RESUMO

Ultrasonography is important in the clinical examination of the foal. The ultrasonographic appearance and size of the neonatal kidneys were defined and an imaging protocol established in 6 normal Thoroughbred foals (mean age +/- s.d. 5.0 +/- 3.2 days). Characteristically, in both the heart-shaped right kidney and bean-shaped left kidney, the renal cortex was more echogenic than the medulla. The terminal recesses, renal crest and pelvis were identified, as was the ureter, which contained anechoic urine in its lumen. The renal, interlobar and arcuate vessels were seen. For the right kidney, the ultrasonographic probe was placed at the 14-17th intercostal spaces and paralumbar fossa. For the left kidney, the probe was at the 16th or 17th intercostal spaces and paralumbar fossa. Perirenal structures, including the caudate lobe of the liver, the dorsal extremity of the spleen, the adrenals, the aorta and caudal vena cava were also identified. An understanding of the ultrasonographic appearance of the normal neonatal kidney, accompanied by a routine imaging protocol to ensure that all regions of each kidney are examined, permit a more informed interpretation of renal images in the first few days postpartum.

RESUMO

A description is given of the arteries within the abdominal cavity of the live permanently fistulated dairy cow which are palpated through the wall of the rumen. It is considered that transruminal palpation of arteries within the abdominal cavity of the live fistulated animal is a realistic aid to the teaching of veterinary anatomy after the student has performed the relevant routine dissection of the embalmed cadaver; and that this technique, as a method of instruction may complement rectal palpation of the live cow, where a majority of the branches of the celiac artery and the cranial mesenteric artery are not routinely within reach of the operator. The possibility of taking blood samples at any time from specified arteries supplying the ruminorecticulum and spleen in the fistulated dairy cow is considered.

A detailed description is given of the abdominal viscera which are palpated through the walls of the rumen and reticulum of the live dairy cow surgically prepared with a permanent fistula. Transruminal and transreticular palpation can be complementary to transrectal and transcolonic examination in gaining detailed knowledge of the topography of abdominal viscera in the live dairy cow.

A detailed description is given of 5 types of surface ripples occurring on the ventral part of the lateral abdominal region of the healthy dairy cow. Results of endoruminal palpation and concurrent visual observation of healthy cows which have permanent ruminal fistulae indicate that the more apparent ripples are due to the movements and changes in tension of the underlying cranial and caudal parts of the left longitudinal pillar and the left accessory pillar of the rumen. The ripples occur during both the primary (or mixing) cycle and the secondary (or eructation) cycle of ruminal motility. The cause of the rising and sinking of the left paralumbar fossa is discussed, and a correlation is made between their occurrence and that of the surface rippling. The possible clinical significance of the rippling is considered.

RESUMO

The anatomy of each feature and structure of the laryngeal and adjacent regions, as perceived by palpation, is described for clinically normal standing horses. Visible skin contours produced by some of the superficial structures are also described. Concurrent dissection was performed on fresh cadavers to confirm initial findings. The procedure of systematic palpation in relation to clinical diagnosis and surgical procedure is discussed.

In an anatomic and radiologic study of the clavicle of 50 adult dogs of 10 breeds, the clavicle had ossified in 96% of the dogs. The clavicles studied had various shapes, and each clavicle was attached to the caudomedial part of the clavicular intersection of the brachiocephalic muscle, to the mastoid part of the cleidocephalic muscle, and to 4 radiating bands of connective tissue fasciculi. One band was attached to the caudal border of the scapula and fascia deep to the latissimus dorsi muscle, 1 was attached to the manubrium of the sternum, and 1 each was attached to the epimysia of the superficial pectoral and sternocephalic muscles. We concluded that, during movements of the thoracic limb, the clavicle and the 4 fasciculated connective tissue bands associated with it stabilize the position of the brachiocephalic muscle with relation to the crest of the greater tubercle of the humerus. Also, the fasciculated band attached to the caudal border of the scapula provides protection for nerves from the brachial plexus and axillary blood vessels that supply the thoracic limb.

