BACKGROUND AND OBJECTIVE: Therapeutic patient education (TPE) is effective and essential in the context of the growing prevalence of chronic diseases, in which tools are needed for planning structured programs. The objective of this project was to develop guidelines for designing and assessing a TPE program. METHODS: 1) We assembled a multidisciplinary group of 8 leaders in TPE, chronicity, quality and safety from the hospital and the university. 2) We conducted an exhaustive review of the scientific literature on the planning of TPE programs directed at chronically ill patients, their relatives and caregivers. 3) The final text underwent comments and suggestions by participants from the hospital and primary care centre during a course on information and TPE methodology. The recommendations were unanimously agreed upon by the writing group. RESULTS: We obtained a standardised work procedure targeted at professionals involved in planning TPE programs, based on international recommendations. The document is structured into sections: a) Definition of the health problem and analysis of the situation; b) Program structure (human resources and materials); objectives (health-related, behaviour-related and educational) and methodology; c) Path the patient and family/caregiver follows in the program; and d) Assessment and indicators. The assessment of the procedure, in the framework of the methodology courses, was favourable. CONCLUSIONS: The methodology provided by this document serves as an instrument for the standardised and systematic planning of educational programs and unifies the criteria in their drafting. However, the document needs to be adapted to the condition and population to which each program is directed.

BACKGROUND AND PURPOSE: During the first wave of the epidemic caused by SARS-CoV-2, hospitals have come under significant pressure. This scenario of uncertainty, low scientific evidence, and insufficient resources, has generated significant variability in practice between different health organisations. In this context, it is proposed to develop a standards-based model for the evaluation of the preparedness and response system against COVID-19 in a tertiary hospital. MATERIALS AND METHODS: The study, carried out at the University Hospital of Vall d'Hebron in Barcelona (Spain), was designed in two phases: 1) development of the standards-based model, by means of a narrative review of the literature, analysis of plans and protocols implemented in the hospital, a review process by expert professionals from the centre, and plan of action, and 2) validation of usability and usefulness of the model through self-assessment and hospital audit. RESULTS: The model contains 208 standards distributed into nine criteria: leadership and strategy; prevention and infection control; management of professionals and skills; public areas; healthcare areas; areas of support for diagnosis and treatment; logistics, technology and works; communication and patient care; and information and research systems. The evaluation achieved 85.2% compliance, with 42 areas for improvement and 96 good practices identified. CONCLUSIONS: Implementing a standards-based model is a useful tool to identify areas for improvement and good practices in COVID-19 preparedness and response plans in a hospital. In the current context, it is recommended to repeat this methodology in other non-hospital and public health settings.

RESUMEN Introducción: Con el panorama cambiante de la educación médica, es crucial examinar críticamente los enfoques y marcos existentes. La educación médica basada en competencias (EMBC) surge como un prometedor cambio de paradigma, que prioriza el desarrollo de habilidades, conocimientos y actitudes explícitas centrado en el alumno y orientado a los resultados. Objetivo: Identificar los ajustes estructurales y procedimentales esenciales necesarios para una implementación eficaz de la EMBC. Se subraya la importancia de reorganizar las instituciones educativas, adaptar las técnicas de instrucción y evaluación y fomentar la adopción de la EMBC en el aula. Resultados: Para impartir con eficacia la EMBC, es imperativo reestructurar las instituciones educativas para que se centren en planes de estudios basados en competencias y en la enseñanza personalizada. Asimismo, es necesario modificar las estrategias de enseñanza y evaluación para facilitar la evaluación continua y el aprendizaje activo. Conclusiones: Adoptar los principios de EMBC puede permitir que la educación médica produzca profesionales equipados con las habilidades necesarias para satisfacer las demandas dinámicas de la medicina moderna.


ABSTRACT Introduction: With the changing landscape of medical education, it is crucial to critically examine existing approaches and frameworks. competency-based medical education (CBME) emerges as a promising paradigm shift, which prioritizes learner-centered, outcome-oriented development of skills, knowledge, and attitudes. Objective: To identify the essential structural and procedural adjustments needed for effective implementation of CBME. It underscores the importance of reorganizing educational institutions, adapting instructional and assessment techniques, and fostering acceptance of CBME in the classroom. Results: To effectively deliver CBME, it is imperative to restructure educational institutions to focus on competency-based curricula and personalized instruction. Also, there is a need to modify teaching and assessment strategies to facilitate continuous assessment and active learning. Conclusions: Adopting CBME principles can enable medical education to produce professionals equipped with the skills necessary to meet the dynamic demands of modern medicine.

Resumo Objetivo Compreender os desafios enfrentados pela educação permanente para o alcance da melhoria da qualidade e da segurança do paciente em um hospital público submetido à acreditação hospitalar. Métodos Estudo descritivo, transversal e com abordagem qualitativa. Realizaram-se entrevistas semiestruturadas com 22 profissionais, durando, em média, 22 minutos, as quais posteriormente foram analisadas e interpretadas por meio da análise de conteúdo temática de Bardin. Adotaram-se os softwares Iramuteq para a análise de corpus textual, e o BioEstat 5.3, para análise do perfil dos participantes. A coleta de dados ocorreu em junho de 2022, após aprovação nos Comitês de Ética em Pesquisa. Resultados Aplicou-se a análise de classificação hierárquica descendente, gerada pelo Iramuteq. Obtiveram-se três categorias: Desafios da Educação Permanente mediante o Processo de Melhoria Contínua; Educação Permanente para a Promoção da Qualidade e da Segurança do Paciente no Contexto da Acreditação Hospitalar; e Estratégias Educativas para a Melhoria da Qualidade e da Segurança do Paciente. Conclusão Identificaram-se desafios inerentes às ações de educação permanente em saúde, tais como resistência à mudança de cultura, adesão às atividades, alta rotatividade de profissionais e dificuldade para liberação da equipe de enfermagem para participar das atividades relacionadas à demanda de trabalho.


