PURPOSE: To characterize changes in the functioning aspects, in the perception of children and adolescents with speech and language disorders under speech-language follow-up, using the ICF. METHODS: Descriptive, analytical and longitudinal research, with a qualitative and quantitative approach, whose sample consisted of 60 children and adolescents: 30 with speech and language disorders and 30 with typical speech and language development. Data collection was carried out in two moments: beginning of the research and six months later. A semi-structured questionnaire was administered to the participants, and a medical records analysis was performed. From these data, functioning was classified using he ICF categories. The Wilcoxon test and thematic content analysis were used to compare the interviews. RESULTS: The use of ICF allowed characterizing changes resulting from speech-language follow-up. Participants with speech and language disorders presented a decrease in the magnitude of the qualifiers in the categories: articulation and fluency, social relationships, daily activities, engagement in play, people's attitude barriers, and how to handle stress. CONCLUSION: The findings show changes in components of Body Functions, Activities and Participation, and the influence of Environmental Factors after speech-language follow-up, in the perception of the studied group, which brings relevant subsidies for a greater understanding of functioning and therapeutic intervention. The use of the ICF enabled the longitudinal analysis in a biopsychosocial approach, contemplating, in addition to biological aspects, the social impact of speech and language disorders in the lives of these children and adolescents.

PURPOSE: To investigate participation, and functionality of children and adolescents with speech disorders using the International Classification of Functioning, Disability and Health (ICF). METHOD: Descriptive and analytical research of qualitative and quantitative approach. The sample was composed of 30 participants with speech disorders and 30 with typical speech development. For data collection, it was conducted semi-structured interviews with the participants, medical record review, observation of speech aspects' participants, and analyzed study of medical records. The collected data were used to qualify the framework codes. The Mann-Whitney Test was used for comparison between groups, and thematic content analysis for the interviews. RESULTS: Participants with speech disorders reported more difficulties than the participants with typical speech development. Speech disorders significantly impacted on the Body Functions (articulation and fluency), Activities and Participation (conversation, relationships, carrying out the routine and handling of stress) and Environmental Factors (attitudes of family, friends and acquaintances). CONCLUSION: The results show the impact of speech disorders and their social consequences for these children and adolescents. The ICF allowed us to comprehend health in all its complexity and integrality, making possible to plan strategies to soften the disorders impact in an individual and collective perspective. Then, ICF can be used, in the future, for elaboration of public policies and actions that will improve the quality of life and promote the health of this population.

OBJECTIVE: To identify risk factors for insufficient birth weight, in the Brazilian city of Campinas from the Live Birth Certificates. METHODS: A cross-sectional study was conducted consulting data from 14,444 Live Birth Certificates from the year 2001. These newborn birth weights ranged from 285g to 5,890g (mean 3,142g), 65.1% of them weighted 3,000g or more, while 25.7% had insufficient weight. A sample of 12,779 term newborn from single pregnancies, with a birth weight > 2,500g was selected. Birth weight was the dependent variable and maternal, pregnancy, delivery and newborn characteristics were the independent variables. Association among variables was assessed by Chi-square test, crude odds-ratio (OR) was calculated. For adjusted odds-ratio (adjOR) values, a logistic regression model was used. RESULTS: Results showed association between insufficient birth weight and newborn of the female gender (adjOR = 1.57 [95%CI 1.44 - 1.70]), birth from primiparous mothers (adjOR = 1.47 [95%CI 1.35 - 1.60]), inadequate prenatal care (adjOR = 1.42 [95%CI 1.29 - 1.58]) and vaginal delivery (adjOR = 1.23 [95%CI 1.13 - 1.34]). CONCLUSION: The risk for insufficient birth weight was higher for female newborn, offspring of primiparous women, of women who had inadequate prenatal care and had vaginal delivery.

OBJECTIVE: To understand the process of resilience (social support and resources of the family environment) and the chance of mental health problems in children and adolescents (9-16 years) who have been victims of domestic violence, assisted in specialized services (Group 1 - G1) and in school services without reports of domestic violence (Group 2 - G2). METHODS: Various semi-structured instruments were applied to the pairs (guardian and child or adolescent): the Strengths and Difficulties Questionnaire (SDQ); the Resiliency Scales for Children and Adolescents (RSCA), including Scale I (SI - sense of control), Scale II (SII - relationship skills) and Scale III (SIII - emotional reactivity); the Social Support Appraisals; the Home Environment Resources Scale and a questionnaire created by the authors to characterize the population. RESULTS: There was no difference in the prevalence of resilience between G1 and G2. Children and adolescents of both groups had a higher chance of low resilience in the absence of perception of social support from the teacher (SI; SIII) and other people in the community (SI; SII). Girls had higher chance of low resilience (SIII). The establishment of routine or rules in the lives of the children and adolescents facilitated the development of resilience (SIII). In G1, the prevalence of mental health problems was 65% for the self-application version of the SDQ for children and adolescents (SDQ/CA) and 54% for the version answered by the guardians (SDQ/G). In G2, it was 33% for SDQ/CA and 37.9% for SDQ/G. Domestic violence against children and adolescents was a risk factor for the development of mental disorders (SDQ/G). Subjects with low resilience (SI) had a higher chance of developing mental health problems (SDQ/CA). Despite originating from the same regions, the groups had socioeconomic differences, which showed no relationship with resilience. CONCLUSIONS: The quality and perception of social support and resources present in the home environment may have facilitated the development of resilience in the studied children and adolescents. Violence may have increased the chance of mental health problems, domestic violence being an aggravating factor. There is need for research on aspects that predict resilience and investment in intervention strategies for this population, as a way to promote mental health.

