RESUMO

The efficient construction of cyclopropyl spiroindoline skeletons and the exploration of related follow-up synthetic transformations have elicited considerable interest amongst members of the chemistry community. Here, we describe a formal (2 + 1) annulation and three-component (1 + 1 + 1) cascade cyclisation via sulphur ylide cyclopropanation under mild conditions. The spiro-cyclopropyl iminoindoline moiety can be readily transformed into another medicinally interesting pyrrolo[3,4-c]quinoline framework through a novel rearrangement process.

PURPOSE: Elderly patients are vulnerable to malnutrition and we have started systematic screening for preoperative malnutrition risk in our institution. This study aims to determine the prevalence and risk factors of preoperative malnutrition risk among elderly surgical patients, and its impact on surgical outcomes. METHODS: In this retrospective cohort study of patients ≥ 65 yr old undergoing elective surgery, we recorded demographics, medications, preoperative effort tolerance, Malnutrition Universal Screening Tool (MUST) score, American Society of Anesthesiologists physical status (ASA), Charlson Comorbidity Index (CCI), and Edmonton Frailty Score (EFS). Postoperative complications based on the Clavien-Dindo (CD) classification and hospital length of stay (LOS) were also recorded. RESULTS: Of the 1,033 patients studied, 123 (11.9%) were at risk of malnutrition (MUST ≥ 1), with 48 (4.6%) at high risk (MUST ≥ 2). Unadjusted predictors for high malnutrition risk included ASA ≥ III, higher EFS, higher CCI, polypharmacy (≥ ten medications), poor effort tolerance (metabolic equivalent of tasks < 3), malignancy, general surgery patients, and lower hemoglobin. These patients had higher odds of CD grade ≥ 1 complications compared with those without risk (adjusted odds ratio, 2.30; 95% confidence interval [CI], 1.11 to 4.78; P = 0.025) and 22% longer hospital LOS (adjusted incidence rate ratio,1.22; 95% CI, 1.00 to 1.49; P = 0.049) after multivariate adjustment for sex, severity of surgery, comorbidities, frailty, malignancy, and anemia. CONCLUSION: Preoperative malnutrition risk is prevalent among the elderly. Patients at high malnutrition risk have increased risk of postoperative complications and longer hospital LOS. Patients with high comorbidity burden and frailty should be screened for malnutrition so that nutritional optimization can be sought.


RéSUMé: OBJECTIF: Les patients âgés sont vulnérables à la malnutrition, c'est pourquoi nous avons commencé un dépistage systématique du risque de malnutrition préopératoire dans notre établissement. Cette étude vise à déterminer la prévalence et les facteurs de risque du risque de la malnutrition préopératoire chez les patients chirurgicaux âgés, ainsi que l'impact de la malnutrition sur les devenirs chirurgicaux. MéTHODE: Dans cette étude de cohorte rétrospective portant sur des patients âgés de ≥ 65 ans subissant une chirurgie non urgente, nous avons enregistré les données démographiques, les médicaments, la tolérance préopératoire à l'effort, le score sur l'Outil de dépistage universel de la malnutrition (MUST), le statut physique selon l'American Society of Anesthesiologists (ASA), l'indice de comorbidité de Charlson (ICC) et le score sur l'Échelle de fragilité d'Edmonton (EFS). Les complications postopératoires basées sur la classification de Clavien-Dindo (CD) et la durée de séjour à l'hôpital ont également été enregistrées. RéSULTATS: Parmi les 1033 patients étudiés, 123 (11,9 %) étaient à risque de malnutrition (MUST ≥ 1), et 48 (4,6 %) à haut risque (MUST ≥ 2). Les prédicteurs non ajustés d'un risque élevé de malnutrition comprenaient un statut ASA ≥ III, un score plus élevé à l'EFS, un ICC plus élevé, la polypharmacie (≥ dix médicaments), une tolérance faible à l'effort (équivalent métabolique des tâches < 3), la malignité, les patients de chirurgie générale, et un taux d'hémoglobine plus bas. Ces patients couraient un risque plus élevé de complications de catégorie CD ≥ 1 comparés aux patients sans risque (rapport de cotes ajusté, 2,30; intervalle de confiance [IC] 95 %, 1,11 à 4,78; P = 0,025) et des durées de séjour hospitalier 22 % plus longues (taux d'incidence ajusté,1,22; IC 95 %, 1,00 à 1,49; P = 0,049) après ajustement multivarié pour tenir compte du sexe, de la sévérité de la chirurgie, des comorbidités, de la fragilité, de la malignité et de l'anémie. CONCLUSION: Le risque de malnutrition préopératoire est répandu chez les personnes âgées. Les patients présentant un risque élevé de malnutrition courent un risque accru de complications postopératoires et d'hospitalisation prolongée. Les patients présentant un fardeau de comorbidité et de fragilité élevé devraient être dépistés pour la malnutrition afin de pouvoir optimiser leur nutrition.

