RESUMO

Mortality from tuberculous meningitis (TBM) remains around 30%, with most deaths occurring within 2 months of starting treatment. Mortality from drug-resistant strains is higher still, making early detection of drug resistance (DR) essential. Targeted next-generation sequencing (tNGS) produces high read depths, allowing the detection of DR-associated alleles with low frequencies. We applied Deeplex Myc-TB-a tNGS assay-to cerebrospinal fluid (CSF) samples from 72 adults with microbiologically confirmed TBM and compared its genomic drug susceptibility predictions to a composite reference standard of phenotypic susceptibility testing (pDST) and whole genome sequencing, as well as to clinical outcomes. Deeplex detected Mycobacterium tuberculosis complex DNA in 24/72 (33.3%) CSF samples and generated full DR reports for 22/24 (91.7%). The read depth generated by Deeplex correlated with semi-quantitative results from MTB/RIF Xpert. Alleles with <20% frequency were seen at canonical loci associated with first-line DR. Disregarding these low-frequency alleles, Deeplex had 100% concordance with the composite reference standard for all drugs except pyrazinamide and streptomycin. Three patients had positive CSF cultures after 30 days of treatment; reference tests and Deeplex identified isoniazid resistance in two, and Deeplex alone identified low-frequency rifampin resistance alleles in one. Five patients died, of whom one had pDST-identified pyrazinamide resistance. tNGS on CSF can rapidly and accurately detect drug-resistant TBM, but its application is limited to those with higher bacterial loads. In those with lower bacterial burdens, alternative approaches need to be developed for both diagnosis and resistance detection.

We report on a unique photonic quantum source chip highly integrating four-stage photonic elements in a lithium niobate (LN) waveguide circuit platform, where an aperiodically poled LN (APPLN) electro-optic (EO) polarization mode converter (PMC) is sandwiched between two identical type-0 PPLN spontaneous parametric down-converters (SPDCs), followed by an EO phase controller (PC). These core nonlinear optic and EO building blocks on the chip are systematically characterized stage by stage to show its high performance as an integrated quantum source. The APPLN EO PMC, optimally constructed by a genetic algorithm, is characterized to have a broad bandwidth (>13 nm), benefiting an efficient control of broadband type-0 SPDC photon pairs featuring a short correlation time. We demonstrate an efficient conversion of the |VV> photon-pair state generated from the first PPLN SPDC stage to the |HH> state through the APPLN EO PMC stage over its operating bandwidth, a broadband or broadly tunable polarization-entangled state can thus be possibly produced via the superposition of the |VV> state generated from the other PPLN SPDC on the third stage of the chip. Such a state can be further manipulated into two of the Bell states if the relative phases between the two polarization states can be properly modulated through the EO PC on the fourth stage of the chip. Such a multifunction integrated quantum photonic source chip can be of high value to developing a compact, efficient, and high-speed quantum information processor.

Extensive removal of 2,4,5-trichlorophenoxyacetic acid (2,4,5-T) using titania (TiO2) nanoparticles by adsorption and photocatalysis with a surface coating by cetyltrimethylammonium bromide (CTAB) is reported. The CTAB-coated TiO2 nanoparticles (CCTN) were characterized by FT-IR, zeta-potential measurements, and UV-vis diffuse reflectance spectroscopy (UV-vis-DRS). 2,4,5-T removal increased significantly after surface modification with CTAB compared with bare TiO2 nanoparticles. Optimal parameters affecting 2,4,5-T removal were found to be pH 4, CCTN dosage 10 mg/mL, and adsorption time 180 min. The maximum adsorptive removal of 2,4,5-T using CCTN reached 96.2% while highest adsorption capacity was 13.4 mg/g. CCTN was also found to be an excellent photocatalyst that achieved degradation efficiency of 99.2% with an initial concentration of 25 mg/L. The removal mechanisms of 2,4,5-T using CCTN by both adsorption and photocatalysis are discussed in detail based on changes in functional group vibrations and surface charge. Our results indicate that CCTN is an excellent material for 2,4,5-T removal in water by both adsorption and photocatalysis.

