Objective: Systematic reviews and other evidence synthesis projects require systematic search methods. Search systems require several essential attributes to support systematic searching; however, many systems used in evidence synthesis fail to meet one or more of these requirements. I undertook a qualitative study to examine the effects of these limitations on systematic searching and how searchers select information sources for evidence synthesis projects. Methods: Qualitative data were collected from interviews with twelve systematic searchers. Data were analyzed using reflexive thematic analysis. Results: I used thematic analysis to identify two key themes relating to search systems: systems shape search processes, and systematic searching occurs within the information market. Many systems required for systematic reviews, in particular sources of unpublished studies, are not designed for systematic searching. Participants described various workarounds for the limitations they encounter in these systems. Economic factors influence searchers' selection of sources to search, as well as the degree to which vendors prioritize these users. Conclusion: Interviews with systematic searchers suggest priorities for improving search systems, and barriers to improvement that must be overcome. Vendors must understand the unique requirements of systematic searching and recognize systematic searchers as a distinct group of users. Better interfaces and improved functionality will result in more efficient evidence synthesis.

BACKGROUND: Given the increasing volume of published research in bibliographic databases, efficient retrieval of evidence is crucial and represents an opportunity to integrate novel techniques such as text mining. OBJECTIVES: To develop and validate a geographic search filter for identifying research from the United States (US) in Ovid MEDLINE. METHODS: US and non-US citations were collected from bibliographies of evidence-based reviews. Citations were partitioned by US/non-US status and randomly divided to a training and testing set. Using text mining, common one- and two-word terms in title/abstract fields were identified, and frequencies compared between US/non-US citations. RESULTS: Common US-related terms included (as ratio of frequency in US/non-US citations) US populations and geographic terms [e.g., 'Americans' (15.5), 'Baltimore' (20.0)]. Common non-US terms were non-US geographic terms [e.g., 'Japan' (0.04), 'French' (0.05)]. A search filter was developed with 98.3% sensitivity and 82.7% specificity. DISCUSSION: This search filter will streamline the identification of evidence from the US. Periodic updates may be necessary to reflect changes in MEDLINE's controlled vocabulary. CONCLUSION: Text mining was instrumental to the development of this search filter. A novel technique generated a gold standard set comprising >20,000 citations. This method may be adapted to develop subsequent geographic search filters.

RESUMO

Germany has a long tradition of health libraries. From their origin in monasteries, they became centres of knowledge and education. In modern times, this tradition has been continued by the Central Library of Medicine. In addition, as a specialty in Germany, special collection areas and special libraries that focus on one topic were established. Those services were transformed to specialized information services and portals as part of the digital transformation process. One of such projects is OPEN-CAM, which provides literature on integrative medicine in a specialist library and hosts the literature database CAMbase. Based on this example, we show how digital transformation has influenced the library landscape and its structures in Germany.

BACKGROUND: Information specialists conducting searches for systematic reviews need to consider key questions around which and how many sources to search. This is particularly important for public health topics where evidence may be found in diverse sources. OBJECTIVES: The objective of this review is to give an overview of recent studies on information retrieval guidance and methods that could be applied to public health evidence and used to guide future searches. METHODS: A literature search was performed in core databases and supplemented by browsing health information journals and citation searching. Results were sifted and reviewed. RESULTS: Seventy-two papers were found and grouped into themes covering sources and search techniques. Public health topics were poorly covered in this literature. DISCUSSION: Many researchers follow the recommendations to search multiple databases. The review topic influences decisions about sources. Additional sources covering grey literature eliminate bias but are time-consuming and difficult to search systematically. Public health searching is complex, often requiring searches in multidisciplinary sources and using additional methods. CONCLUSIONS: Search planning is advisable to enable decisions about which and how many sources to search. This could improve with more work on modelling search scenarios, particularly in public health topics, to examine where publications were found and guide future research.

Health science libraries have been using information technology since the late 1960s, shaping both the profession and the mission of these libraries. To explore the impact of technology, a series of articles has been commissioned for the HILJ Regular Feature, International Perspectives and Initiatives. This editorial sets the scene for this series of articles, which starts in this issue. These articles, written by health science librarians from around the globe, will explore the impact of technology on the way health science libraries provide information in the digital age. Some articles will look at national trends and others will focus on a particular library. A key theme is how technology is being used to support the mission of health science libraries and whether technology has altered that mission. This editorial provides a brief overview of the technologies libraries have adopted, from the 1970s to the present day. From this, it is clear that information technology has transformed the way health information is collected, catalogued, and disseminated to users. And it is certain that in the coming decade new technologies will be incorporated into health science libraries, which will pose challenges for both users and librarians. However, librarians will continue to find ways to adapt and use these tools to meet the needs of their users.