Objective To develop an ideal surgical procedure for neobladder reconstruction in experimental porcine models. Methods Six experimental female pigs weighting 28-33 kg underwent transplantation of autologous peritoneum for bladder reconstruction under general anesthesia.The flaps were used to reconstruct the orthotopic neobladder by suturing with the edges of the triangle and neck of the remnant bladder.The ureteral catheters were removed on the 5 th postoperative day and the balloon catheter was removed on the 7 th postoperative day.Voiding behaviour was monitored.The animals were euthanized at week 12 for routine pathology,immunohistochemistry,and electron microscopy. Results All the pigs survived after the surgery,and no postoperative complication such as peritonitis,intestinal obstruction,or urinary fistula was observed.All the peritoneum-ileum composite free valves survived after transplantation.Voiding behaviour was normal after catheter removal,and the urine was clear.At autopsy,reconstructed bladders were healthy.Pathological examination showed the neobladder had been covered by continuous urothelium while the peritoneum disappeared and showed no ileal mucosa regrowth and residual.Scanning electron microscope showed the transitional cells of neobladder were complete and orderly,and the urothelium around suture border was continuous and showed no malposition. Conclusions Reconstruction of bladder by autologous peritoneum and ileal seromuscular flaps is an ideal approach in the experimental pigs as it can prevent regrowth of ileal epithelial cells and avoid the complications of conventional enterocystoplasty.Its clinical application deserves further investigations.

KEY MESSAGE: Six Sec14-like PITP genes from sugarcane were identified, two of them were cloned, and their biological functions were characterized indicating their involvement in plant defense against biotic and abiotic stresses. Sec14, a phosphatidylinositol transfer protein (PITP) is widely present in eukaryotes. In this study, the structure and expression patterns of six Sec14-like PITP genes (ScSEC14-1, ScSEC14p, ScSFH1, ScSFH2, ScPATL1, and ScPATL2) from sugarcane were analyzed, and two of them (ScSEC14-1 and ScSEC14p) were cloned and functionally verified. Phylogenetic analysis divided these genes into four groups, including group I (ScSFH1 and ScSFH2), group II (ScPATL1 and ScPATL2), Group III (ScSEC14p), and group V (ScSEC14-1). qRT-PCR analysis showed tissue-specific expression of these genes, primarily in the root, leaf, and bud tissues. They responded differently to SA, MeJA, and ABA stresses. ScSEC14-1, ScSEC14p, and ScSFH2 were upregulated by CuCl2 and CdCl2, while ScSEC14-1, ScSFH1, ScSFH2, and ScPATL1 were upregulated by PEG and NaCl. When infected by Sporisorium scitamineum, the transcripts of ScSFH1, ScSFH2, ScPATL1, and ScPATL2 were upregulated in the resistant genotype Yacheng 05-179, while those of ScSEC14-1 and ScSEC14p were upregulated in the susceptible genotype ROC22. Subcellular localization showed that ScSEC14-1 and ScSEC14p were mainly localized in the plasma membrane and cytoplasm. Enhanced growth of Escherichia coli BL21 cells expressing ScSEC14-1 and ScSEC14p showed high tolerance to NaCl and mannitol stresses. The transient overexpression of ScSEC14-1 and ScSEC14p in Nicotiana benthamiana leaves enhanced its resistance to the infection of tobacco pathogens Ralstonia solanacearum and Fusarium solani var. coeruleum. We can conclude the involvement of ScSEC14-1 and ScSEC14p in the defense against biotic and abiotic stresses, which should facilitate further research on Sec14-like PITP gene family, especially its regulatory mechanisms in sugarcane.

