RESUMO

The COVID-19 pandemic brings to the forefront the complex interconnected dilemmas of globalization, health equity, economic security, environmental justice, and collective trauma, severely impacting the marginalized and people of color in the United States. This lack of access to and the quality of healthcare, affordable housing, and lack of financial resources also continue to have a more significant impact on documented and undocumented immigrants. This paper aims at examining these critical issues and developing a framework for family therapists to address these challenges by focusing on four interrelated dimensions: cultural values, social determinants of health, collective trauma, and the ethical and moral responsibility of family therapists. Given the fact that family therapists may unwittingly function as the best ally of an economic and political system that perpetuates institutionalized racism and class discrimination, we need to utilize a set of principles, values, and practices that are not just palliative or after the fact but bring forth into the psychotherapeutic and policy work a politics of care. Therefore, a strong call to promote and advocate for the broader continuum of health and critical thinking preparing professionals to meet the challenges of health equity, as well as economic and environmental justice, is needed. The issues discussed in this paper are specific to the United States despite their relevance to family therapy as a field. We are mindful not to generalize the United States' reality to the rest of the world, recognizing that issues discussed in this paper could potentially contribute to international discourse.


La pandemia de la COVID-19 ha puesto en primer plano los dilemas complejos e interconectados de la globalización, la equidad sanitaria, la seguridad económica, la justicia ambiental y el trauma colectivo, afectando gravemente a las personas marginadas y de color de los Estados Unidos. Esta falta de acceso a asistencia sanitaria de calidad, a viviendas asequibles, y la falta de recursos económicos también continúan teniendo un efecto más significativo en los inmigrantes documentados e indocumentados. Este artículo tiene como finalidad analizar estas cuestiones críticas y desarrollar un marco para que los terapeutas familiares aborden estas dificultades centrándose en cuatro dimensiones interrelacionadas: valores culturales, determinantes sociales de salud, trauma colectivo, y responsabilidad ética y moral de los terapeutas familiares. Teniendo en cuenta el hecho de que los terapeutas familiares pueden funcionar inconscientemente como los mejores aliados de un sistema económico y político que perpetúa el racismo institucionalizado y el clasismo, necesitamos utilizar un conjunto de principios, valores y prácticas que no sea solo paliativo o a posteriori, sino que genere en el trabajo político y psicoterapéutico una política de asistencia. Por lo tanto, es necesario un reclamo firme de promover y abogar por un continuo más amplio de la salud y un pensamiento crítico que prepare a los profesionales para responder a las dificultades de la equidad sanitaria, así como de la justicia económica y ambiental, si fuera necesario. Las cuestiones tratadas en este artículo son específicas de los Estados Unidos a pesar de su relevancia para la terapia familiar como área. Somos conscientes de no generalizar la realidad de los Estados Unidos para el resto del mundo, y reconocemos que las cuestiones tratadas en este artículo podrían contribuir al discurso internacional.

We sought to take a first step toward better integration of social concerns into empirical ecosystem service (ES) work. We did this by adapting cognitive anthropological techniques to study the Clayoquot Sound social-ecological system on the Pacific coast of Canada's Vancouver Island. We used freelisting and ranking exercises to elicit how locals perceive ESs and to determine locals' preferred food species. We analyzed these data with the freelist-analysis software package ANTHROPAC. We considered the results in light of an ongoing trophic cascade caused by the government reintroduction of sea otters (Enhydra lutris) and their spread along the island's Pacific coast. We interviewed 67 local residents (n = 29 females, n = 38 males; n = 26 self-identified First Nation individuals, and n = 41 non-First Nation individuals) and 4 government managers responsible for conservation policy in the region. We found that the mental categories participants-including trained ecologists-used to think about ESs, did not match the standard academic ES typology. With reference to the latest ecological model projections for the region, we found that First Nations individuals and women were most likely to perceive the most immediate ES losses from the trophic cascade, with the most certainty. The inverse was found for men and non-First Nations individuals, generally. This suggests that 2 historically disadvantaged groups (i.e., First Nations and women) are poised to experience the immediate impacts of the government-initiated trophic cascade as yet another social injustice in a long line of perceived inequities. Left unaddressed, this could complicate efforts at multistakeholder ecosystem management in the region.

