BACKGROUND: Currently, no nationwide objective physical activity data exists for children and adolescents living in Germany. The German Health Interview and Examination Survey for Children and Adolescents (KiGGS) and the Motorik-Modul study (MoMo) is a national cohort study that has incorporated accelerometers in its most recent data collection wave (wave 2, since 2014). This wave 2 marks the first nationwide collection of objective data on the physical activity of children and adolescents living in Germany. OBJECTIVE: The purpose of this protocol is to describe the methods used in the KiGGS and MoMo study to capture the intensity, frequency, and duration of physical activity with accelerometers. METHODS: Participants (N=11,003, aged 6 to 31 years) were instructed to wear an ActiGraph GT3X+ or wGT3X-BT accelerometer laterally on the right hip. Accelerometers were worn on consecutive days during waking hours, including at least 4 valid weekdays and 1 weekend day (wear time >8 hours) in the evaluation. A nonwear time protocol was also implemented. RESULTS: Data collection was completed by October 2017. Data harmonization took place in 2018. The first accelerometer results from this wave were published in 2019, and detailed analyses are ready to be submitted in 2020. CONCLUSIONS: This study protocol provides an overview of technical details and basic choices when using accelerometers in large-scale epidemiological studies. At the same time, the restrictions imposed by the specified filters and the evaluation routines must be taken into account. INTERNATIONAL REGISTERED REPORT IDENTIFIER (IRRID): DERR1-10.2196/14370.

BACKGROUND & AIMS: With high prevalence of overweight and obesity in developed countries, the assessment of body composition in children and adolescents is of growing interest. The aim of this study was (i) to provide up-to-date population wide percentiles for meaningful diagnostic bioelectrical impedance outcome parameters and (ii) to specify reference values for these measures in a normal weight subpopulation. METHODS: A representative nationwide sample of 4 to 24-year-old children and adolescents (n = 3593) was drawn out of 167 primary sample points across Germany from 2009 to 2012 (Motorik-Modul study wave 1). Fat free mass index (FFMI), fat mass index (FMI), body cell mass index (BCMI) and phase angle (PA) were derived from four electrodes 50 kHz bioelectrical impedance analyses. Sex-specific percentiles were calculated using the LMS-method for the total study population and a normal weight subsample (defined according to the international BMI-standard by Cole). RESULTS: Percentile curves for body composition parameters are similar between boys and girls until puberty. Subsequently, girls show a higher FMI than boys, and boys increase their FFM, BCM, and PA time-shifted, in that order. Differences in FMI between the overall and the normal weight sample increase with age, showing an age-dependent prevalence for overweight and obesity among the representative German sample. CONCLUSIONS: Results from the normal weight sample can be used as reference values to define a healthy body composition. Differences in body composition between the representative German sample and the normal weight subsample reveal a high prevalence of elevated fat mass in the population of children and adolescents living in Germany.

In this study, we aimed to measure the development of physical fitness (PF) of 10 different cohorts in grade 4 and 8 different cohorts in grade 7 at 18 sport schools of North Rhine-Westphalia, Germany. A total of 11,451 subjects (3979 female, 7472 male) aged 8-12 from the past 10 years were assessed using the German Motor Test (DMT) in grade 4. We tested 2614 subjects (1032 girls, 1582 boys) aged 11-15 from the past eight years using the DMT in grade 7. PF talents were defined as the top 10 boys and top 10 girls of each cohort. Linear regression was calculated to assess the development of PF. The PF of all subjects remained stable in grade 4 and declined in grade 7. The PF of the top 10 boys and top 10 girls increased in both grades. The improvements were stronger in grade 7 (female: rates of change (ß) = 0.80; male: ß = 0.76) than in grade 4 (female: ß = 0.36; male: ß = 0.32). Sit-ups and push-ups showed the highest change rates. The increase in PF of the top 10 boys and girls can be interpreted as a success for sport schools. Due to the increasing number of test participants, the likelihood of finding top talent increased. However, the increase in PF in the top talents was only partly explained by an increase in the number of tested individuals.

Background: Physical activity is an important contributor to reducing the risk for a variety of diseases. Understanding why people are physically active contributes to evidence-based planning of public health interventions because successful actions will target factors known to be related to physical activity (PA). Therefore the aim of this study is to identify the most meaningful correlates of PA in children and adolescents using a large, representative data set. Methods: Among n = 3539 (1801 boys) 6 to 17-year-old participants of the German representative Motorik-Modul baseline study (2003⁻2006) a total of 1154 different demographic, psychological, behavioral, biological, social and environmental factors were ranked according to their power of predicting PA using least absolute shrinkage and selection operator (LASSO) regressions. Results: A total of 18 (in girls) and 19 (in boys) important PA predictors from different, personal, social and environmental factors have been identified and ranked by LASSO. Peer modeling and physical self-concept were identified as the strongest correlates of PA in both boys and girls. Conclusions: The results confirm that PA interventions must target changes in different categories of PA correlates, but we suggest to focus particularly on the social environment and physical self-concept for interventions targeting children and adolescents in Germany nowadays. We also strongly recommend to repeatedly track correlates of PA, at least every 10 years, from representative samples in order to tailor contemporary PA interventions.
