RESUMO

Objective:Based on the concept of outcomes-based education (OBE), aiming to improve students' medical mathematical modeling ability, the teaching design and practice of mathematical modeling module for the course Medical Advanced Mathematics are carried out and the direction of improvement is defined. Methods:Taking infectious disease model as an example, teaching practice was carried out, and then questionnaires from 44 students were collected for feedback opinions. Statistical description, Mann-Whitney U test, one-way ANOVA and Scheffe method, as well as chi-square test and z-test were used to analyze data. Results:The results showed that 90.91% of the students were interested in the subject of the course, and 65.91% of the students thought that this mode significantly improved the learning effect. In this course, there was no significant difference in the learning initiative of students on the two levels of being or not being interested in the course topic, but it had a significant impact on their mastery of knowledge points and their willingness to continue to carry out flipped classroom.Conclusion:The teaching mode is generally effective, but it needs to be further improved in three aspects: emphasizing modeling and downplaying computation, enhancing students' participation and strengthening the classroom learning effect.

Objective:To investigate the effects of miR-363-5p on the proliferation and apoptosis of nasopharyngeal carcinoma cells and the possible mechanism.Methods:miR-363-5p expression in human normal nasopharyngeal epithelial cells NP-69 and nasopharyngeal carcinoma cells 5-8F was detected using quantitative real-time polymerase chain reaction. Proliferation and apoptosis of 5-8F cells overexpresing miR-365-5p were determined. At the same time, Caspase3 and BRD4 protein expression in 5-8F cells were also detected.Results:miR-365-5p expression in 5-8F cells (0.71 ± 0.45) was significantly lower than that in NP-69 cells ( t = 2.68, P < 0.05). After overexpressing miR-363-5p, the proliferation of 5-8F cells was significantly decreased ( F = 22.68, P < 0.05). The apoptotic rate in the 5-8F cells was significantly higher than that in the control group [(24.45 ± 5.38)% vs. (18.23 ± 2.41)%, t = 4.13, P < 0.05]. Bax and Caspase3 protein levels in the 5-8F cells were (1.35 ± 0.24) and (1.44 ± 0.34) respectively, which were significantly higher than those in the NP-69 cells [(1.00 ± 0.08), (1.00 ± 0.23), t = 3.12, 5.12, P < 0.05]. BRD4 protein level in the 5-8F cells was significantly lower than that in the control group [(0.42 ± 0.24) vs. (1.00 ± 0.37), t = 2.98, P <0.05]. Conclusion:miR-365-5p can inhibit proliferation of nasopharyngeal carcinoma cells and promote their apoptosis. The negative regulatory effects of miR-363-5p on tumor cells are achieved possibly through inhibiting BRD4 protein expression.

Objective:To explore the efficacy and clinical significance of Eltrombopag combined with immunosuppression therapy(IST) in the treatment of severe aplastic anemia (SAA) in children.Methods:Clinical data of 63 children with initially diagnosed SAA in the Department of Hematology of Xinxiang Central Hospital from May 2017 to June 2019 were retrospectively analyzed.All of them were all donors without siblings and they were classified into observation group (31 cases) and control group (32 cases). Patients in the observation group received IST combined with Eltrombopag treatment, and those in the control group received IST treatment.The Chi- square test was used to compare the overall remission (OR) rate and complete remission (CR) rate at 3 months, 6 months and 12 months, and incidence of infection and significant bleeding between groups.The t-test was used to compare the application of gra-nulocyte colony stimulating factor, the mean red blood cell transfusion, and the mean platelet infusion volume between groups.Kaplan-Meier method was adopted to analyze the 2-year overall survival (OS) rate and failure-free survival (FFS) rate, followed by the Log- rank test. Results:The 3-month and 6-month OR rate of the observation group were 61.29%(19/31 cases) and 80.64% (25/31 cases), respectively, which was significantly higher than that of the control group [37.50%(12/32 cases) and 59.38%(19/32 cases), χ2=45.27, 43.81, respectively, all P<0.05]. The 3-month and 6-month CR rate of the observation group were 32.26%(10/31 cases) and 45.16%(14 /31 cases), respectively, which was significantly higher than that of the control group [15.62%(5/32 cases) and 28.13%(9/32 cases), χ2=47.02, 48.35, respectively, all P<0.05]. The 12-month OR rate and CR rate in the observation group were 83.87%(26/31 cases), and 64.52%(20/31 cases), respectively, which were 81.25%(26/32 cases), and 59.38%(19/32 cases), respectively in the control group, and no significant differences in them were detected between the two groups (all P>0.05). The total amount of granulocyte colony stimulating factors [(13.58±4.28) doses vs.(23.24±6.68) doses, t=2.591], and the mean infusion volume of red blood cells [(5.48±1.67) U vs.(10.58±3.67) U, t=2.040] and platelets (4.15±2.47) bags vs.(9.15±3.87) bags, t=2.744) used in observation group within 6 months of treatment were significantly lower than those of the control group (all P<0.05). The rate of infection (16.13% vs.43.75%, χ2=47.12) and significant bleeding (16.13% vs.37.50%, χ2=44.52) in the observation group were significantly lower than those of the control group (all P<0.05). The 2-year OS rate of the observation group and control group were 93.55% (29/31 cases), and 87.50% (28/32 cases), respectively.No significant difference in the OS rate was found between groups ( P=0.407 3), nor as the 2-year FFS rate(87.10% vs.78.13%, P=0.326 6). Conclusions:IST combined with Eltrombopag can significantly improve the early treatment response rate of SAA children without a sibling identical donor, which can reduce red blood cell and platelet transfusion, and the incidence of infection and bleeding.