RESUMO Objetivo Caracterizar mudanças nos aspectos de funcionalidade, segundo a percepção de crianças e adolescentes com alterações de fala e linguagem em acompanhamento fonoaudiológico, utilizando a Classificação Internacional de Funcionalidade, Incapacidade e Saúde (CIF). Método Pesquisa de delineamento descritivo-analítico, longitudinal e de abordagem qualitativa-quantitativa, cuja amostra se constituiu de 60 crianças e adolescentes: 30 com alteração de fala e linguagem e 30 com desenvolvimento típico de fala. Coleta foi realizada em dois momentos: início da pesquisa e seis meses depois. Foi realizada entrevista com questionário semiestruturado com os participantes, e análise de prontuário. A partir desses dados, a funcionalidade foi caracterizada com as categorias da CIF. Para comparação entre as entrevistas utilizou-se o Teste Wilcoxon e análise de conteúdo temática. Resultados O uso da CIF possibilitou caracterizar mudanças ocorridas com o acompanhamento fonoaudiológico. Os participantes com alterações de fala e linguagem apresentaram diminuição na magnitude dos qualificadores nas categorias de funções de articulação e fluência, relacionamentos sociais, atividades do cotidiano, envolvimento no brincar, atitudes barreiras das pessoas, modo de lidar com o estresse. Conclusão Os achados mostram mudanças nos componentes de Funções do Corpo, Atividades e Participação e a influência dos Fatores Ambientais após o acompanhamento fonoaudiológico na percepção do grupo estudado, o que traz subsídios relevantes quanto à funcionalidade para maior compreensão e intervenção terapêutica. A utilização da CIF possibilitou estudo e análise longitudinal em uma abordagem biopsicossocial, contemplando além dos aspectos biológicos, o impacto social das alterações de fala e linguagem na vida dessas crianças e adolescentes.

PURPOSE: To investigate the perception of family members regarding linguistic conditions and social participation of children and adolescents with speech and language impairments using the International Classification of Functioning, Disability and Health - Children and Youth Version (ICF-CY). METHODS: Quali-quantitative approach research, in which a survey of medical records of 24 children/adolescents undergoing speech-language therapy and interviews with their family members was conducted. A descriptive analysis of the participants' profiles was performed, followed by a categorization of responses using the ICF-CY. RESULTS: All family members mentioned various aspects of speech/language categorized by the ICF-CY. Initially, they approached it as an organic issue, categorized under the component of Body Functions and Structures. Most reported different repercussions of the speech-language impairments on the domains, such as dealing with stress and speaking, qualified from mild to severe. Participants reported Environmental Factors categorized as facilitators in the immediate family's attitudes and as barriers in the social attitudes. CONCLUSION: These findings, according to the use of the ICF-CY, demonstrate that the children/adolescents' speech-language impairments, from the families' perception, are primarily understood in the body dimension. However, guided by a broader approach to health, the findings in the Activities and Participation and Environmental Factors demonstrate a broader understanding of the participants of the speech-language impairments. The results corroborate the importance of using the ICF-CY as a health care analysis tool, by incorporating functionality and participation aspects and providing subsidies for the construction of unique therapeutic projects in a broader approach to the health of the group studied.

RESUMO Objetivo: Investigar a participação e funcionalidade de crianças e adolescentes com alterações de fala utilizando a Classificação Internacional de Funcionalidade, Incapacidade e Saúde. Método: Pesquisa descritiva-analítica de abordagem qualitativa-quantitativa com 30 participantes com alteração de fala (CAF) e 30 com desenvolvimento típico de fala (DTF). Para a coleta de dados, realizaram-se entrevistas semiestruturadas com os participantes, estudo de prontuário, observação dos participantes quanto aos aspectos de fala e aplicação de questionário estruturado com os responsáveis. Os dados coletados foram utilizados para seleção de qualificadores das categorias da Classificação Internacional de Funcionalidade, Incapacidade e Saúde. Para comparação dos qualificadores entre grupos, utilizou-se o Teste Mann-Whitney, e a análise de conteúdo temática, para as entrevistas. Resultados: Os participantes do grupo CAF relataram maior grau de dificuldade do que o grupo DTF. As alterações de fala repercutiram significativamente em Funções do Corpo (articulação e fluência), Atividades e Participação (conversa, relacionamentos, realização de rotinas e lidar com estresse) e Fatores Ambientais (atitudes de familiares, amigos e conhecidos). Conclusão: Os resultados evidenciam o impacto das alterações de fala e suas implicações sociais para estas crianças e adolescentes. A CIF permitiu compreender a saúde em complexidade e integralidade, possibilitando assim o planejamento de estratégias para amenizar o impacto dessas alterações individual e coletivamente. Podendo ser base, futuramente, para a criação de políticas públicas e ações que poderão melhorar a qualidade de vida e promover a saúde dessa população.

