BACKGROUND: Estimates of energy intake are required for an understanding of growth and disease; however, few methods of energy intake in children have been validated. OBJECTIVE: Our objective was to validate energy intake estimated by the Youth-Adolescent Food-Frequency Questionnaire (YAQ) against the criterion total energy expenditure (TEE) by doubly labeled water (DLW). DESIGN: Twenty-three boys and 27 girls (8.6-16.2 y of age) completed the YAQ and TEE measurements in 1 y. RESULTS: Energy intake by the YAQ (10. 03 +/- 3.12 MJ) and energy expenditure by DLW (9.84 +/- 1.79 MJ) were similar (P: = 0.91) with large lower (-6.30 MJ) and upper (6.67 MJ) +/-2 SD limits of agreement. When within-subject CVs of repeated measures of the DLW and YAQ methods were used, 25 of the 50 subjects were deemed to have misreported their energy intake. The discrepancy in energy intake (YAQ - TEE) was related to body weight (r = -0.25, P: = 0.077) and percentage body fat (r = -0.24, P: = 0.09) but not to age (r = -0.07, P: = 0.63) or the time between measures. From logistic regression, fatter boys were more likely to underreport energy intake than were fatter girls. CONCLUSION: The YAQ provides an accurate estimation of mean energy intake for a group but not for an individual.

A 33 year-old woman underwent lobectomy of the middle and lower lobes of the right lung because of inflammatory fibrous histiocytoma of the bronchus. To our knowledge, this is the first description of such a tumor originating in the bronchus.

A case is described in which massive rectal bleeding was controlled by selective embolization of the superior hemorrhoidal arteries. Selective therapeutic arterial embolization in colonic bleeding represents an important alternative to procedures such as surgery and infusion of vasospastic agents which carry a prohibitive risk in severely ill patients.

The visualisation of the small bowel by double contrast can be easily obtained by introduction of a suspension of barium sulphate, air insufflation, and parenteral administration of metoclopramide. Satisfactory demonstration of the entire small bowel by air and barium was obtained in 38 out of 40 unselected adult patients. There were no complications and no significant adverse reactions.

Biliary tract visualisation in 50 patients examined by infusion of methylglucamine iodipamide was compared, by double-blind technique, to cholangiograms obtained in 50 other patients by a single bolus injection. The infusion technique did not give better results than the single bolus method. In another group of 20 patients, each individual was examined by both methods. The radiographs, read in a double-blind manner, showed both methods to be comparable in demonstrating the biliary system. It seems that other means have to be designed and employed in order to improve the radiographic delineation of the biliary tract.

Two patients with the XXXXY syndrome are presented. Both boys are mentally retarded with short stature, muscular hypotonia, and hypogonadism. A constant feature of this syndrome is a varying degree of epiphysial dysplasia probably secondary to hypotonia and growth deceleration.

Intravenous pyelography carried out with a previously distended bladder in order to produce partial urinary flow obstruction offers superior delineation of the pelvi-caliceal system and ureters. Often, these structures are visible in their entirety on a single film. In 100 adult patients of both sexes who had undergone conventional i.v. pyelography, a second examination was carried out without external compression, after preliminary distension of the urinary bladder (DUB). DUB was achieved by three different methods. In 25 patients the urinary bladder was distended by retrograde instillation of saline. Another 25 patients retained their overnight urine and 50 others retained their urine following a water load of 1,000 ml. Best results were obtained in patients examined by the retrograde and water-load methods. The vast majority of examinations in these groups were of superior quality compared with that obtained by the routine technique. Pyelography with a distended bladder proved to be a valuable diagnostic adjunct in all instances in which application of external compression is undesirable, particularly in cases of trauma.

A case of left subclavian steal syndrome with transient ischemic attacks of left carotid artery distribution is presented. An attempt to explain this uncommon symptomatology is based on a rare patent cervical arterial network, stealing blood from the left common carotid artery and supplying the distal portion of the obstructed left subclavian artery.

We report three cases of serious renal injuries due to blunt abdominal trauma within the context of multiple organ trauma. The renal involvement was not initially suspected, with resulting serious consequences. We suggest that infusion pyelography be performed prior to or shortly after surgery in all cases with extensive abdominal trauma. A retroperitoneal hematoma on explorative laparotomy in cases of multiple trauma should raise the suspicion of a concomitant serious renal injury, and an immediate diagnostic investigation should be conducted.

This report describes a 19-year-old girl with a chest radiograph strongly suggestive of cardiomegaly. Although she did not consent to haemodynamic studies, cardiomegaly wasru led out on the basis of the clinical course. Careful radiological study proved the existence of a large mass in the anterior mediastinum, simulating cardiomegaly. At thoracotomy a thymolipoma weighing 450 g was resected. The pathogenesis of thymolipoma is discussed.

The recently described thin ribs seen in a neonate suffering from myotonic dystrophy represents a valuable sign for early detection of this condition. Thin ribs were found on chest radiographs of two siblings a short time after birth. This led in retrospect to the diagnosis of neonatal myotonic dystrophy. The diagnosis was confirmed by clinical and electromyographic data of myotonic dystrophy in the mother who was not previously known to have the disease. Both children died at the age of 2 days from respiratory distress. All children of affected mothers should have a chest radiograph soon after birth in order to help in diagnosis and to establish the frequency of this sign.