Objective Retrospectively investigate 76 cases of traumatic amputation and limb salvage so as to provide clinical reference for children amputation .Methods We retrospevtively investigated 38 cases of traumatic amputation admitted during July 1996 to May 2013(experimental group) ,and 38 cases of limb salvage cases at the same time(control group) ,and re‐evaluated them according to the MESS standards and LSI score .Statistical analysis of the two scoring systems for the inosculation of amputation was conducted .Results Both Mangled Extremity Severity Score and Limb‐salvage index system can be used as the estimation for the traumatic amputation .Conclusion Mangled Extremity Severity Score (MESS) and Limb‐salvage index (LSI) can be used as an evaluation of the traumatic amputation in children ,and LSI was more suitable for the children .

Objective To discuss the effective treatments for fracture of the medial humeral epicondyle in children.Methods Twenty-five children with fracture of the medial humeral epicondyle treated from January 2008 to June 2011 were studied,including 18 males and 7 females at age of 7-14 years (mean,11.4 years).There were 7 cases accompanied with elbow joint dislocation and 2 lateral humeral epicondyle fracture.Patients were assigned to medial elbow approach to open reduction and internal fixation with absorbable rods (Group A,n =13) and open reduction and percutaneous K-wire fixation (Group B,n =12) according to the random number table.A plaster support was applied after surgery.Functional training was initiated 2-3 weeks after operation for Group A and 4-6 weeks after adequate callus formation for Group B.Moreover,follow-up was performed at regular intervals.Results All patients were followed up for 6-41 months (mean,22 months).Mean healing time was 3.8 weeks (range,3-5 weeks) in Group A and 5.6 weeks (range,4-8 weeks) in Group B.At postoperative 6 months,Bede elbow performance score was (93.7 ± 3.3) points in Group A with 10 excellent and 3 good results and (85.3 ±6.3) points in Group B with 3 excellent,7 good,and2 fair results (t=-4.24,P<0.05).Conclusion Open reduction and internal fixation with absorbable rods gains advantages of rigid immobilization,functional recovery and minor complication and hence is effective for treatment of medial humeral epicondyle fracture in children.

BACKGROUND:Neuroblastoma is a common solid tumor in children. Pediatric tumors are little affected by environmental factors, but closely related to child development. The suspension method is an effective and reliable method to harvest neoplastic stem cel s. OBJECTIVE:To culture the cel clones of human neuroblastoma cel line SK-N-SH and to assess the biological properties of the cel clones. METHODS:Using the suspension method with no serum media, tumor cel clones were obtained. Immunofluorescence method was used to identify whether tumor cel clones exhibit stem cel properties. SK-N-SH neuroblastoma was labeled by luciferase, and tumor cel clones and tumor cel s were seeded onto the back of nude mice to monitor cel proliferative properties in nude mice using in vivo imaging. RESULTS AND CONCLUSION:Using the suspension culture method, SK-N-SH neuroblastoma cel s could successful y develop into cloning bal s. Under serum-free culture, cloning bal s were immunofluorescently used to detect molecular markers that showed strong positive expression. Cloning bal s subcutaneously implanted into nude mice showed the strong ability of self-renewal and differentiation as stem cel s. The cel clones cultured by the suspension method strongly expressed Nestin, but weakly expressed glial fibril ary acidic protein, neuron-specific tubulin, possessing stem cel characteristics and strong proliferation and metastasis in nude mice.

OBJECTIVE@#This paper presents a method for treating maxillary neuralgia, the second division of trigeminal nerve.@*METHOD@#One hundred and thirty six cases with 136 trigeminal neuralgia were treated from 2004 to 2011. All patients were treated with endoscopic surgery at the pterygopalatofossa through approach to the maxillary sinus for resecting maxillary nerve and infraorbital nerve.@*RESULT@#One hundred and eighteen patients were relieved after operation and no recurrence of neuralgia was occurred after 2 to 8 years of follow-up.@*CONCLUSION@#This method had the advantages of avoiding to operate craniotomy with no complications, which was performed easily with valid efficacy.

