RESUMO

PURPOSE: To create a metric for evaluating the degree of laterality of the patella's entry into the trochlea, the entry point-trochlear groove (EP-TG) angle, and to evaluate if this laterality is associated with recurrent patella instability. METHODS: The time frame of the study was January 2020 to February 2023. The inclusion criteria were patients treated by the senior author (J.P.F.) (with the exception of 2 patients who were treated by another provider at the institution who was aware of the study) who have been diagnosed with recurrent atraumatic patellar dislocations. Controls without knee pathology were selected from the New Mexico Decedent Imaging Database (NMDID). Simpleware ScanIP was used to create 3-dimensional (3D) models of the distal femurs from computed tomography scans. Anteroposterior images of these 3D models were uploaded to a custom EP-TG angle measuring tool. Three measurers used the tool to measure the EP-TG angle of the distal femurs. RESULTS: Twenty-eight patients were included for the recurrent dislocator group. Twenty-four decedents from the NMDID were selected for the control group, each with a left or right knee chosen randomly for measurement. A 1-sided Mann-Whitney U test, used to evaluate whether the recurrent dislocators had higher EP-TG angle values, yielded a P value <.001, demonstrating a high level of significance. A Bayesian mixed-effect model, used to determine how different the EP-TG angles are between the 2 groups, gave a posterior predictive interval of [11.93°, 19.12°] for the EP-TG angle shift of dislocators. The intraclass correlation coefficient was 0.648. CONCLUSIONS: The morphologic entry point of the patella into the proximal trochlea is more lateral in recurrent patella dislocators than in controls. This increased laterality can be measured by the EP-TG angle, which may be useful information for optimizing treatment of recurrent patella instability. LEVEL OF EVIDENCE: Level III, case control study.

RESUMO

BACKGROUND: Open online forums like Reddit provide an opportunity to quantitatively examine COVID-19 vaccine perceptions early in the vaccine timeline. We examine COVID-19 misinformation on Reddit following vaccine scientific announcements, in the initial phases of the vaccine timeline. METHODS: We collected all posts on Reddit (reddit.com) from January 1 2020 - December 14 2020 (n=266,840) that contained both COVID-19 and vaccine-related keywords. We used topic modeling to understand changes in word prevalence within topics after the release of vaccine trial data. Social network analysis was also conducted to determine the relationship between Reddit communities (subreddits) that shared COVID-19 vaccine posts, and the movement of posts between subreddits. RESULTS: There was an association between a Pfizer press release reporting 90% efficacy and increased discussion on vaccine misinformation. We observed an association between Johnson and Johnson temporarily halting its vaccine trials and reduced misinformation. We found that information skeptical of vaccination was first posted in a subreddit (r/Coronavirus) which favored accurate information and then reposted in subreddits associated with antivaccine beliefs and conspiracy theories (e.g. conspiracy, NoNewNormal). CONCLUSIONS: Our findings can inform the development of interventions where individuals determine the accuracy of vaccine information, and communications campaigns to improve COVID-19 vaccine perceptions, early in the vaccine timeline. Such efforts can increase individual- and population-level awareness of accurate and scientifically sound information regarding vaccines and thereby improve attitudes about vaccines, especially in the early phases of vaccine roll-out. Further research is needed to understand how social media can contribute to COVID-19 vaccination services.

RESUMO

A nested neutron spectrometer was used to measure the stray neutron fluence and ambient dose equivalent produced by protons from a 4 MV tandem accelerator on various target materials at the Reactor Materials Testing Laboratory (RMTL), Queen's University, Kingston, Ontario. The nested neutron spectrometer utilizes a Helium-3 proportional counter and measures the thermal neutron count rate as a function of moderator shell-thickness. Using custom unfolding software, neutron spectra and neutron dosimetric quantities were derived at various locations within the RMTL. The nested neutron spectrometer can be operated in both a pulse mode and current mode allowing for measurements over a wide range of accelerator beam currents. All measurements conducted at the facility were within the main accelerator chamber and experimental target rooms. Target room measurements were conducted with a variety of target materials representative of those to be used for research planned at the RMTL.

RESUMO

The Royal Canadian Navy has conducted a comprehensive programme of safety, security and environmental monitoring since the first visits of nuclear powered and nuclear capable vessels (NPV/NCVs) to Canadian harbours in the late 1960s. The outcomes of baseline monitoring and vessel visit sampling for the period 2003-2012 are described for vessel visits to Halifax (NS), Esquimalt (BC) and Nanoose (BC). Data were obtained by gamma-ray spectroscopy using high purity germanium detectors. No evidence was found for the release of radioactive fission or activation products by NCV/NPVs during the study period, although anthropogenically produced radionuclides were observed as part of the study's baseline monitoring. Background activities of Cs-137 can be observed in sediments from all three locations which are derived from well-documented radioactivity releases. The detection of I-131 in aquatic plants is consistently observed in Halifax at activities as high as 15,000 Bq kg(-1) dry weight. These data are tentatively assigned to the release of medical I-131, followed by bioaccumulation from seawater. I-131 was also observed in aquatic plants samples from Esquimalt (33 Bq kg(-1)) and Nanoose (20 Bq kg(-1)) for a single sampling following the Fukushima Daiichi accident.