OBJECTIVES: To assess the extent of socio-economic inequity in coverage and timeliness of key childhood immunisations in Ghana. METHODS: Secondary analysis of vaccination card data collected from babies born between January 2008 and January 2010 who were registered in the surveillance system supporting the ObaapaVita and Newhints Trials was carried out. 20 251 babies had 6 weeks' follow-up, 16 652 had 26 weeks' follow-up, and 5568 had 1 year's follow-up. We performed a descriptive analysis of coverage and timeliness of vaccinations by indicators for urban/rural status, wealth and educational attainment. The association of coverage with socio-economic indicators was tested using a chi-square-test and the association with timeliness using Cox regression. RESULTS: Overall coverage at 1 year of age was high (>95%) for Bacillus Calmette-Guérin (BCG), all three pentavalent diphtheria-pertussis-tetanus-haemophilus influenzae B-hepatitis B (DPTHH) doses and all polio doses except polio at birth (63%). Coverage against measles and yellow fever was 85%. Median delay for BCG was 1.7 weeks. For polio at birth, the median delay was 5 days; all other vaccine doses had median delays of 2-4 weeks. We found substantial health inequity across all socio-economic indicators for all vaccines in terms of timeliness, but not coverage at 1 year. For example, for the last DPTHH dose, the proportion of children delayed more than 8 weeks were 27% for urban children and 31% for rural children (P < 0.001), 21% in the wealthiest quintile and 41% in the poorest quintile (P < 0.001), and 9% in the most educated group and 39% in the least educated group (P < 0.001). However, 1-year coverage of the same dose remained above 90% for all levels of all socio-economic indicators. CONCLUSIONS: Ghana has substantial health inequity across urban/rural, socio-economic and educational divides. While overall coverage was high, most vaccines suffered from poor timeliness. We suggest that countries achieving high coverage should include timeliness indicators in their surveillance systems.

The SES variables that have the greatest impact are educational level, income level, and cohabitation status. Significant disparities exist among racial and ethnic minorities in access to osteoporosis screening and treatment. In Spain, a higher risk of fractures has been described in people with a low-income level, residence in rural areas during childhood and low educational level. The civil war cohort effect is a significant risk factor for hip fracture. There is significant geographic variability in hip fracture care, although the possible impact of socioeconomic factors has not been analyzed. It would be desirable to act on socioeconomic inequalities to improve the prevention and treatment of osteoporotic fractures.

OBJECTIVE: To explore menstrual knowledge, menstrual management, the use of menstrual products, the prevalence of menstrual poverty and to assess the acceptability of a menstrual equity intervention among students in the fourth grade of compulsory secondary education in Catalonia (Spain). METHOD: Post-intervention mixed-methods study (cross-sectional study and qualitative study with focus groups) with a critical and gender perspective. It was conducted between July 2022 and March 2023. Descriptive and bivariate statistical analyses stratified by gender were carried out. Qualitative data were analysed using thematic analysis. RESULTS: Women and people who menstruate rated the intervention favourably, while some men were reluctant. The intervention promoted the use of some reusable menstrual products, although some barriers to use menstrual cups were identified. Participants reported institutional barriers to menstrual management in the school setting and 19.4% stopped attending school during menstruation in the 6 months prior to the study. Between 10.9-16.4% reported menstrual poverty in the 6 months prior to the study, and 29,0% took actions to reduce the environmental impact of menstrual products. CONCLUSIONS: This study highlights the need for co-designing menstrual interventions that consider gender dynamics and sexist attitudes with students, as well as targeting it to teachers. The provision of reusable menstrual products can be helpful in promoting their use, although accompaniment should be provided. In parallel, it is crucial to strengthen menstrual education, as well as to reduce menstrual poverty and school absenteeism during menstruation.

