[Nutritional management of kidney diseases in children].

The prevalence of various kidney diseases in children remains high in recent decades. Adequate nutrition management can enhance the effectiveness of drug treatment, slow the frequency of relapses andprevent the progression of the disease. The article is devoted to modern approaches to diet therapy in various kidney diseases in children with the defeat of tubular and glomerular appa ratus. For the first time the therapeutic diets for children with various kidney diseases are presented. Particular attention is paid to diet therapy in nephrotic syndrome (steroid-responsive and steroid-refractory). Dietary approaches with modern formulas for enteral nutrition in cases of steroid therapy complications in children with renal insufficiency (in predialysis stage and on dialysis) are described. Differentiated nutritional approaches for patients with different types of crystalluria are separately presented.

Characteristics of the nutrition organization and nutrient consumption in toddlers' living in the Central Federal District, Russian Federation (Moscow, Ivanovo, Yaroslavl).

A multicenter cross-sectional study with the participation of 356 healthy children aged 1 to 3 years from the cities of the Central Federal District: Moscow (n=106), Ivanovo (n=126), Yaroslavl (n=124) has been carried out. Questionnaire method, the method of 24-hour diet recall for 3 days (2 weekdays and 1 weekend) and the method of diary recording of food intake were used. The average daily volumes of food were consistent with the recommended age norms only in 31.2% of the surveyed children; after 1 year of life children actively started to transfer to nutrition from the family table and only 51.7% of children continued to receive individually prepared for baby food. The recommended dietary allowances were inadequate in macro- and micronutrients: 48.6% of all children received excessive amounts of protein and fat, and 74.1% were less in carbohydrates. The consumption of vitamins D and A and minerals (calcium, iron and zinc) did not correspond to existing recommendations in the majority of cases: only 10-30% of the examined children received physiological norms of vitamins from food and 13-34% - some minerals and trace elements. These results indicate that the nutrition of young children living in the Central Federal District is imbalanced on daily caloric, macro- and micronutrient composition, does not fully meet the requirements of the balanced diet and needs serious optimization.

[Study composition of lactoflora of the large intestine in patients with food allergy and irritable bowel syndrome].

Patients of different ages with various forms of food allergy and also with the irritable bowel syndrome with locks are studied. The quantitative levels of bifido- and lactoflora of large intestine, the qualitative characteristics of lactoflora population, including specific gravity of individual representatives in the sum of the isolates, are investigated, species composition is identified. Shown that food allergy, irrespective of the disease manifestation form induces the balance disturbance of bifido- and lactoflora in large intestine, significantly affects the reduction in the number of species of Lactococcus and Leuconostoc spp. and on the narrowing of the enzymatically active species of Lactobacillus spp.

[Evaluation of the function of mitral valve prosthesis by non-invasive methods]. / Otsenka funktsii protezov mitral'nogo klapana serdtsa neinvazivnymi metodami.

The study was undertaken to examine the correlation between the parameters of central hemodynamics and mitral valve prosthesis functions which were obtained by invasive techniques, as well as these of physical working capacity, hemodynamics, and blood by noninvasive methods at rest and during bicycle ergometry in the graded exercise test. An examination was made of 24 patients every 0.5-8 years (mean 3.2 years) after replacement of mitral valves by xenobioprostheses. "Oxygen pulse" and cardiac index defined during exercise highly correlated with prosthetic mid-diastolic pressure gradients. A moderate correlation was found between dry erythrocytic mass and total pulmonary blood flow resistance. The findings may be useful for the early diagnosis of prosthetic dysfunctions.

[Rhabditiasis in tamarisk gerbils]. / Rabditoz grebenchukovykh peschanok.

The paper deals with infection of Meriones (M.) tamariscinus Pallas, 1773 caused by Rhabditis strongyloides Schn., 1866 first recorded in the USSR. The clinical picture of the disease is described and data of pathoanatomical autopsy of dead animals are given. The way of infection of the animals in nature and in laboratory conditions is suggested. Under favourable conditions domestic animals can be infected with R. strongyloides too.

[The species composition and ecological characteristics of trematodes from reptiles in the Volga delta]. / Vidovoi sostav i ékologicheskie osobennosti trematod reptilii del'ty Volgi.

In the result of parasitological researches of reptiles of the Volga delta, 17 species of trematodes belonging to 10 families and 5 orders have been recovered. The dynamics of trematodes in reptiles is regulated by transformations of biocenoses of the delta depending on fluctuations of the Caspian sea level.

