RESUMO
The influence of whitening toothpastes on the surface of nanocomposite dental restorative materials (24 samples divided in 3 groups according to type of toothpaste used) was studied using optical profilometry. The most significant changes on the surface roughness of the material caused abrasive whitening toothpaste in comparison to enzymes-based paste. Selection and use of whitening toothpastes in patients with dental restorations must be carried out under dentist's supervision.

The study focuses on complicated and non-complicated tooth decay course and prognosis in smokers. Oral status, prevention and treatment effectiveness was assessed in 330 non-smokers and 345 smoking patients. The results allowed concluding with guidelines for tooth decay prevention and treatment in smokers.

The objective of this study was to characterize dental status and oral mucosa blood flow in patients with chronic heart failure and amyloid deposits in oral mucosa. Histological and immunohistochemical analysis of 80 oral mucosa biopsies taken from patients aged 32-72 years with chronic heart failure I-IV NYHA functional class was carried out. It detected a systemic amyloidosis in 15.7% of cases; a local amyloid deposition in oral mucosa was found in 58.5% of cases. Amyloid deposition in oral mucosa was associated with severe chronic generalized periodontitis in more than a half of cases. Amyloid deposits in oral mucosa were revealed more often in patients with metabolic syndrome (63.5%). The article describes dental status and oral mucosa blood flow in patients with heart failure.

An approach to development of recommendations for the use of preventive and curative modalities in cases of inflammatory diseases of the parodontium is studied basing on mathematical modeling. It is suggested to use the mathematical apparatus of Markov's processes. The effectiveness of the suggested approach was demonstrated on the example of prognostication of the parodontium status in the staff of one of the plants of Saint-Petersburg.

Generalized periodontitis is a polyetiological disease, pathogenetically bound with internal pathology. The study was made to reveal common somatic mechanisms of generalized periodontitis pathogenesis. The examination of 574 patients has established immunological unbalances, mineral deficiencies and high activity of free radical oxidation of lipids and proteins. The usage of systemic multienzyme remedies, replacement mineral therapy, antioxidants, had correcting effects. This evidences for the presence of close links between pathogenetic mechanisms of generalized periodontitis, functioning according to the mobile's principle.

The article describes the results of examination of 42 patients with inflammatory periodontal disease (IPO) and insulin-dependent diabetes mellitus (IDDM). It is shown, that the effect of treatment of IPO depends on a degree of compensation of carbohydrate metabolism: it is quite satisfactory in compensated and unsatisfactory in decompensated IDDM. In saliva of patients with IPO the antigen of damaged periodontal tissues, antibody to it, and also immune complexes Ag-Ab are found. The frequency of these parameters and their dynamics after treatment of IPO depends on a degree of compensation of carbohydrate metabolism in IDDM. The antibodies and immune complexes do not play a pathogenic role and can be used for prognosis and success of IPO treatment.

The composition OSTEOSIT at a surgical stage of combined treatment of chronic generalized periodontitis was examined. The results in the control and study groups prove, that the composition OSTEOSIT allows to reduce time of treatment, to achieve remission of disease, to reduce amount of relapses.

The authors analyzed published data and their own findings which confirm the unity of systemic mechanisms of pathogenesis in combined visceral diseases and generalized periodontitis. The data proving the pathogenetic role of free-radical oxidation of lipids, proteins, and low-molecular-weight thiols, immunological imbalances, and deficiencies of the major macro- and trace elements (Ca, Mg, K, Na, Zn, Cu, Fe) are presented. The detected metabolic disorders and homeostatic shifts characterize the universal processes mediating alteration of cell structures in different organs and tissues in combined diseases.