Although systematic studies have demonstrated that acupuncture or electroacupuncture (EA) analgesia is based on their accelerating endogenous opioid release to activate opioid receptors and that EA of different frequencies is mediated by different opioid receptors in specific areas of the central nervous system, there is little direct, real-time evidence to confirm this in vivo. The present study was designed to investigate the effects of transcutaneous electrical acupoint stimulation (TEAS), an analogue of EA, at low and high frequencies on µ-opioid receptor (MOR) availability in the brain of rhesus monkeys. Monkeys underwent 95-min positron emission tomography (PET) with (11) C-carfentanil three times randomly while receiving 0, 2, or 100 Hz TEAS, respectively. Each TEAS was administered in the middle 30 min during the 95-min PET scan, and each session of PET and TEAS was separated by at least 2 weeks. The results revealed that 2 Hz but not 100 Hz TEAS evoked a significant increase in MOR binding potential in the anterior cingulate cortex, the caudate nucleus, the putamen, the temporal lobe, the somatosensory cortex, and the amygdala compared with 0 Hz TEAS. The effect remained after the end of TEAS in the anterior cingulate cortex and the temporal lobe. The selective increase in MOR availability in multiple brain regions related to pain and sensory processes may play a role in mediating low-frequency TEAS efficacy.

RESUMO

The popularity of mobile devices has produced a set of image data with geographic information, time information, and text description information, which is called geotagged photo data set. The division of this kind of data by its behavior and the location not only can identify the user's important location and daily behavior, but also helps users to sort the huge image data. This paper proposes a method to build an index based on multiple classification result, which can divide the data set multiple times and distribute labels to the data to build index according to the estimated probability of classification results in order to accomplish the identification of users' important location and daily behaviors. This paper collects 1400 discrete sets of data as experimental data to verify the method proposed in this paper. The result of the experiment shows that the index and actual tagging results have a high inosculation.

OBJECTIVE: How maximal safe resection (MSR) of glioblastoma is implemented in the clinical setting remains understudied. Here, we utilized a survey-based approach to understand physician perspectives on this matter. METHODS: Scenarios involving glioblastomas were presented to physicians who were asked to select from planned sub-total resection (STR), gross total resection (GTR), medical therapy only, or palliative care. Demographic, experience, and Likert scales of value assessment were collected. RESULTS: In the scenario involving a corpus callosum glioblastoma, 2.33% opted for GTR. For a right frontal glioblastoma, 91.7% opted for GTR. In contrast, only 30.8% chose GTR of a right motor strip glioblastoma (p< 0.001). When presented with a left motor strip glioblastoma, fewer respondents (12.7%,p < 0.001) opted for GTR. Physicians who placed a high value on preserving physical independence were more likely to forgo GTR for right motor glioblastomas (HR=0.068,95% CI:0.47-0.97,p=0.035), and physicians who placed a high value on their faith were more likely to opt for surgical treatments that differ from the general consensus, for instance opting for GTR of the corpus callosum glioblastoma (HR=4.18,95%CI:1.63-10.74,p=0.003). No other associations were found between the choice for GTR and other variables collected. INTERPRETATION: Our results suggest that while maximal safe resection remains a guiding principle for glioblastoma resection, physician preference in terms of the extent of resection varies significantly as a function of tumor location and personal values.

PURPOSE: To investigate the expression profiles of IL-6 and STAT3 in Wilms' tumor (WT) and their relationship with disease progression. METHODS: Immunohistochemistry was used to examine IL-6 and STAT3 expression in 58 primary tumors and 18 invasive/metastatic ones. RESULTS: Positive expression rate of IL-6/STAT3 was 39.7% (23/58)/29.3% (17/58) in primary WT tissues, while 61.1% (11/18)/33.3% (6/18) in associated invasive/metastatic tissues. The expression rate of IL-6 and STAT3 was higher in primary WT tumors of invasive/metastatic group than that of non-invasive/metastatic group (P=0.033; P=0.012). There was a positive correlation between IL-6 and STAT3 expression in 76 WT tissues (P<0.001, r=0.444). The expression of IL-6 /STAT3 between primary WT and matched invasive/metastatic tissues was concordance (P=0.727; P=0.99). IL-6 expression status and histopathological type were associated with disease-free survival (DFS) and overall survival (OS) (P<0.05), while STAT3 was only correlated with DFS (P<0.05). CONCLUSIONS: IL-6 and STAT3 expression in WT might be correlated with progression and predict unfavorable prognosis, highlighting a new therapy target for invasive or metastatic WTs.