Resumen Objetivo Comprender los desafíos enfrentados por la educación permanente para lograr mejorar la calidad y la seguridad del paciente en un hospital público sometido a acreditación hospitalaria. Métodos Estudio descriptivo, transversal y con enfoque cualitativo. Se realizaron entrevistas semiestructuradas a 22 profesionales, con duración promedio de 22 minutos, que luego se analizaron e interpretaron mediante el análisis de contenido temático de Bardin. Se utilizaron los softwares Iramuteq para el análisis de corpus textual y BioEstat 5.3 para el análisis del perfil de los participantes. La recopilación de datos se llevó a cabo en junio de 2022, después de la aprobación de los Comités de Ética en Investigación. Resultados Se aplicó el análisis de clasificación jerárquica descendente, generado por Iramuteq. Se obtuvieron tres categorías: Desafíos de la educación permanente mediante el proceso de mejora continua, Educación permanente para la promoción de la calidad y de la seguridad del paciente en el contexto de la acreditación hospitalaria, y Estrategias educativas para la mejora de la calidad y la seguridad del paciente. Conclusión Se identificaron desafíos inherentes a las acciones de educación permanente en salud, tales como resistencia a cambios de cultura, adherencia a las actividades, alta rotación de profesionales y dificultad de autorizar al equipo de enfermería para participar en las actividades relacionadas con la demanda de trabajo.


Abstract Objective To understand the challenges faced in terms of permanent education in health, for achieving quality improvements and patient safety at a public hospital undergoing hospital accreditation. Methods This was a descriptive, cross-sectional study with a qualitative approach. Semi-structured interviews were conducted with 22 professionals, lasting an average of 22 minutes. The interviews were subsequently analyzed and interpreted using Bardin's thematic content analysis. The software Iramuteq was used to analyze the textual corpus, and BioEstat 5.3 was used to analyze the profile of the participants. The data collection took place in June 2022, following approval by the Research Ethics Committees. Results The descending hierarchical classification analysis, generated by Iramuteq, was applied, resulting in three categories: Challenges of Permanent Education through the Continuous Improvement Process, Permanent Education for the Promotion of Quality and Patient Safety in the Context of Hospital Accreditation, and Educational Strategies for Improving Quality and Patient Safety. Conclusion Challenges inherent to the actions of permanent education in health were identified, such as resistance to cultural change, adherence to activities, high turnover of professionals, and difficulty in releasing the nursing team to participate in activities, due to work demand.

OBJECTIVES: Nursing work environments are key determinants of care quality. Our study aimed to evaluate the characteristics of nursing environments in primary care settings in the Canary Islands, and identify crucial components of such environments to improve quality. METHOD: We conducted a cross-sectional study in primary care organisations using the Practice Environment Scale - Nursing Work Index tool. We collected sociodemographic variables, scores, and selected the essential items conducive to optimal care. Appropriate parametric and non-parametric statistical tests were used to analyse relations between variables (CI = 95%, error = 5%). RESULTS: One hundred and forty-four nurses participated. The mean total score was 81.6. The results for the five dimensions included in the Practice Environment Scale - Nursing Work Index ranged from 2.25 - 2.92 (Mean). Twelve key items for quality of care were selected; six were positive in the Canary Islands, two were mixed, and four negative. 7/12 items were included in Dimension 2 (fundamentals of nursing). Being a manager was statistically associated with higher scores (p<.000). Years of experience was inversely associated with scores in the 12 items (p<.021). CONCLUSIONS: Nursing work environments in primary care settings in the Canary Islands are comparable to others previously studied in Spain. Areas to improve were human resources and participation of nurses in management decisions. Nurse managers must be knowledgeable about their working environments so they can focus on improvements in key dimensions.

Fundamento: constituyó una necesidad el incremento de acciones de preparación a los estudiantes de ciencias médicas para realizar la pesquisa "casa a casa", teniendo en cuenta que el municipio Santa Clara de la provincia Villa Clara reportó la mayor cantidad de casos de COVID-19, entre los primeros meses de 2021. Objetivo: determinar la efectividad de un programa de capacitación para estudiantes de ciencias médicas en las acciones de prevención contra la COVID-19, durante las pesquisas activas "casa a casa". Métodos: se elaboró y ejecutó un programa de capacitación a partir de la investigación-acción, insertado en las acciones de prevención realizadas por los estudiantes en los escenarios de la comunidad; fueron agrupados en 132 Consultorios Médicos de la Familia (CMF), pertenecientes a los seis policlínicos del municipio Santa Clara. La intervención se efectuó del 8 de marzo al 10 abril de 2021. El universo estuvo constituido por 809 estudiantes, quienes pesquisaron 126 475 personas. Resultados: al finalizar el programa, 508 estudiantes obtuvieron calificaciones de Muy Bien y 132 de Bien, con diferencias significativas entre el nivel de conocimientos antes y después de su aplicación. Conclusiones: el programa de capacitación demostró efectividad porque se modificó el nivel de conocimientos de los estudiantes que participaron, mejoraron sus conocimientos y habilidades para realizar acciones de prevención; ellos lograron la detección precoz de pacientes en riesgo y enfermos de COVID-19.


Background: the increase in preparation actions for medical science students to carry out the "house to house" research was a necessity, taking into account that Santa Clara municipality in Villa Clara province reported the highest number of COVID-19 cases, in the first months of 2021. Objective: to determine the effectiveness of a training program for medical science students in preventive actions against COVID-19, during active "house-to-house" investigations. Methods: a training program based on action research was developed and implemented, inserted in the prevention actions carried out by the students in community settings; they were grouped into 132 Doctor´s Offices, belonging to the six polyclinics of Santa Clara municipality. The intervention was carried out from March 8 to April 10, 2021. The universe consisted of 809 students, who investigated 126,475 people. Results: at the end of the program, 508 students obtained ratings of Very Good and 132 of Good, with significant differences between the level of knowledge before and after its implementation. Conclusions: the training program demonstrated effectiveness because the level of knowledge of the students who participated was modified, their knowledge and skills to carry out preventive actions improved; they achieved early detection of patients at risk and sick with COVID-19.

