RESUMO

The Ouest Data Hub (ODH) a project lead by GCS HUGO which is a cooperation group of University Hospitals in the French Grand Ouest region represents a groundbreaking initiative in this territory, advancing health data sharing and reuse to support research driven by real-world health data. Central to its structure are the Clinical Data Warehouses (CDWs) and Clinical Data Centers (CDCs), essential for analytics and as the linchpin of the ODH's status as an interregional Learning Health System. Aimed at fostering innovation and research, the ODH's collaborative and multi-institutional model effectively utilizes both local and shared resources. Yet, the path is not without challenges, especially in data quality and interoperability, where ongoing harmonization and standard adherence are critical. In 2023, this facilitated access to extensive health data from over 9.3 million patient records, demonstrating the ODH's capacity for both monocentric and multicentric research across various clinical fields, in close collaboration with physicians. The integration of healthcare professionals is crucial, ensuring data's clinical relevance and guiding accurate interpretations. Future expansions of the ODH to new hospitals and data types promise to enhance its model further, already inspiring similar frameworks across France. This scalable model for health data ecosystems showcases the ODH's potential as a foundation for national and supranational data sharing efforts.

RESUMO

i2b2 data-warehouse could be a useful tool to support the enrollment phase of clinical studies. The aim of this work is to evaluate its performance on two clinical trials. We developed also an i2b2 extension to help in suggesting eligible patients for a study. The work showed good results in terms of ability to implement inclusion/exclusion criteria, but also in terms of identified patients actually enrolled and high number of patients suggested as potentially enrollable.

BACKGROUND: When developed jointly with clinical information systems, clinical data warehouses (CDWs) facilitate the reuse of healthcare data and leverage clinical research. OBJECTIVE: To describe both data access and use for clinical research, epidemiology and health service research of the "Hôpital Européen Georges Pompidou" (HEGP) CDW. METHODS: The CDW has been developed since 2008 using an i2b2 platform. It was made available to health professionals and researchers in October 2010. Procedures to access data have been implemented and different access levels have been distinguished according to the nature of queries. RESULTS: As of July 2016, the CDW contained the consolidated data of over 860,000 patients followed since the opening of the HEGP hospital in July 2000. These data correspond to more than 122 million clinical item values, 124 million biological item values, and 3.7 million free text reports. The ethics committee of the hospital evaluates all CDW projects that generate secondary data marts. Characteristics of the 74 research projects validated between January 2011 and December 2015 are described. CONCLUSION: The use of HEGP CDWs is a key facilitator for clinical research studies. It required however important methodological and organizational support efforts from a biomedical informatics department.

RESUMO

Recently, we have experienced with various changes in the health insurance. Korean government is implementing the prospective payment system(PPS) as a pilot project to reduce medical expenditure. Case management is an effective way to reduce costs, while not sacrificing quality of service, under the PPS. In this study, the integrated decision support system(DSS) for case management was presented to support two case management activities ; utilization management and discharge planning. This system can be used in three ways ; prospective, concurrent, and retrospective management. By monitoring cases from medical record database, the DSS provides basic patient information, payment information, medical service information to the case manager. In the future, data warehouse should be introduced to integrate various databases related to case management and to improve quality of information produced by the DSS.