When the COVID-19 pandemic arrived in the United States in early 2020, it caused an information explosion in the health science literature. Researchers wanted to share their results quickly, so they utilized sources that were not indexed in conventional databases. Hospital librarians stepped up to meet the information and public health challenges of the pandemic. They developed alternate strategies to provide services and resources remotely at a time when their physical libraries were closed to comply with the need for social distancing and compliance with public health recommendations.

Teaching students how to conduct bibliographic searches in health sciences' databases is essential training. One of the challenges librarians face is how to motivate students during classroom learning. In this article, two hospital libraries, in Spain, used Escape rooms as a method of bringing creativity, teamwork, communication and critical thinking into bibliographic search instruction. Escape rooms are a series of puzzles that must be solved to exit the game. This article explores the methods used for integrating escape rooms into training programmes and evaluates the results. Escape Rooms are a useful tool that can be integrated into residents' training to support their instruction on bibliographic searches. This kind of learning stablishes competences like logical thinking and deductive approaching. These aspects aid participants to make their own decision and to develop social and intellectual skills.

BACKGROUND: PubMed is one of the most important basic tools to access medical literature. Semantic query expansion using synonyms can improve retrieval efficacy. OBJECTIVE: The objective was to evaluate the performance of three semantic query expansion strategies. METHODS: Queries were built for forty MeSH descriptors using three semantic expansion strategies (MeSH synonyms, UMLS mappings, and mappings created by the CISMeF team), then sent to PubMed. To evaluate expansion performances for each query, the first twenty citations were selected, and their relevance were judged by three independent evaluators based on the title and abstract. RESULTS: Queries built with the UMLS expansion provided new citations with a slightly higher mean precision (74.19%) than with the CISMeF expansion (70.28%), although the difference was not significant. Inter-rater agreement was 0.28. Results varied greatly depending on the descriptor selected. DISCUSSION: The number of citations retrieved by the three strategies and their precision varied greatly according to the descriptor. This heterogeneity could be explained by the quality of the synonyms. Optimal use of these different expansions would be through various combinations of UMLS and CISMeF intersections or unions. CONCLUSION: Information retrieval tools should propose different semantic expansions depending on the descriptor and the search objectives.

Indigenous people constitute an important section of society in many countries. Despite being a numerically smaller section, they are culturally diverse and distributed mostly in valuable natural resources-rich regions worldwide. In the era of globalization, industrialization, and trade liberalization, indigenous communities have become more vulnerable to displacement, land alienation, cultural erosion, and social exclusion. During the last few decades, researchers have tried to evaluate and document their problems and prospects. The present study analyzes the trends and characteristics of research and development conducted about indigenous communities. The research hotspots based on keywords, productive researchers, and journals during 1979-2020 were mapped using the Scopus database. The analysis was carried out using the bibliometrix R-package and VOSviewer software tool. Consistent growth in the number of studies and citations on indigenous communities concerning environmental conservation, natural resources, and economic development was observed during the last four decades. The present findings reveal that research on the indigenous community has attracted the attention of the scientific community in recent years. Qualitative studies with methodological rigor, having potential for social and policy implications, are warranted to understand and respect ingrained cultural and socio-economic diversity among these communities. The history of European health technology assessment (HTA) goes back more than 30 years. Almost as old as HTA agencies themselves is the desire to achieve European collaboration. This gained further impetus with the establishment of the European Network of Health Technology Assessment (EUnetHTA) in 2006. In this context, the field of information management faced specific challenges. Although these services are an integral part of HTA and information specialists play a key role here, this field is often not adequately represented in the HTA agencies within EUnetHTA. Furthermore, the organization of HTA production, including the types of HTAs produced, as well as funding, varies considerably. In order to meet these different conditions, information specialists have created various products and defined processes. With the EUnetHTA guideline, a common methodological understanding for the production of rapid Relative Effectiveness Assessments now exists. Furthermore, the Standard Operating Procedures map the complex information retrieval processes within EUnetHTA in a hands-on manner. The newly established Information Specialist Network (ISN) will in future ensure that information specialists are involved in all EUnetHTA assessments and that the methods are applied consistently in all assessments. In addition, the steering committee of the ISN manages enquiries and can be contacted to discuss methodological issues. Major barriers such as heterogeneity in the daily work of the EUnetHTA members can only be overcome through more collaboration and training.

