It is estimated that circumcision concerns approximately 30% of the world male population. Most operations are done for religious or traditional reasons. In clinics, the operation is not without any risk, it has advantage as well as disadvantage. 0.6 to 2% of patients will present complications. However, the risk of contracting a sexually transmitted disease or a Human Immunodeficiency Virus infection is found considerably reduced by circumcised men. Female partners are less risky to develop HIV, Human Papilloma Virus or Chlamydia infection. Circumcision reduced the incidence of penile carcinoma. These data raise the question of the indication of prophylactic circumcision.

In prostatic cancer, PSA velocity is a reliable sign of cancer agressivity. In the metastatic prostatic cancer, there is no difference on survival between an early and late hormonal treatment. In the invasive bladder cancer néo-adjuvant chemotherapy offers a light advantage. In the kidney cancer, anti-angiogenic agents increase the survival. In the non neurogenic overactive bladder, no studies have led to relevant results in using antimuscarinic agent in the first line. The 5 phophodiesterase inhibitors used in the treatment of erectile dysfunction are also effective in the treatment of trouble induced by benign prostatic hyperplasia.

An 18-year-old man presented with a growing painless left scrotal mass. Sonography showed a hydrocele and a homogeneous, well-encapsulated left extratesticular mass with similar echogenicity as the normal testis, suggestive of a splenogonadal fusion. To substantiate the diagnosis, the patient underwent Tc-99m heat-denatured red blood cell scintigraphy showing normal physiological hyperactivity in the spleen but activity similar to the blood pool projecting on the upper part of the left testis. This made testicular splenic tissue less likely. The patient underwent resection and histopathology revealed a well-differentiated papillary mesothelioma. Inguinal orchidectomy was subsequently performed and the patient was free of recurrence at 18 months.

The systemic treatment of kidney cancers is promising regarding the first results of the inhibiting molecules of the angiogenesis. Projections in research are encouraging for more specific and sensitive markers of the prostate cancer. For this last the intermittent hormonotherapy improves the quality of life of the patients. The overweight control in infertility allows greater chances of giving birth. The morbidity of the kidney percutaneous surgery is decreased by the use of smaller tools. Reduction rate of reobstruction thanks to new manufactoring stents. The botulinic toxin for the hyperactive bladder refunded by the health insurances.

Syringocele is a poorly known and rarely diagnosed disease due to congenital or acquired dilatation of Cowper's glands that can cause urinary disorders in men. The authors report a clinical case of syringocele and review its aetiology, clinical features, diagnosis and treatment.

Lower urinary tract symptoms of benign prostatic hyperplasia, erectile dysfunction and ejaculatory disorders are often associated. The severity of sexual dysfunction is positively associated with increasing severity of urinary tract symptoms. Current treatment of BHP produce some effects, positive as well as negative, on the sexual function. It is important to remember this, when you choose a treatment for a patient who report regular sexual activity.

A fungal mass in the urinary tract (fungus ball), mainly occurring in compromised patients, is a rare and dangerous complication of candiduria. We report 2 cases of fungus ball associated with hydronephrosis and sepsis. As reported in the literature, we treated the first patient by prompt relief of obstruction by nephrostomy and local and systemic antifungal agent. The second patient failed to respond to this treatment due to a distal ureteral stenosis and required open surgery with fungus ball removal and ureteral reimplantation. Despite a large success in urinary tract drainage with antifungal treatments, some cases need a modified approach due to anatomical modification.