In Greyhounds, 2 plantar sesamoid bones were identified in radiographs taken of the intact tarsus and also of the tarsometatarsal fibrocartilage, that had been dissected free from the joints. Anatomic dissections defined the precise position of each sesamoid. A prominent sesamoid was present on the lateral side of the tarsometatarsal articulation in 50% of the tarsi; it is proposed that this bone be named the lateral plantar tarsometatarsal sesamoid bone. A smaller bone was detected on the medial side of the tarsometatarsal articulation in 27% of the tarsi; it is proposed that this bone be named the intra-articular tarsometatarsal sesamoid bone.

Ultrasonographic or anatomic observations or both were made of the kidneys of 26 dogs. The anatomic studies established precise correlations between the gross anatomic features of the organ and its ultrasonographic images obtained in transverse, sagittal, dorsal, and 2 oblique planes. Uniformly mottled echogenicity of the renal cortex could be clearly differentiated from the less echogenic renal medulla. In the mid-dorsal plane, the papillae of the renal pyramids were directed towards the renal sinus. The bases of the pyramids were almost circular in outline in the midsagittal images and the renal crest was seen as an echogenic line. Although the renal sinus was highly echogenic, neither the renal pelvis nor its recesses were detected. The walls of each of the interlobar arteries provided echogenic parallel lines, passing in the renal recesses between the renal pyramids. Arcuate arteries were demonstrated at the corticomedullary junction and interlobular arteries were detected within the renal cortex. For the right kidney, transverse images were obtained with the ultrasonographic transducer at the last 2 intercostal spaces; images in the dorsal, sagittal, and oblique planes were obtained with the transducer placed over the caudal extremity of the kidney. In the left kidney, transverse images were made with the transducers at, and caudal to, the last intercostal space; images in the dorsal, sagittal, and oblique planes were obtained with the transducer placed over the lateral border of the kidney. The use of such a protocol ensures that the entire organ is inspected and a diagnosis of either a normal or pathologic kidney is made.

Sonographic and/or anatomic observations were made of the spleen in 27 dogs. Anatomic studies were used to establish precise correlations between the gross anatomic features of the organ and its ultrasonographic image. In 8 anesthetized dogs, ultrasonographic images of the spleen were made in dorsal, transverse, and sagittal planes. When it was incident to the ultrasonic beam, the splenic capsule was represented by a fine echogenic line that defined the boundaries of the organ. The splenic substance had a uniformly mottled echogenicity apart from the anechoic lumen of the splenic venous rami, which were detected at and near the hilus of the spleen. Less regularly, splenic arterial rami were detected at the hilus, but not within the splenic substance. Dorsal and transverse images were made with the ultrasonic transducer perpendicular to the left thoracic and abdominal wall at the 11th intercostal space and caudoventrad to it. Sagittal images were produced with the transducer's face directed craniad, placed parallel to the left lateral abdominal wall, and pushed under the costal arch. The adoption of such an ultrasonographic imaging protocol ensures that all of the spleen is inspected. A definitive opinion can then be given as to whether the spleen is normal or abnormal. Pathologic changes in the spleen must also be differentiated from changes in adjacent organs or structures.

Radiologic and anatomic studies of the elbow of 100 dogs of 17 different breeds showed that a sesamoid bone may be located in a constantly present sesamoid cartilage associated with the tendon of origin of the supinator muscle. The sesamoid was observed in 31% of the radiographs of 100 supinator tendons dissected from 50 of the dogs. The bone was usually bilateral and was best demonstrated in craniocaudal medial-to-lateral oblique radiographs of the elbow where it articulated with the craniolateral aspect of the head of the radius.

