RESUMO

The Earth's mantle is isotopically heterogeneous on length scales ranging from centimetres to more than 10(4) kilometres. This heterogeneity originates from partial melt extraction and plate tectonic recycling, whereas stirring during mantle convection tends to reduce it. Here we show that mid-ocean ridge basalts from 2,000 km along the southeast Indian ridge (SEIR) display a bimodal hafnium isotopic distribution. This bimodality reveals the presence of ancient compositional striations (streaks) in the Indian Ocean upper mantle. The number density of the streaks is described by a Poisson distribution, with an average thickness of approximately 40 km. Such a distribution is anticipated for a well-stirred upper mantle, in which heterogeneity is continually introduced by plate tectonic recycling, and redistributed by viscous stretching and convective refolding.

The reasons why the Western Mediterranean, especially Carthage and Rome, resisted monetization relative to the Eastern Mediterranean are still unclear. We address this question by combining lead (Pb) and silver (Ag) isotope abundances in silver coinage from the Aegean, Magna Graecia, Carthage and Roman Republic. The clear relationships observed between 109Ag/107Ag and 208Pb/206Pb reflect the mixing of silver ores or silver objects with Pb metal used for cupellation. The combined analysis of Ag and Pb isotopes reveals important information about the technology of smelting. The Greek world extracted Ag and Pb from associated ores, whereas, on the Iberian Peninsula, Carthaginians and Republican-era Romans applied Phoenician cupellation techniques and added exotic Pb to Pb-poor Ag ores. Massive Ag recupellation is observed in Rome during the Second Punic War. After defeating the Carthaginians and the Macedonians in the late second century bce, the Romans brought together the efficient, millennium-old techniques of silver extraction of the Phoenicians, who considered this metal a simple commodity, with the monetization of the economy introduced by the Greeks.

Modern basalts have seemingly lost all 'memory' of the primitive Earth's mantle except for an ambiguous isotopic signal observed in some rare gases. Although the Earth is expected to have reached a thermal steady state within several hundred million years of accretion, it is not known how and when the initial chemical fractionations left over from planetary accretion (and perhaps a stage involving a magma ocean) were overshadowed by fractionations imposed by modern-style geodynamics. Because of the lack of samples older than 4 Gyr, this early dynamic regime of the Earth is poorly understood. Here we compare published Hf-Nd isotope data on supracrustals from Isua, Greenland, with similar data on lunar rocks and the SNC (martian) meteorites, and show that, about 3.8 Gyr ago, the geochemical signature of the Archaean mantle was partly inherited from the initial differentiation of the Earth. The observed features seem to indicate that the planet at that time was still losing a substantial amount of primordial heat. The survival of remnants from an early layering in the modern deep mantle may account for some unexplained seismological, thermal and geochemical characteristics of the Earth as observed today.

The neodymium isotope and samarium-neodymium systematics of 2.7-billion-year-old mantle-derived magmas indicate that the lifetime of chemical heterogeneities was much shorter in the Archean mantle than in the modern mantle. Isotopic evidence is compatible with a Rayleigh number 100 times larger and convection 10 times faster in the Late Archean compared with the present-day mantle. Modern plate tectonics thus may be an improbable analog for the Archean. Chemical heterogeneities in the mantle may originate upon magma migration and mineralogical phase changes rather than by recycling of oceanic and continental crust.

We report a novel heteroplasmic T-->C mutation at nt position 582 within the mitochondrial tRNA(Phe) gene of a 70-year-old woman with mitochondrial myopathy. No other family members were affected, suggesting that our patient was a sporadic case. The muscle showed frequent ragged red fibers and 43% cytochrome c oxidase deficient fibers. The mutation alters a conserved base pairing in the aminoacyl acceptor stem. The mutation load was 70% in muscle homogenate and varied from 0 to 95% in individual muscle fiber segments. Cytochrome c oxidase-negative fibers showed significantly higher levels of mutated mtDNA (>75%) than Cytochrome c oxidase-positive fibers (<55%). This mutation adds to the previously described four pathogenic mutations in the tRNA(Phe) gene.