OBJECTIVE: To evaluate the effectiveness of dynamic SPECT (99m)Tc-galactosyl human serum albumin (GSA) scintigraphy on the assessment of reserve function of cirrhosis liver. METHODS: From January 2010 to December 2011, 55 patients with cirrhosis liver were enrolled in this study. The case numbers of male and female were 43 and 12 respectively and the age was (51 ± 9) years (ranging from 35 to 69 years). After routine biochemistry test, CT scan and (99m)Tc-GSA dynamic SPECT scan were performed in turn using a juxtaposed SPECT/CT system. Then the morphologic volume of liver parenchyma (MLV), functional liver volume (FLV) and the hepatic cell absorption rate constant (GSA-K) were calculated. The correlations between GSA-K and routine biochemistry test, Child-Pugh score, indocyanine green clearance rate (ICG-K) were analyzed. The patients were further divided into 3 groups according to whether there was occlusion or stenosis in the main branch of left portal vein (group 1, n = 5), right portal vein (group 2, n = 13) or not (group 3, n = 37) and the regional hepatic functions index of the 3 groups were compared. RESULTS: The value of FLV of the whole, left and right liver was (594 ± 152) ml, (244 ± 119) ml and (356 ± 171) ml, respectively. There were correlations between GSA-K and total bilirubin, prothrombintime, Child-Pugh score and ICG-K (r = -0.730--0.298, P < 0.05). The FLV and MLV ratios of involved hemiliver to uninvolved hemiliver were 0.09 ± 0.06 and 0.30 ± 0.14 in group 1, 0.57 ± 0.43 and 1.08 ± 0.63 in group 2, 0.71 ± 0.30 and 0.71 ± 0.48 in group 3. The difference in MLV-FLV ratio was signifcant between group 1 and group 3, between group 2 and group 3 (P = 0.000). CONCLUSIONS: The dynamic SPCECT (99m)Tc-GSA scintigraphy can not only assess the whole liver function of cirrhosis liver effectively, but also evaluate the variation of regional liver function accurately.

BACKGROUND: During early embryonic development, maternal exposure to hyperthermia induces neural tube defects (NTDs). Connexins are essential for the formation of gap junctions and Connexin43 (Cx43) is crucially involved in neural tube development. This study was designed to explore the potential role of Cx43 in NTDs induced by hyperthermia. METHODS: Using PCR, the Cx43 cDNA was screened from the cDNA library of the neural tube from golden hamsters treated with hyperthermia. By Northern blot, the expression of Cx43 in heat-treated and control groups of the golden hamsters at day 8.5 after mating was detected. Finally, by in situ hybridization and RT-PCR, the expression of Cx43 was examined in the neural tube at different time points after heat treatment. RESULTS: Cx43 was stably expressed in heat-treated and control groups of the golden hamsters, whereas the expression was evidently higher in the heat-treated group. Cx43 expression in the neural tube at different time points after heat treatment was significantly higher than in control groups (p < 0.01). Hyperthermia did not induce any mutations in Cx43 cDNA. CONCLUSIONS: Our data provide the first evidence that hyperthermia induces upregulation of Cx43 in the golden hamster neural tube. NTDs caused by hyperthermia may be intimately related with the overexpression of Cx43.

Under the condition of different pH (7-11) and different ethanol volume fraction (45% to 85%), the ultraviolet-visible absorption spectra of malachite green were studied in neutral and alkaline ethanol solution, the maximum absorption wavelength at 620 nm was found, and the matching degree of standard curve was better established. In low power trench-type ultrasound apparatus, the absorption of the malachite green solution was measured under ultrasound and non-ultrasound, respectively. the difference values of the ultraviolet absorption of the malachite green solution under low power trench-type ultrasound were measured results of the hydroxyl free radical oxidation degrading malachite green, therefore hydroxyl free radical from low power trench-type ultrasound was determined indirectly. Then the contents of hydroxyl free radical in four conditions were measured. The detection limit of the method of 8.4 x 10(-6) mmol x L(-1) and the relative standard deviation of the method of 9.4 x 10(-5) - 3.7 x 10(-4) mmol x L(-1) were determined, a higher testing precision and good reproducibility were confirmed. It can be applied for fast detection of neutral and alkaline ethanol solution system in the case of very low concentration of hydroxyl free radicals. Since malachite green is heat sensitive, so compared to measuring temperature, the method possessed better functions for thermal effects of ultrasound.

The Suzuki-Miyaura cross-coupling of C(sp3)-hybridised boronic compounds still remains a challenging task, thereby hindering the broad application of alkyl boron substrates in carbon-carbon bond-forming reactions. Herein, we developed an NHC/photoredox dual catalytic cross-coupling of alkyl trifluoroborates with acid fluorides, providing an alternative solution to the classical acylative Suzuki coupling chemistry. With this protocol, various ketones could be rapidly synthesised from readily available materials under mild conditions. Preliminary mechanistic studies shed light on the unique radical reaction mechanism.

