RESUMO

This study explores the dual challenges of ageism and professional recognition faced by senior nurses in healthcare settings. Utilizing a phenomenological approach, we conducted semi-structured interviews with 20 registered nurses aged 50 and older in Abha City, Saudi Arabia. The analysis revealed four key themes: experiences of ageism, its impact on professional roles, discrimination and stigmatization, and the need for organizational support. Findings indicate that age-related biases lead to significant professional marginalization, reduced job satisfaction, and emotional distress among senior nurses. The study highlights the critical need for comprehensive policies, organizational support, and cultural changes to address ageism and recognize the valuable contributions of senior nurses. Promoting an inclusive work environment and equitable professional development opportunities can enhance the overall quality of care and job satisfaction for older nurses.

RESUMO

Over the last fifteen years or so in France, the deployment of peer professionals in mental health seems to have accelerated. The benefits of peer support have been widely demonstrated. It is based on the experiential knowledge acquired by certain patients who have recovered during their course of treatment, which can be passed on by positive identification, stimulating hope and motivation. These unique skills are then complemented by more theoretical knowledge sanctioned by a diploma that legitimises the future professionals and gives them access to undisputed recognition to work alongside pre-existing systems. However, it is not always easy to integrate them into health and social teams.

OBJECTIVE: Health research is an accessory activity for most health professionals in the national health service of Spain. One way to recognize your worth is through professional careers. The objective of this paper has been to compare how research activity is valued in the professional careers of the health services of the different regions of Spain. DESIGN: Review of agreements, resolutions and decrees that define the professional careers of each community collected from the corporate websites of health services and health ministries. SITE: Health Research and Innovation Commission of the Extremadura Health Service. PARTICIPANTS: Members of said commission. METHODS: The following was considered for comparison: if the research activity is valued in isolation from other concepts, if it is considered to advance in different levels, if it is a requirement for any of them, and the percentage of score assigned to the block to which it belongs. RESULTS: Almost all the regions consider research as merit at all career levels. The score of the research block is between 4 and 50% of overall. They are only considered independently, of others activities, in the Canary Islands, Castilla-La Mancha, Castilla-León, Cataluña, Madrid and Murcia. They are required to climb a certain level in the Canary Islands and Castilla-León. CONCLUSIONS: There is a high variability in the recognition of research activity between regions. The careers that best value research would be those of the Canary Islands and Castilla-León, which consider it an independent activity and it becomes a requirement. The regions that have a more negative approach are those that can be promoted to the highest level, without making any contribution to research.

Professional identities research in medical education has made significant contributions to the field. However, what comprises professional identities is rarely interrogated. This research tackles this relatively understudied component of professional identities research by understanding emergency medicine physicians' perspectives on the important elements that comprise their professional identities. Q-methodology was used to identify different clusters of viewpoints on professional identities; by extension, the core components that comprise emergency medicine physicians' professional identities are disclosed. Thirty-three emergency medicine physicians were recruited, through purposive sampling, from five hospitals across Taiwan. R software was used to analyse the Q-sorts, determine loadings on each viewpoint and formulate the viewpoint array. Analysis of interview data enhanced our understanding of these viewpoints. In total, twenty-five emergency medicine physicians loaded onto four distinct viewpoints, reflecting dominant perspectives of emergency medicine physicians' understanding of their professional identities. These distinct viewpoints demonstrated what emergency medicine physicians deemed significant in how they understood themselves. The viewpoints comprised: skills acquisition, capabilities and practical wisdom; coping ability and resilience; professional recognition and self-esteem; and wellbeing and quality of life. All viewpoints stressed the importance of trust between colleagues. These findings demonstrate the multitude of ways in which seemingly unified professional identities diverge across groups of individuals. An enhanced understanding of speciality work culture is gained. By understanding facets of professional identities, the development of future educational interventions and departmental initiatives, which might support key components of professional identities, can be explored.

Colloquium talks at prestigious universities both create and reflect academic researchers' reputations. Gender disparities in colloquium talks can arise through a variety of mechanisms. The current study examines gender differences in colloquium speakers at 50 prestigious US colleges and universities in 2013-2014. Using archival data, we analyzed 3,652 talks in six academic disciplines. Men were more likely than women to be colloquium speakers even after controlling for the gender and rank of the available speakers. Eliminating alternative explanations (e.g., women declining invitations more often than men), our follow-up data revealed that female and male faculty at top universities reported no differences in the extent to which they (i) valued and (ii) turned down speaking engagements. Additional data revealed that the presence of women as colloquium chairs (and potentially on colloquium committees) increased the likelihood of women appearing as colloquium speakers. Our data suggest that those who invite and schedule speakers serve as gender gatekeepers with the power to create or reduce gender differences in academic reputations.

CONTEXT: Physicians who migrate globally face a daunting series of time-consuming, labor- and resource-intensive procedures to prove their clinical competency before being allowed to practice medicine in a new country. ISSUES: In this commentary, we describe licensing barriers faced by physician-migrants based on the authors' experiences, and reflect also on rapidly implemented measures to address COVID-19 pandemic related workforce shortages. We offer recommendations for potential reductions in bureaucratic regulatory barriers that prohibit mobilization of international medical graduate talent. LESSONS LEARNED: Licensing boards and authorities should strive for standardized, competency-based basic professional recognition. Professional medical societies are well-positioned to guide such competency-based recognition as a more organized, international collaborative effort across specialties. The COVID-19 pandemic facilitated cross-state and international licensing in some regions, highlighting a key opportunity: streamlining professional recognition requirements is achievable.