BACKGROUND: Quality improvement is an important component of hospital operations. AIM: To prioritise clinical quality and safety problems in Chilean hospitals according to their severity, frequency, and detectability. MATERIAL AND METHODS: The study was conducted between December 2018 and June 2019. To identify quality and safety problems, an exploratory study was conducted using an online survey aimed to those responsible for clinical quality and safety in Chilean hospitals. The survey was sent to 94 hospitals and completed by quality management personnel at 34 hospitals, yielding a total of 25 valid surveys for analysis. Based on the information gathered, a risk priority score was computed to rank the problems surveyed. Focus groups were held to find the root causes of the quality and safety problem with the highest risk priority score. RESULTS: The three highest risk priorities were:1 ineffective interprofessional communication,2 lack of leadership for addressing frequently recurring safety issues, and3 antimicrobial resistance due to inappropriate use of antibiotics. For the communication problem, the focus group found two main root causes: those due to personnel and those relating to the hospitals themselves. CONCLUSIONS: Hospitals can systematically use the proposed approach to categorize their main clinical quality and safety problems, analyze their causes, and then design solutions.


ANTECEDENTES: La mejora continua de la calidad es un componente importante en las actividades hospitalarias. OBJETIVO: Priorizar los problemas de calidad y seguridad en hospitales chilenos de acuerdo a su severidad, frecuencia y detectabilidad. MATERIAL Y MÉTODOS: Se efectuó un estudio exploratorio con una encuesta en línea para detectar problemas de calidad y seguridad, dirigida a quienes están a cargo de los problemas de calidad y seguridad en los hospitales. La encuesta fue enviada a 94 hospitales y respondida por los encargados de calidad y seguridad en 34 de ellos, lográndose 25 encuestas válidas para análisis. El estudio se llevó a cabo entre diciembre de 2018 y junio de 2019. Se diseñó una escala de prioridades de riesgo para determinar la importancia relativa de los problemas detectados. Se llevaron a cabo grupos focales para determinar las causas del problema más importante. RESULTADOS: En Chile, los problemas de calidad y seguridad más importantes son la falta de comunicación interprofesional, falta de liderazgo para abordar los problemas de seguridad y calidad, y resistencia a antibióticos debido a su uso inapropiado. Problemas relacionados al personal y relacionados al hospital fueron las causas primarias de la falta de comunicación. CONCLUSIONES: Los hospitales podrían utilizar este enfoque de forma sistemática para categorizar sus principales problemas de calidad y seguridad, analizar las causas y diseñar soluciones.

Introducción: La calidad de los datos facilita garantizar la fiabilidad de los estudios observacionales. Objetivo: Describir el aseguramiento y el control de calidad para mantener la fiabilidad y la validez del dato en un estudio de cohorte. Métodos: Presentar el manejo de datos implementado dentro de un seguimiento de enfermos renales crónicos cuya exposición fue un programa de protección renal comparado con el tratamiento convencional y su asociación con desenlaces clínicos. Se evaluó el cambio en la frecuencia de errores después de implementar el plan y la reproducibilidad del ingreso de registros a las bases de datos. Resultados: Se documentó una disminución progresiva en los errores cometidos en la captación de datos. El valor de Kappa entre los recolectores de la información para las variables clínicas más importantes fue 0,960 para la depuración de creatinina 150 mg/dL; 0,730 para la alteración del sedimento urinario; 0,956 para la asignación de estadio al ingreso. Los coeficientes de correlación intraclase para la identificación de las cifras de presión arterial sistólica fue 0,996; para la de presión arterial diastólica 0,993 y para los niveles de creatinina sérica al diagnóstico 0,995. Discusión: La calidad de los datos comienza con el reconocimiento de los retos y dificultades que implica su responsable captación, de ahí el aporte de la estandarización de los procesos y el personal que los lleve a cabo en forma idónea. Estudios evidencian que muchos procesos de mejora surgen en el desarrollo de la investigación sin protocolos preestablecidos. Conclusión: La reducción en la proporción y el tipo de error durante el proceso de captación de datos se debe a su identificación temprana y la corrección de instructivos, del instrumento de control de diligenciamiento y de la capacitación continua del personal. El análisis mostró una buena concordancia interevaluador.


Introduction: Data quality makes it easier to ensure that observational studies are reliable. Objective: To describe assurance and quality control to maintain data reliability and validity in a cohort study. Methodology: We present the data management strategies implemented in a study that followed patients of chronic kidney disease who were in a renal protection program and compared them with those undergoing conventional treatment to observe its association with clinical outcomes. We assessed the changes in error frequency after implementing the plan along with the reproducibility of the strategies for entering records into the databases. Results: We documented a progressive decrease of data collection errors. The Kappa values among data collectors for the most important variables were: 0.960 for creatinine clearance 150 mg/dl; 0.730 for urinary sediment alteration and 0.956 for stage allocation upon admission. The intraclass correlation coefficient for the identification of systolic blood pressure was 0.996; for diastolic blood pressure, the coefficient was 0.993 and for serum creatinine levels at diagnosis, the value was 0.995. Discussion: Data quality begins with the recognition of the challenges and difficulties involved in responsible data collection, hence the contribution of standardized processes and personnel to carry them out in a suitable manner. Studies show that many improvement processes arise in the development of research without pre-established protocols. Conclusion: The reduction in error ratio and type during the data collection process are the result of the early identification of erroneously entered or missing data, the correction of the guidelines for completing forms as well as of the instruments for detecting errors and continuous training of the staff. The analysis showed good inter-rater reliability.

Introducción: La evaluación de la gestión de calidad de los servicios de Enfermería se perfila como una herramienta útil para trazar estrategias de mejora de los procesos organizacionales y satisfacer las necesidades del cliente. Objetivo: Evaluar la gestión de calidad de los servicios de Enfermería. Métodos: Estudio cuantitativo, descriptivo, de corte transversal, en el Centro Internacional de Restauración Neurológica, La Habana, en el año 2022. Participaron 60 enfermeras con 5 o más años de servicio, pertenecientes a los servicios lesiones estáticas, restauración biológica cerebral, atención a cubanos, lesiones raquimedulares, trastornos del movimiento y neurocirugía. Se aplicó una encuesta adaptada al contexto hospitalario basada en el Modelo Europeo de excelencia European Foundation for Quality Management. Se utilizó la media como medida de resumen. Se consideró la evaluación punto fuerte (aceptable) cuando el valor real alcanzado fue igual o superior al deseado y área de mejora (no aceptable) cuando el valor real no alcanzó el deseado. Resultados: La puntuación global fue de 977,88 de 1000 puntos que exige el Modelo. Se detectaron dos áreas de mejora a expensas de los criterios 3 (implicar los grupos de interés) y 5 (gestionar el funcionamiento y la transformación). Se evidenciaron puntos fuertes con énfasis en una cultura de calidad y elevada capacitación en los profesionales. Conclusión: La evaluación de gestión de la calidad en los servicios de Enfermería se considera aceptable. Los hallazgos obtenidos permiten plantearse acciones de mejora para fortalecer la calidad de la atención que se brinda a los clientes. La evaluación de la gestión de calidad en los servicios de enfermería es una estrategia metodológica útil para identificar errores y ayudar a identificar el camino hacia la excelencia(AU)


