OBJECTIVE: Despite numerous studies suggesting the role of insular cortex in the processing of gustatory and olfactory inputs, the exact location of olfactogustatory representation in the insula remains controversial. Here we provide a functional mapping of olfactory-gustatory responses to stimulation of the human insular cortex. METHODS: We reviewed 651 electrical stimulations of the insula that were performed in 221 patients, using stereotactically implanted depth electrodes, during the presurgical evaluation of drug-refractory epilepsy. RESULTS: Gustatory sensations were evoked in 15 (2.7%) of the 550 stimulations that elicited a clinical response. They were exclusively obtained after stimulation of a relatively delimited zone of insula, located in its mid-dorsal part (posterior short gyrus). Six olfactory sensations (1.1%) could be obtained during stimulations of an insular region that partially overlapped with the gustatory representation. INTERPRETATION: Our study provides a functional mapping of gustatory representation in the insular posterior short gyrus and the first detailed description of olfactory sensations obtained by direct stimulation of mid-dorsal insula. Our data also show a spatial overlap between gustatory, olfactory, and oral somatosensory representation in the mid-dorsal insula, and suggest that this part of the insula may be an integrated oral sensory region that plays a key role in flavor perception. It also indicates that dysfunction in this region should be considered during the evaluation of gustatory and olfactory epileptic seizures. Ann Neurol 2017;82:360-370.

Graph theory enables the study of systems by describing those systems as a set of nodes and edges. Graph theory has been widely applied to characterize the overall structure of data sets in the social, technological, and biological sciences, including neuroscience. Modular structure decomposition enables the definition of sub-networks whose components are gathered in the same module and work together closely, while working weakly with components from other modules. This processing is of interest for studying memory, a cognitive process that is widely distributed. We propose a new method to identify modular structure in task-related functional magnetic resonance imaging (fMRI) networks. The modular structure was obtained directly from correlation coefficients and thus retained information about both signs and weights. The method was applied to functional data acquired during a yes-no odor recognition memory task performed by young and elderly adults. Four response categories were explored: correct (Hit) and incorrect (False alarm, FA) recognition and correct and incorrect rejection. We extracted time series data for 36 areas as a function of response categories and age groups and calculated condition-based weighted correlation matrices. Overall, condition-based modular partitions were more homogeneous in young than elderly subjects. Using partition similarity-based statistics and a posteriori statistical analyses, we demonstrated that several areas, including the hippocampus, caudate nucleus, and anterior cingulate gyrus, belonged to the same module more frequently during Hit than during all other conditions. Modularity values were negatively correlated with memory scores in the Hit condition and positively correlated with bias scores (liberal/conservative attitude) in the Hit and FA conditions. We further demonstrated that the proportion of positive and negative links between areas of different modules (i.e., the proportion of correlated and anti-correlated areas) accounted for most of the observed differences in signed modularity. Taken together, our results provided some evidence that the neural networks involved in odor recognition memory are organized into modules and that these modular partitions are linked to behavioral performance and individual strategies.

Olfactory processes were reported to be lateralized. The purpose of this study was to further explore this phenomenon and investigate the effect of the hemispheric localization of epileptogenic foci on olfactory deficits in patients with temporal lobe epilepsy (TLE). Olfactory functioning was assessed in 61 patients and 60 healthy control (HC) subjects. The patients and HC subjects were asked to rate the intensity, pleasantness, familiarity, and edibility of 12 common odorants and then identify them. Stimulations were delivered monorhinally in the nostril ipsilateral to the epileptogenic focus in TLE and arbitrarily in either the left or the right nostril in the HC subjects. The results demonstrated that regardless of the side of stimulation, patients with TLE had reduced performance in all olfactory tasks compared with the HC subjects. With regard to the side of the epileptogenic focus, patients with left TLE judged odors as less pleasant and had more difficulty with identification than patients with right TLE, underlining a privileged role of the left hemisphere in the emotional and semantic processing of odors. Finally, irrespective of group, a tendency towards a right-nostril advantage for judging odor familiarity was found in agreement with a prominent role of the right hemisphere in odor memory processing.