Adsorption of essential amino acid, Tryptophan (Tryp) on synthesized gibbsite nanoparticles and their applications in eliminating of antibiotic ciprofloxacin (CFX) and bacteria Escherichia coli (E. coli) in aqueous solution. Nano-gibbsite which was successfully fabricated, was characterized by XRD, TEM-SAED, FT-IR, SEM-EDX and zeta potential measurements. The selected parameters for Tryp adsorption on nano-gibbsite to form biomaterial, Tryp/gibbsite were pH 11, gibbsite dosage 20 mg/mL and 1400 mg/L Tryp. The optimum conditions for CFX removal using Tryp/gibbsite were adsorption time 60 min, pH 5, and 20 mg/mL Tryp/gibbsite dosage. The CFX removal significantly raised from 63 to 90% when using Tryp/gibbsite. The Freundlich and pseudo-second-order models achieved the best fits for CFX adsorption isotherm and kinetic on Tryp/gibbsite, respectively. The amount of CFX increased with increasing ionic strength, suggesting that both electrostatic and non-electrostatic interactions were important. After four reused time, CFX removal was greater than 66%, demonstrating that Tryp/gibbsite is reusable with high performance in removing CFX. The application in bacterial activity in term of E. coli reached greater than 98% that was the best material for bacteria inactivation. The present study reveals that Tryp/gibbsite is an excellent bio-material for removing CFX and E. coli.

Offering patients with tuberculosis (TB) an optimal and timely treatment regimen depends on the rapid detection of Mycobacterium tuberculosis (Mtb) drug resistance from clinical samples. Finding Low Abundance Sequences by Hybridization (FLASH) is a technique that harnesses the efficiency, specificity, and flexibility of the Cas9 enzyme to enrich targeted sequences. Here, we used FLASH to amplify 52 candidate genes probably associated with resistance to first- and second-line drugs in the Mtb reference strain (H37Rv), then detect drug resistance mutations in cultured Mtb isolates, and in sputum samples. 92% of H37Rv reads mapped to Mtb targets, with 97.8% of target regions covered at a depth ≥ 10X. Among cultured isolates, FLASH-TB detected the same 17 drug resistance mutations as whole genome sequencing (WGS) did, but with much greater depth. Among the 16 sputum samples, FLASH-TB increased recovery of Mtb DNA compared with WGS (from 1.4% [IQR 0.5-7.5] to 33% [IQR 4.6-66.3]) and average depth reads of targets (from 6.3 [IQR 3.8-10.5] to 1991 [IQR 254.4-3623.7]). FLASH-TB identified Mtb complex in all 16 samples based on IS1081 and IS6110 copies. Drug resistance predictions for 15/16 (93.7%) clinical samples were highly concordant with phenotypic DST for isoniazid, rifampicin, amikacin, and kanamycin [15/15 (100%)], ethambutol [12/15 (80%)] and moxifloxacin [14/15 (93.3%)]. These results highlighted the potential of FLASH-TB for detecting Mtb drug resistance from sputum samples.

This Letter demonstrates a high-speed fiber-terahertz-fiber system in the 355-GHz band using stable optical frequency comb generation and a photonics-enabled receiver. At the transmitter, a single dual-drive Mach-Zehnder modulator is employed to generate a frequency comb by driving the modulator under an optimal condition. At the antenna site, a photonics-enabled receiver consisting of an optical local oscillator signal generator, a frequency doubler, and an electronic mixer is used to downconvert the terahertz-wave signal to the microwave band. Simple intensity modulation and a direct detection method are used for transmitting the downconverted signal to the receiver over the second fiber link. To demonstrate the proof of concept, we transmitted a 16-quadrature amplitude modulation (QAM) orthogonal frequency-division multiplexing signal over a system consisting of two radio-over-fiber links and a 4 m wireless link in the 355-GHz band and achieved a line rate of 60 Gb/s. We also successfully transmitted a 16-QAM subcarrier multiplexing single-carrier signal over the system and achieved a capacity of 50 Gb/s. The proposed system can facilitate the deployment of ultra-dense small cells in high-frequency bands in beyond-5G networks.