Introduction: The evaluation of quality management of Nursing services is emerging as a useful tool to outline strategies to improve organizational processes and meet customer needs, Objective: To evaluate the quality management of nursing services. Methods: Quantitative, descriptive, cross-sectional, cross-sectional study at the International Center for Neurological Restoration, Havana, Cuba, in the year 2022. Sixty nurses with 5 or more years of service, belonging to the services Static Injuries, Biological Brain Restoration, Care for Cubans, Rachimedullary Injuries, Movement Disorders and Neurosurgery participated. A survey was applied, adapted to the hospital context based on the European Model of Excellence EFQM. The mean was used as a summary measure. The evaluation was considered a strong point (acceptable) when the actual value achieved was equal to or higher than the desired value, and an area for improvement (not acceptable) when the actual value did not reach the desired value. Results: The overall score was 977.88 out of 1000 points required by the Model. Two areas for improvement were detected at the expense of criteria 3 (involve stakeholders) and 5 (manage operation and transformation). Strengths were evidenced with emphasis on a culture of quality and high qualification of professionals. Conclusion: The evaluation of quality management in nursing services is considered acceptable(AU)

Participaron 60 enfermeras con 5 o más años de servicio, pertenecientes a los servicios lesiones estáticas, restauración biológica cerebral, atención a cubanos, lesiones raquimedulares, trastornos del movimiento y neurocirugía. Se aplicó una encuesta adaptada al contexto hospitalario basada en el Modelo Europeo de excelencia European Foundation for Quality Management. Se utilizó la media como medida de resumen. Se consideró la evaluación punto fuerte (aceptable) cuando el valor real alcanzado fue igual o superior al deseado y área de mejora (no aceptable) cuando el valor real no alcanzó el deseado. Resultados: La puntuación global fue de 977,88 de 1000 puntos que exige el Modelo. Se detectaron dos áreas de mejora a expensas de los criterios 3 (implicar los grupos de interés) y 5 (gestionar el funcionamiento y la transformación). Se evidenciaron puntos fuertes con énfasis en una cultura de calidad y elevada capacitación en los profesionales. Conclusión: La evaluación de gestión de la calidad en los servicios de Enfermería se considera aceptable. Los hallazgos obtenidos permiten plantearse acciones de mejora para fortalecer la calidad de la atención que se brinda a los clientes. La evaluación de la gestión de calidad en los servicios de enfermería es una estrategia metodológica útil para identificar errores y ayudar a identificar el camino hacia la excelencia.


Introduction: The evaluation of quality management of Nursing services is emerging as a useful tool to outline strategies to improve organizational processes and meet customer needs, Objective: To evaluate the quality management of nursing services. Methods: Quantitative, descriptive, cross-sectional, cross-sectional study at the International Center for Neurological Restoration, Havana, Cuba, in the year 2022. Sixty nurses with 5 or more years of service, belonging to the services Static Injuries, Biological Brain Restoration, Care for Cubans, Rachimedullary Injuries, Movement Disorders and Neurosurgery participated. A survey was applied, adapted to the hospital context based on the European Model of Excellence EFQM. The mean was used as a summary measure. The evaluation was considered a strong point (acceptable) when the actual value achieved was equal to or higher than the desired value, and an area for improvement (not acceptable) when the actual value did not reach the desired value. Results: The overall score was 977.88 out of 1000 points required by the Model. Two areas for improvement were detected at the expense of criteria 3 (involve stakeholders) and 5 (manage operation and transformation). Strengths were evidenced with emphasis on a culture of quality and high qualification of professionals. Conclusion: The evaluation of quality management in nursing services is considered acceptable.

Objetivo: construir uma matriz de análise para identificação dos problemas relacionados à cultura de segurança do paciente em uma unidade de pronto atendimento no sul do Brasil. Método: a primeira etapa, com delineamento transversal descritivo, ocorreu de julho a agosto de 2018, com enfermeiros, por meio da autoaplicação do instrumento "Safety Attitudes Questionaire - SAQ Short Form 2006". Na segunda etapa, com delineamento exploratório descritivo, as questões do SAQ passaram por análise interpretativa, realizada por enfermeiras dedicadas ao estudo da temática, em abril de 2021. Resultados: os problemas na cultura de segurança do paciente na unidade de pronto atendimento relacionaram-se à estrutura, processos de gestão e processos clínicos, principalmente de caráter organizacional. Conclusão: a utilização da análise de cadeia causal permitiu a construção de uma matriz de análise, facilitando a identificação dos principais problemas relacionados à cultura de segurança do paciente na unidade de pronto atendimento.


Objetivo: to develop an analysis matrix to identify problems related to the patient safety culture in an emergency care unit in southern Brazil. Método: the first stage, with a descriptive cross-sectional design, occurred from July to August 2018, with nurses, through the self-application of the instrument "Safety Attitudes Questionaire - SAQ Short Form 2006". In the second stage, with a descriptive exploratory design, the SAQ questions underwent an interpretative analysis, carried out by nurses dedicated to the study of the subject, in April 2021. Resultados: the problems in the patient safety culture in the emergency care unit were related to the structure, management processes and clinical processes, mainly of an organizational nature. Conclusión: the use of causal chain analysis allowed the construction of an analysis matrix, facilitating the identification of the main problems related to the patient safety culture in the emergency care unit.