Paramesotriton aurantius (Caudata: Salamandridae) is a new species that found in southeastern China. Its complete mitochondrial genome (mitogenome) sequence was 16,313 bp in length with with A + T contents of 60.9%, and contained 13 protein-coding genes, 22 tRNA genes, 2 rRNA genes, one control region (D-loop), and one non-coding region. Our molecular tree showed that P. aurantius was positioned near P. hongkongensis, and formed a clade with other Paramesotriton species. The first complete mitogenome sequence of P. aurantius could provided fundamental data for resolving phylogenetic and genetic problems related to genus Paramesotriton.

Chitosan is a popular biomaterial used in tissue engineering. Fibers of chitosan could provide a favorable anatomical substrate for cell growth which provides a promising application for axonal regeneration during peripheral injury. Neuroepithelial stem cells (NEPs) are the most primitive neural stem cells with multipotential for neuronal and glia differentiation. To assess the biocompatibility between NEPs and chitosan fibers, and to explore whether the NEPs have the ability to differentiation on chitosan fibers, NEPs were harvested from the neural tube and seeded on chitosan fibers in in vitro culture. The biocompatibility of chitosan fibers was tested by MTT assays. The growth and survival were observed by light and scanning electronic microscope at different times in culture. And, the differentiation of NEPs was examined by immunocytochemical staining. The results indicated that NEPs could grow on the chitosan fibers and attach firmly to the surface of fibers. On chitosan fibers, NEPs could differentiate into neurons and glia. Our study demonstrated that chitasan fibers had a good biocompatibility with NEPs which affords a potential alternative for the repair of peripheral nerve injury.

OBJECTIVE: To simulate and assess the clinical effect of intracoronary infusion of bone marrow mononuclear cells or peripheral endothelial progenitor cells on myocardial reperfusion injury in mini-swine model. METHODS: Twenty-three mini-swine with myocardial reperfusion injury were used as designed in the study protocol. About (3.54 +/- 0.90) x 10(8) bone marrow mononuclear cells (MNC group, n = 9) or (1.16 +/- 1.07) x 10(7) endothelial progenitor cells (EPC group, n = 7) was infused into the affected coronary segment of the swine. The other mini-swine were infused with phosphate buffered saline as control (n = 7). Echocardiography and hemodynamic studies were performed before and 4 weeks after cell infusion. Myocardium infarction size was calculated. Stem cell differentiation was analyzed under a transmission electromicroscope. RESULTS: Left ventricular ejection fraction dropped by 0% in EPC group, 2% in MNC group, and 10% in the control group 4 weeks after cell infusion, respectively (P < 0.05). The systolic parameters increased in MNC and EPC groups but decreased in the control group. However, the diastolic parameters demonstrated no significant change in the three groups (P > 0.05). EPC decreased total infarction size more than MNC did (1.60 +/- 0.26 cm2 vs. 3.71 +/- 1.38 cm2, P < 0.05). Undermature endothelial cells and myocytes were found under transmission electromicroscope. CONCLUSIONS: Transplantation of either MNC or EPC may be beneficial to cardiac systolic function, but might not has obvious effect on diastolic function. Intracoronary infusion of EPC might be better than MNC in controlling infarction size. Both MNC and EPC may stimulate angiogenesis, inhibit fibrogenesis, and differentiate into myocardial cells.

RESUMO

Efficient construction of medium-sized lactones has attracted considerable interest over several decades, but remains a formidable challenge in synthetic chemistry. Here, we describe an unprecedented palladium-catalysed regioselective [5 + n] cyclisation (n = 5, 6, and 7) between vinylethylene carbonates and various anhydrides. Catalytic transformation occurs under mild, room-temperature conditions and offers an exceptional substrate scope. A broad spectrum of medium-sized bislactones with skeletal diversity can be obtained easily.