ObjectiveTo investigate the clinical effects of minimally invasive incision in anterior traverse approach for Gartland type Ⅲ humeral supracondylar fractures in children.Methods Forty-two patients with Gartland type Ⅲ displaced supracondylar fractures of the humerus were treated through anterior traverse approach between January 2008 and April 2011.The fractures were fixed using two Kirschner wires from the medial and lateral epicondyles placed crossing each other.There were 31 males and 11 females,at age of 2.6-12 years (average 5.7 years).Three patients were with open fractures.Seven patients were combined with other fractures,four with radial nerve injury and one with median nerve injury,with time from injury to hospitalization for a range of 1 hour to 3 days.Of all the patients,30 patients were with fractures on the right side and 12 with fractures on the left side.All fractures were treated by minimally invasive incision in anterior traverse approach,when the neurovascular and muscular probing and repair were performed. Results Forty patients were followed up for 3-32 months ( average 13 months),which showed incision healing at one stage,with fracture healing time for 3-4 weeks (average 3.6 weeks).No cubitus varus or incision scar were found after operation.According to Flynn' s criteria,the effect of treatment three months after operation were excellent in 32 patients,good in four and fair in four,with excellence rate of 90%.ConclusionsThe minimally invasive incision of anterior traverse approach is characterized by minor trauma,small incision,nil scar and convenient neurovascular and muscular exploration and hence is a safe and reliable treatment for open reduction of Gartland type Ⅲ humeral supracondylar fractures in children.

Objective To investigate the effect of intramedullary stabilization of lower limb fractures with titanium elastic nails (TENs) in children.Methods From June 2004 to October 2010,278 children were stabilized with TENs in our hospital,including 181 boys and 97 girlsat average age of 7.6 years (range,3.0-14 years).There were 162 patients with femoral fractures,of which 35 underwent open reduction and the rest closed reduction.There were 116 patients with tibial fractures,which was treated with closed reduction.Postoperative fixation lasted for 3-4 weeks.The mean hospital stay was seven days.Of all the patients,272 patients received the removal of the TENs six months after operation and 242 received more than one year of follow-up to have a further observation on the limb length,hip and knee mobility and gait condition in children.Results All the patients were followed up for 6-66 months (mean,33 months).All fractures were healed in good alignment without severe complications.There were no intraoperative complications like infections,delayed union,nonunion,TEN broken and femoral or tibial epiphysis injuries.Ten patients suffered from swell and skin irritation in the nail' s position 3-4 months after operation,and the nails were removed in advance.Nineteen patients showed unequal lengths ( within 1 cm) of the lower limbs,with normal function of the lower limb joint but with no obvious limp.According to the Flynn evaluation standard,the excellent and good rate was 100%.Conclusions Characterized by good stability,rapid healing,small incision,anatomic reduction,unimpaired periosteum,short hospital stay and few complications,intramedullary stabilization with titanium elastic nails can achieve effective fixation in the treatment of lower limb fractures in children.

In order to detect the relationships of human laryngeal squamous cell carcinoma (LSCC) with the expression of Vascular Endothelial Growth Factor C (VEGF-C) and Vascular Endothelial Growth Factor Receptor-3 (VEGFR-3), we examined the level of VEGF-C and VEGFR-3 gene expression in the samples from 30 cases of normal laryngeal mucosa tissue (NLM), primary laryngeal carcinoma (PLC) and cervical lymph node (CLN) by using reverse transcription polymerase chain reaction (RT-PCR). The protein levels of VEGF-C and VEGFR-3 expression were also determined by Western blotting in 10 cases of NLM, PLC, CLN,respectively. We found that in the samples from the same patient, the VEGF-C and VEGFR-3 expression level is different among normal laryngeal mucosa tissue, primary laryngeal carcinoma and cervical lymph node. In primary laryngeal carcinoma, the level of VEGF-C and VEGFR-3 expression was significantly higher in lymph node positive group than in lymph node negative group, and this difference was the associated with the histological grade of differentiation (P < 0.05). The expression levels of VEGF-C and VEGFR-3 were not associated with age, sex, site and T stage (P > 0.5). These results suggest that there is a close relationship of LSCC with VEGF-C/VEGFR-3; and VEGF-C/VEGFR-3 may have the potential for use as an indicator with prognostic significance in dealing with LSCC.

Objective:To establish the pulmonary metastasis model of osteosarcoma in rats.Methods:UMR106 cells(rat osteosarcoma strain) were cultured in RPMI1640 media and collected before transplantation and resuspended in serum-free media or collagen gel to a final concentration of 1?107 cells/ml.About 200?l cell suspensions were injected into the right proximal tibia of 3-week-old Sprague-Dawley rats.Twelve rats were used in every group sorted by resuspension fluid.All rats were treated by cyclosporin A and SMZ-TMP after transplantation.The dates of tumor appearance and rat death were noted,the size of tumor was measured once a week,the metastasis foci on the lung surface was counted and the limbs afflicted by tumor were observed by X-ray after death.Both the tumors in limb and lung were stained with hematoxylin and eosin,and analyzed by microscopy.Results:All rats developed local intratibial bone tumors that were radiographically and histologically similar to primary human osteosarcoma.The pulmonary metastasis foci were found in all rats at autopsy.The tumor formation time of serum-free media group was(10.0?1.65) days,shorter than that of collagen gel group,which was(17.8?0.87) days(P