Objective: to evaluate the clinical predictors for dry eye and the nursing diagnosis Risk of dry eye in postoperative patients in the Post-Anesthesia Care Unit. Method: cross-sectional study conducted between May and August, 2017. The sample was 157 adult patients undergoing elective surgical procedures. Data were analyzed using descriptive and inferential statistics. Results: the clinical diagnosis of dry eye prevailed in 85.35% of patients, and the nursing diagnosis Risk of dry eye in 14.65%. Statistical differences were observed between hyperemia, excessive tearing and insufficient Schirmer test with the diagnosis of ocular dryness in the right eye and with the diagnosis Risk of dry eye. In the left eye the Schirmer test was insufficient. Conclusão: hyperemia, excessive tearing and insufficient Schirmer test are relevant perioperative clinical predictors to promote preventive measures and/or early detection of dry eye.

ABSTRACT Objectives: to describe the Lean Six Sigma implementation process to improve the discharge process in a Brazilian health institution's ICU. Methods: prospective study following the Define-Measure-Analyse-Improve-Control project development method. This method consists of five phases, namely: project definition, measurement of the starting point and data collection, analysis of results, improvement in processes, and statistical control. Results: applying Lean Six Sigma methodology following the Define-Measure-Analyse-Improve-Control in the discharge process from the intensive care unit to the inpatient unit was effective in improving processes. This improvement represented a reduction in the mean patient transfer time to the inpatient unit from 189 minutes to 75 minutes, representing a 61% improvement in discharge time. Conclusions: this article demonstrates the effectiveness of applying Lean Six Sigma methodology to improve the discharge flow in a critical unit, resulting in time and waste reduction.


RESUMO Objetivos: descrever o processo de implementação do Lean Seis Sigma para melhoria do processo de alta em uma unidade de terapia intensiva brasileira. Método: foi realizado um estudo prospectivo, que seguiu o método de desenvolvimento de projetos intitulado DMAIC (Define-Measure-Analyze-Improve-Control). Este método constituiu cinco fases, sendo elas: a definição do projeto, mensuração do ponto inicial e coleta de dados, análise dos resultados, melhoria dos processos e controle estatístico. Resultados: a aplicação da metodologia Lean Seis Sigma foi efetiva para melhoria do processo de alta da unidade de terapia intensiva para a unidade de internação. Esta melhoria representou uma redução no tempo médio de alta de 189 para 75 minutos, totalizando uma melhoria de 61%. Conclusões: este artigo demonstra a efetividade da aplicação da metodologia Lean Seis Sigma para melhoria do fluxo de alta em uma unidade crítica, possibilitando ganhos na redução de tempo e desperdícios.


RESUMEN Objetivos: describir el proceso de implementación de Lean Six Sigma para mejorar el proceso de alta en una unidad de cuidados intensivos brasileña. Métodos: estudio prospectivo siguiendo el método de desarrollo de proyectos denominado DMAIC (Define-Measure-Analyze-Improve-Control). Este método consta de cinco fases, a saber: definición del proyecto, medición del punto de partida y recolección de datos, análisis de resultados, mejora en los procesos y control estadístico. Resultados: la aplicación de la metodología Lean Six Sigma fue efectiva para mejorar el proceso de alta de la unidad de cuidados intensivos a la unidad de hospitalización. Esta mejora representó una reducción en el tiempo promedio de alta de 189 a 75 minutos, totalizando una mejora del 61%. Conclusiones: este artículo demuestra la efectividad de la aplicación de la metodología Lean Six Sigma para mejorar el flujo de descarga en una unidad crítica, lo que resulta en la reducción de tiempo y desperdicio.

ABSTRACT Objectives. The aim of this initiative was to assess whether a novel training program - Understanding Stigma and Strengthening Cognitive Behavioral Interpersonal Skills - could improve primary health care providers' confidence in the quality of mental health care they provide in the Caribbean setting by using the Plan-Do-Study-Act rapid cycle for learning improvement. Methods. We conducted a prospective observational study of the impact of this training program. The training was refined during three cycles: first, the relevance of the program for practice improvement in the Caribbean was assessed. Second, pilot training of 15 local providers was conducted to adapt the program to the culture and context. Third, the course was launched in fall 2021 with 96 primary care providers. Pre- and post-program outcomes were assessed by surveys, including providers' confidence in the quality of the mental health care they provided, changes in stigma among the providers and their use of and comfort with the tools. This paper describes an evaluation of the results of cycle 3, the official launch. Results. A total of 81 participants completed the program. The program improved primary care providers' confidence in the quality of mental health care that they provided to people with lived experience of mental health disorders, and it reduced providers' stigmatization of people with mental health disorders. Conclusions. The program's quality improvement model achieved its goals in enhancing health care providers' confidence in the quality of the mental health care they provided in the Caribbean context; the program provides effective tools to support the work and it helped to empower and engage clients.


RESUMEN Objetivos. La finalidad de esta iniciativa fue determinar si un nuevo programa de capacitación, titulado Understanding Stigma and Strengthening Cognitive Behavioral Interpersonal Skills (Comprender la estigmatización y fortalecer las competencias interpersonales cognitivas y conductuales), podría mejorar la confianza de los prestadores de atención primaria de salud en la calidad de la atención de salud mental que proporcionan en los países del Caribe, mediante el uso del ciclo rápido Planificación-Realización-Estudio-Acción para la mejora del aprendizaje. Métodos. Realizamos un estudio de observación prospectivo sobre el impacto de este programa de capacitación. La formación se perfeccionó a lo largo de tres ciclos: en primer lugar, se evaluó la pertinencia del programa para la mejora de las prácticas en el Caribe; en segundo lugar, se llevó a cabo una capacitación piloto de 15 prestadores locales para adaptar el programa a la cultura y el contexto; en tercer lugar, en el otoño del 2021 se puso en marcha el curso con 96 prestadores de atención primaria. Los resultados anteriores y posteriores al programa se evaluaron mediante encuestas que incluían la confianza de los prestadores en la calidad de la atención de salud mental que proporcionaban, los cambios aparecidos en los prestadores en cuanto a la estigmatización, y su uso y conocimiento de las herramientas. En este artículo se describe una evaluación de los resultados del ciclo 3, correspondiente a la puesta en marcha oficial. Resultados. Un total de 81 participantes completaron el programa. El programa mejoró la confianza de los prestadores de atención primaria en la calidad de la atención de salud mental que proporcionaban a las personas que presentaban trastornos de la salud mental y redujo la estigmatización de las personas con este tipo de trastornos por parte de los prestadores de la atención. Conclusiones. El modelo de mejora de la calidad del programa logró sus objetivos en cuanto a mejorar la confianza de los prestadores de atención de salud en la calidad de la atención de salud mental que proporcionan en los países del Caribe; el programa aporta herramientas eficaces para sustentar esta tarea y ayudó a empoderar e involucrar a las personas usuarias.