Aerogels are becoming a promising platform to fabricate photothermal materials for use in solar steam generation (SSG), which have remarkable application potential in solar desalination, due to their excellent thermal management, salt resistance, and considerable water evaporation rate. In this work, a novel photothermal material is fabricated by forming a suspension between sugarcane bagasse fibers (SBF) and poly(vinyl alcohol), tannic acid (TA), and Fe3+ solutions via hydrogen bonds of hydroxyl groups. After freeze drying, the fabricated SBF aerogel-based photothermal (SBFAP) material possesses a 3D interconnected porous microstructure, which could enhance water transportation ability, reduce thermal conductivity, and quickly dissolve salt crystals on the SBFAP surface. Thanks to the formation of micro/nanosized complexes between TA and Fe3+ ions on the SBFAP material, the SBFAP exhibits high light capture and water evaporation rate (2.28 kg m-2 h-1). In particular, due to strong hydrogen bonding and the SBF, the SBFAP material is reinforced, thereby exhibiting excellent structural stability in seawater. Moreover, the high salt tolerance of SBFAP favors its high desalination performance for at least 76 days of continuous evaporation under actual conditions. This research paves the way for the fabrication of natural cellulose fiber-based photothermal materials for application in solar desalination.

BACKGROUND: Existing research on health equity falls short of identifying a comprehensive set of indicators for measurement across health systems. Health systems in the ASEAN region, in particular, lack a standardised framework to assess health equity. This paper proposes a comprehensive framework to measure health equity in the ASEAN region and highlights current gaps in data availability according to its indicator components. METHODS: A comprehensive literature review was undertaken to map out a core set of indicators to evaluate health equity at the health system level. Secondary data collection was subsequently conducted to assess current data availability for ASEAN states in key global health databases, national health accounts, and policy documents. RESULTS: A robust framework to measure health equity was developed comprising 195 indicators across Health System Inputs and Processes, Outputs, Outcomes, and Contextual Factors. Total indicator data availability equated to 72.9% (1423/1950). Across the ASEAN region, the Inputs and Processes sub-component of Health Financing had complete data availability for all indicators (160/160, 100%), while Access to Essential Medicine had the least data available (6/30, 20%). Under Outputs and Outcomes, Coverage of Selected Interventions (161/270, 59.63%) and Population Health (350/350, 100%) respectively had the most data available, while other indicator sub-components had little to none (≤ 38%). 72.145% (384/530) of data is available for all Contextual Factors. Out of the 10 ASEAN countries, the Philippines had the highest data availability overall at 77.44% (151/195), while Brunei Darussalam and Vietnam had the lowest data availability at 67.18% (131/195). CONCLUSIONS: The data availability gaps highlighted in this study underscore the need for a standardised framework to guide data collection and benchmarking of health equity in ASEAN. There is a need to prioritise regular data collection for overlooked indicator areas and in countries with low levels of data availability. The application of this indicator framework and resulting data availability analysis could be conducted beyond ASEAN to enable cross-regional benchmarking of health equity.

Uranium is considered as one of the most perilous radioactive contaminants in the aqueous environment. It has shown detrimental effects on both flora and fauna and because of its toxicities on human beings, therefore its exclusion from the aqueous environment is very essential. The utilization of metal-organic frameworks (MOFs) as an adsorbent for the removal of uranium from the aqueous environment could be a good approach. MOFs possess unique properties like high surface area, high porosity, adjustable pore size, etc. This makes them promising adsorbents for the removal of uranium from contaminated water. In this paper, sources of uranium in the water environment, human health disorders, and application of the different types of MOFs as well as the mechanisms of uranium removal have been discussed meticulously.

The adsorption and transformation of tetracyclines (TCs) antibiotics, including oxytetracycline (OTC), chlortetracycline (CTC), and tetracycline (TC), on the sodium dodecyl sulfate (SDS) surfactant-modified α-Al2O3 particles were comprehensively investigated in this study. The TCs adsorption was significantly enhanced by using the modified adsorbents compared with the use of the unmodified adsorbents. The experimental conditions were systematically optimized and found to be pH 4, NaCl 1 mM, the contact time of 180 min, and the adsorbent dosage of 25 mg. mL-1. The high maximum adsorption capacities were approximately 320, 85, and 91 mg. g-1 for TC, OTC, and CTC, respectively. Meanwhile, the great removal efficiencies of the three antibiotics TC, OTC, and CTC were correspondingly 91.85, 88.4, and 98.3%. The TCs adsorption isotherm and kinetics on the SDS-modified α-Al2O3 particles mainly governed by the electrostatic and hydrophobic interactions were clarified by a suitable two-step model, the Fourier transform infrared spectroscopy (FT-IR) and zeta potential measurements. Meanwhile, the TCs structural transformation determined by the liquid chromatography with tandem mass spectrometry (LC-MS/MS) measurement was promoted through the adsorption on the α-Al2O3 surface. The TCs transformation rates strongly affected by the TCs adsorption were in the order of CTC > TC > OTC. The found results are promised that the SDS-modified α-Al2O3 particles might behave as high-performance adsorbents to remove the TCs from aqueous solutions.