OBJECTIVE: To delve deeper from a gender perspective into the lessons learned during the COVID-19 pandemic to address future health crises. METHOD: Study with key informants with experience in public health and gender from the Ministerio de Sanidad, ministries of the autonomous communities, Institut Català de la Salut, Hospital de La Princesa, Escuela Andaluza de Salud Pública and Universidad País Vasco. SOURCE OF INFORMATION: individual open-ended questionnaire on health and health inequalities/gender inequalities related to COVID-19. After presenting the findings, the key informants group discussed them in a meeting until reaching a consensus on the lessons learned. RESULTS: The lack of clinical statistics by sex could compromise epidemiological surveillance, losing the opportunity to characterize the disease. The performance of essential services fell more on women, exhausting them with double and triple shifts; with the differences according to sex in the clinical presentation of COVID-19, and the criteria for hospitalization/admission to the intensive care unit, their access to health care decreased. Increased: gender violence and mental health problems; delaying recognition of the second effects of vaccines in women; partially due to information biases in clinical trials. The gender perspective was lacking in academic, healthcare, and health management areas. CONCLUSIONS: Women's gender dimensions determined their higher frequency of COVID-19 and played a fundamental role in its control. Broadly considering the lessons learned will strengthen prevention systems and be able to provide effective responses to future health crises.

BACKGROUND: Gender-based differences among cardiology professionals have been reported in North America and Europe. However, the perspective of Latin American cardiologists remains unexplored. OBJECTIVES: The objectives of the study were to analyze the gender gap perspective among cardiologists from Latin America. METHODS: A cross-sectional study using an online survey directed to Latin American cardiologists from five different cardiology societies. The survey included questions on demographic data, professional development in the field of cardiology, and perceived barriers in clinical practice. RESULTS: A total of 240 professionals were surveyed, of which 41.7% were women (100) and 58.3% were men (140). The majority of women tend to work in the subfields of clinical cardiology and cardiovascular imaging. Women were underrepresented as heads of departments, earn less, and report less work satisfaction than men. The barriers that female cardiologists face at their workplace include labor discrimination, sexual harassment, family-related concerns, and lack of career development. CONCLUSIONS: The survey points toward the prevalence of a gender gap among cardiologists in Latin America, which is primarily driven by labor discrimination, sexual harassment, family-related concerns, and lack of career development among female cardiologists. Actions aimed at addressing this issue should be considered by different parties.


ANTECEDENTES: Existen diferencias de género entre los profesionales en cardiología en Europa y Norteamérica. La perspectiva de este suceso en América Latina permanece inexplorado. OBJECTIVOS: Analizar la perspectiva de la diferencia de género entre cardiólogos en América Latina. METODOS: Estudio transversal en el que se usó un cuestionario en línea dirigido a profesionales en cardiología en América Latina. El cuestionario incluía datos demográficos, desarrollo profesional en el campo de la campo de la cardiología y la percepción de las barreras en la práctica clínica. RESULTADOS: un total de 240 profesionales fueron analizados, de los cuales 41.7% (100) fueron mujeres y 58.3% (140) fueron hombres. La mayoría de las mujeres suelen trabajar en el área de imagen cardiovascular. Se encontró subrepresentación del género femenino en jefaturas de departamento, tienen un menor salario y reportan menor satisfacción laboral en comparación con los hombres. Las barreas que enfrentan en su lugar de trabajo fueron discriminación laboral, acoso sexual y falta de desarrollo profesional. CONCLUSIONES: El cuestionario señala la prevalencia de la diferencia de género en América Latina, principalmente por discriminación laboral, acoso sexual y falta de desarrollo profesional. Se necesita tomar acciones interinstitucionales que vayan dirigidas a reducir y eliminar esta brecha.

OBJECTIVE: To perform an updated meta-analysis of cataract surgical coverage (CSC) data in Latin American studies to confirm that gender equity exists in terms of receiving cataract surgery. MATERIALS AND METHODS: A literature search of Rapid Assessment of Avoidable Blindness (RAAB) studies in Latin American published since 2011 was done. Older studies from countries that did not have newer data published were also included. Using summary original study data of CSC rates on an individual basis, a random effects model of meta-analysis was performed to evaluate the differences in CSC between men and women. RESULTS: Nineteen studies from 17 countries were included (Mexico data were pooled). The odds ratios at a visual acuity (VA) of <3/60 and <6/18 were 1.04 [95% confidence interval (CI): 0.82-1.32] and 1.04 (95% CI: 0.90-1.19), respectively, without heterogeneity. There were no significant gender differences for CSC at any VA level. CONCLUSIONS: This updated meta-analysis of CSC data from Latin American countries supports that gender inequity in terms of receiving cataract surgery is not an issue in the region. The results do not provide insight into gender inequity in terms of the quality of cataract surgery and other types of eye care services.