ABSTRACT Purpose: to discuss the implementation and use of the International Classification of Functioning, Disability and Health (ICF) and International Classification of Functioning, Disability and Health, Children and Youth Version (ICF-CY) among children and adolescents, within the Augmentative and Alternative Communication (AAC) field. Methods: an integrative literature review. PubMed, Web of Science and VHL databases were searched for papers published between 2006 and 2017 that reported on the use of ICF and ICF-CY within the AAC context. Eighteen papers were reviewed and sorted into: Category i) papers which reported on the use of the ICF or ICF-CY with people who rely on AAC; and Category ii) theoretical papers or papers that used the ICF and ICF-CY to organize the results. Results: papers used the frameworks with different purposes, including the characterization of the children and their environment, goal setting and measurement of the results of therapeutic intervention. The papers drew on all elements, however, Activities and Participation were the components most used. Parents or caregivers were most commonly consulted in classifying the children's and young people's profiles of functioning, followed by the Educators and speech and language pathologists. Conclusion: classifications have shown advantages when used in the AAC field. Therefore, it is necessary to provide training in order for professionals to implement them in services.

UNLABELLED: Body mass index (BMI) has been considered a criterion to define and analyse obesity in adults and children. BACKGROUND: the purpose of this study was to evaluate the correlation between BMI and triceps skinfold (TSF). METHODS: there were studied 4,236 children (3.1-10.9y); 48.3%M:51.6%F, from four studies made in Paul nia, SP-Brazil. Height, weight and TSF (Holtain caliper) were measured. For each children BMI was calculated and transformed in SDS, according to North American data (Frisancho, 1993). Multiple linear regression analysis (stepwise) was used for the whole population and in three groups according to BMI: A) SDS < or = -1.0; B) -1.0 < SDS< 1.0) SDS(3) 1.0. Data were processed with SPSS software. RESULTS: in group A, the TSF (7.8 +/- 2.3) variability was lower when compared with the groups B (10.1 +/- 4.0) and C (17.8 +/- 6.2). In multiple linear regression with the whole population, R = 0.478 for TSF. In groups B and C, R = 0.364 and 0.368 respectively for TSF, and in group A it was only 0.032. CONCLUSIONS: these observations demonstrated a height correlation between BMI and TSF in children with obesity risk (group C). Therefore, we conclude that, in Brazil BMI can be used for children's research of obesity in population studies, instead of TSF.

RESUMO OBJETIVO: Compreender o processo de resiliência (suporte social e recursos do ambiente familiar) e a chance de problemas de saúde mental em crianças e adolescentes (9-16 anos) vítimas de violência doméstica acompanhados em serviços especializados (Grupo 1 - G1) e em escolares sem relatos de situações de violência doméstica (Grupo 2 - G2). MÉTODOS: Diversos instrumentos semiestruturados foram aplicados às díades (responsável e criança ou adolescente): Strengths and Difficulties Questionnaire (SDQ); Resiliency Scales for Children and Adolescents (RSCA), incluindo a Escala I (EI - sentido do controle), a Escala II (EII - capacidade de relacionamento) e a Escala III (EIII - reatividade emocional); Social Support Appraisals; Inventário de Recursos no Ambiente Familiar e um questionário elaborado pelos autores para caracterizar a população. RESULTADOS: Não houve diferença na prevalência de resiliência entre G1 e G2. As crianças e adolescentes de ambos os grupos tiveram maior chance de baixa resiliência na ausência de percepção do suporte social do professor (EI; EIII) e de outras pessoas da comunidade (EI; EII). Meninas apresentaram maior chance de baixa resiliência (EIII). O estabelecimento de rotina ou regras na vida das crianças e adolescentes facilitou o desenvolvimento da resiliência (EIII). No G1 a prevalência de problemas de saúde mental foi de 65% pela versão de autoaplicação do SDQ para crianças e adolescentes (SDQ/CA) e de 54% pela versão respondida pelos responsáveis (SDQ/R). No G2 foi de 33% pelo SDQ/CA e de 37,9% pelo SDQ/R. A violência doméstica infanto-juvenil foi fator de risco para o desenvolvimento de transtornos mentais (SDQ/R). Os sujeitos com baixa resiliência (EI) apresentaram maior chance de problemas de saúde mental (SDQ/CA). Embora provenientes das mesmas regiões, os grupos apresentaram diferenças socioeconômicas, as quais não apresentaram relação com a resiliência. CONCLUSÕES: A qualidade e percepção do suporte social e de recursos presentes no ambiente familiar podem ter facilitado o desenvolvimento da resiliência nas crianças e adolescentes estudados. A violência pode ter aumentado a chance de problemas de saúde mental, sendo a violência doméstica um agravante. Há necessidade de pesquisas sobre os aspectos preditores de resiliência e de investimento em estratégias de intervenção para esta população, como forma de promover a saúde mental.

