RESUMO

Introduction and importance: Limbic encephalitides (LE) have symptoms and signs of new-onset seizures accompanied by cognitive impairment and signal changes in the MRI of the limbic system in the brain. Numerous antibodies against the neurons and synapses have been detected so far. Of those, antiglutamic acid decarboxylase antibody (Anti-GAD Ab) impairs the gamma amino butyric acid, one of the primary mediators that naturally prevents abnormal neuronal activity causing seizure. Case presentation: The authors have reported a case of anti-GAD Ab LE in a diabetic male adolescent who responded dramatically to intravenous immunoglobulin and reviewed all similar pediatric cases for 15 years now. Clinical discussion: The symptoms in children suffering from anti-GAD LE in three categories, systemic, psychiatric, and neurological, are heterogeneous. The most common manifestations were seizures followed by altered mental status and behavioral changes, respectively. The two main clinical scenarios described in GAD65-mediated autoimmune epilepsy are (1) an acute/subacute onset of seizures alone or seizures (including new-onset refractory status epilepticus, NORSE) accompanied by some degrees of cognitive and psychiatric manifestations, including amnesia and mesiotemporal inflammatory involvement consistent with LE and (2) epilepsy without clinical or MRI evidence of active central nervous system inflammation. Conclusion: Although rare, the neurologist should consider the potential role of anti-GAD ab-associated encephalitis in the presence of diabetes mellitus.

RESUMO

When the chyle, the liquid that is formed from the connection of the body's lymphatic flows, leaks to the pleural space, chylothorax expression comes into existence. Traumatic types are due to penetrating wounds or iatrogenic during heavy thoracic oncology surgeries. To the best of our knowledge, we have reported the first case of left-sided chylothorax following an isolated stab wound in the fifth intercostal space of the same side treated by tube drainage and 'nil per os' dietary condition.

Introduction and importance: Tracheostomy has been done since ancient Greece, but in today's modern format, it has been accepted for over one and a half centuries. Improvements in medical care and treatments have survived disabled patients who died in the past decades. Among these, tracheostomy tube (TT) segment aspiration was as rare as less than 100 cases have already been documented. We reported a case of aspirated metal TT to the right main bronchus from a 12-year-old teenage girl and then reviewed the available literature. Case presentation: The patient suffered from subglottic stenosis affected after adenotonsillectomy remained for 6 years. She was referred to the emergency department for acute respiratory distress and cyanosis. Immediately underwent rigid bronchoscopy, and on the way to the operating room, she was oxygenated from the tracheal orifice. The detached part was successfully removed. Clinical discussion: The common tracheostomy indications in children involve congenital or acquired airway stenosis, some conditions needing long-term ventilation, bilateral vocal fold insufficiency, and infectious compromise of the upper airway. Metal TTs for long-lasting are preferred to synthetic ones. Nevertheless, it has some potential complications. Conclusion: Careful inspecting, especially metallic TTs, after every disinfection and regular follow-up by the surgeon, is highly recommended to prevent potential casualties.

Lymphatic malformations (LM) refer to very rare hamartomatous benign lymphatic ectasias with an incidence of 1:250 000. They almost involve children more than adults, and among them, infants under one age are mostly affected. Macrocytic LM is found to be more than 2 cm in diameter or 2×2 cm2 in volume. The proper treatment for mesenteric LM is complete surgical excision unless there is vital structure involvement. Case presentation: The authors report a mesenteric macrocytic LM in a 2-year-old girl complaining of vague abdominal discomfort and persistent vomiting in which ultrasonography revealed a cystic masse with seromucous components. She then underwent exploratory laparotomy. The operation and the follow-up duration were uneventful. Discussion: LMs are rare benign lesions of vascular origin with lymphatic differentiation, according to the latest International Society for the Study of Vascular Anomalies (ISSVA 2018). Under light microscopy, these malformations are characterized by their thin-walled endothelium and lymphatic tissue.These mobile lesions are incidentally found or appear with intestinal obstruction or acute abdomen scenarios. Conclusion: Although benign, the LMs have the potential for invasion and recurrence. Thus, the examiner physician must keep such intra-abdominal lesions in mind.

The synchronicity of situs inversus totalis (SIT) and infantile hypertrophic pyloric stenosis (IHPS) is rare. We have reported a case of this concurrency in a 3-day-old newborn with classic symptoms.

INTRODUCTION: Penetrating traumas to the thorax could be potentially serious. Vena caval wounds are highly lethal, so that half of the patients die before reaching the hospital, and another 50% may die perioperatively. Although rare, most of them are the result of gunshot wounds. PRESENTATION OF CASE: We report a 13-year-old boy shot by an air gun through his right hemithorax. During surgery, an air gun bullet appeared right within the wall of the intrapericardial inferior vena cava (IVC). DISCUSSION: Traumas to the thoracic contents as vena cava are inevitable, presenting mostly with haemo or pneumothorax. If the victim reaches the operating room alive, the approach to his or her vena cava rupture remains a challenge in the hands of surgeons. In this case, the surgeon, following the bullet removal, repaired the penetration immediately by direct suturing because clamping the inferior vena for its thin wall can expand the rupture, also blocking systemic venous return. CONCLUSION: The surgeon in a general trauma center that is almost lacking cardiopulmonary pump can repair the vital injuries to the IVC with the technique of direct suturing.
