RESUMO

In this review, we would like to demonstrate the case of a 6-year-old girl who presented with progressive ankle pain and eventual inability to weight bear. She was shown to have primary acute osteomyelitis of the talus caused by Pasteurella canis, a commensal organism usually found in the oropharynx of dogs, despite the absence of any history of a dog bite or other zoonotic risk factors. We characterise the symptoms, signs, radiographic appearances and result of both the medical and surgical management, including a review of the literature. This review aims to increase awareness of this rare pathology and help guide other clinicians in accurately diagnosing and managing the condition.

Pasteurella canis is a Gram-negative facultative anaerobic bacterium that is a typical commensal of the oral cavity and enteric tract of domestic animals. Human infections caused by this species are most often associated with dog bites and rarely with other animal bites. In this report, we present a case of a 34-year-old man who developed P. canis soft tissue infection of the left hand after a cat bite. The patient was successfully treated by a surgical intervention and antibiotics. The species identification of the isolate was performed by a conventional automatic system and nucleic acid-based methods. The nucleic acid-based methods provide a powerful alternative to the conventional microbiological diagnostic of these bacteria.

The biotypic and genotypic features of Pasteurella canis isolated from dogs, cats, and humans were clarified by repetitive sequence-based fingerprinting and nucleotide sequences encoding trehalose-6-phosphate hydrolase (treC). Thirty P. canis and 48 P. multocida isolates were collected from dogs, cats, and humans to perform biotyping. The genotyping of P. canis by fingerprinting was followed by dendrogram construction. The whole-genome sequences (WGSs) were searched for the enzyme-coding nucleotide sequences around the main and adjacent loci constituting the operon. Full-length nucleotide sequences encoding the enzyme were determined using polymerase chain reaction and direct sequencing. Biotypic results were compared to the dendrogram and nucleotide sequence data. We observed a difference in trehalose fermentation with a positivity rate of 46.7%. Two (A-1/A-2) and three (B-1/B-2/B-3) clades were located on the dendrograms generated based on two repetitive sequence-based fingerprinting techniques, showing no association between trehalose fermentation and the clades. Based on the WGSs, two variants of the gene, namely, a 1,641 bp gene treC and a pseudogene (1,335 bp) of treC with its first 306 nucleotides deleted, were observed. Trehalose-positive isolates harbored treC, whereas trehalose-negative isolates lacked treC with or without the pseudogene. Our observations suggest biotypic and genotypic diversity among the P. canis isolates from animal and human hosts, with respect to trehalose fermentation and treC nucleotide sequences. This is the first report on the diversity of treC nucleotide sequences among these isolates.

Background: Comparative analysis of virulence factors (VFs) between Pasteurella canis and Pasteurella multocida are lacking, although both cause zoonotic infections. We determined the virulence-associated genome sequence characteristics of P. canis and assessed the toxin gene prevalence unique to P. canis among clinical isolates of P. canis and P. multocida. Methods: We selected 10 P. canis and 16 P. multocida whole-genome sequences (WGSs) from the National Center for Biotechnology database. The VFanalyzer tool was used to estimate P. canis-characteristic VFs. Amino acid sequences of VFs were compared with multiple-aligned sequences. The genome structure containing P. canis-characteristic and adjacent loci was compared to the corresponding P. multocida genome structure. After designing primer sequences and assessing their accuracy, we examined the gene prevalence of the P. canis-characteristic VFs using PCR among clinical isolates of P. multocida and P. canis. Results: Using VFanalyzer, we found virulence-associated cytolethal distending toxin (cdt)A-cdtB-cdtC loci common to all P. canis WGSs that were not found in P. multocida WGSs. Similarities in the multiple alignments of CdtA-CdtB-CdtC amino acid sequences were found among the 10 P. canis WGSs. Shared or similar loci around cdtA-cdtB-cdtC were identified between the P. canis and P. multocida genome structures. The PCR-based cdtA-cdtB-cdtC prevalence differed for P. canis and P. multocida clinical isolates. Conclusions: P. canis-specific cdtA-cdtB-cdtC prevalence was identified among clinical isolates. These three loci may be unique toxin genes and promising targets for the rapid identification of P. canis in clinical settings.

RESUMO

Pasteurella canis is a Gram-negative coccobacilli from the Pasteurellaceae family. The most common form of transmission to humans is a bite from a dog or a cat. We report a case of a 90-year-old woman who presented with septic arthritis in the right knee and bacteremia two weeks after a cat bite. The patient was treated with arthrocentesis and directed antimicrobial therapy. Human Pasteurella canis infection is a rare occurrence, making this a case of note.