OBJETIVO: Realizar un metaanálisis actualizado de la cobertura de cirugía de catarata en estudios latinoamericanos para confirmar que existe equidad de género en términos del acceso a cirugía de catarata. MATERIAL Y MÉTODOS: Se realizó una búsqueda de la literatura de "Rapid Assessments of Avoidable Blindness (Encuestas Rápida de Ceguera Evitable)" publicados en Latinoamérica desde el 2011. Se incluyeron también estudios anteriores de países que no tenían publicados datos más recientes. Utilizando el compendio de datos de los estudios originales de la cobertura de cirugía de catarata (Cataract Surgery Coverage, CSC por sus siglas en inglés) en base individual, se realizó un modelo de efectos aleatorios de metaanálisis para evaluar las diferencias en la CSC entre hombres y mujeres. RESULTADOS: Se incluyeron 19 estudios de 17 países (los datos de México fueron agrupados). La oportunidad relativa (razón de probabilidades) para una agudeza visual (AV) de <3/60 y <6/18 fueron de 1.04 [95% Intervalo de confianza (IC): 0.82-1.32] y 1.04 (95% IC: 0.90-1.19), respectivamente, sin heterogeneidad. No hubo diferencias significativas para la CSC a ningún nivel de AV. CONCLUSIONES: Este metaanálisis actualizado de la CSC de Latinoamérica, confirma que no existe inequidad de género en términos de acceso a cirugía de catarata en esta región.

OBJECTIVE: To analyse the prevalence of unwanted loneliness in our context and its association with health from a life cycle and social inequalities perspective. METHOD: Cross-sectional study based on data from the Health Survey of the Basque Country (n = 7.228; response rate 79%). The prevalence of unwanted loneliness, its association with perceived health and mental health, and social inequalities according to social class were analysed. To analyse the association, age-adjusted prevalence ratios (PR) calculated from robust Poisson models were calculated. RESULTS: The feeling of unwanted loneliness affects 23.3% of men and 29.7% of women, being higher in older people and young adults, and in the most disadvantaged social classes. Feeling alone is associated with worse health, both general (PRa men 2.11, 95% confidence interval [95%CI] 1.77-2.51; PRa women 2.10, 95%CI 1.83-2.41) and mental (PRa men 3.95, 95%CI 3.15-4.95; RPa women 3.50, 95%CI 2.99-4.12). Social inequalities in loneliness, as well as the association between it and poor health, was greater among young adults. CONCLUSIONS: The results of the study have important implications for intervention in the reduction and prevention of unwanted loneliness, underlining the importance of the life cycle and social inequalities.

In most European countries, facemasks use is recommended or mandatory in enclosed spaces where physical distancing is not possible. In Spain, this measure was first extended to open public spaces and later made mandatory regardless of whether or not the interpersonal safety distance can be kept. At present, there is no evidence on the effectiveness of universal masking of healthy people in the community to prevent infection with respiratory viruses, including SARS-CoV-2. The mandatory use of masks poses some ethical questions. Firstly, it entails a paternalistic action. Secondly, application of the principle of precaution becomes questionable when there is no clear benefit-risk relationship. Thirdly, compulsoriness can interfere with equity of public health actions. Fourthly, it can result in social stigma and discrimination against those who do not wear one, even though they well may have good reasons for doing so. Lastly, this measure may generate confusion in the population, along with an altered perception of the risk. The World Health Organization recommends its use in public places with a high potential risk of transmission and where other prevention measures, such as physical distancing, are not possible. Mandatory use of masks in public open spaces, regardless of the risk of transmission or of whether or not the interpersonal safety distance can be kept, is an intrusive measure that restricts individual freedoms, and would not appear to be justified on the basis of available scientific evidence. What we need are recommendations explaining where, when, how and what type of mask to wear.