Sonographic and anatomic observations were made of the kidneys of 23 Thoroughbreds or Standardbreds. In an in vitro study of 16 horses, precise correlations were established between the gross anatomic features of the kidneys and their sonographic appearance in images obtained in dorsal, sagittal, transverse, and transverse oblique anatomic planes. The renal cortex had a uniformly mottled echogenicity, and the renal medulla was relatively hypoechogenic, compared with the cortex. Acoustic anisotropy was observed in the cortex and medulla of the cranial and caudal extremities of each kidney. The distinctive renal pelvis was seen in the transverse plane as an echogenic pair of diverging lines that lead to the crescent shaped renal crest in the lateral half of the kidney. In images made in the sagittal plane, the renal pelvis was seen as a pair of parallel echogenic lines separated by the moderately echogenic line of the renal crest. The terminal recesses were best seen in the transverse oblique views of each extremity, where they appeared as moderately echogenic lines in the medulla of the cranial and caudal extremities. The interlobar vessels were represented as irregular echogenic lines in the medulla, and the arcuate vessels were seen as echogenic points at the corticomedullary junction. At the hilus, the renal artery or its branches was located cranial to the renal vein, which in turn was cranial to the position of the proximal portion of the ureter. In an in vivo study of 7 horses, sonographic images of the right kidney were obtained in the sagittal, transverse, and transverse oblique anatomic planes in all horses, with the transducer positioned at the 15th, 16th, or 17th intercostal space; images in the dorsal plane were obtained, however, in only 3 of the horses. For the left kidney, sonographic images were obtained in each of the anatomic planes when the transducer was positioned at the 16th or 17th intercostal space or the paralumbar fossa; rectal location of the transducer gave images in the dorsal and sagittal planes. In this study, a routine sonographic imaging protocol, using standard anatomic planes, enabled each kidney to be examined in its entirety. The protocol provided definition of normal renal sonographic anatomic features and may permit a more informed and accurate recognition of renal pathologic change.

Results of radiologic and anatomic studies of each cubital articulation (elbow) of a group of 50 adult cat cadavers indicated that a sesamoid bone may be located in a constantly present sesamoid cartilage associated with the tendon of origin of the supinator muscle. Radiography revealed a sesamoid bone in 40 of the 100 tendons of origin of the supinator muscles dissected from the elbows. The sesamoid bone articulated with the craniolateral aspect of the head of the radius, and the larger sesamoid cartilage, which contained the bone, articulated with the head of the radius and the capitulum of the humerus. Of several possible functions of the sesamoid cartilage (bone), it was considered that protection of the craniolateral part of the humeroradial articulation and maintenance of the complex anatomic system during joint movement were important. In radiographic views of the elbows of lame cats, the sesamoid bone should not be mistaken for a chip fracture or an osteocartilaginous loose body.

The prevalence of hepatitis C virus (HCV) infection in adult patients with a congenital bleeding disorder (CBD) approaches 95% and is a major cause of morbidity and mortality. Histological examination of the liver remains the cornerstone of management decisions in patients without a CBD. The reluctance to perform liver biopsies in patients with a CBD has been a major limitation in the management of these patients. We are currently the only haemophilia centre in Australasia performing liver biopsies in patients with a CBD for the purpose of guiding prognostic and therapeutic decisions. We report here the results of our centre's experience with transjugular liver biopsy (TJLB) in patients with a CBD. An adequate specimen for histological assessment was attained from all of the patients. There were no major complications recorded. Patients were hospitalized for < or = 48 h for haemostasis prophylaxis. The diagnostic specimen obtained from patients was integral in guiding their future management. We suggest that with a coordinated multidisciplinary approach, TJLB can be performed in patients with a CBD.

A detailed description is given of the methods to locate and palpate a subcutaneous part of the zygomatic branch of the auriculopalpebral nerve in 9 out of 10 adult horses examined. This permits the exact placing of local anaesthetic along the nerve branch at the described site, resulting in the akinesis of the m orbicularis oculi and the elimination of the blink reflex. This facilitates ophthalmologic examination and possible treatment of some eye conditions.