Analyzing ambiguous bite marks using conventional morphological approaches to identify attackers is difficult; thus, applying molecular biological methods for identifying an attacker from their saliva is a possible approach in a forensic investigation. This study aimed to establish oral bacterial DNA-based human and canine saliva markers and develop a practical method for their discrimination. We considered Streptococcus oralis and Pasteurella canis as human and canine saliva marker candidates, respectively. Duplex bacterial DNA detection using melting curve analysis was designed and evaluated for forensic applicability using proof-of-concept experiments. S. oralis DNA was detected from human saliva samples from 30 out of 30 individuals, and P. canis DNA was detected from canine saliva samples from 73 out of 77 individuals (26 dog breeds). Additionally, both bacterial DNA markers were accurately detected from human blood-contaminated saliva samples and mock indistinct bite marks. Our results indicate that both bacterial DNA markers were sensitive, robust, and discriminating saliva markers. We consider that our duplex bacterial DNA examination is a simple, practical, and useful method for the detection of saliva from indistinct bite marks and discrimination between human and canine saliva.

Pasteurella multocida, which colonizes upper respiratory and digestive tracts, is a leading cause of respiratory diseases in many host species. Here, we describe a case of P. multocida pneumonia with hemoptysis. A 72-year-old female diagnosed with bronchiectasis with a 36-year history presented with a worsened infiltrative and granular shadow in the lower right lobe and lingular segment. Bronchial lavage fluid culturing suggested Pasteurella pneumonia. P. multocida was confirmed by 16S rRNA sequencing. The patient was readmitted to our hospital because of hemoptysis, and she was treated successfully with antibiotic therapy. The possibility of P. multocida infection must be considered in patients who own pets.

Osteomyelitis of metatarsal bones is a dreaded complication of foot ulcers of various etiologies, often resulting in amputation of part of the affected limb. In this article, a case is presented in which a spontaneous expulsion of the necrotic bone part occurred, after which the ulcer could heal completely.

Infective endocarditis, an infrequent clinical syndrome in dogs, is typically associated with nondescript clinical signs such as fever, malaise and loss of appetite. Although an uncommonly reported infection in dogs, Pasteurella canis is an emerging pathogen with increasing relevance in the human microbiology literature. The goal of this study is to detail the clinical presentation and microbiological findings associated with a novel causative agent of infective endocarditis in the dog. Diagnostic evaluation as well as conventional, automated and molecular microbiological methods are highlighted. The recent literature regarding P. canis and infective endocarditis in companion animals and humans is reviewed. Although an unusual etiologic agent of infective endocarditis, awareness of P. canis as a diagnostic possibility is crucial to accurate microbial surveillance.

RESUMO

Pasteurella species are a component of the normal oropharyngeal flora in canine animals. In humans, the skin and soft tissue are the most common sites for Pasteurella infection and, less commonly, the respiratory tract can also be affected. We report a case of pneumonia by Pasteurella canis in a chronic respiratory disease patient with a history of permanent contact with a dog.

INTRODUCTION: We describe a case of refractory Pasteurella Canis keratitis that initially failed antimicrobial treatment but was successfully treated with a conjunctival flap followed by a penetrating keratoplasty. CASE DISCUSSION: A 70-year-old male had persistent corneal edema related to previous herpes zoster keratouveitis with visual acuity of 20/400. An endothelial keratoplasty was performed with improvement in vision to 20/25. Several months after this surgery the patient developed a corneal infiltrate and cultures grew Pasturella canis. The infection persisted despite topical treatment with broad spectrum fortified vancomycin, tobramycin and fluoroquinolones. A conjunctival flap combined with corneal cryotherapy was performed to help mitigate symptoms and to allow the ulcer to heal. A penetrating keratoplasty was done several months later. DISCUSSION: The patient achieved pain relief immediately after the flap was placed. Medications were tapered off in the following few months after the procedure. A penetrating keratoplasty was performed 4 months later. The patient has remained almost entirely symptom free, and the graft remained clear one year after surgery. CONCLUSIONS: A keratitis caused by Pasteurella canis that is resistant to medical therapy can be treated successfully with a conjunctival flap and cryotherapy followed by a penetrating keratoplasty.

Resumen Se comunica el caso de una paciente que sufrió mordedura de perro. Solicitó atención médica inmediatamente en donde realizaron asepsia y antisepsia de la herida para posteriormente tratar la lesión. A las 12 horas la paciente comenzó a mostrar cambios de la coloración, aumento de temperatura, volumen y dolor de severa intensidad, por lo que acudió a nuestro hospital para valoración. Se tomaron cultivos y se realizó resonancia magnética, que mostró datos compatibles con infección de tejidos blandos y osteomielitis. En los cultivos se aisló Pasteurella canis. Se dio tratamiento con antibióticos orales con lo que se logró un desenlace favorable. El género Pasteurella lo constituyen cocobacilos gramnegativos, inmóviles, anaerobios facultativos. La cepa más aislada es P. canis posterior a mordeduras de perro y P. multocida y P. septicum en mordeduras de gato. La manifestación más frecuente es celulitis en el sitio de mordedura o arañazo. El diagnóstico se establece mediante métodos microbiológicos. El tratamiento de elección es la penicilina.