The human brain's ability to adapt to environmental changes is obvious in specific sensory domains of experts, and olfaction is one of the least investigated senses. As we have previously demonstrated that olfactory expertise is related to functional brain modifications, we investigated here whether olfactory expertise is also coupled with structural changes. We used voxel-based morphometry to compare the gray-matter volume in student and professional perfumers, as well as untrained control subjects, and accounted for all methodological improvements that have been recently developed to limit possible errors associated with image processing. In all perfumers, we detected an increase in gray-matter volume in the bilateral gyrus rectus/medial orbital gyrus (GR/MOG), an orbitofrontal area that surrounds the olfactory sulcus. In addition, gray-matter volume in the anterior PC and left GR/MOG was positively correlated with experience in professional perfumers. We concluded that the acute olfactory knowledge acquired through extensive olfactory training leads to the structural reorganization of olfactory brain areas.

Areas of expertise that cultivate specific sensory domains reveal the brain's ability to adapt to environmental change. Perfumers are a small population who claim to have a unique ability to generate olfactory mental images. To evaluate the impact of this expertise on the brain regions involved in odor processing, we measured brain activity in novice and experienced (student and professional) perfumers while they smelled or imagined odors. We demonstrate that olfactory imagery activates the primary olfactory (piriform) cortex (PC) in all perfumers, demonstrating that similar neural substrates were activated in odor perception and imagination. In professional perfumers, extensive olfactory practice influences the posterior PC, the orbitofrontal cortex, and the hippocampus; during the creation of mental images of odors, the activity in these areas was negatively correlated with experience. Thus, the perfumers' expertise is associated with a functional reorganization of key olfactory and memory brain regions, explaining their extraordinary ability to imagine odors and create fragrances.

Alliesthesia is the modulation of the rewarding value of a stimulus according to the internal state (hungry or satiated). This study aimed to evaluate this phenomenon as a function of the nature of the stimulus (odors evoking edible and non-edible items, and the food odors evoking fatty and non-fat foods) and to compare the effectiveness of two reward evaluations (measures of pleasantness and appetence) to reveal alliesthesia. The results showed that both fatty and non-fat food odors were judged as less pleasant and less appetent when the subjects were satiated than when they were hungry, whereas no such difference was observed for non-food odors. There was a greater decrease in appetence than there was in pleasantness. Moreover, the decrease in appetence was greater for fatty than for non-fat food odors, whereas the decrease in pleasantness was similar for both fatty and non-fat food odors. Our study allows for the definition of a more comprehensive pattern of alliesthesia based on odor category. It demonstrates that alliesthesia is specific to food odors and that it is more pronounced when odors are associated with fatty rather than non-fat foods. It also reveals that an appetence measure is more sensitive than a pleasantness measure for describing an acute reward modulation process.

Brain reward dysfunction in eating disorders has been widely reported. However, whether the neural correlates of hedonic and motivational experiences related to food cues are differentially affected in anorexia nervosa of restrictive type (ANr), bulimia nervosa (BN), and healthy control (HC) participants remains unknown. Here, 39 women (14 ANr, 13 BN, and 12 HC) underwent fMRI while smelling food or non-food odors in hunger and satiety states during liking and wanting tasks. ANr and BN patients reported less desire to eat odor-cued food and odor-cued high energy-density food (EDF), respectively. ANr patients exhibited lower ventral tegmental area (VTA) activation than BN patients to food odors when rating their desire to eat, suggesting altered incentive salience attribution to food odors. Compared with HC participants, BN patients exhibited decreased activation of the caudate nucleus to food odors in the hunger state during the wanting task. Both patient groups also showed reduced activation of the anterior ventral pallidum and insula in response to high EDF odors in the hunger state during the wanting task. These findings indicate that brain activation within the food reward-regulating circuit differentiates the three groups. ANr patients further exhibited lower activation of the precuneus than other participants, suggesting a possible role of body image distortion in ANr. Our study highlights that food odors are relevant sensory probes to gain better insight into the dysfunction of the mesolimbic and striatal circuitry involved in food reward processing in patients with EDs.

What neural mechanism underlies the capacity to understand the emotions of others? Does this mechanism involve brain areas normally involved in experiencing the same emotion? We performed an fMRI study in which participants inhaled odorants producing a strong feeling of disgust. The same participants observed video clips showing the emotional facial expression of disgust. Observing such faces and feeling disgust activated the same sites in the anterior insula and to a lesser extent in the anterior cingulate cortex. Thus, as observing hand actions activates the observer's motor representation of that action, observing an emotion activates the neural representation of that emotion. This finding provides a unifying mechanism for understanding the behaviors of others.

