RESUMO

Target-controlled infusion pumps and depth of anaesthesia monitors have made total intravenous anaesthesia (TIVA) easy, safe, and precise. The merits of TIVA were highlighted during the coronavirus disease 2019 (COVID-19) pandemic, confirming its potential further in the post-COVID clinical practice as well. Ciprofol and remimazolam are newer drugs that are being tried with a hope to upgrade the practice of TIVA. While research on safe and effective drugs continues, TIVA is being practised with a combination of drugs and adjuncts to overcome the disadvantages of each and to provide complete and balanced anaesthesia with additional benefits in recovery and pain relief postoperatively. Modulation of TIVA for the special population groups is still under process. Advancement in digital technology with mobile apps has increased the scope of TIVA in day-to-day use. The formulation and update of guidelines can establish a safe and efficient practice of TIVA.

RESUMO

Extant legislation, professional and societal ethics provide fundamental guidance to professionals in practising their profession. Legal rules are enforced by courts of law whereas rules of ethics are enforced by professional bodies like the Medical Council of India (now National Medical Commission). In this article, we explore some of the principal guidelines that regulate medical ethics. The role of informed written consent is vital in safeguarding the patient as well the doctor's interests; the approach for getting consent will differ depending on patient and procedure. Medical records document every aspect of the patient's history and details of the procedure and are the only means to settle disputes as they provide legitimate, real-time and accurate details of events that have occurred in the past. Although not explicitly listed as a 'service' in the Consumer Protection Act (CPA), healthcare-related complaints can be filed under the Consumer court and the precedent has been established that it is a service. CPA of 2019 provides detailed regulations for performing scrutiny of cases, fines that can be levied, the directives for seeking compensation, etc., Every anaesthesiologist must have some knowledge of law and ethics in order to protect himself/herself from avoidable litigation or disciplinary action.

RESUMO

INTRODUCTION: An important aspect of creating a competent medical practitioner is the teaching-learning environment that shapes the mentality and instigates an urge among the students to learn. To facilitate a motivating learning setup, the faculty members need to inadvertently establish a successful and safe learning environment which is extensively feasible, given that the faculty gets to control their own style of teaching. A lot of research focuses on student's perception of the learning environment, seldom studies have reported the teacher's perspective. This present study focuses on the teacher's side of the story regarding the current scenario of the workplace that is one of the essential factors to alter the approach toward andragogy. MATERIALS AND METHODS: A cross-sectional questionnaire study was carried out using the Assessment of Medical Education Environment by Teachers (AMEET) questionnaire. The AMEET questionnaire was administered to 50 staff members at MA Rangoonwala College of Dental Sciences and Research Centre, Pune. All the participants were assured of confidentiality of their personal details and they all signed a written informed consent before participating in the study. RESULTS: Majority of the participants had a positive attitude which was seen more in the seniors compared to the juniors. The teacher's perception of teaching is that there needs to be a greater emphasis on what the student learns rather than what the teacher does. Junior staff members felt that students do not prepare and come for classes nor are the expected outcomes clear to them. Assignments are also a task for the students that are never on time. CONCLUSION: It was observed that more senior faculties were positive toward the existing education environment. In addition, even though opportunities are available for the staff as well as students for upgrading and imbibing newer information, they were not being benefitted by the same due to underutilization.