Abstract This paper reports a case of osteomyelitis due to Pasteurella canis after a dog bite. The patient requested medical care immediately after the bite, there were performed asepsis and antisepsis of the wound to later close the injury. After 12 hours, the patient's wound started changing colour with increase in temperature, volume and severe pain, so patient went to our hospital for evaluation. Cultures were taken, and magnetic resonance imaging showed images related to soft tissue infection and osteomyelitis. In cultures, Pasteurella canis was isolated. Treatment was given with oral antibiotics, achieving a favourable outcome. The genus Pasteurella are gram-negative, immobile, anaerobic facultative coccobacilli. The most isolated strain is P. canis after dog bites, and P. multocida and P. septicum in cat bites. The most frequent manifestation is cellulitis at the site of a bite or scratch. The diagnosis is made by microbiological methods. The treatment of choice is penicillin.

We report the case of a 56-year-old female patient, with a three-day history of hematemesis, melena, abdominal wall hematoma and epistaxis associated with thrombocytopenia and anemia. Idiopathic thrombocytopenic purpura was diagnosed and she was treated with dexamethasone for four days. The patient developed acute respiratory failure with signs of systemic inflammatory response. Blood and pleural fluid cultures grew Pasteurella canis. This is the first case, to our knowledge, of P. canis empyema associated with hemorrhagic septicemia without epidemiological background and the third case of septicemia caused by P. canis reported in the literature.


Comunicamos el caso de una mujer de 56 años de edad, con un cuadro clínico de tres días de evolución caracterizado por hematemesis, melena, hematoma en la pared abdominal y epistaxis, asociado a trombocitopenia y anemia. Con un probable diagnóstico de un púrpura trombocitopénico idiopático, se trató con dexametasona por cuatro días. Evolucionó con una insuficiencia respiratoria aguda con signos de respuesta inflamatoria sistémica, por un empiema pleural izquierdo con aislamiento de Pasteurella canis en hemocultivos y líquido pleural. Este es el primer caso, según nuestro conocimiento, de un empiema por P. canis asociado a una septicemia hemorrágica, sin antecedentes epidemiológicos; y tercero de una sepsis por P. canis publicado en el mundo.

La meningitis bacteriana es una enfermedad importante de distribución mundial, causa mayor y sustancial de mortalidad y morbilidad en países en desarrollo. La Organización Mundial de la Salud (OMS) sostiene que la meningitis es una de las diez afecciones principales del ser humano y debe ser considerada como una emergencia infectológica; por eso es fundamental reconocer que esta enfermedad es causa de muerte en niños de todo el mundo, sin distinción de raza, nivel económico o sociocultural. Se realizó una investigación de caso en menor de 53 días de nacido, que cumplía con los criterios clínicos y de laboratorio compatible con meningitis bacteriana, con el propósito de analizar y fortalecer la toma de decisiones en salud pública por parte de la secretaría local de salud del municipio de Valledupar (Colombia). Entre los hallazgos se encontró antecedentes infecciosos en el menor, coloración de Gram y cultivo de LCR, en el que se identificó cocobacilos Gram negativos, que fueron aislados como agente causal Pasteurella canis. Este estudio pretende sensibilizar a los prestadores de salud para que cuenten con personal altamente capacitado para brindar tratamientos adecuados y prevenir complicaciones en la meningitis bacteriana en niños, y así disminuir la posibilidad de secuelas o muerte, tanto en pacientes con compromiso inmunológico o sin este.


Bacterial meningitis is an important substantial worldwide disease, and a major cause of mortality and morbidity in developing countries. The World Health Organization (WHO) says that meningitis is one of the top ten diseases that affect humans and should be considered as an infectious diseases specialist emergency, it is essential to recognize that this disease causes death in children worldwide regardless of race, economic status or social and cultural. An investigation of the case in less than 53 days old, who met the clinical criteria and compatible laboratory with bacterial meningitis, in order to analyze and strengthen decision making in public health by the local health department came forward the municipality of Valledupar (Colombia). Among the findings infectious history was found in the lower, Gram stain and culture of CSF where Gram negative coccobacillary identified, isolated as a causative agent Pasteurella canis. This study aims to sensitize health care providers to have highly trained staff to provide adequate treatment and prevent complications of bacterial meningitis in children and lessen the possibility of sequelae or death, both in patients with and without immune compromise.