Millimeter wave fixed wireless systems in future backhaul and access network applications can be affected by weather conditions. The losses caused by rain attenuation and antenna misalignment due to wind-induced vibrations have greater impacts on the link budget reduction at E-band frequencies and higher. The current International Telecommunications Union Radiocommunication Sector (ITU-R) recommendation has been widely used to estimate rain attenuation, and the recent Asia Pacific Telecommunity (APT) report provides the model to estimate the wind-induced attenuation. This article provides the first experimental study of the combined rain and wind effects in a tropical location using both models at a frequency in the E band (74.625 GHz) and a short distance of 150 m. In addition to using wind speeds for attenuation estimation, the setup also provides direct antenna inclination angle measurements using the accelerometer data. This solves the limitation of relying on the wind speed since the wind-induced loss is dependent on the inclination direction. The results show that the current ITU-R model can be used to estimate the attenuation of a short fixed wireless link under heavy rain, and the addition of wind attenuation via the APT model can estimate the worst-case link budget during high wind speeds.

Previous studies have investigated the effects of the pandemic on quality of life; both economical and psychosocial effects were explored. Some studies also mentioned mediating factors as mechanisms in this relationship; however, the mediating role of anxiety has not been examined. The present study examined the mediating effects of anxiety on the relationship between socioeconomic impact of COVID-19 and quality of life. An online survey of 280 Vietnamese residents was conducted in the context of the pandemic outbreak. Anxiety was found to be a completely mediating variable in the association between the socioeconomic impact of the pandemic and quality of life during the lockdown period. This finding contributes to a better understanding of how the pandemic affects people's quality of life and serves as the foundation for lessening the negative effects of the epidemic on people's lives.

We demonstrate an electro-optic (EO) switch or in general, an EO controllable power divider based on a periodically poled lithium niobate (PPLN) polarization mode converter (PMC) and a five-waveguide adiabatic coupler integrated on a Ti:LN photonic circuit chip. In this integrated photonic circuit (IPC) device, the PPLN works as an EO controllable polarization rotator (and therefore a PMC), while the adiabatic coupler functions as a broadband polarization beam splitter (PBS). The 1-cm long PPLN EO PMC of the IPC device is characterized to have a half-wave (or switching) voltage of Vπ∼20 V and a conversion bandwidth of ∼2.6 nm. The splitting ratios of the adiabatic coupler PBS in the IPC device are >99% for both polarization modes over a broad spectral range from 1500-1640 nm. The EO mode of the implemented IPC device is activated when the PPLN EO PMC section is driven by an external voltage; the characterized EO switching/power division behavior of the device is in good agreement with the theoretical fit. The tunability of the EO IPC device in the 100-nm experimental spectral range is also demonstrated via the temperature tuning. The featured broad tunability and high integrability of the EO device presented in this study facilitates it to be an advantageous building block for realizing an on-chip photonic system.

We report the demonstration of an electro-optic (EO) switchable dual-wavelength (1064- and 1342-nm) Nd:YVO4 laser based on an aperiodically poled lithium niobate (APPLN) chip whose domain structure is designed using aperiodic optical superlattice (AOS) technology. The APPLN works as a wavelength-dependent EO polarization-state controller in the polarization-dependent laser gain system to enable switching among multiple laser spectra simply by voltage control. When the APPLN device is driven by a voltage-pulse train modulating between a VHQ (in which target laser lines obtain gain) and a VLQ (in which laser lines are gain suppressed), the unique laser system can produce Q-switched laser pulses at dual wavelengths 1064 and 1342 nm, single wavelength 1064 nm, and single wavelength 1342 nm, as well as their non-phase-matched sum-frequency and second-harmonic generations at VHQ = 0, 267, and 895 V, respectively. A laser can benefit from such a novel, to the best of our knowledge, simultaneous EO spectral switching and Q switching mechanisms to increase its processing speed and multiplexity for versatile applications.