OBJECTIVE: To compare worldwide differences in scientific production on fatherhood and motherhood (quantity, start time and evolution over time), to determine the international geographic distribution of articles on fatherhood, and, to examine the relationship between the human development index (HDI), the global gender gap index (GGGI), and the number of articles on fatherhood. METHOD: Descriptive analysis of articles on fatherhood and motherhood from the Scopus database 1788-2016, and longitudinal analysis 2006-2015 of the relationship between scientific production on fatherhood, the HDI and the GGGI, by means of a multilevel model with Poisson distribution and extra-Poisson parameter. RESULTS: We observed four times fewer articles on fatherhood than on motherhood. Articles on fatherhood were developed later than those on motherhood, and most (85%) were published in the last two decades, when they increased more than articles on motherhood. We identified geographical inequalities, with North America, Europe and Oceania leading the way. There is a statistically significant relationship between the increase in the HDI and the GGGI in world countries, and the increase in the articles on fatherhood. CONCLUSIONS: Socially built knowledge around fatherhood and motherhood is unequal. It is essential to develop non-parcelled, undivided and non- reductionist knowledge in the reproductive field. It is necessary to make men visible as fathers in the scientific sphere, to break gender stereotypes, and to incorporate childrearing co-responsibility in social policies and practices as a matter of right.

RESUMEN La inasistencia a citas dentales es un problema relevante en la atención sanitaria debido a las consecuencias negativas que implica. El presente artículo tiene como objetivo revisar las causas de la inasistencia y las estrategias implementadas para su prevención a nivel internacional. Los factores socioeconómicos, culturales, geográficos y demográficos son identificados como las principales causas del ausentismo. Se ha observado una relación inversamente proporcional entre el nivel socioeconómico y el ausentismo, donde aquellos pacientes con mayores recursos tienden a tener una menor inasistencia. El factor socioeconómico y cultural, junto con la educación en salud oral, influyen en la preocupación y la frecuencia de los controles dentales. Se han propuesto diversas soluciones, como el recordatorio telefónico o mediante mensajes de texto (SMS), aunque su efectividad puede variar debido a la familiaridad y el acceso de los pacientes a la tecnología. Se requiere abordar este problema de manera integral y considerar diferentes enfoques para reducir la inasistencia a citas dentales.


ABSTRACT Non-attendance at dental appointments is a relevant problem in health care due to the negative consequences it entails. This article aims to review the causes of nonattendance and the strategies implemented for its prevention at the international level. Socioeconomic, cultural, geographic, and demographic factors are identified as the main causes of absenteeism. An inversely proportional relationship has been observed between socioeconomic status and absenteeism, with those patients with greater resources tending to have lower absenteeism. Socioeconomic and cultural factors, together with oral health education, influence the concern and frequency of dental checkups. Various solutions have been proposed, such as telephone or SMS reminders, although their effectiveness may vary due to patient familiarity and access to technology. There is a need to address this problem comprehensively and consider different approaches to reduce missed dental appointments.


RESUMO A não comparência às consultas de medicina dentária é um problema relevante nos cuidados de saúde pelas consequências negativas que acarreta. Este artigo tem como objetivo rever as causas do absentismo e as estratégias implementadas para a sua prevenção a nível internacional. Os fatores socioeconómicos, culturais, geográficos e demográficos são identificados como as principais causas do absentismo. Foi observada uma relação inversamente proporcional entre o estatuto socioeconómico e o absentismo, sendo que os doentes com mais recursos tendem a ter menos absentismo. Os fatores socioeconómicos e culturais, juntamente com a educação para a saúde oral, influenciam a preocupação e a frequência das consultas dentárias. Têm sido propostas várias soluções, como lembretes por telefone ou SMS, embora a sua eficácia possa variar devido à familiaridade do paciente e ao acesso à tecnologia. É necessário abordar este problema de forma abrangente e considerar diferentes abordagens para reduzir a não comparência às consultas dentárias.