O objetivo da iniciativa foi avaliar se um novo programa de capacitação, Compreensão do estigma e fortalecimento das competências cognitivas e comportamentais interpessoais, seria capaz de aumentar a confiança dos profissionais de atenção primária à saúde na qualidade da atenção à saúde mental que oferecem a pacientes no Caribe, utilizando o ciclo rápido Plan-Do-Study-Act (planejar, fazer, avaliar e agir) para melhorar o aprendizado. Métodos. Este foi um estudo observacional prospectivo sobre o impacto desse programa de capacitação. A capacitação foi aperfeiçoada ao longo de três ciclos. Inicialmente, avaliou-se a relevância do programa para o aprimoramento da prática no Caribe. A seguir, foi realizada uma capacitação-piloto de 15 profissionais locais para adaptar o programa à cultura e ao contexto. No terceiro ciclo, realizado no outono de 2021, o curso foi lançado, com a participação de 96 profissionais de atenção primária. Os resultados antes e depois do programa foram avaliados por meio de questionários, que incluíam a confiança dos provedores na qualidade da atenção à saúde mental oferecida, mudanças no estigma entre os profissionais e a utilização das ferramentas e o conforto dos profissionais em usá-las. Este documento apresenta a avaliação dos resultados do ciclo 3, o lançamento oficial do curso. Resultados. Oitenta e um participantes completaram o programa. O programa melhorou a confiança dos profissionais de atenção primária na qualidade dos cuidados de saúde mental que ofereciam às pessoas com experiência vivida relacionada a problemas de saúde mental. Além disso, reduziu a estigmatização das pessoas com problemas de saúde mental pelos profissionais de saúde. Conclusão. O modelo de melhoria da qualidade do programa atingiu suas metas de aumentar a confiança dos prestadores de serviços de saúde na qualidade dos serviços de saúde mental que prestavam no Caribe; o programa fornece ferramentas efetivas de apoio ao trabalho e ajudou a empoderar e envolver os clientes.

Resumo Enquadramento: Existe evidência de que a sistematização da abordagem terapêutica à pessoa com deglutição comprometida após o acidente vascular cerebral (AVC) tem um impacte significativo na redução de complicações. Objetivo: Conceber e implementar uma intervenção multimodal para a implementação da evidência, na sistematização da abordagem à pessoa com deglutição comprometida. Metodologia: Projeto de melhoria contínua da qualidade dos cuidados de enfermagem desenvolvido num serviço de internamento de reabilitação, em cinco fases: análise do modelo em uso, construção da intervenção, implementação, avaliação e partilha dos resultados. Para a recolha de dados utilizaram-se a análise à documentação, a auditoria e grupos focais. Resultados: Verificou-se um incremento de 39,59% nos focos de enfermagem identificados e de 45,33% de intervenções, assim como a transposição para a prática da evidência ao nível da avaliação, da sistematização das práticas, acompanhamento durante as refeições e higiene oral. Conclusão: Este projeto contribuiu para a transposição do conhecimento para a prática, nomeadamente para o sucesso do plano terapêutico da pessoa alvo dos cuidados, concretamente no âmbito da deglutição comprometida.


Abstract Background: Evidence shows that systematizing the therapeutic approach to patients with post-stroke dysphagia reduces complications significantly. Objective: To design and implement a multimodal intervention to systematize an evidence-based approach to patients with dysphagia. Methodology: A continuous quality improvement project for nursing care was implemented in a rehabilitation inpatient unit in five phases: analysis of the model in use, design of the intervention, implementation, assessment, and dissemination of results. Data were collected through documentary analysis, audits, and focus groups. Results: There was an increase of 39.59% in the identified nursing foci and 45.33% in interventions. The translation of evidence into practice was also observed at the level of assessment, systematization of practices, and monitoring during meals and oral hygiene. Conclusion: This Project contributed to translating knowledge into practice, particularly concerning the success of the therapeutic plan, specifically in terms of dysphagia, for patients being cared for.


Resumen Marco contextual: Está demostrado que la sistematización del enfoque terapéutico de las personas con trastorno de la deglución tras un accidente cerebrovascular (ACV) tiene un impacto significativo en la reducción de las complicaciones. Objetivo: Diseñar e implementar una intervención multimodal para aplicar la evidencia en la sistematización del enfoque de la persona con trastorno de la deglución. Metodología: Proyecto de mejora continua de la calidad de los cuidados de enfermería desarrollado en una unidad hospitalaria de rehabilitación, en cinco fases: análisis del modelo en uso, diseño de la intervención, implementación, evaluación y puesta en común de los resultados. Los datos se recogieron mediante análisis de documentos, auditorías y grupos de discusión. Resultados: Se observa un incremento del 39,59% en los focos de enfermería identificados y del 45,33% en las intervenciones, así como la transposición a la práctica de la evidencia a nivel de la evaluación, la sistematización de las prácticas, el seguimiento durante las comidas y la higiene bucodental. Conclusión: Este proyecto contribuyó a la transposición de los conocimientos a la práctica, es decir, al éxito del plan terapéutico para la persona atendida, concretamente en el contexto del trastorno de la deglución.

Resumo Objetivo Analisar as notificações de incidentes ocorridos durante a pandemia de COVID-19. Métodos Estudo com delineamento transversal de abordagem quantitativa do tipo descritivo exploratório. Foram analisadas 1.466 notificações à gerência de risco de um hospital privado, no período de setembro de 2020 a setembro de 2021. Utilizou-se a análise estatística descritiva, aplicando o teste Qui-quadrado de Pearson ou o teste da Razão de Verossimilhança. A margem de erro utilizada foi de 5%. Resultados Identificou-se como incidentes prevalentes a falha na comunicação (358 - 24,5%), falha no uso de sondas e cateteres (232 - 15,9%) e falha no uso de artigos e equipamentos (132 - 9,1%). A circunstância notificável totalizou (55,9%) dos relatos e destas, (33,4%) eram falha na comunicação. Os eventos adversos foram em número de 416 (28,6%) e a queda esteve relacionada a dano leve (43,9%); Infecção relacionada à assistência à saúde ao dano moderado (31%) e a falha no uso de medicamentos a (50%) como dano grave e óbito. Conclusão Falha na comunicação foi a circunstância de risco mais notificada, seguida de falha no uso de medicamentos como evento adverso com dano grave. A unidade de enfermaria evidenciou a possibilidade de maior número de eventos adversos; enquanto que nas unidades de terapia intensiva o grau de dano dos eventos adversos foi superior.

