (état nutritionnel p=0,004 ; anémie RR=3,1 IC p=0,003 et diabète), ceux liés à l'intervention chirurgicale (la durée de l'intervention sup ≥2H p=0,0001 ; la classe d'Altemeier 3 et 4 ; RR=4,24 ; IC p=0,005 ; le nombre d'intervenants au blocs ≥7 ; p=0,000 ; le score de NNISS 1et 2 p=0,0009), les actes invasifs (sondage vésical ≥ 4 jours p=0,0000). Le Traitement a été local (pansement biquotidien avec des antiseptiques), chirurgical (nécrosectomie 16% et ré-intervention 10%) et général (adapté à l'antibiogramme). Les conséquences des IAS ont été le prolongement de la durée totale d'hospitalisation (supérieur à 7 jours) avec p= 0,0000, la morbidité 3% et la mortalité 5%. CONCLUSION: Les IAS demeurent préoccupantes dans notre pays comme à l'échelle mondiale. Elles prolongent le séjour hospitalier. La mise en œuvre d'un programme de prévention, de contrôle et de surveillance permettra d'améliorer la qualité des soins en réduisant considérablement les IAS.