BACKGROUND: Several studies have explored the feasibility of using myocardial perfusion imaging to detect allograft vasculopathy after heart transplantation. We undertook the present prospective consecutive study to comparatively evaluate the role of serial myocardial perfusion single-photon emission computed tomography (SPECT) scanning and coronary arteriography (CAG) in detecting coronary artery stenosis suitable for coronary angioplasty in heart transplant recipients. METHODS: Within a 2-week interval during a follow-up period of 5.6 (95% confidence limits 2.1 to 12) years, 255 serial CAGs and myocardial perfusion scintigraphies were performed in 67 patients. Arteriography and scintigraphy were performed once yearly after heart transplantation. We retrospectively analyzed the data. RESULTS: Myocardial scintigraphy showed pathologic reversible defects in 9 out of 67 patients. Four of these patients had significant (>50% and also >70%) focal segmental stenosis in the middle and proximal parts of the coronary arteries (Type A lesions), 1 had diffuse and circumferential narrowing in the distal parts (Type B lesions), whereas CAG showed no lesions in the remaining 4 patients. The patients with significant Type A lesions were revascularized with percutaneous coronary angioplasty. Coronary arteriography showed that 1 patient had extensive Type A and Type B lesions, whereas myocardial perfusion scans detected no. The predictive value of a negative (normal) SPECT was 98% (95% confidence limits 94% to 100%) for the detection of lesions suited for revascularization. CONCLUSIONS: Annual myocardial SPECT seems well suited to screen for significant coronary artery stenosis. A SPECT study without reversible defects virtually excludes lesions suitable for coronary artery revascularization.

Odor perception plays an important role in nutrition. In the present study, the effect of aging and health status on detection of food odors is shown and interrelations with nutritional status are explored. We have tested 26 healthy young (20-25 yrs) and 23 elderly (61-74 yrs) subjects who were screened according to the SENIEUR protocol. Anthropometric measures and blood samples provided 20 parameters of nutritional status. A validated measurement procedure under forced choice conditions was used to quantify the detection thresholds of two food odors of which one had a trigeminal effect and the other mainly had an olfactory effect. There is a significant declining sensitivity for both odors. Our observations indicate that a relation between nutrition and odor perception in the elderly population exists. Whether olfactory deficits cause or are caused by increased nutritional risk deserves further study.

Methylene blue trihydrate is used widely as a dye and therapeutic agent. Methylene blue was administered by gavage to 30 animals/sex/dose group in a 0.5% aqueous methylcellulose suspension at doses of 0, 25, 50, 100, and 200 mg/kg. Blood samples from 10 animals/sex/dose group were collected at the end of study weeks 1, 6, and 13. Methylene blue treatment resulted in methemoglobin formation and oxidative damage to red blood cells, leading to a regenerative anemia and a variety of tissue and biochemical changes secondary to erythrocyte injury. An early change was a dose-related increase in methemoglobin, where the response of rats and mice was similar in magnitude. Mice appeared to be more sensitive than rats to the formation of Heinz bodies and the development of anemia that was characterized by a decrease in hemoglobin, hematocrit, and erythrocyte count. Splenomegaly was apparent in all treated mice and in the 100 mg/kg (males only) and 200 mg/kg rats at necropsy. There was a dose-related increase in absolute and relative spleen weight for both species. Microscopic examination revealed increased splenic hematopoiesis in all mice treatment groups and in rats at the 50 mg/kg dose level and above. Splenic congestion and bone marrow hyperplasia were also observed in these rat-dose groups. Mice at the higher doses showed hematopoiesis in the liver and accumulation of hemosiderin in Kupffer cells. These gross and microscopic findings are consistent with the development of hemolytic anemia. A dose-related increase in the reticulocyte count during study weeks 6 and 13 suggested a compensatory response to anemia.