OBJECTIVE: To study the relationship between inflammation and neointimal proliferation after coronary stent implantation in porcine model. METHODS: Twenty normal minipigs were randomly divided into group A (implanted with 316L bare metal stents), group B (implanted with 605L bare metal stents), group C (implanted with PLGA coating 605L stents), and group D (implanted with rapamycin-loaded PLGA coating 605L stents). Each minipig was implanted with two same stents in left anterior descending artery and right coronary artery. Four weeks later, the animals were sacrificed and histomorphometric measurements on the stent-segment coronary arteries were made to calculate the correlation between inflammation area and neointimal area. RESULTS: Group D had the smallest neointimal area [(0.64 +/- 0.38) mm2, P < 0. 001] and inflammation area (median 0.00 mm2, P = 0.009) among all the groups, while there were no statistical differences among group A, B, and C in neointimal area [(2.09 +/- 0.90), (2.11 +/- 1.07), and (1.42 +/- 0.35) mm2 respectively] and in inflammation area (0.22 , 0.21, and 0.09 mm2, respectively). Bivariate correlation analysis showed that the inflammation area was positively correlated with the neointimal area (P < 0.001, correlation coefficient = 0.719). When stent type, mean injury score, and EEL area were adjusted, partial correlations analysis showed that the inflammation area was still positively correlated with the neointimal area (P = 0.01, correlation coefficient = 0.498). CONCLUSION: Inflammation promotes the neointimal proliferation after coronary stent implantation. Sirolimus-eluting stent may reduce the inflammatory response.

The aim of this investigation was to evaluate the clinical significance of the various optional surgical approaches for giant and irregular pituitary adenomas and to summarize the optimal surgical protocols for the adenomas in terms of different growth morphologies. Fifty-four cases with giant and irregular pituitary adenomas were treated by studying their clinical features and image examinations, designing the specific surgical protocols, and choosing the optimal approaches according to the various growth morphologies. Neuro-endoscope and neuronavigation-assisted techniques were applied intraoperatively. Postoperative MRI and endocrine function were re-examined routinely to judge the therapeutic efficacy of various single approaches, combined approaches and staged operations. Application of the six protocols resulted in total removal of the tumours in 18 cases, subtotal removal in 28 cases, partial removal in five cases and three deaths. The most appropriate surgical approaches, which were adopted after comprehensive analyses of the morphological characteristics presented in image examinations, those involving anatomical spaces and the clinical symptoms, can achieve the improved therapeutic results and reduce injuries to the vital anatomic structures. The tumour removal rate can be increased with the help of neuro-endoscope, neuronavigation techniques and intraoperative MRI.

Neuroepithelial stem cells (NEPs) demonstrate a high potential for self-renewal and differentiation during embryonic development. To explore the survival and differentiation of NEPs in vivo, we isolated NEPs from green fluorescence protein (GFP) transgenic embryos and transplanted into the lateral ventricle of rats. In vitro culture, NEPs proliferated into neurospheres and differentiated into both neurons and glia. When transplanted into the lateral ventricle of rats, these GFP positive NEPs (GFP+ NEPs) survived and attached to the wall of ventricle. Moreover, grafted cells differentiated into neuron-specific enolase (NSE) positive neurons and glial fibrillary acidic protein (GFAP) positive astrocytes and migrated into the host brain. Thus, our results indicate that NEPs can survive and differentiate into neurons and astrocytes in the lateral ventricle following transplantation.

OBJECTIVE: To investigate the therapeutic effectiveness of intracoronary implantation of autologous bone marrow mononuclear cells (BM-MNC) in miniswine model of reperfused myocardial infarction. METHODS: Sixteen miniswine myocardial ischemic reperfusion injury models made by ligation of the distal one third segment of left anterior descending artery for 90 minutes were randomized into 2 groups. In BM-MNC group (n = 9), (3.54 +/- 0.90) X 10(8) BM-MNC were intracoronary injected, and in the control group (n = 7), phosphate buffered saline was injected by the same way. Echocardiographic and hemodynamic results, vessel density, and myocardial infarction size were evaluated and compared before and 4 weeks after cell transplantation. RESULTS: In BM-MNC group, there were no differences between before and 4 weeks after transplantation in aspects of left ventricular ejection fraction (LVEF), interventricular septal thickness, left ventricular lateral and anterior septal wall thickness, cardiac output, or +dp/dtmax. In control group, LVEF, interventricular septal thickness, left ventricular lateral and anterior septal wall thickness, cardiac output, and +dp/dtmax decreased significantly 4 weeks after transplantation (P < 0.05). Left ventricular end-diastolic pressure and -dp/dtmax, did not change significantly before and after cell transplantation in both groups. Capillary density in BM-MNC group was greater than that in control group [(13.39 +/- 6.96)/high power field vs. (3.50 +/- 1.90)/high power field, P < 0.05]. Infarction area assessed by tetrazolium red staining and the infarction percentage decreased in BM-MNC group compared with those in control group (P < 0.05). CONCLUSIONS: Transplantation of BM-MNC into myocardium with ischemic reperfusion injury increases capillary density and decreases infarction area. It has significantly beneficial effect on cardiac systolic function rather than on diastolic function.