Las circunstancias que pueden ser notificadas totalizaron (55,9 %) de los relatos y, entre ellas, (33,4 %) era una falla en la comunicación. Los eventos adversos totalizaron 416 (28,6 %) y la disminución estuvo relacionada con el daño leve (43,9 %); infección relacionada con la atención a la salud al daño moderado (31 %) y a la falla al usar medicamentos (50 %) como daño grave y defunción. Conclusión La falla en la comunicación fue la circunstancia de riesgo más notificada, seguida de falla al usar medicamentos como evento adverso con daño grave. La unidad de enfermería evidenció la posibilidad de un número más elevado de eventos adversos; mientras que en las unidades de terapia intensiva el grado de daño de los eventos adversos fue superior.


Abstract Objective To review notification of incidents that occurred during the COVID-19 pandemic. Methods This is a cross-sectional, exploratory descriptive quantitative study. A total of 1,466 notifications to risk management of a private hospital were analyzed from September 2020 to September 2021. Descriptive statistical analysis was used, applying Pearson's chi-square test or the likelihood ratio test. The margin of error used was 5%. Results Communication failure (358 - 24.5%), probe and catheter use failure (232 - 15.9%) and article and equipment use failure (132 - 9.1%) were identified as prevalent incidents. The notifiable circumstance totaled 55.9% of reports, and, of these, 33.4% were communication failure. Adverse events were 416 (28.6%), and fall was related to mild damage (43.9%), health care-associated infections, to moderate harm (31%), and medication use failure (50%), to severe harm and death. Conclusion Communication failure was the most reported risk circumstance, followed by medication use failure as an adverse event with severe harm. The nursing unit showed the possibility of a greater number of adverse events, while in Intensive Care Units, the degree of harm from adverse events was higher.

RESUMEN Fundamento: es necesario incrementar la preparación de los estudiantes de ciencias médicas para la promoción de salud y prevención de la COVID-19. Objetivo: determinar la efectividad de un programa de capacitación dirigido a estudiantes de ciencias médicas para las acciones de prevención de la COVID-19en Santa Clara. Métodos: se realizó un estudio de intervención educativa a través de la investigación-acción en el periodo marzo-abril de 2021, en las áreas de salud del municipio de Santa Clara. Se utilizaron métodos teóricos para la fundamentación de la investigación, y empíricos: análisis documental y cuestionario antes y después de aplicado el programa, el cual fue valorado por cinco especialistas con más de 20 años de experiencia y categorías científica y docente. Se emplearon métodos matemático-estadísticos de cálculo porcentual y análisis descriptivo. Resultados: al finalizar la implementación del programa, 744 (91,9%) estudiantes se mostraron satisfechos con la capacitación recibida entre ellos 509 (62,9%) y 235 (29%) obtuvieron calificaciones de Muy Bien y Bien respectivamente, con diferencias significativas entre el nivel de conocimiento antes y después. Conclusiones: los estudiantes mejoraron su preparación para realizar acciones de promoción y prevención en las pesquisas casa a casa para la prevención de COVID 19 y manifestaron su satisfacción con las herramientas utilizadas. El programa fue valorado satisfactoriamente por los especialistas consultados.


ABSTRACT Background: it is necessary toin crease the preparation of medical ciences students for health promotion and prevention of COVID-19. Objective: to determine the effectiveness of a training program aimed at medical science students for COVID-19 prevention actions in Santa Clara. Methods: an educational intervention study was carried out through action research in the period from March to April 2021, in the health areas of the municipality of Santa Clara. Theoretical and empirical methods were used to support the research: documentary analysis and questionnaire before and after the program was applied, which was assessed by five specialists with more than 20 years of experience and scientific and teaching ranks. Mathematical-statistical methods of percentage calculation and descriptive analysis we reused. Results: at the end of the implementation of the program, 744 (91.9%) students were satisfied with the training received; among them, 509 (62.9%) and 235 (29%) obtained ratings of Very Good and Good, respectively, with significant differences between the level of know ledge before and after. Conclusions: the students improved their preparation to carry out promotion and prevention actions in house-to-house investigations for the prevention of COVID 19 and expressed their satisfaction with the tools used. The program was satisfactorily assessed by the specialists consulted.

RESUMEN Fundamento: el enfoque curricular por competencias favorece el aprendizaje activo de los discentes, lo que permite desarrollar competencias que pueden proyectarse más allá del aula. Objetivo: determinar el impacto de la aplicación de metodologías activas en el desarrollo de las competencias de trabajo colaborativo y aprendizaje conceptual en estudiantes de la carrera de Medicina. Métodos: se utilizó un diseño cuasiexperimental de corte longitudinal con un grupo control y tres grupos experimentales, realizado en 2019. La muestra fue 84 alumnos (cohortes 2018 y 2019) de la carrera de Medicina. Se aplicó metodologías activas a tres grupos experimentales y se utilizó la clase expositiva tradicional en el grupo control. El Autoinforme de Interacción Grupal (AIG) fue el instrumento utilizado para medir el trabajo colaborativo y el mapa conceptual para evidenciar el desarrollo del aprendizaje conceptual, con un de Alfa de Cronbach de 0,92 y 0,88, respectivamente. El análisis estadístico consideró la Prueba de Rangos de Friedman para más de dos muestras relacionadas. Resultados: la aplicación de las metodologías activas de intervención favorecieron el desarrollo del trabajo colaborativo y el nivel de aprendizaje conceptual en comparación al grupo control (metodología expositiva), el cual no logró resultados estadísticamente significativos (p> ,05). Conclusiones: el aprendizaje activo en los discentes fue favorecido por metodologías activas de intervención, que permitieron un desarrollo del aprendizaje conceptual con un mayor grado de reflexión en la medida en que se desarrolló el trabajo colaborativo, lo que a su vez se fortaleció en el tiempo.