Negative alliesthesia to olfactory and visual stimuli was assessed in 29 normal-weight women who, on alternate days, were either fasting or in a postprandial state after an ad libitum lunch. The participants were alternatively exposed to food and non-food pictures and odorants, and then rated for their hedonic appreciation (liking) and their desire to ingest (wanting) the evoked foods. While negative alliesthesia was observed only for food stimuli, it did not equally affect all food categories in either sensory modality. The stimuli representing foods eaten in typical local main dishes or having high energy density (e.g., pizza, bacon, beef, cheese) evoked clear negative alliesthesia, whereas this was not the case for those less consumed within a customary meal or associated with desserts (i.e., fruits). Furthermore, the visual food stimuli triggered a more negative shift in liking than did the food odours. Finally, the shift in wanting between pre- and post-meal state was more important than the shift in liking. These results suggest that alliesthesia may be influenced by both metabolic and non-metabolic factors.

BACKGROUND: As evidenced in the DSM-V, autism spectrum disorders (ASD) are often characterized by atypical sensory behavior (hyper- or hypo-reactivity), but very few studies have evaluated olfactory abilities in individuals with ASD. METHODS: Fifteen adults with ASD and 15 typically developing participants underwent olfactory tests focused on superficial (suprathreshold detection task), perceptual (intensity and pleasantness judgment tasks), and semantic (identification task) odor processing. RESULTS: In terms of suprathreshold detection performance, decreased discrimination scores and increased bias scores were observed in the ASD group. Furthermore, the participants with ASD exhibited increased intensity judgment scores and impaired scores for pleasantness judgments of unpleasant odorants. Decreased identification performance was also observed in the participants with ASD compared with the typically developing participants. This decrease was partly attributed to a higher number of near misses (a category close to veridical labels) among the participants with ASD than was observed among the typically developing participants. CONCLUSIONS: The changes in discrimination and bias scores were the result of a high number of false alarms among the participants with ASD, which suggests the adoption of a liberal attitude in their responses. Atypical intensity and pleasantness ratings were associated with hyperresponsiveness and flattened emotional reactions, respectively, which are typical of participants with ASD. The high number of near misses as non-veridical labels suggested that categorical processing is functional in individuals with ASD and could be explained by attention-deficit/hyperactivity disorder. These findings are discussed in terms of dysfunction of the olfactory system.

The study of food aversion in humans by the induction of illness is ethically unthinkable, and it is difficult to propose a type of food that is disgusting for everybody. However, although cheese is considered edible by most people, it can also be perceived as particularly disgusting to some individuals. As such, the perception of cheese constitutes a good model to study the cerebral processes of food disgust and aversion. In this study, we show that a higher percentage of people are disgusted by cheese than by other types of food. Functional magnetic resonance imaging then reveals that the internal and external globus pallidus and the substantia nigra belonging to the basal ganglia are more activated in participants who dislike or diswant to eat cheese (Anti) than in other participants who like to eat cheese, as revealed following stimulation with cheese odors and pictures. We suggest that the aforementioned basal ganglia structures commonly involved in reward are also involved in the aversive motivated behaviors. Our results further show that the ventral pallidum, a core structure of the reward circuit, is deactivated in Anti subjects stimulated by cheese in the wanting task, highlighting the suppression of motivation-related activation in subjects disgusted by cheese.

Brain reward systems mediate liking and wanting for food reward. Here, we explore the differential involvement of the following structures for these two components: the ventral and dorsal striatopallidal area, orbitofrontal cortex (OFC), anterior insula and anterior cingulate. Twelve healthy female participants were asked to rate pleasantness (liking of food and non-food odors) and the desire to eat (wanting of odor-evoked food) during event-related functional magnetic resonance imaging (fMRI). The subjective ratings and fMRI were performed in hunger and satiety states. Activations of regions of interest were compared as a function of task (liking vs wanting), odor category (food vs non-food) and metabolic state (hunger vs satiety). We found that the nucleus accumbens and ventral pallidum were differentially involved in liking or wanting during the hunger state, which suggests a reciprocal inhibitory influence between these structures. Neural activation of OFC subregions was correlated with either liking or wanting ratings, suggesting an OFC role in reward processing magnitude. Finally, during the hunger state, participants with a high body mass index exhibited less activation in neural structures underlying food reward processing. Our results suggest that food liking and wanting are two separable psychological constructs and may be functionally segregated within the cortico-striatopallidal circuit.