Scientific knowledge constitutes a complex system that has recently been the topic of in-depth analysis. Empirical evidence reveals that little is known about the dynamic aspects of human knowledge. Precise dissection of the expansion of scientific knowledge could help us to better understand the evolutionary dynamics of science. In this paper, we analyzed the dynamic properties and growth principles of the MEDLINE bibliographic database using network analysis methodology. The basic assumption of this work is that the scientific evolution of the life sciences can be represented as a list of co-occurrences of MeSH descriptors that are linked to MEDLINE citations. The MEDLINE database was summarized as a complex system, consisting of nodes and edges, where the nodes refer to knowledge concepts and the edges symbolize corresponding relations. We performed an extensive statistical evaluation based on more than 25 million citations in the MEDLINE database, from 1966 until 2014. We based our analysis on node and community level in order to track temporal evolution in the network. The degree distribution of the network follows a stretched exponential distribution which prevents the creation of large hubs. Results showed that the appearance of new MeSH terms does not also imply new connections. The majority of new connections among nodes results from old MeSH descriptors. We suggest a wiring mechanism based on the theory of structural holes, according to which a novel scientific discovery is established when a connection is built among two or more previously disconnected parts of scientific knowledge. Overall, we extracted 142 different evolving communities. It is evident that new communities are constantly born, live for some time, and then die. We also provide a Web-based application that helps characterize and understand the content of extracted communities. This study clearly shows that the evolution of MEDLINE knowledge correlates with the network's structural and temporal characteristics.

BACKGROUND: Healthcare professionals and researchers in the field of palliative care often have difficulties finding relevant articles in online databases. Standardized search filters may help improve the efficiency and quality of such searches, but prior developed filters showed only moderate performance. AIM: To develop and validate a specific search filter and a sensitive search filter for the field of palliative care. DESIGN: We used a novel, objective method for search filter development. First, we created a gold standard set. This set was split into three groups: term identification, filter development, and filter validation set. After creating the filters in PubMed, we translated the filters into search filters for Ovid MEDLINE, Embase, CINAHL, PsychINFO, and Cochrane Library. We calculated specificity, sensitivity and precision of both filters. RESULTS: The specific filter had a specificity of 97.4%, a sensitivity of 93.7%, and a precision of 45%. The sensitive filter had a sensitivity of 99.6%, a specificity of 92.5%, and a precision of 5%. CONCLUSION: Our search filters can support literature searches in the field of palliative care. Our specific filter retrieves 93.7% of relevant articles, while 45% of the retrieved articles are relevant. This filter can be used to find answers to questions when time is limited. Our sensitive filter finds 99.6% of all relevant articles and may, for instance, help conducting systematic reviews. Both filters perform better than prior developed search filters in the field of palliative care.

Health and Life studies are well known for the huge amount of data they produce, such as high-throughput sequencing projects (Stephens et al., PLoS Biol 13(7):e1002195, 2015; Hey et al., The fourth paradigm: data-intensive scientific discovery, vol 1. Microsoft research Redmond, Redmond, 2009). However, the value of the data should not be measured by its amount, but instead by the possibility and ability of researchers to retrieve and process it (Leonelli, Data-centric biology: a philosophical study. University of Chicago Press, Chicago, 2016). Transparency, openness, and reproducibility are key aspects to boost the discovery of novel insights into how living systems work (Nosek et al., Science 348(6242):1422-1425, 2015).

Scientific hypotheses are essential for progress in rapidly developing academic disciplines. Proposing new ideas and hypotheses require thorough analyses of evidence-based data and predictions of the implications. One of the main concerns relates to the ethical implications of the generated hypotheses. The authors may need to outline potential benefits and limitations of their suggestions and target widely visible publication outlets to ignite discussion by experts and start testing the hypotheses. Not many publication outlets are currently welcoming hypotheses and unconventional ideas that may open gates to criticism and conservative remarks. A few scholarly journals guide the authors on how to structure hypotheses. Reflecting on general and specific issues around the subject matter is often recommended for drafting a well-structured hypothesis article. An analysis of influential hypotheses, presented in this article, particularly Strachan's hygiene hypothesis with global implications in the field of immunology and allergy, points to the need for properly interpreting and testing new suggestions. Envisaging the ethical implications of the hypotheses should be considered both by authors and journal editors during the writing and publishing process.

In order to identify current (and relevant) evidence for a specific clinical question within the unmanageable amount of information available, solid skills in performing a systematic literature search are essential. An efficient approach is to search a biomedical database containing relevant literature citations of study reports. In this article, we explain step by step how to perform a systematic literature search via PubMed (MEDLINE) by means of an example research question in the field of ophthalmology. First, we demonstrate how to translate the clinical problem into a well-framed and searchable research question, how to identify relevant search terms and how to conduct a text word search and a search with keywords in MeSH terms. We then show how to limit the number of search results if the search yields too many irrelevant hits and how to increase the number in the case of too few citations. Finally, we summarize all essential principles that guide a literature search via PubMed.