[Changes in the proteolytic enzyme activity of the blood against the background of a high-protein diet in children with pancreatitis]. / Izmenenie aktivnosti proteoliticheskikh fermentov syvorotki krovi na fone diety s povyshennym soderzhaniem belka u detei, bol'nykh pankreatitom.

Fifty-one children aged 5 to 14 years suffering from pancreatic diseases received the diet enriched with protein. The control group included 44 children of the same age with analogous pathology. These children were kept on the diet with common protein content. The dietetic treatment lasted 3 weeks. The effect of the high-protein diet on pancreatic function was evaluated from the time course of the activity of blood serum proteolytic enzymes (trypsin, trypsin inhibitor, elastase). The data obtained suggest that in case of administering the high-protein diet to children with inflammatory pancreatic diseases one should consider the stage of the inflammatory process and compensatory potentialities of the pancreas.

[A clinical and experimental study of the food and biological value of dietetic canned chicken meat for children]. / Kliniko-éksperimental'noe izuchenie pishchevoi i biologicheskoi tsennosti detskikh dieteticheskikh konservov iz miasa tsypliat.

The results are presented of clinico-experimental investigations of 4 different variants of dietetic canned chicken meat for children. The products were enriched with blood stimulants. Certain clinical and hematological parameters were used for the evaluation of the effectiveness of the canned chicken meat. The results obtained have evidenced that the products studied are tolerated well and produce a pronounced antianemic effect.

[Specialized products for nutrition of children with mucoviscidosis]. / Spetsializirovannye produkty dlia pitaniia detei, bol'nykh mukovistsidozom.

Mucoviscidosis is serious inherited disease with lesions of almost all secretory cells of mucosa and exocrine glands, following by disturbances of metabolism, digestion and nutritional status. Using of modern high effective pancreatic enzymes does not allow to reach a normalization of protein metabolism and digestion especially of fats. For success treatment of this disease the effective dietetic methods should be used including dietary intake of special food products which can supply enough energy and nutrients in small volume. The nutrient contents of a few foreign substitution of breast milk and food product for older children are given. The results of treatment of 86 children suffering from mucoviscidosis are presented. Milk proteins (casein, whey proteins), hydrolysates of milk protein and soy protein isolates are main resources of protein component in special food. Fat components are produced from middle-chain triglycerides (60-70%) and vegetable oils. Carbohydrates in special food are made from glucose polymers, glucose, fructose and sucrose. Positive results of clinical trials of foreign special food have been an example for creation of two home-produced food for treatment of children with mucoviscidosis: 1) adapted version "A" for children in age under 1 year, and 2) version "B" for older children. The products were created on basis of native milk protein with ratio casein to whey protein as 60:40. Middle-chain triglycerides compose 70-75% of fats. Carbohydrates are presented by dextrin-maltose (85%), glucose and sucrose. In present time these special products are under clinical trial in scientific medico-genetic center of Russia' Academy of Medical Science.

[DNA immobilization on modified inorganic sorbents]. / Immobilizatsiia DNK na modifitsirovannykh neorganicheskikh sorbentakh.

Fragments of denaturated DNA having the length of about 500 nucleotides were immobilized, using silica carriers modified by organic polymers. It was shown that the organic coating allows to obtain sorbents, which practically exclude non-specific sorption with respect to DNA. The immobilization was carried out on hydroxyl-containing sorbents and on their phrosphorylated derivatives by means of water-soluble carbodiimide. The resulting preparations contained up to 60 units at A260 of DNA per 1 g of carrier. The effects of endonuclease A236, exonuclease A5 and pancreatic DNAse on the immobilized RNA were studied.

[Use of spirometry and bicycle ergometry in the assessment of the stability of mitral valve bioprosthesis]. / Ispol'zovanie spiroveloérgometrii dlia otsenki stabil'nosti rezul'tatov bioprotezirovaniia mitral'nogo klapana serdtsa.

Muscular and cardiac performance was assessed a spirometry and bicycle ergometry in two groups of patients aged 25-55 with mitral xenobioprostheses functioning normally. 36 patients were examined 1-3 years following surgery (group I), 23 patients in 3.5-8 years. Functional results were found not inferior in group II. Stable parameters were more characteristic for patients having preoperative functional classes I-III.