Objetivo: compreender o comportamento materno no cuidado à criança com asma. Método: estudo qualitativo, orientado pelo referencial do método etnográfico, realizado no período de julho de 2013 a janeiro de 2014. Participaram 12 mães de crianças com asma grave, de 6 a 7 anos de idade, selecionadas por equipes da estratégia saúde da família do município de Teresina-PI. O projeto de pesquisa foi aprovado por Comitê de Ética em Pesquisa. Resultados: as categorias extraídas dos discursos foram: a mãe diante do diagnóstico de asma infantil; a mãe e a crise de asma infantil; a mãe e o serviço de saúde do bairro. Conclusão: identificou-se que as mães apresentaram reduzido conhecimento sobre manejo adequado da doença e demonstraram insatisfação em relação ao serviço de saúde do bairro. Evidenciou-se a necessidade do desenvolvimento de atividades educativas para a redução da morbidade da asma na população estudada.

Results: the categories extracted from the accounts were: the mother and the child's diagnosis of asthma; the mother and the child's asthma crisis; the mother and the neighborhood health service. Conclusion: mothers were found to know little about proper management of the disease and displayed dissatisfaction with the neighborhood health service. There emerged a need for educational activities to reduce asthma morbidity in the study population.


Objetivo: comprender el comportamiento materno en el cuidado del niño con asma. Método: estudio cualitativo, orientado por el referencial del método etnográfico, realizado en el período de julio de 2013 a enero de 2014. Participaron 12 madres de niños con asma grave, de 6 a 7 años de edad, seleccionadas por equipos de la estrategia salud de la familia del municipio de Teresina-PI. El proyecto de investigación fue aprobado por Comité de Ética en Investigación. Resultados: las categorías extraídas de los discursos fueron: la madre ante el diagnóstico de asma del niño; la madre y la crisis de asma del niño; la madre y el servicio de salud del barrio. Conclusión: se identificó que las madres tenían poco conocimiento sobre el cuidado adecuado de la enfermedad y demostraron insatisfacción en cuanto al servicio de salud del barrio. Quedó evidente la necesidad del desarrollo de actividades educativas para la reducción de la morbilidad del asma en la población estudiada.

RESUMO Conclusão As diretrizes curriculares propõem um perfil desejado para o médico formado, adequando o currículo às necessidades da população, com abordagem integral do indivíduo. Os profissionais da área da saúde frequentemente participam da integração ensino-serviço como preceptores. O curso de Medicina da Universidade Estadual de Campinas, em 2001, reformulou seu currículo para uma abordagem ampliada de cenários de aprendizagem, levando seus estudantes também para o ambiente em que residem seus clientes e trabalhando com a perspectiva de promover saúde. Em 2004, teve início a disciplina Atenção Integral à Saúde, em parceria com a Prefeitura Municipal de Campinas (SP), para que médicos das áreas de Pediatria, Clínica Médica e Ginecologia que atuam em Unidades Básicas de Saúde (UBS) utilizassem quatro horas de sua carga horária para supervisão de estudantes no sétimo e oitavo semestres do curso de Medicina, em conjunto com docentes da universidade. O objetivo deste estudo foi avaliar a satisfação do cliente atendido na UBS por médicos que atuam como preceptores e conhecer se estes profissionais relacionam a preceptoria com sua qualificação. De 22 preceptores, 20 aceitaram responder a uma entrevista semiestruturada sobre como se atualizam para a supervisão dos estudantes do quarto ano de Medicina da Unicamp e se, por estarem vinculados à universidade, passam por educação médica permanente, refletindo-se em um atendimento integral a seus clientes, sendo a preceptoria um instrumento para qualificar sua atuação profissional. Seus clientes foram entrevistados com um questionário fechado utilizando Smilles (emotions) para classificar o preceptor nos itens: qualidade do atendimento médico, valorização da queixa, tempo da consulta, resolutividade da consulta, confiança e aderência à orientação, e percepção geral sobre o atendimento médico. Mais de 90% dos clientes atendidos pelos preceptores apresentam satisfação quanto à assistência recebida e 80% dos médicos entrevistados relacionam a preceptoria como um instrumento para qualificar sua atividade profissional.


