Perspectives in scintigraphic detection of gynecologic tumors using labeled estrogen.

After having succeeded in imaging the prostate with 131I-Estracyt in our former experiments we tried to use the same method for scanning gynecological tumors. In about fifty per cent of the patients (34 cases) there was a convenient accumulation of the labeled estrogen, first of all the adenoid tumors, myomas of uteri and carcinomas can take up this radiopharmacon. Though the number of cases is rather low it is to be concluded that tumors of hormonal dependence can be scanned using estrogen hormon. Further investigations are in progress.

Gestagens in the management of prostatic carcinoma.

Two different hormonal gestagens, cyproterone acetate and norethisterone, were studied in prostatic carcinoma, for the greatest part in advanced cases. Cyproterone acetate was administered to 80, norethisterone to 38 unselected patients. The period of study extended over 2 years or more. The mechanism of action of gestagens and the role of the hormone receptors are discussed. The significance of serial measurements of plasma testosterone by providing a monitor of hormonal therapy is emphasized. The relevant literature is reviewed. The value of gestagens as an alternative to the traditional hormone therapy of prostatic carcinoma is pointed out.

A simplified surgical technique for the therapy of stress incontinence.

After a review of the literature on stress incontinence the importance of selecting the appropriate surgical procedure is emphasized. The long-term results of a modified and simplified method of retropubic urethropexy which has been used by the authors for 5 years are presented. The technique has been found equal in efficiency to other, more complicated, surgical methods.

Renal tubular acidosis: its types and role in renal calculosis.

In the light of published observations attention is called to the various forms of renal tubular acidosis (RTA), its etiology, pathologic significance, excretion of an "alkaline-persistent" urine in the presence of hyperchloremic acidosis, and to the possible consequences: nephrocalcinosis and renal calculosis. The frequency of incomplete RTA of various causes in calculosis is pointed out. The patient material of the authors and the results of phosphate therapy are presented, stressing the importance of alkalizing therapy and its prophylactic value.

In patients with urate calculi as well as in endangered persons--formation of calculi in the anamnesis, excretion of calculi or sand, urate diathesis--a regular permanent control and care is also of importance as in every other patient suffering from a chronic renal disease. It would be desirable to establish the actual frequency of urolithiasis by introducing the duty of notification. On this way a network of care units could be established within the leading regional institutions, which would further the elaboration of unitary directives for the examination and treatment of patients.
