RESUMO

Children with autism spectrum conditions (ASC) experience difficulties recognizing others' emotions and mental states. It has been shown that serious games (SG) can produce simplified versions of the socio-emotional world. The current study performed a cross-cultural evaluation (in the UK, Israel and Sweden) of Emotiplay's SG, a system aimed to teach emotion recognition (ER) to children with ASC in an entertaining, and intrinsically motivating way. Participants were 6-9 year olds with high functioning ASC who used the SG for 8-12 weeks. Measures included face, voice, body, and integrative ER tasks, as well as parent-reported level of autism symptoms, and adaptive socialization. In the UK, 15 children were tested before and after using the SG. In Israel (n = 38) and Sweden (n = 36), children were randomized into a SG or a waiting list control group. In the UK, results revealed that 8 weeks of SG use significantly improved participants' performance on ER body language and integrative tasks. Parents also reported their children improved their adaptive socialization. In Israel and Sweden, participants using the SG improved significantly more than controls on all ER measures. In addition, parents in the Israeli SG group reported their children showed reduced autism symptoms after using the SG. In conclusion, Emotiplay's SG is an effective and motivating psycho-educational intervention, cross-culturally teaching ER from faces, voices, body language, and their integration in context to children with high functioning ASC. Local evidence was found for more generalized gains to socialization and reduced autism symptoms.

A new approach for doping of Cu2S nanocrystal arrays using thermal treatment at moderate temperatures (T < 400 K) is presented. This thermal doping process yields conductance enhancement by 6 orders of magnitude. Local probe measurements prove this doping is an intraparticle effect and, moreover, tunneling spectroscopy data signify p-type doping. The doping mechanism is attributed to Cu vacancy formation, resulting in free holes. Thermal-doping temperature dependence exhibits an Arrhenius-like behavior, providing the vacancy formation energy of 1.6 eV. The moderate temperature conditions for thermal doping unique to these nanocrystals allow patterned doping of nanocrystal films through local heating by a focused laser beam, toward fabrication of nanocrystal-based electronic devices.

RESUMO

A 55-year-old Jewish patient was simultaneously diagnosed as having Gaucher's disease and IgA multiple myeloma. Serum protein electrophoresis and immunoelectrophoresis showed two different IgA kappa type monoclonal spikes. After four years of observation, a rapid fatal course of disease developed together with expression of J chain protein and an additional IgA lambda paraprotein. Although dysproteinaemias and multiple myeloma have occasionally been reported in Gaucher's patients, such a complex gammopathy has to the best of our knowledge not yet been described.

RESUMO

A case of a patient suffering from Waldenstrom's macroglobulinemia who developed diarrhea and mild steatorrhea is described. Laboratory studies revealed low serum IgA, intestinal secretory IgA deficiency, and small intestine bacterial overgrowth as demonstrated by the C14-cholylglycine breath test. These findings suggest that selective IgA deficiency and secretory component deficiency may be contributing factors in the development of diarrhea in Waldenstrom's macroglobulinemia.

In order to identify patients who benefit most from a cardiac rehabilitation program, we studied retrospectively all patients who completed a 3-month comprehensive cardiac rehabilitation program during a 2-year period. Questionnaires regarding physical exercise habits were sent to 122 patients and returned by 117 (96%) of them (53 post-acute myocardial infarction, 50 post-coronary artery bypass surgery, 14 post-infarction and surgery, 2 post-angioplasty). Exercise capacity (subset of 66 patients) improved by 19% after rehabilitation (7.8 +/- 3.1 to 9.3 +/- 2.7 METs, p < 0.0001). Univariate and multivariate analysis identified initial exercise capacity as the only independent variable predicting improvement in exercise performance (inverse relationship) (r2 = 0.24, p < 0.0001). The improvement was not related to age, sex, left ventricular function or time from cardiac event to rehabilitation. Patients recovering from both infarction and coronary artery bypass surgery showed a greater improvement (delta exercise capacity 2.8 +/- 1.4 METs) than patients after myocardial infarction alone (delta exercise capacity 0.8 +/- 2 METs, p < 0.02). Improvement was sustained for up to 2 years after completion of the program.

Purified, formaldehyde-inactivated and alum-adjuvanted hepatitis A virus (HAV) vaccines have recently become available for clinical trials. The vaccine is administered intramuscularly in a schedule of 0, 1, and 6 months. The aim of the study was to evaluate the reactogenicity and immunogenicity of an inactivated hepatitis A (HA) vaccine. Three groups of volunteers comprised the study population: 28 volunteers without antibody to HAV were given HA vaccine and, for comparison, 43 subjects received hepatitis B vaccine for possible adverse reactions to the HA vaccine; 12 other subjects received immunoglobulin alone. Each 1 ml dose of HA vaccine contained 720 enzyme units or about 100 ng of antigen. Anti-HAV was determined by means of a commercial assay (Abbott Laboratories: HAV-EIA), and by a more sensitive ELISA. No significant adverse reactions were reported. In the group that received HA vaccine, 4 weeks following the first dose all had detectable antibodies (> or = 20 mIU/ml) by the sensitive ELISA. By commercial HAV-EIA, at 20 weeks following the second dose 75.0% had detectable antibodies, and after the third vaccine all had detectable antibodies. This new inactivated HA vaccine is highly immunogenic and had no significant side effects.

Inactivated hepatitis A virus (HAV) vaccine is given in a three-dose schedule. When rapid protection is needed, injection of immune globulin (IG) concomitantly with the first dose could provide passive protection until adequate active antibody response has developed. A possible effect of IG on the immune response to the vaccine was studied in healthy volunteers; 28 received vaccine alone, and 34 received the first dose simultaneously with 5 mL of IG. A control group received hepatitis B vaccine, and a fourth group received IG alone. Four weeks after the first vaccine dose, all subjects had detectable ELISA anti-HAV antibodies. Several weeks after each vaccine dose, the geometric mean titer of antibodies was significantly lower in those who received vaccine with IG but higher than in those who received IG alone. Results for neutralizing antibodies yielded a similar trend. If IG is given with HAV vaccine, a further booster vaccine dose may be required to ensure long-lasting immunity.