Pentachlorophenol (PCP) has been used as an herbicide, algaecide, defoliant, wood preservative, germicide, fungicide, and molluscicide. A 28-day toxicity study of PCP in F344/N rats of both sexes was conducted to select dose levels for a carcinogenicity study. Groups of 10 male and 10 female rats were given 0, 200, 400, 800, 1600, or 3200 ppm PCP in feed for 28 days. The incidences of minimal to mild hepatocyte degeneration in males and females exposed to 400 ppm or greater and the incidences of centrilobular hepatocyte hypertrophy in the 3200-ppm groups were increased. For carcinogenicity studies, groups of 50 male and 50 female F344/N rats were fed diets containing 200, 400, or 600 PCP for 2 years. A stop-exposure group of 60 male and 60 female rats received 1000 ppm of PCP in feed for 52 weeks and control feed thereafter for the remainder of the 2-year studies; 10 male and 10 female rats were evaluated at 7 months. Survival of 600-ppm males was significantly greater than that of the controls; survival of all other exposed groups was similar to that of the control groups. Mean body weights of the 400- and 600-ppm groups were generally less than those of the controls throughout the studies. There was no evidence of carcinogenic activity of PCP in male or female rats fed diets containing 200, 400, or 600 ppm for 2 years. Stop-exposure study males and females regained a transitory body weight reduction by the end of the 2 year study, and males had better survival than the controls. At a 7-month interim evaluation, the incidences of centrilobular hypertrophy in stop-exposure males and females exceeded those in the controls. At 2 years, malignant mesothelioma originating from the tunica vaginalis was present in 9 1000-ppm males and 1 control male (p = 0.014). Nasal squamous cell carcinomas were present in five 1000-ppm males and 1 control male. This incidence was not significantly increased but exceeded the historical control range (0-4%). Based on the increased incidences of mesotheliomas and nasal tumors, there was some evidence of carcinogenic activity of PCP in male rats given a diet containing 1000 ppm for 1 year followed by control diet for 1 year. There was no evidence of PCP carcinogenic activity in stop-exposure female rats.

A comprehensive chronic toxicity and carcinogenicity study was conducted on a series of Aroclors (1016, 1242, 1254, and 1260). Each Aroclor was assessed at multiple dietary concentrations, ranging from 25 to 200 ppm, for 24 months in male and female Sprague-Dawley rats. Liver toxicity was indicated by elevated serum enzyme activity (AST, ALT, and GGT), elevated serum cholesterol concentration, decreases in hematologic parameters (RBC, Hb, and Hct), hepatocellular hypertrophy, an increased incidence of altered hepatocellular foci, and an increased incidence of hepatocellular neoplasms (primarily adenomas). Liver toxicity was distinctly more severe in females than in males. The incidence of hepatocellular neoplasms was highly sex-dependent (females >> males), differed between Aroclor mixtures and, for females, increased with dose and followed the general incidence pattern of Aroclor 1254 > Aroclor 1260 approximately Aroclor 1242 > Aroclor 1016. A significant response (p < 0.05) in males was seen only for the high dose of Aroclor 1260. A small increase in the incidence of thyroid gland follicular cell adenomas was noted in males for Aroclors 1242, 1254, and 1260, with the incidence being uniform across dose groups and Aroclor mixtures. For females, increased survival relative to controls was observed for all Aroclor treatment groups. A significantly decreased trend in the incidence of mammary gland neoplasms compared to control was also noted for females receiving Aroclors 1242, 1254, and 1260.

We evaluated left ventricular mass and function in 15 oarsmen aged 78 (65-82) yr (median and range) and in 15 sedentary males aged 72 (65-81) yr by 2-D and M-mode echocardiography and cycle ergometry. The weekly time spent exercising among the oarsmen was 6 (2-18) h and two of the oarsmen were former national and international champions. The two groups of subjects had similar weight, height, and resting blood pressure. The oarsmen reached a maximal work rate of 142 (117-174) vs 113 (75-150) W for the sedentary group (P < 0.01). The internal diameters of the left ventricle were not significantly different, but the septum and posterior wall thicknesses were larger in the oarsmen (11 (8-20) vs 9 (7-11) mm, and 9 (8-13) vs 8 (7-19) mm, respectively, P < 0.023). Thus, the left ventricular mass index of the oarsmen was 19% larger (127 (101-284) vs 103 (74-134) g.m-2, P < 0.01). Also, the systolic function appeared to be superior in the oarsmen as the fractional shortening was 0.45 (0.28-0.55) vs 0.36 (0.18-0.49) in the controls (P < 0.05). In conclusion, we found that long-term rowing in the senior oarsman is associated with enlarged myocardial wall thickness, a normal systolic function, and a high work capacity.