BACKGROUND: The most-cited papers help to better understand important characteristics of this specific science field. OBJECTIVE: To analyse the 100 most-cited papers in the field of Paediatric Dentistry. DESIGN: A search of the most-cited papers in Paediatric Dentistry journals was performed using journals included in the category of 'Dentistry, Oral Surgery & Medicine' in the Thompson Reuters Web of Science citation indexing database up to December 2018. Two researchers performed the data extraction, which included: number of citations, title, authors, country, year, journals, study design, and thematic area. RESULTS: The number of citations of each paper included in the top 100 most-cited ranged from 42 to 182 (mean: 64.51). Seven papers were cited more than 100 times. Most of the papers were published in the International Journal of Paediatric Dentistry (36%), between 2006 and 2015 (55%), with a cross-sectional design (39%). Twenty-six authors participated in two or more papers. The countries with the highest number of most-cited papers were the United States (25%), Australia (11%), and Brazil (9%). Cariology was the most studied thematic area. CONCLUSION: The evaluation of the top 100 most-cited papers in Paediatric Dentistry journals allowed for a better understanding of the world scenario regarding this research field.

BACKGROUND: There are few publications on search strategies to identify diagnostic test accuracy (DTA) studies in lilacs. OBJECTIVE: To translate and customise medline search strategies for use in lilacs and assess their retrieval of studies in Cochrane DTA systematic reviews. METHOD: We developed a six-step process to translate and customise medline search strategies for use in lilacs (iAHx interface). We identified medline search strategies of published Cochrane DTA reviews, translated/customised them for use in lilacs, ran searches in lilacs and compared the retrieval results of our translated search strategy versus the one used in the published reviews. RESULTS: Our lilacs search strategies translated/customised from the medline strategies retrieved studies in 70 Cochrane DTA reviews. Only 29 of these reviews stated that they had searched the lilacs database and 21 published their lilacs search strategies. Few had used the lilacs database search tools, none exploded the subject headings, and 86% used only English terms. CONCLUSION: Translating and tailoring a medline search strategy for the lilacs database resulted in the retrieval of DTA studies that would have been missed otherwise.

BACKGROUND: Information specialists frequently translate search filters from one interface to another. Publications advise that translation can be complex and should be undertaken carefully. OBJECTIVES: To investigate the issues arising when translating the Cochrane Embase RCT search filter from one interface (Ovid) to another (Embase.com). METHODS: We drafted a translation of the Cochrane Ovid RCT filter to run in Embase.com. We compared the line-by-line results of the Ovid filter with the results of the translation. We revised the filter. We identified differences between database versions including records with different publication years and subject headings. Some records were in Embase in one interface but not in the other. We encountered expected interface differences relating to proximity operators. We also encountered unexpected interface issues around truncation and the use of the original title or original abstract field. DISCUSSION: Filter conversion is challenging and time consuming revealing unexpected differences in interfaces and databases. Careful planning can pre-empt some issues, but others may only emerge during testing. We identified interface anomalies that have led database publishers to review aspects of the way their interfaces work. CONCLUSIONS: Translators should be vigilant for known and unknown differences in both interfaces and database versions.

BACKGROUND: Searching for topics within large biomedical databases can be challenging, especially when topics are complex, diffuse, emerging or lack definitional clarity. Experimentally derived topic search filters offer a reliable solution to effective retrieval; however, their number and range of subject foci remain unknown. OBJECTIVES: This systematic scoping review aims to identify and describe available experimentally developed topic search filters. METHODS: Reports on topic search filter development (1990-) were sought using grey literature sources and 15 databases. Reports describing the conception and prospective development of a database-specific topic search and including an objectively measured estimate of its performance ('sensitivity') were included. RESULTS: Fifty-four reports met inclusion criteria. Data were extracted and thematically synthesised to describe the characteristics of 58 topic search filters. DISCUSSION: Topic search filters are proliferating and cover a wide range of subjects. Filter reports, however, often lack clear definitions of concepts and topic scope to guide users. Without standardised terminology, filters are challenging to find. Information specialists may benefit from a centralised topic filter repository and appraisal checklists to facilitate quality assessment. CONCLUSION: Findings will help information specialists identify existing topic search filters and assist filter developers to build on current knowledge in the field.