ABSTRACT Conclusion Medical curricula guidelines outline the desired profile for graduate physicians, adapting to the needs of the population by means of an integrated approach to the individual. Healthcare professionals often participate in initiatives integrating teaching and service as preceptors. In 2001, Unicamp Medical School reformulated its curriculum to adopt an extended approach to learning scenarios, taking their students into the environment in which their users reside and working in the aim of promoting health. In 2004, the discipline entitled “Comprehensive Healthcare” was introduced in partnership with the municipality of Campinas (SP), in order that physicians in the field of pediatrics, general practice, and gynecology working in basic healthcare units (BHU) could use four of their scheduled hours to supervise students in the seventh and eighth semesters of medical school completing preceptorships, in a joint initiative with university professors. The objective of this study was to assess the satisfaction of users served in BHUs by physicians acting as preceptors and to determine if the professionals linked the preceptorship to their qualifying as professionals. Of a total of 22 preceptors, 20 submitted to a semi-structured interview, with one of the questions asking if the preceptorship helped conceptualize the service offered to the users in the BHUs in which they were acting as physicians. Users were interviewed by means of a close-ended questionnaire using emoticons to classify the preceptor on factors including quality of medical care, appreciation of the complaint made, duration of the consultation, resolution rate, trust and adherence to orientation, and general perception of the care received. Over 90% of users rated the care positively, with 80% of the physicians interviewed regarding the preceptorship as contributing to their qualifying as professionals.

RESUMO Objetivo Investigar a percepção de familiares acerca das condições linguísticas e da participação social de crianças e adolescentes com alterações de fala/linguagem utilizando a Classificação Internacional de Funcionalidade, Incapacidade e Saúde – Versão Crianças e Jovens (CIF-CJ). Método Pesquisa de abordagem quali-quantitativa, na qual se realizou levantamento dos prontuários de 24 crianças/adolescentes, em acompanhamento fonoaudiológico, e entrevistas com seus familiares. Foi feita análise descritiva dos perfis dos participantes e categorização das respostas, utilizando a CIF-CJ. Resultados Todos os familiares abordaram diversos aspectos de fala/linguagem categorizados pela CIF-CJ. Inicialmente, trataram-nos como um problema orgânico, categorizado no componente de Funções e Estruturas do Corpo. A maioria referiu diferentes repercussões das alterações de fala/linguagem em domínios, como, lidar com estresse e atividade do falar, qualificados de leve a grave. Os participantes relataram Fatores Ambientais categorizados como facilitadores, em atitudes da família imediata, e como barreiras, em atitudes sociais. Conclusão Os achados, utilizando-se a CIF-CJ, evidenciam que as alterações de fala/linguagem das crianças e adolescentes, na percepção dos familiares, são compreendidas, inicialmente, na dimensão do corpo. Contudo, pautados numa abordagem mais ampla de saúde, os achados, em Atividades e Participação e Fatores Ambientais, demonstram ampliação do olhar dos participantes quanto às alterações de fala e linguagem. Os resultados reiteram a importância do uso da CIF-CJ como instrumento de análise na atenção à saúde, ao incorporar aspectos de funcionalidade e participação, proporcionando subsídios para a construção de projetos terapêuticos singulares, em uma abordagem mais ampla de saúde do grupo estudado.


ABSTRACT Purpose To investigate the perception of family members regarding linguistic conditions and social participation of children and adolescents with speech and language impairments using the International Classification of Functioning, Disability and Health - Children and Youth Version (ICF-CY). Methods Quali-quantitative approach research, in which a survey of medical records of 24 children/adolescents undergoing speech-language therapy and interviews with their family members was conducted. A descriptive analysis of the participants’ profiles was performed, followed by a categorization of responses using the ICF-CY. Results All family members mentioned various aspects of speech/language categorized by the ICF-CY. Initially, they approached it as an organic issue, categorized under the component of Body Functions and Structures. Most reported different repercussions of the speech-language impairments on the domains, such as dealing with stress and speaking, qualified from mild to severe. Participants reported Environmental Factors categorized as facilitators in the immediate family’s attitudes and as barriers in the social attitudes. Conclusion These findings, according to the use of the ICF-CY, demonstrate that the children/adolescents’ speech-language impairments, from the families’ perception, are primarily understood in the body dimension. However, guided by a broader approach to health, the findings in the Activities and Participation and Environmental Factors demonstrate a broader understanding of the participants of the speech-language impairments. The results corroborate the importance of using the ICF-CY as a health care analysis tool, by incorporating functionality and participation aspects and providing subsidies for the construction of unique therapeutic projects in a broader approach to the health of the group studied.

Brazil is developing a Family Health Program in the Primary Health Care. The Family Health Program's team composition has been criticized as insufficient. One of the criticisms has been about the absence of the pediatrician. This study analyzed the experience of the city of Campinas, São Paulo State, which implemented this program with adaptations specifically with the presence of the pediatrician in its 140 teams. Using qualitative methodology and semi-structured interviews, pediatricians and ten family physicians teams were interviewed. The objective was to know their visions about their practice with children's primary health care, the role of each one and about the pediatrician's presence in the team. It was almost an unanimity about the importance of the pediatrician's participation in Primary Health Care, not as a specialty, but because of the improvement of the team's resolubility. Though, the pediatrician's work was considered too focused in the biomedical paradigm and in the office. There is a need for the pediatrician to have more engagement with the psychosocial aspects, the family and territory. It is essential the improvement of the training of pediatricians and family physicians to deal with the complex mission of the primary health care of nowadays.