RESUMO

Social determinants of health can mean difficulties in access to diagnosis, management and adequate treatment of IBD. Income, educational level, age, sex, and rurality, among other factors, can influence the health outcomes of specific populations, such as IBD patients. It is essential to be aware of these gaps and advocate for public policies that address socioeconomic disparities and promote a healthy environment for all people. This includes providing equitable access to quality healthcare and supporting patients in managing their disease.


Los determinantes sociales de la salud pueden significar dificultades en el acceso a diagnóstico, manejo y trata- miento adecuado de la EII. El ingreso, el nivel educacional, la edad, el sexo y la ruralidad, entre otros factores pueden influir en los resultados en salud de poblaciones determinadas, como los pacientes con EII. Es fundamental, conocer estas brechas y abogar por políticas públicas que aborden las disparidades socioeconómicas y promuevan un entorno saludable para todos. Esto incluye brindar acceso equitativo a la atención médica de calidad y apoyar a los pacientes en la gestión de su enfermedad.

Resumen Introducción : El objetivo de este estudio fue exami nar cómo la variabilidad geográfica y los determinantes sociales de la salud influyen en la tasa de letalidad por COVID-19 en Bariloche. Métodos : Se utilizó una base de datos del Sistema Nacional de Vigilancia Epidemiológica para analizar los casos positivos de COVID-19 desde enero de 2020 hasta diciembre de 2021. Los datos se geo-codificaron y se incorporaron en un sistema de información geográfica (SIG). Se aplicó un marco de análisis en tres pasos para medir la inequidad en salud, utilizando indicadores socioeconómicos y de acceso a servicios. Se realizó un análisis multivariado para predecir la letalidad. Resultados : Se diagnosticaron un total de 25 020 casos de COVID-19 en Bariloche durante el período de estudio. La letalidad fue del 2.1%. Se observó una variabilidad significativa en indicadores socioeconó micos entre las diferentes delegaciones territoriales de la ciudad. Discusión : Los resultados mostraron inequida des en salud y una asociación entre determinantes sociales y letalidad por COVID-19 en Bariloche. Las personas que vivían en áreas con mayor vulnerabili dad socioeconómica presentaron un mayor riesgo de mortalidad. Estos hallazgos resaltan la importancia de abordar las inequidades en salud en la respuesta a una pandemia.


Abstract Introduction : The objective of this study was to ana lyze the geographic variability and the relationship between social determinants of health and COVID-19 lethality in Bariloche. Methods : A database from the National Epidemiologi cal Surveillance System was used to analyze COVID-19 positive cases from January 2020 to December 2021. The data were geocoded and incorporated into a geo graphic information system (GIS). A three-step analytical framework was applied to measure health inequity, us ing socioeconomic indicators and access to services. A multivariate analysis was conducted to predict fatality. Results : A total of 25 020 COVID-19 cases were diag nosed in Bariloche during the study period. The fatality rate was 2.1%. Significant variability in socioeconomic indicators was observed among different territorial delegations of the city. Discussion : The results showed health inequities and an association between social determinants and COVID-19 lethality in Bariloche. Individuals living in areas with higher socioeconomic vulnerability had a higher risk of mortality. These findings highlight the importance of addressing health inequities in a pan demic response.

Objetivo: Profundizar desde la perspectiva de género en las lecciones aprendidas durante la pandemia de COVID-19 para hacer frente a futuras crisis de salud. Método: Estudio con informantes clave con experiencia en salud pública y género del Ministerio de Sanidad, Consejerías de las comunidades autónomas, Institut Català de la Salut, Hospital de La Princesa, Escuela Andaluza de Salud Pública y Universidad del País Vasco. Fuente de información: cuestionario individual de respuestas abiertas sobre inequidades o desigualdades de género sanitarias y en salud relacionadas con la COVID-19. Tras presentar los hallazgos, el grupo de informantes clave se reunió y debatió hasta llegar a un consenso sobre las lecciones aprendidas. Resultados: La falta de estadísticas clínicas por sexo pudo comprometer la vigilancia epidemiológica, perdiendo una oportunidad para caracterizar la enfermedad. El desempeño de los servicios esenciales recayó más en las mujeres, extenuándolas con dobles y triples jornadas, lo cual, junto con las diferencias según sexo en la presentación clínica de la COVID-19 y los criterios de hospitalización y de ingreso en la unidad de cuidados intensivos, disminuyó su acceso a la atención sanitaria. Aumentaron la violencia de género y los problemas de salud mental, retrasando el reconocer los efectos secundarios de las vacunas en las mujeres parcialmente por sesgos de información en los ensayos clínicos. La perspectiva de género faltó en los ámbitos académicos, asistenciales y de gestión sanitaria. Conclusiones: Las dimensiones de género de las mujeres determinaron su mayor frecuencia de COVID-19 y desempeñaron un papel fundamental en su control. Considerar las lecciones aprendidas fortalecerá los sistemas de prevención y permitirá poder dar respuestas eficaces a futuras crisis de salud.(AU)


