RESUMO

Surgical treatment of vertigo is performed with in-depth study of inner ear diseases. Achieving an effective control of vertigo symptoms while reducing damage to hearing and reducing surgical complications is the principle followed by scholars studying surgical modalities. Semicircular canal occlusion is aimed at treatment of partial peripheral vertigo disease and has attracted the attention of scholars because of the above advantages. This article provides a review of the origins of semicircular canal occlusion, related basic research, clinical applications, and the effects of surgery on vestibular and hearing function.

OBJECTIVES: Carcinoma of the hypopharynx and cervical esophagus is a very aggressive cancer with a high incidence of multifocal mucosal involvement and a high incidence of submucosal lymphatic spread. Total pharyngolaryngoesophagectomy and gastric pull-up reconstruction are often the procedures of choice. The aim of this study is to review the complication after gastric pull-up reconstruction in patients with advanced hypopharyngeal or cervical esophageal cancer. MATERIALS AND METHODS: A total of 208 patients undergoing gastric pull-up reconstruction for squamous cell carcinoma of the hypopharynx invading the cervical esophagus and cervical esophagus at the Affiliated Provincial Hospital of Anhui Medical University in China from 1988 to 2007 were reviewed. Of 208 patients, 124 patients had hypopharyngeal carcinoma invading cervical esophagus; and 84 patients had cervical esophageal carcinoma. The analysis focused on the most common complications and the survival following gastric pull-up reconstruction. This study and its methods have been approved by the institutional review board. RESULTS: Of the 208 patients, 87 (41.8%) developed some complications, including anastomotic leak (19, 9.1%), pneumonitis (23, 11.1%), pleural effusion (15, 7.2%), wound infection (8, 3.9%), heart failure (4, 1.9%), anastomosis stricture (7, 3.4%), chylous fistula (4, 1.9%), hemothorax (3, 1.4%), hemoperitoneum (2, 1.0%), and burst abdomen (2, 1.0%); there was no gastric necrosis. In our cases, there was no immediate operative mortality; but there were 4 hospital deaths. The average hospital stay was 15 days. CONCLUSIONS: Gastric pull-up reconstruction is a relatively safe and effective method and can be performed with low mortality and acceptable morbidity and result in good quality of lives.

Objective To investigate the effects of phonosurgery combined with botulinumtoxinof type A in-jection in adults with vocal nodules .Methods A total of 76 adults with diagnosis of vocal nodules were studied in this experiment .They were divided into two group according to the different treatments :the surgery group (n=40) ,and the group with the combined botulinum toxin of type A injection (n=36) .Preoperative and postoperative examinations consisted of electronic laryngoscope ,stroboscopic laryngoscope ,voice assessments of subjective and objective evaluations using GRBAS classification and voice disorder index (VHI) .The evaluation parameters were the longest phonation time (MPT) ,jitter and shimmer and dysphonia severity index (DSI) .The results from the two groups were compared .Results There were 5 cases cured ,28 cases improved and 7 cases of now effects in the surgery group .The effective rate was 82 .5% .There were 30 cases cured and 6 cases improved in the combined in-jection group with the effective rate of 100% .There were 22 cases with good glottis closure and 18 cases with incom-plete closure in the surgery group postoperatively ;in the combined injection group ,30 cases glottis closed well and 6 cases still closed incompletely .The differences of postoperative VHI ,MPT ,jitter ,shimmer and DSI in addition to G were significant different between the two groups .Conclusion Phonosurgery combined with botulinum toxin of type A injection is an effective way for the treatment of vocal fold nodules in adults .

Objective@#To investigate the significance of circulating tumor cells (CTC) in squamous cell carcinoma of the head and neck (HNSCC).@*Methods@#Twenty-four patients with HNSCC treated between October 2016 and July 2017 in our department were selected (experimental group), including 23 males and 1 females, aged 47-81 years. There were 14 cases of squamous cell carcinoma of larynx and 10 cases of hypopharynx, including I-Ⅱ stage (5 cases) and Ⅲ- Ⅳ stage (19 cases). All patients were primary and/or relapsed after treatment. Nine healthy volunteers were selected as control group. A novel in vivo capture technique (CellCellector system) was used to detect CTC. SPSS23.0 was used for statistical analysis.@*Results@#The total capture rate of CTC in patients with HNSCC before treatment was 70.8% (17/24), with 40% (2/5) for patients at I-Ⅱ stage, and 78.9% (15/19) for patients at Ⅲ- Ⅳ stage, and was 0 in patients of control group. The total capture rate of CTC in patients with HNSCC after treatment was 50% (8/16). There was no significant correlation between CTC and age, sex, location of tumor or lymph node metastasis (P>0.05). CTC was related to tumor staging and tumor differentiation (P<0.05). The positive rate of EGFRVⅢ in CTC was 26.3% (5/19).@*Conclusions@#The CellCollector system is a very efficient way of detecting CTC, and CTC plays an important role in the occurrence, progression and metastasis of HNSCC.

Objective To observe the correlation between postoperative sputum properties and pharyngeal fistula in laryngeal cancer patients and summarize the nursing points. Methods Fifty laryngeal cancer patients with partial laryngectomy but no radio-therapy were enrolled in the study to record the sputum properties change,pharyngeal fistula.The relevant care measures were taken. Results On day 3 to 5 after partial laryngectomy,the sputum from the patients was stickier and more odorous,and the stench intensity was increased.1 case and another 7 ones were diagnosed as pharyngeal fistula on days 3 and 7,respectively.3 of them were healed by local open medication for 2 to 4 weeks.The other 5 cases which were not cured with routine medication for 4 to 6 weeks were treated with repair of pectoralis major myocutaneous flap and then healed on days 10 to 104 days,averaged (56.03 ±16.87)d. Conclusions The postoperative sputum properties like color,stickiness and odor can indicate the local healing condition.The relevant nursing mea-sures can promote their recovery.

OBJECTIVE To explore the dry ears and improving the auditory level of postaural bipedicle musculo-periosteal flap to obliterate the mastoid cavity. METHODS 78 cases(ears) of chronic otitis media were treated with radical mastoidectomy,open-ended tympanoplasty and conchaplasty,and received mastoid plombabge by postaural bipedicle musculo-periosteal flap.RESULTS In 78 cases(ears),the time of dry ears were reduced to 10 or 15 days and the auditory level in 58 ears was impoved(17.2?4.9) dB after treatment.CONCLUSION The surgery with postaural bipedicle musculo-periosteal flap to plombabge the mastoid cavity can achieve the epithelization in the operation cavity more quickly,achieve a short duration to dry ear shortly and improve the auditory level.