This Letter describes a high-speed yet simple fiber-wireless-fiber system in the 100-GHz band using a photonics-enabled receiver and optical phase modulator. At the antenna site, a millimeter-wave signal is down-converted to the microwave band using an electronic mixer with a local oscillator signal generated remotely using photonic technology. The down-converted signal is converted to an optical signal using an optical phase modulator. At the receiver, a simple direct detection of the phase-modulated signal is performed using optical filtering technology. To demonstrate the proof-of-concept, we successfully transmitted a 64-quadrature amplitude modulation orthogonal frequency-division multiplexing signal with a record line rate of 80 Gb/s (net data rate of 55 Gb/s) over a system consisting of two radio-over-fiber links and a 5 m wireless link in the 100-GHz band. The proposed system with simple antenna sites and optical transceivers can facilitate the deployment of ultra-dense small cells in high-frequency bands in beyond-5G networks.

The spin-forbidden and spin-allowed reactions of the excited and ground electronic state O(1D, 3P) + N2O(X1Σ+) systems have been studied theoretically. Quantum calculations at the UCCSD(T)/CBS(T, Q, 5)//CCSD/aug-cc-pVTZ level have located two crossing points, MSX1 and MSX2, with energies of 11.2 and 22.7 kcal mol-1 above O(3P) + N2O, respectively. The second-order P-independent rate constants for the adiabatic and non-adiabatic thermal reactions predicted by adiabatic TST/VTST and non-adiabatic TST, respectively, agree closely with the available literature results. The second-order rate constant, k2a = 9.55 × 10-11 exp(-26.09 kcal mol-1/RT) cm3 molecule-1 s-1, for the O(3P) + N2O → 2NO reaction, contributed by both the dominant MSX2 and the minor TS1-a channels, is in reasonable accord with prior experiments and recommendations, covering the temperature range of 1200-4100 K. The calculated rate constant, k2b = 4.47 × 10-12 exp(-12.9 kcal mol-1/RT) cm3 molecule-1 s-1, for the O(3P) + N2O → N2 + O2(a1Δg) reaction, occurring exclusively via MSX1, is also in good agreement with the combined experimental data measured in a shock tube study at T = 1940-3340 K (ref 16) and the result measured by Fourier transform infrared spectroscopy in the temperature range of 988-1083 K (ref 17). Moreover, the spin-allowed rate constants predicted for the singlet-state reactions, k1a = (7.06-7.46) × 10-11 cm3 molecule-1 s-1 for O(1D) + N2O → 2NO and k1b = (4.36-4.66) × 10-11 cm3 molecule-1 s-1 for O(1D) + N2O → N2 + O2(a1Δg) in the temperature range of 200-350 K, agree quantitatively with the experimentally measured data, while the total rate constant k1 = k1a + k1b was also found to be in excellent accordance with many reported values.

This study aims to investigate the adsorption characteristics of cationic surfactant, cetyltrimethylamonium bromide (CTAB) onto negatively nanosilica rice husk surface and the application for antibiotic treatment in water environment. Adsorption of CTAB onto nanosilica increased with an increase of solution pH, due to an enhancement of the electrostatic attraction between cationic methylamomethylamonium groups and negatively charged nanosilica surface enhanced at higher pH. Adsorption of CTAB decreased with a decrease of ionic strength while a common intersection point (CIP) was observed for adsorption isotherm at different ionic strengths, suggesting that hydrophobic interactions between alkyl chains in CTAB molecules significantly induced adsorption and admicelles with bilayer formation were dominant than monolayer of hemimicelles. The CTAB functionalized nanosilica (CFNS) was applied for removal of beta-lactam amoxicillin (AMX). The best conditions for AMX treatment using CFNS were selected as pH 10, contact time 60 min and CFNS dosage 10 mg/mL. Removal efficiency of AMX using CFNS reached to 100% under optimum conditions while it was only 25.01% using nanosilica without CTAB. The maximum AMX adsorption capacity using CFNS of about 25 mg/g was much higher than other adsorbents. The effects of different organics such as humic acid, anionic surfactant, and other antibiotics on AMX removal using CFNS were also studied. A two-step model can fit CTAB uptake isotherms onto nanosilica and AMX onto CFNS well at different KCl concentrations. Based on the desorption of CTAB with AMX adsorption as well as adsorption isotherms, the change in surface charge and functional vibration groups after adsorption, we indicate that AMX adsorption onto CFNS was mainly controlled by electrostatic interaction. We reveal that CFNS is an excellent adsorbent for antibiotic treatment from aqueous solution.