Objetivo: Compreender influência das avós no exercício e duração do aleitamento materno de suas filhas adolescentes, durante os primeiros seis meses de vida da criança. Métodos: Estudo qualitativo, realizado com 25 futuras avós, que conviviam com suas filhas ou noras adolescentes grávidas, antes e após o nascimento de seus netos, no período de março de 2011 a janeiro de 2014. As entrevistas foram guiadas por um roteiro semiestruturado, e uma vez transcritas foram analisadas de forma temática, da qual emergiu a categoria: "O processo de amamentar para as avós". Resultados: As avós mostraram-se presentes na amamentação de seus netos desde os momentos iniciais até a introdução de alimentos complementares e as suas experiências pessoais de sucesso no aleitamento materno foram importantes para a construção do apoio à nutriz adolescente. Com passar do tempo, as avós se posicionaram na retaguarda, permitindo que as adolescentes assumissem a responsabilidade de principais cuidadoras dos bebês. Conclusão: A influência das avós na amamentação de seus netos se deu pela crença compartilhada que amamentar é importante, saudável e uma obrigação da mãe.


Objective: To understand the influence of grandmothers on the effectiveness and duration of breastfeeding of their teenage daughters during the child's first six months of life. Methods: Qualitative study conducted with 25 new grandmothers who lived with their pregnant teenage daughters or daughters-in-law before and after the birth of their grandchildren from March 2011 to January 2014. The interviews were guided by a semi-structured questionnaire; after transcription, data underwent thematic analysis, which yielded the category: "the breastfeeding process for grandmothers." Results: Grandmothers were present during the breastfeeding of their grandchildren from the earliest moments to the introduction of complementary foods; successful personal experiences with breastfeeding were important for building support for the nursing teenage mother. Over time, they stepped back and allowed the teenagers to take responsibility as the primary caregivers of the babies. Conclusion: The influence of grandmothers on the breastfeeding of their grandchildren is a result of the shared belief that breastfeeding is important, healthy and a mother's obligation.


Objetivo: Comprender la influencia de las abuelas en el ejercicio y la duración del amamantamiento materno de sus hijas adolescentes durante los primeros seis meses de vida del niño. Métodos: Estudio cualitativo realizado con 25 futuras abuelas que convivían con sus hijas o nueras adolescentes embarazadas antes y después del nacimiento de sus netos en el período entre marzo de 2011 y enero de 2014. Se utilizó para las entrevistas um guión semiestructurado y tras la transcripción las mismas fueron analizadas a través de temática de la cual emergió la categoría: "El proceso del amamantamiento para las abuelas". Resultados: Las abuelas estuvieron presentes durante el amamantamiento de sus nietos desde los primeros momentos hasta la introducción de los alimentos complementarios. Las experiencias personales de éxito para el amamantamiento materno fueron importantes para la construcción del apoyo a la adolescente y a lo largo del tiempo se pusieron en retaguardia permitiendo que las adolescentes asumieran la responsabilidad de principales cuidadoras de los bebés. Conclusión: La influencia de las abuelas para el amamantamiento de sus nietos se dio por la creencia compartida de que amamantar es importante y saludable y es una obligación de la madre.

OBJECTIVE: To verify the prevalence of obesity, systemic arterial hypertension (SAH), waist circumference and acanthosis nigricans (AN) in school children from Sorocaba, in 2009 and associate them with risk factors. METHODS: A probabilistic sample study was carried out with 680 children (7-11 years) from 13 public schools from the city of Sorocaba, SP. A questionnaire containing questions on physical activity, time spent watching television, playing with video games and computers (TV/VG/PC), student and parental antecedents of arterial hypertension, renal or cardiac disease, and economic level was applied. On physical examination, weight, height, waist circumference (WC) and blood pressure (BP) were measured; presence of AN was observed. The prevalence of nutritional disorders, SAH, WC increase and presence of AN were calculated. To associate body mass index (BMI) > P85 and BP > P90 with the other variables, chi square or Fisher's exact test (significance p < 0.05) and crude and adjusted prevalence odds ratio (POR) were used. RESULTS: The prevalence of BMI > P85 was 22.1% [95% CI: 19.0-25.3%], of BP > P90 10.9% [95% CI: 8.6- 13.5%], increased WC 15.4% [95% CI: 12.9-17.9%] and AN 3.8% [95% CI: 2.6-5.6%]. Paternal antecedents were associated with weight excess in both analysis (POR: 1.76; 95% CI: 1.05-2.95; p = 0.02). High blood pressure was associated with female sex (POR: 1.90; 95% CI: 1.12-3.23; p = 0.01), more time spent with TV/VG/PC (POR: 1.82; 95% CI: 1.00-3.36; p = 0.03), AN (POR: 8.18; 95% CI: 3.37-19.80; p < 0.00), obesity (POR: 4.09; 95% CI: 2.41-6.94; p < 0.00) and WC (POR: 4.83; 95% CI: 2.77-8.41; p < 0.00). After the multivariate analysis, the female sex (adjusted POR = 2.15; 95% CI: 1.17-3.93) and obesity (adjusted POR = 9.51; 95% CI: 4.77-18.97) remained. CONCLUSION: The prevalence of weight excess, SAH, increased WC and AN in these school children was relevant. This fact justifies the use of these measurements.