Episodic memory is defined as the conscious retrieval of specific past events. Whether accurate episodic retrieval requires a recollective experience or if a feeling of knowing is sufficient remains unresolved. We recently devised an ecological approach to investigate the controlled cued-retrieval of episodes composed of unnamable odors (What) located spatially (Where) within a visual context (Which context). By combining the Remember/Know procedure with our laboratory-ecological approach in an original way, the present study demonstrated that the accurate odor-evoked retrieval of complex and multimodal episodes overwhelmingly required conscious recollection. A feeling of knowing, even when associated with a high level of confidence, was not sufficient to generate accurate episodic retrieval. Interestingly, we demonstrated that the recollection of accurate episodic memories was promoted by odor retrieval-cue familiarity and describability. In conclusion, our study suggested that semantic knowledge about retrieval-cues increased the recollection which is the state of awareness required for the accurate retrieval of complex episodic memories.

The effects of odor processing were investigated at various analytical levels, from simple sensory analysis to deep or semantic analysis, on a subsequent task of odor naming. Students (106 women, 23.6 +/- 5.5 yr. old; 65 men, 25.1 +/- 7.1 yr. old) were tested. The experimental procedure included two successive sessions, a first session to characterize a set of 30 odors with criteria that used various depths of processing and a second session to name the odors as quickly as possible. Four processing conditions rated the odors using descriptors before naming the odor. The control condition did not rate the odors before naming. The processing conditions were based on lower-level olfactory judgments (superficial processing), higher-level olfactory-gustatory-somesthetic judgments (deep processing), and higher-level nonolfactory judgments (Deep-Control processing, with subjects rating odors with auditory and visual descriptors). One experimental condition successively grouped lower- and higher-level olfactory judgments (Superficial-Deep processing). A naming index which depended on response accuracy and the subjects' response time were calculated. Odor naming was modified for 18 out of 30 odorants as a function of the level of processing required. For 94.5% of significant variations, the scores for odor naming were higher following those tasks for which it was hypothesized that the necessary olfactory processing was carried out at a deeper level. Performance in the naming task was progressively improved as follows: no rating of odors, then superficial, deep-control, deep, and superficial-deep processings. These data show that the deepest olfactory encoding was later associated with progressively higher performance in naming.

Odors are powerful cues that trigger episodic memories. However, in light of the amount of behavioral data describing the characteristics of episodic odor memory, the paucity of information available on the neural substrates of this function is startling. Furthermore, the diversity of experimental paradigms complicates the identification of a generic episodic odor memory network. We conduct a systematic review of the literature depicting the current state of the neural correlates of episodic odor memory in healthy humans by placing a focus on the experimental approaches. Functional neuroimaging data are introduced by a brief characterization of the memory processes investigated. We present and discuss laboratory-based approaches, such as odor recognition and odor associative memory, and autobiographical approaches, such as the evaluation of odor familiarity and odor-evoked autobiographical memory. We then suggest the development of new laboratory-ecological approaches allowing for the controlled encoding and retrieval of specific multidimensional events that could open up new prospects for the comprehension of episodic odor memory and its neural underpinnings. While large conceptual differences distinguish experimental approaches, the overview of the functional neuroimaging findings suggests relatively stable neural correlates of episodic odor memory.

We behaviorally explore the link between olfaction, emotion and memory by testing the hypothesis that the emotion carried by odors facilitates the memory of specific unique events. To investigate this idea, we used a novel behavioral approach inspired by a paradigm developed by our team to study episodic memory in a controlled and as ecological as possible way in humans. The participants freely explored three unique and rich laboratory episodes; each episode consisted of three unfamiliar odors (What) positioned at three specific locations (Where) within a visual context (Which context). During the retrieval test, which occurred 24-72 h after the encoding, odors were used to trigger the retrieval of the complex episodes. The participants were proficient in recognizing the target odors among distractors and retrieving the visuospatial context in which they were encountered. The episodic nature of the task generated high and stable memory performances, which were accompanied by faster responses and slower and deeper breathing. Successful odor recognition and episodic memory were not related to differences in odor investigation at encoding. However, memory performances were influenced by the emotional content of the odors, regardless of odor valence, with both pleasant and unpleasant odors generating higher recognition and episodic retrieval than neutral odors. Finally, the present study also suggested that when the binding between the odors and the spatio-contextual features of the episode was successful, the odor recognition and the episodic retrieval collapsed into a unique memory process that began as soon as the participants smelled the odors.