Objective: To delve deeper from a gender perspective into the lessons learned during the COVID-19 pandemic to address future health crises. Method: Study with key informants with experience in public health and gender from the Ministerio de Sanidad, ministries of the autonomous communities, Institut Català de la Salut, Hospital de La Princesa, Escuela Andaluza de Salud Pública and Universidad País Vasco. Source of information: individual open-ended questionnaire on health and health inequalities/gender inequalities related to COVID-19. After presenting the findings, the key informants group discussed them in a meeting until reaching a consensus on the lessons learned. Results: The lack of clinical statistics by sex could compromise epidemiological surveillance, losing the opportunity to characterize the disease. The performance of essential services fell more on women, exhausting them with double and triple shifts; with the differences according to sex in the clinical presentation of COVID-19, and the criteria for hospitalization/admission to the intensive care unit, their access to health care decreased. Increased: gender violence and mental health problems; delaying recognition of the second effects of vaccines in women; partially due to information biases in clinical trials. The gender perspective was lacking in academic, healthcare, and health management areas. Conclusions: Women's gender dimensions determined their higher frequency of COVID-19 and played a fundamental role in its control. Broadly considering the lessons learned will strengthen prevention systems and be able to provide effective responses to future health crises.(AU)

Introducción. A lo largo de los años se ha observado una importante brecha de género en la especialidad de ortopedia y traumatología. Su persistencia en otros países sugiere una evolución similar en Colombia. Objetivo. Determinar la distribución por sexo de los residentes de ortopedia y traumatología en Colombia en tres momentos fundamentales: la inscripción al proceso de selección, la admisión a la especialidad y la terminación del programa académico y obtención del correspondiente título. Metodología. Estudio transversal en el que se analizaron los datos registrados entre 2001 y 2021 en el Sistema Nacional de Información de la Educación Superior - SNIES para los programas de posgrado de ortopedia y traumatología. Resultados. En comparación con 2001, la proporción de mujeres egresadas de especialidades quirúrgicas en 2021 aumentó (26,56% vs. 44,84%). En el caso de los programas de postgrado en ortopedia y traumatología, entre 2007 y 2021, 16 979 individuos se postularon al proceso de admisión, 975 fueron admitidos y 702 se graduaron como ortopedistas, siendo la proporción de mujeres en cada etapa 22,47%, 22,56% y 16,80%, respectivamente. Además, en relación con el número de admitidos estratificado por sexo, el porcentaje de mujeres que se graduaron como ortopedistas fue notablemente menor que en los hombres (53,64% vs. 77,35%).Conclusiones. A pesar del aumento en la proporción de mujeres egresadas de las especialidades médicas, en el caso de ortopedia y traumatología, las mujeres siguen teniendo una participación muy baja en términos de aspirantes inscritos, admitidos y egresados