A literatura demonstra que sofrer violência doméstica na infância e juventude é um risco para o desenvolvimento das psicopatologias. Este estudo utilizou o Strenghts and Difficulties Questionnarie (SDQ) e encontrou prevalência de 65,5% de possibilidades de problemas de saúde mental em crianças e adolescentes (4-16 anos), vítimas de violência doméstica, acompanhados em Serviços de Referência, todos residentes em um município brasileiro. O resultado encontrado foi considerado elevado, se comparado à população geral. A violência doméstica foi observada como fator de risco para problemas de saúde mental nas crianças e adolescentes estudados, agravada pelo uso preocupante de bebida alcoólica pelo responsável, ou este estar fora do processo produtivo. Não foram encontrados fatores de proteção efetivos, principalmente em relação à rede de proteção. Esses resultados sinalizam a necessidade de implementação de mecanismos que avaliem a efetividade de programas específicos nas áreas de saúde mental e violência doméstica.


Literature shows that experiencing domestic violence in childhood and young adulthood is a risk for the development of psychopathology. This study used the Strengths and Difficulties Questionnaire (SDQ) and found a prevalence of 65.5% of possibility of mental health problems in children and adolescents (4-16 years old) who were victims of domestic violence followed in Referral Services in a Brazilian city. The result found was considered high when compared to the general population. Domestic violence was observed as a risk factor for mental health problems among the studied children and adolescents, aggravated by caregivers' alcoholism or unemployment. No effective protective factors were found, mainly regarding the protection network. These results indicate the need for reassessing the effectiveness of specific programs in the areas of mental health and domestic violence.

The lack of a clear correlation between the levels of antibody to pertussis antigens and protection against disease lends credence to the possibility that cell-mediated immunity provides primary protection against disease. This phase I comparative trial had the aim of comparing the in vitro cellular immune response and anti-pertussis toxin (anti-PT) immunoglobulin G (IgG) titers induced by a cellular pertussis vaccine with low lipopolysaccharide (LPS) content (wP(low) vaccine) with those induced by the conventional whole-cell pertussis (wP) vaccine. A total of 234 infants were vaccinated at 2, 4, and 6 months with the conventional wP vaccine or the wP(low) vaccine. Proliferation of CD3(+) T cells was evaluated by flow cytometry after 6 days of peripheral blood mononuclear cell culture with stimulation with heat-killed Bordetella pertussis or phytohemagglutinin (PHA). CD3(+), CD4(+), CD8(+), and T-cell receptor gammadelta-positive (gammadelta(+)) cells were identified in the gate of blast lymphocytes. Gamma interferon, tumor necrosis factor alpha, interleukin-4 (IL-4), and IL-10 levels in supernatants and serum anti-PT IgG levels were determined using enzyme-linked immunosorbent assay (ELISA). The net percentage of CD3(+) blasts in cultures with B. pertussis in the group vaccinated with wP was higher than that in the group vaccinated with the wP(low) vaccine (medians of 6.2% for the wP vaccine and 3.9% for the wP(low) vaccine; P = 0.029). The frequencies of proliferating CD4(+), CD8(+), and gammadelta(+) cells, cytokine concentrations in supernatants, and the geometric mean titers of anti-PT IgG were similar for the two vaccination groups. There was a significant difference between the T-cell subpopulations for B. pertussis and PHA cultures, with a higher percentage of gammadelta(+) cells in the B. pertussis cultures (P < 0.001). The overall data did suggest that wP vaccination resulted in modestly better specific CD3(+) cell proliferation, and gammadelta(+) cell expansions were similar with the two vaccines.

Objetivo: elaborar uma proposta de assistência fisioterapêutica na rede básica de saúde objetivando sistematizar esse serviço para a saúde da criança e do adolescente. Método: revisão bibliográfica/pesquisa nas bases de dados Scielo, Lilacs e Medline. Resultados: o fisioterapeuta não apenas restaura, desenvolve e conserva a capacidade física do paciente, como também previne doenças e promove orientações. Assim, propõe-se sua atuação com base em três estratégias: orientação, assistência e acompanhamento. Conclusão: o fisioterapeuta na atenção primária pode efetivar sua integração na equipe multiprofissional das UBS, no apoio matricial e programas de saúde da família.