Coastal sediments in the Mong Cai area were collected and analyzed for grain size, heavy metals, total organic carbon, and isotopes (210Pb, 226Ra, δ15N, δ13C) to assess sediment quality. The most common sediments were fine sand in surface sediment, very fine sand in core C1, and very coarse and coarse silt in core C2. The total organic carbon was highest in C2 next to the surface and lowest in C1, with content levels of 1.81%, 0.40%, and 0.31%, respectively. The chronology in C1 was 1877-2019 (142 years, 0-5 0 cm), with an average sedimentation rate of 0.71 cm/year. In C2, the chronology was 1944-2019 (75 years, 0-14 cm), with an average sedimentation rate of 0.27 cm/year. These δ13C and δ15N in the sediment reflect the source of the organic matter mix from the marine and terrigenous sediments. All studied heavy metals were lower than the ISQGs, with the exception of As in C1 and C2, which were higher. C1 showed a decline in As over time, while C2 As levels increased between 1996 and 2019. In terms of heavy metal pollution indexes, the geoaccumulation index (Igeo) showed that C1 and C2 were unpolluted to moderately polluted with As, with Li and Pb in C2; the enrichment factor (EF) was moderately enriched with As; the contamination factor (CF) was moderately contaminated (Pb, Cd, Fe, Mo, and Li) in C2 and C1 (Cd, As, Li) and considerably contaminated (As) in C2. The risk factor (ER) of As showed a moderate potential ecological risk in C2. The degree of contamination (CD) ranged from moderate to considerable (C1, C2), and the ecological risk (RI) was low. Although CD ranged from moderate (C1) to considerable (C2), most contamination was concentrated at the bottom of the cores. RI was low. The Mong Cai sediment quality does not currently affect the coastal area's ecosystem and fauna.

Arsenic intake can cause human health disorders to the lungs, urinary tract, kidney, liver, hyper-pigmentation, muscles, neurological and even cancer. Biochar is potent, economical and ecologically sound adsorbents for water purification. After surface modifications, adsorption capacity of biochar significantly increased due to high porosity and reactivity. Adsorption capacities of the biochar derived from the municipal solid waste and KOH mixed municipal solid waste were increased from 24.49 and 30.98 mg/g for arsenic adsorption. Complex formation, electrostatic behavior and ion exchange are important mechanisms for arsenic adsorption. Organic arsenic removal using biochar is a major challenge. Hence, more innovative research should be conducted to achieve one of the 17 sustainable development goals of the United Nations i.e. "providing safe drinking water for all". This review is focused on the arsenic removal from water using pristine and modified biochar adsorbents. Recent advances in production methods of biochar adsorbents and mechanisms of arsenic removal from water are also illustrated.

The COVID-19 pandemic has caused enormous detrimental impacts on a global scale. Street vendors are one of the most heavily affected groups since they lack the skills and resources to overcome shocks. This study examines the economic burden facing this group during the pandemic and their coping strategies and mitigation mechanisms in response to these adverse effects. We utilized a mixed-methods approach, wherein 91 women vendors completed a survey questionnaire and 15 women vendors were interviewed. These vendors were found to experience a large reduction in business and consumption. The businesses of immigrant vendors suffered more adverse effects than those of local vendors. Also, the vendors selling in wet market areas incurred greater economic burdens than those selling near schools or recreation centers. The vendors lacked coping strategies to sustain their businesses and adopted various mitigation mechanisms to ensure essential consumption. This study highlights the need for urban social policies that can support this vulnerable group amid a pandemic. We also discuss policy implications for cities and economic development with a focus on street vendors.