In agreement with the theoretical framework stipulating that mental images arise from neural activity in early sensory cortices, the primary olfactory cortex [i.e., the piriform cortex (PC)] is activated when non-olfactory-experts try to generate odor mental images. This finding strongly contrasts with the allegation that it is typically impossible to mentally imagine odors. However, other neurophysiological or cognitive processes engaged in the endeavor of odor mental imagery such as sniffing, attention, expectation, and cross-modal interactions involve the PC and could explain this paradox. To unambiguously study the odor mental imagery, we first argued the need to investigate odor experts who have learned to specifically reactivate olfactory percepts. We then assert the necessity to explore the network dedicated to this function by considering variations in both the activity level and the connection strength of the areas belonging to this network as a function of the level of expertise of the odor experts.

Olfactory expertise remains poorly understood, most likely because experts in odor, such as perfumers, sommeliers, and oenologists, are much rarer than experts in other modalities, such as musicians or sportsmen. In this review, we address the specificities of odor expertise in both odor experts and in a priori untrained individuals who have undergone specific olfactory training in the frame of an experiment, such as repeated exposure to odors or associative learning. Until the 21st century, only the behavioral effects of olfactory training of untrained control individuals had been reported, revealing an improvement of olfactory performance in terms of sensitivity, discrimination, memory, and identification. Behavioral studies of odor experts have been scarce, with inconsistent or inconclusive results. Recently, the development of cerebral imaging techniques has enabled the identification of brain areas and neural networks involved in odor processing, revealing functional and structural modifications as a function of experience. The behavioral approach to odor expertise has also evolved. Researchers have particularly focused on odor mental imagery, which is characteristic of odor experts, because this ability is absent in the average person but is part of a perfumer's professional practice. This review summarizes behavioral, functional, and structural findings on odor expertise. These data are compared with those obtained using animals subjected to prolonged olfactory exposure or to olfactory-enriched environments and are discussed in the context of functional and structural plasticity.

Episodic memory is defined as the conscious recollection of a personal event (What) in its spatial (Where) and contextual (Which context) environment. In existing approaches, human episodic memory is either explored separately from real-life situations or is not fully controlled. In this study, we propose an intermediate approach, inspired by animal studies, that permits the control of the encoding and recall phases, while still being ecologically valid. As odors are known to be especially evocative reminders, we explored the memory of olfactory episodes. During trial-unique encoding, participants freely explored three episodes, one episode per day, each composed of three unnamable odors (What) that were positioned at specific locations on a board (Where) within a visual context (Which context). On the fourth day, both old and new odors were presented, and when an odor was recognized, the participants had to remember both its spatial location and the visual context in which it occurred. In Experiment 1, the participants were highly proficient at recognizing odors, and they recall the spatio-contextual environment associated with these odors in approximately half of the trials. To adapt the recall procedure to the constraints of fMRI, we conducted Experiment 2 demonstrating that trial repetition did not disturb the memory process. Thus, we first validated our protocol, which investigates the memory of olfactory episodes in a fully controlled way that is as close as possible to real-life situations. Then, we demonstrated the adaptability of our protocol for the future exploration of the neural networks implicated in episodic recall.

Though it has been suggested that hedonic processing is altered in anorexia nervosa (AN), few studies have used objective measures to assess affective processes in this eating disorder. Accordingly, we investigated facial electromyographic, autonomic and subjective reactivity to the smell and sight of food and non-food stimuli, and assessed more particularly rapid facial reactions reflecting automatic processing of pleasantness. AN and healthy control (HC) women were exposed, before and after a standardized lunch, to pictures and odorants of foods differing in energy density, as well as to non-food sensory cues. Whereas the temporal profile of zygomatic activity in AN patients was typified by a fast drop to sensory cues within the 1000 ms following stimulus onset, HC showed a larger EMG reactivity to pictures in a 800-1000 ms time window. In contrast, pleasantness ratings discriminated the two groups only for high energy density food cues suggesting a partial dissociation between objective and subjective measures of hedonic processes in AN patients. The findings suggest that the automatic processing of pleasantness might be altered in AN, with the sensitivity to reward being modulated by controlled processes.