Objective: To develop a proposal for physical therapy in primary health aiming to systematize this service to child health and adolescent. Method: literature / research in Scielo, Lilacs and Medline. Results: the therapist not only restores, develops and retains the physical ability of the patient, also prevents diseases and promotes guidelines. Thus, we propose a performance based on three strategies: guidance, assistance and support. Conclusion: the physiotherapist in primary care can accomplish their integration into multidisciplinary team of basic health units in the matrix support programs and family health.

INTRODUÇÃO: Visando atualizar suas práticas pedagógicas, atender as exigências da comunidade, da reestruturação do sistema de saúde e os avanços tecnológicos, a Faculdade de Ciências Médicas da Universidade Estadual de Campinas implementou uma grande reforma curricular para alunos ingressantes de 2001. OBJETIVO: Descrever uma experiência de ensino voltada à integração dos conhecimentos para atenção aos indivíduos nas diversas fases da vida, dentro da realidade de assistência primária à saúde, com ênfase no conhecimento, nas habilidades clínicas, na responsabilização e nas atitudes humanísticas e éticas. MÉTODOS: No novo currículo, a integração intra, inter e transdisciplinar foi estruturada em módulos interdepartamentais, inserção progressiva das disciplinas clínicas, contato mais cedo e progressivo do aluno com a sistema de saúde, preservando módulos integradores horizontais e verticais. A iniciação da prática clínica em Centros de Saúde tem, no quarto ano, 432 horas destinada a atendimentos clínico-ambulatoriais de assistência à criança, à mulher, ao adulto e ao idoso num contexto de saúde da família. A supervisão é realizada por professores, médicos assistentes da Faculdade e tutores selecionados entre os profissionais da rede primária de saúde. O Programa Nacional de Reorientação da Formação Profissional em Saúde Pró-Saúde facilitou a inserção e a parceria do curso de medicina com as UBS. O conteúdo teórico é integrado em seminários ministrados em dois períodos semanais e avaliado por meio de provas teóricas (conhecimento cognitivo). As habilidades e competências nas atividades clínicas são avaliadas por meio de discussões teórico-práticas quinzenais ao longo do estágio, avaliações clínicas estruturadas de atendimentos à criança, mulher e adulto, além da composição de portfólio com planilha de atendimentos totais, casos selecionados para revisão e auto-crítica de aprendizado. RESULTADOS: O módulo foi avaliado na forma de fóruns semestrais de discussão, com participação de discentes, docentes, tutores e gestores. Os grupos foram unânimes em considerar plenamente atingidos os objetivos de responsabilização, vínculo e ética, e parcialmente atingida a integração dos conteúdos teórico-práticos e trabalho em equipe. CONCLUSÃO: O currículo integrado propiciou uma visão clínica abrangente da família. Permitiu que o estudante se responsabilizasse e criasse vínculo com o paciente, entendendo a resolutividade e demandas da atenção básica à saúde por meio de sua vivência.


INTRODUCTION: In order to update its teaching practices and meet the community's needs and restructuring of the health system and technological advancements, the School of Medical Sciences at the State University in Campinas implemented a major curricular reform for incoming students in 2001. OBJECTIVE: Describe a teaching experience focused on the integration of knowledge for individual care in the various life phases, within the reality of primary healthcare, with an emphasis on knowledge, clinical skills, accountability, and humanistic and ethical attitudes. METHODS: In the new curriculum, intra-, inter-, and cross-disciplinary integration was organized in inter-departmental modules, progressive participation in clinical disciplines, students' earlier and progressive contact with the health system, maintaining horizontally and vertically integrated modules. Initiation in clinical practice in Health Centers in the fourth year of school, with 432 hours dedicated to outpatient clinical care for children, women, adults, and the elderly in the context of family health. Supervision is provided by faculty, clinical staff from the medical school, and tutors selected from among the primary healthcare system staff. The National Program for Reorientation of Professional Training in Health (Pró-Saúde) facilitated participation and partnership between the medical school and the primary care units. Theoretical content is provided in seminars during two weekly periods, with grading based on written tests (cognitive knowledge). Skills and competencies in clinical activities are evaluated by means of theoretical and practical discussions every other week throughout the internship, structured clinical evaluations of care provided to children, women, and adults, in addition to the portfolio composition with the total number of consultations, selected cases for review, and self-criticism of the learning process. RESULTS: The module was evaluated through discussion forums held once a semester with students, professors, tutors, and administrators. The groups agreed unanimously that the project had fully achieved the goals of accountability, bonds, and ethics, having partially achieved the integration between theoretical and practical course content and teamwork. CONCLUSION: The integrated curriculum fostered a comprehensive view of the family. It allowed students to take responsibility and create ties with patients, understanding the service's case-resolution capacity and demands of primary care through their direct experience.