Introduction. Over the years, a significant gender gap has been observed in the orthopedics and trauma specialty. This gap has persisted in other countries, suggesting a similar trend in Colombia. Objective. To establish the gender distribution of orthopedics and trauma residents in Colombia at three critical moments: enrollment in the selection process, admission to the specialty, and completion of the academic program and awarding of the corresponding degree.Methodology. Cross-sectional study that analyzed the data registered between 2001 and 2021 in the National Information System for Higher Education (SNIES by its Spanish acronym) for postgraduate programs in orthopedics and trauma.Results. In 2021, the proportion of female graduates from surgical specialties increased compared to 2001 (26.56% vs. 44.84%). Between 2007 and 2021, 16 979 people applied for postgraduate programs in orthopedics and trauma, 975 were admitted, and 702 graduated as orthopedists, with the proportion of women at each stage being 22.47%, 22.56%, and 16.80%, respectively. Furthermore, when admissions were stratified by gender, the percentage of women graduating as orthopedists was significantly lower than that of men (53.64% vs. 77.35%).Conclusions. Por otro lado, la provisión de dichos bienes y servicios por la vía de mecanismos de mercado, tiene ante sí, el inconveniente que supone el ambiente inflacionario de su economía, cuyo impacto en los precios limita el acceso del paciente a su consumo. El caso de la enfermedad cardiovascular ofrece algunas claves para la comprensión de este fenómeno en el medio venezolano.


Public-health models of a monopolistic nature, suchas the Venezuelan, bring with them trends in the rationing ofthe goods and services they provide to their users, as well as inthe reduction of their quality. On the other hand, the provisionof these goods and services by means of market mechanisms hasbefore it, in our environment, the enormous impediment posedby the inflationary environment of its economy, whose impacton prices limits the patient's access to consumption. The case of cardiovascular disease offers some keys to understanding thisphenomenon in the Venezuelan environment.

Abstract The sensory and affective impact of a photograph in a news story may mitigate the message of the headline and even the content of the story as a whole, with the potential to lead the reader to countless parallel stories. These stories can be imbricated to the theme explored by the media, but can also expand to several other issues, memories, events, authors unveiling to the reader other situational layers that transcend the sphere of simple reading leading to reflexivity. This text has, therefore, the intention to reflect at length about the inequities in health, especially in relation to the distribution of vaccines against Covid-19 on the planet, from an image published by France Presse in a story about a day with record application of doses of the vaccine against Covid-19 in India. Con 3 de cada 4 personas viviendo con sobrepeso u obesidad, la prevalencia de IMC elevado en Chile es de las más altas del continente, mostrando un patrón de distribución inequitativo mediado por determinantes estructurales que modelan el comportamiento en salud (seguridad social, nivel socioeconómico, educación, género, entre otros). Las características socioeconómicas del país, nación de ingresos altos con marcada inequidad, son poco comunes y representan un desafío adicional a la hora de diseñar intervenciones en salud. Una alta concentración de riqueza permite ser clasificado como país de ingresos altos aun cuando la mayor parte de la población pertenecería a una clase social vulnerable, cuyos ingresos se ven acompañados de recursos sociales y simbólicos que dificultan doblemente la adopción de un "estilo de vida" saludable. A pesar de las múltiples estrategias nutricionales implementadas, la prevalencia de sobrepeso y obesidad continúa en aumento. Se postula como gran responsable al insistente uso de modelos basados en elección y responsabilidad individual, que buscan modificar factores de riesgo conductuales (sedentarismo y alta ingesta calórica) sin neutralizar los determinantes estructurales que predisponen esa conducta. Favorablemente, la última Política Nacional de Nutrición reconoce la "determinación social de la alimentación", representando un cambio de paradigma que confiere cierto optimismo y cuya eficacia deberá ser evaluada en los próximos años.


With 3 out of 4 people living with overweight or obesity, the national prevalence of high BMI is among the highest on the continent, thus showing an inequitable distribution pattern mediated by structural determinants that shape health behavior (social security, socioeconomic status, education, gender, among others). The socioeconomic features of the country, a high-income nation with marked inequity, are unusual and represent an additional challenge when designing health interventions. A high concentration of wealth allows it to be classified as a high-income country even though most of the population would belong to a vulnerable social class, whose income is accompanied by social and symbolic resources that make it doubly challenging to adopt a healthy "lifestyle". Despite the multiple nutritional strategies implemented, the prevalence of overweight and obesity continues to increase. The insistent use of models based on individual choice and responsibility, which seek to modify behavioral risk factors (sedentary lifestyle and high caloric intake) without neutralizing the structural determinants predisposing this behavior, is postulated as highly responsible. Favorably, the latest National Nutrition Policy recognizes the "social determination of food", representing a paradigm shift that confers some optimism and whose effectiveness has to be evaluated in the coming years.