Leishmanial forms of Trypanosoma cruzi with multifocal pseudocysts were identified in a deceased adult female rhesus monkey (Macaca mulatta) that had been maintained in an outdoor primate colony in southern Texas. Subsequent clinical investigation of the remainder of the colony identified 20 additional monkeys seropositive for T cruzi. Concurrent epizootiologic surveys confirmed the presence of sylvatic reservoirs and triatomine vectors of T cruzi infection and identified a natural T cruzi transmission cycle. Veterinarians associated with the research colony were informed of the potential for further T cruzi transmission and appropriate insect vector and reservoir animal control programs were implemented..

Two dogs kenneled at a commercial establishment died of a disease manifested as severe hemorrhagic diarrhea. Histopathologic findings in both dogs bore a striking resemblance to those typical of feline infectious enteritis. Viral particles resembling members of the Parvoviridae family were observed in the contents of the small intestine of one of the dogs. The disease was similar to the recently recognized canine gastroenteritis associated with a parvovirus-like agent.

A hypothyroid patient with severe coronary disease is described. Coronary artery bypass was undertaken without preoperative treatment with thyroid hormone. Coronary artery bypass surgery may be undertaken in hypothyroid patients without significant risks. As a rule, preoperative administration of thyroid hormone should be omitted on account of the risk of aggravating the ischaemic cardiac condition. Treatment with L-thyroxin is instituted rapidly postoperatively and, in the great majority of cases, it is possible to obtain complete thyroid hormone substitution therapy after coronary revascularisation has been performed. It is emphasized that all middle-aged women with raised serum cholesterol should be screened for myxoedema even if they do not appear to be hypothyroid clinically.

Moderate hyperhomocysteinaemia is associated with hypothyroidism. Increased levels of homocysteine might contribute to a greater cardiovascular risk. This paper reports a case of transient myxoedema, owing to postpartum thyroiditis. The fluctuation of P-homocysteine was inversely correlated to the free thyroxine index. We recommend TSH screening of patients with unexplained hyperhomocysteinaemia.

RESUMO

An investigation was made of the presence of drugs and narcotics in samples from Danish drivers having violated the Danish Road Traffic act in 1993. Four hundred and twenty-five cases were received, but only 317 cases were analysed for drugs or narcotics. In 256 cases drugs or narcotics were found present with a total of 531 findings. In 112 of the cases only one compound was found present. Benzodiazepines, morphine, methadone, cannabinoids and amphetamine were the most frequently occurring compounds with 239, 52, 42, 32 and 28 findings, respectively. The study has shown that especially the benzodiazepines together with the illicit drugs seem to constitute a threat to traffic safety. For two countries very much alike, namely Denmark and Norway this study has shown a remarkable difference in the number of samples analysed and the results found.

Six heavy smokers with leucocytosis are described. Except for leucocytosis, laboratory tests were normal. A strong tobacco-induced leucocyte response indicates an increased risk of developing arterial diseases and lung diseases.

In vivo employment of monoclonal antibodies of murine origin (MoAb) with the object of immunotherapy or immunoscintigraphy may result in formation of human antimouse antibodies (HAMA) in a number of the persons exposed. HAMA is of IgG and also IgM types. Allergic reactions may occur but only few are of serious nature (less than 0.3 per thousand). In cases where MoAb is administered on repeated occasions, HAMA forms complexes with the injected which results in resuced quality of immunescintigraphic examinations, increased clearance of MoAb and reduced effect of immunetherapy. "Sandwich" type of immune analyses on blood samples which contain HAMA must be planned so that HAMA interference is eliminated.

RESUMO

Peripheral oedema which is not of cardiac origin may be an early clinical sign of hyperthyroidism. Measurement of the thyroid parameters is recommended in patients with sudden onset of oedema of the lower limbs and tachycardia without any immediate cause.