ABSTRACT Background: the competency-based curricular approach favors the active learning of students, which allows them to develop competencies that can be projected beyond the classroom. Objective: to determine the impact of the application of active methodologies in the development of collaborative work skills and conceptual learning in Medicine students. Methods: a longitudinal quasi-experimental design was used with a control group and three experimental groups, it was carried out in 2019. The sample comprised 84 students (cohorts 2018 and 2019) of the Medicine degree. Active methodologies were applied to three experimental groups and the traditional expository class was used in the control group. The Group Interaction Self-Report (AIG) was the instrument used to measure collaborative work and the conceptual map to evidence the development of conceptual learning, with a Cronbach's Alpha of 0.92 and 0.88, respectively. Statistical analysis considered the Friedman Rank Test for more than two related samples. Results: the application of active intervention methodologies favored the development of collaborative work and the level of conceptual learning compared to the control group (expository methodology), which did not achieve statistically significant results (p> .05). Conclusions: active learning in students was favored by active intervention methodologies, which allowed a development of conceptual learning with a greater degree of reflection to the extent that collaborative work was developed, which in turn was strengthened over time.

Objetivo: avaliar retrospectivamente a incidência de lesão por pressão (LP) em uma UTI Geral em 2018, após aplicação da ciência de melhoria no Hospital Santa Teresa, Petrópolis, RJ. Método: estudo retrospectivo, descritivo e exploratório de natureza quantitativa, com dados de março a dezembro de 2018, sobre a incidência de LP e adesão as medidas implantadas após aplicação ciência da melhoria na UTI Geral São Judas Tadeu no Hospital Santa Teresa, Petrópolis, RJ. Resultados: identificadas 104 lesões em 59 pacientes e uma incidência de LP de 7,33%. A região sacra foi a mais acometida (27,03%). 50% das LP foram de estágio 2. 38,46% das lesões foram relacionadas a dispositivos médicos. A adesão ao bundle de cuidados ficou em 71,16% e a Escala de Braden em 4 horas em 89,4%. Conclusão: estudo demonstra uma menor incidência de LP comparada com os valores nacionais, melhoria significativa no processo de identificação dos estadiamentos, avaliação do risco e notificações das LP(AU)


Objective: to retrospectively evaluate the incidence of pressure injury (PI) in a General ICU in 2018, after application of improvement science at Hospital Santa Teresa, Petrópolis, RJ. Method: a retrospective, descriptive and exploratory study of a quantitative nature, with data from March to December 2018, on the incidence of PI and adherence to measures implemented after scientific application of improvement in the São Judas Tadeu General ICU at Hospital Santa Teresa, Petrópolis, RJ. Results: 104 lesions were identified in 59 patients and an incidence of PI of 7.33%. The sacral region was the most affected (27.03%). 50% of PI were stage 2. 38.46% of injuries were related to medical devices. Adherence to the care bundle was 71.16% and the 4-hour Braden Scale was 89.4%. Conclusion: study demonstrates a lower incidence of PI compared to national values, significant improvement in the process of identifying staging, risk assessment and reporting of PI.(AU)

Introducción. Las funciones del Comité de Ética Clínica (CEC) son educativas, normativas, consultivas, mediadoras y reflexivas. Como cualquier servicio de salud, las consultas de casos a un CEC deben ser sujetas a revisión y mejora de la calidad. Los objetivos fueron evaluar la factibilidad, la efectividad y la satisfacción de las recomendaciones ético-asistenciales dadas por el CEC, y evaluar su impacto en el equipo tratante y en la familia. Población y métodos. Estudio descriptivo, retrospectivo, cualicuantitativo, de casos clínicos presentados en el Comité de Ética Clínica del hospital desde el 1 de enero de 2013 hasta el 31 de diciembre de 2017, según datos del libro de actas, historias clínicas, registros de la Oficina de Comunicación a Distancia y entrevistas semiestructuradas al equipo de salud. Resultados. Se analizaron 108 casos (106 pacientes), 73 casos con encuesta y registros, y 35 solamente con registros. Los motivos principales más frecuentes de consulta fueron los siguientes: adecuación del esfuerzo terapéutico (46/42,6 %), compromiso neurológico grave (15/13,9 %), calidad de vida del paciente (11/10,2 %), conflictos entre el paciente, la familia y el sistema de salud (7/6,5 %), y rechazo del tratamiento por la familia (6/5,6 %). Se hallaron niveles altos de satisfacción (> 95 %) y niveles moderados de factibilidad (> 74 %) y efectividad (> 85 %). Solo en 50/108 casos (46,3 %) quedó registro en la historia clínica de la consulta al CEC y el 44 % de los profesionales opinó que la consulta tuvo un impacto positivo en la familia y en el paciente. Conclusiones. Los resultados contribuyeron a descubrir oportunidades de mejora, especialmente en la documentación y en la comunicación en el proceso de consulta.


Introduction. A clinical ethics committee (CEC) has educational, regulatory, advisory, mediation, and reflexive functions. As any health care service, the consults with the CEC should be subjected to review and quality improvement. The study objectives were to assess the feasibility, effectiveness, and satisfaction with the bioethical recommendations made by the CEC and assess their impact on the treating team and the patient's family. Population and methods. Descriptive, retrospective, qualitative, and quantitative study of clinical cases submitted to the hospital's CEC between January 1 st, 2013 and December 31, 2017 using data from the CEC minute book, medical records, registries from the Office for Remote Communication, and semi-structured interviews with health care team members. Results. A total of 108 cases (106 patients) were analyzed: 73 cases with survey and registries and 35 with registries only. The main most frequent reasons for consultation were adequacy of therapeutic effort (46/42.6%), severe neurological involvement (15/13.9%), patient's quality of life (11/10.2%), patient-family-health system conflict (7/6.5%), and family's refusal of treatment (6/5.6%). High levels of satisfaction (> 95%) and moderate levels of feasibility (> 74%) and effectiveness (> 85%) were observed. In only 50/108 cases (46.3%), the consultation with the CEC was registered in the medical record, 44% of health care providers stated that the consultation had a positive impact on the patient and their family. Conclusions. Results helped to establish improvement opportunities, especially in terms of documentation and communication in the consultation process.
