PURPOSE: Chemotherapy-induced nausea and vomiting (CINV) is a common adverse event with cancer chemotherapy, despite the availability of effective antiemetic agents. This is a prospective observational study of Italian breast cancer patients treated with anthracycline plus cyclophosphamide (AC), assessed CINV incidence, adherence to national antiemetic guidelines (AIOM 2012), and the relationship with CINV outcomes. METHODS: Patients with breast cancer scheduled to receive their first cycle of an AC-based regimen were enrolled at 12 Italian centers and their clinical data prospectively recorded. CINV incidence was assessed from patient diaries after the first chemotherapy cycle. The relationship between guideline adherence and CINV outcomes was examined using multiple logistic regression. RESULTS: The overall incidence rates of nausea and vomiting among 246 evaluable patients were 63.0 and 25.4%, respectively. Most patients received a 5-HT3-RA agent and dexamethasone for acute phase CINV prophylaxis, whereas a triple combination including aprepitant (NK1-RA), consistent with national guidelines, was used in only 45.5% of cases. In the delayed phase, the guideline adherence was 48.8%, while the overall adherence was 43.5%. After adjusting for confounding factors, adherence to antiemetic prophylaxis guidelines was associated with a significant reduction in the odds of three endpoints, namely any nausea, "significant nausea," and vomiting (OR = 0.49, OR = 0.54, and OR = 0.48, respectively), and a 90% increase in the odds of overall complete protection (OR = 1.90). CONCLUSIONS: CINV is still a critical issue in AC-treated patients, despite antiemetic treatment. Non-adherence to antiemetic guidelines may lead to poorer outcomes and indicates the need for strategies to enhance the use of guidelines in clinical practice.

Important therapeutic advances for patients with advanced non-small cell lung cancer (NSCLC) have recently occurred, and the development of targeted therapy has revolutionized the treatment of these patients. We report the case of an NSCLC patient with unknown EGFR mutation status who developed resistance to erlotinib and was treated with afatinib. After 19 months of therapy with afatinib, the patient is still in treatment and maintains stable disease. Emerging resistance to reversible TKIs is a significant problem. Oral irreversible TKIs such as afatinib may offer an effective treatment option after failure with reversible TKIs.

El objetivo de la investigación fue determinar la influencia del nivel del conocimiento en la actividad hacia el uso del preservativo en la prevención del VIH/SIDA en adolescentes de la Institución Educativa José María Arguedas, Chimbote. Se usó un test para medir el conocimiento y una escala para medir las actitudes. El resultado demuestra que el mayor porcentaje de estudiantes se concentra dentro del grupo de 12-14 años; de sexo femenino y en el primer y segundo año respectivamente de 10 a 14 años; la mitad de ellos refirio haber usado preservativos en sus relaciones, cifra similar manifesto que su relación no fue planeada, su encuentro sexual fue en casa, respecto al número de parejas, en su mayoría refirió solo una, sin embargo es preocupante encontrar adolescentes que refieren haber tenido hasta 07 parejas. Del total de estudiantes las tres cuartas partes se ubican dentro del nivel de conocimiento regular 78,7%; el mayor porcentaje 85,3% tienen una actitud positiva hacia el uso del preservativo en la prevención del VIH/SIDA. El nivel de conocimiento no tiene influencia sobre las actitudes hacia el uso de preservativos en la prevención del VIH/SIDA en adolescentes de la Institución Educativa José María Arguedas.


The aim of the present work of investigation was to determine the influence of the level of knowledge in the attitude towards the use of the condom in the prevention of the HIV/AIDS in teenagers of the Educational Institution Jose Maria Arguedas, Chimbote. It was by means of a test to measure the knowledge and a scale to measure attitudes; the same one that was applied inside the area of the Educational Institution before mentioned, obtaining the following results: The major percentage of students centers inside the group of 12-14 years; of femenine sex and in the first and second year respectively. Almost the students' fourth part refried to have begun sexually, between te ages from 10 to 14 years for the most part; the half of them refried to have used condom in his relations, similar number demonstrated that his relation was not planned, his sexual meeting was in house and with regard to the number of pairs, in the main refried only one, nevertheless it is worrying that there are teenagers who recount to have had up to 07 pairs. Of the total of students the fourth three parts are located inside the level of regular knowledge. With regard to the attitude the major number of students have a positive attitudes, towards the use of the condom in the prevention of the HIV/AIDS. The level knowledge does not have influence on the attitudes towards the use of the condom in the prevention of the HIV/AIDs in teenagers of the Educational Institution Jose MAria Arguedas.