BACKGROUND: Clinical ladder programs (CLPs) are often utilized by healthcare organizations to monitor and incentivize staff nurse development. Few studies exist related to this topic, yet implementation of CLPs can be an important factor in staff nurse retention and satisfaction, and therefore requires better understanding. AIM: To identify and examine the factors that contribute to a successful CLP. METHODS: A systematic search of the literature was conducted in PubMed and CINAHL. Studies were not limited by year and were included if the focus was CLP attributes. PRISMA and PICOT were used to guide the process. A matrix of the existing studies was used, and interrater reliability was established at 90% with consensus building for inclusion of studies by the research team. RESULTS: Twenty-nine studies were identified for inclusion. Most were program evaluation. Institution's organizational culture was identified as the overarching attribute contributing to a successful CLP. Common themes identified as important to a supportive organizational culture were (a) education and experience; (b) competence and critical thinking; (c) job satisfaction and retention; and (d) compensation and institutional cost. Each of these components can play a significant role in the overall success of a CLP. LINKING EVIDENCE TO ACTION: While further research of a higher caliber is needed, some recommendations for practice can be made: (a) CLP description and terminology needs to be consistent with intent, (b) mandatory CLP engagement has positive implications, and (c) implementation needs to be driven by bedside nurses and includes both continuous education and refinement of program requirements and incentives.

The aim of this study was to explore the factors influencing registered nurses' pursuit of postgraduate education in specialty nursing practice in Australia. Despite the increased requirement for postgraduate education for advanced practice, little has been reported on the contributory factors involved in the decision to undertake further education. The Nurses' Attitudes Towards Postgraduate Education instrument was administered to 1632 registered nurses from the Nurses and Midwives e-Cohort Study across Australia, with a response rate of 35.9% (n = 568). Data reduction techniques using principal component analysis with varimax rotation were used. The analysis identified a three-factor solution for 14 items, accounting for 52.5% of the variance of the scale: "facilitators," "professional recognition," and "inhibiting factors." Facilitators of postgraduate education accounted for 28.5% of the variance, including: (i) improves knowledge; (ii) increases nurses' confidence in clinical decision-making; (iii) enhances nurses' careers; (iv) improves critical thinking; (v) improves nurses' clinical skill; and (vi) increased job satisfaction. This new instrument has potential clinical and research applications to support registered nurses' pursuit of postgraduate education.

Appointed Director General of the International Council of Nurses (ICN) in February 2019, Howard Catton is committed to ensuring that the ICN can effectively represent nursing around the world. He firmly believes that nurses must be at the heart of health policy decisions. With a nursing diploma and a master's degree in labor relations, he held a number of nursing positions in England and the United States before joining the New Zealand Nurses Organisation. For ten years, he directed the political and international affairs of the UK's Royal College of Nursing, then led ICN's action during the Covid-19 pandemic, while positioning himself in favor of protecting and investing in the nursing profession.

Behavior analysts are not recognized or regulated as a distinct profession in Europe. For the most part, European behavior analysts adhered to the standards set by the U.S.-based Behavior Analyst Certification Board (BACB). However, the BACB certification has not been recognized officially in any European jurisdiction. The recent decision by the BACB to discontinue eligibility of non-U.S. residents to apply for the BCBA exam by the end of 2022 has brought the issue of professional regulation outside of the United States into sharp focus. This article offers a snapshot in time of professional recognition of behavior analysts in 21 European countries. It stems from the Erasmus+ funded EuroBA project and its Professional Advisory Group (PAG). The EuroBA project aims to develop common standards and competences for behavior analysts to facilitate national regulation and mutual recognition across Europe.

The COVID-19 pandemic tested the health system of Western Australia (WA) and the relatively new overarching legislative framework that guided the state-wide public health response to the pandemic. This study aimed to evaluate the experiences and roles of environmental health officers (EHOs) in WA during the management of COVID-19 and to identify any policy changes that are needed to facilitate the rapid deployment of EHOs during a future public health crisis. An online survey with 78 respondents was administered and analysis was both qualitative and quantitative. It was found that participants believed there was inadequate resourcing, workforce shortages, increased workloads, and a lack of recognition and integration of the profession within the COVID-19 response. Notably, more than 65% of the respondents believed they could have been better utilised during the pandemic. This research has demonstrated that the COVID-19 pandemic in WA had clear gaps in its processes for managing responses and resilience to pandemics. Policy recommendations outlining a more efficient and integrated delivery of environmental health services throughout the state during emergencies are discussed.

BACKGROUND: Identifying common factors that influence job satisfaction for midwives working in diverse work settings is challenging. Applying a work design model developed in organisational behaviour to the midwifery context may help identify key antecedents of midwives job satisfaction. AIM: To investigate three job characteristics - decision-making autonomy, empowerment, and professional recognition as antecedents of job satisfaction in New Zealand (NZ) midwives. METHODS: Latent multiple regressions were performed on data from Lead Maternity Carer (LMC) midwives n = 327, employed midwives n = 255, and midwives working in 'mixed-roles' n = 123. FINDINGS: We found that professional recognition is positively linked to job satisfaction for midwives in all three work settings. At the same time, decision-making autonomy and empowerment were shown to influence job satisfaction for midwives working as LMCs only. DISCUSSION: Our main finding suggests that the esteem generated from being acknowledged as an expert and valuable contributor by maternity health colleagues is satisfying across all work contexts. Professional recognition encompasses the social dimension of midwifery work and influences midwives job satisfaction. Decision-making autonomy and empowerment are task and relational job characteristics that may not be similarly experienced by all midwives to noticeably influence job satisfaction. CONCLUSION: Given that job satisfaction contributes to recruitment, retention, and sustainability, our findings show that drivers of job satisfaction differ by midwifery work context. We present evidence to support tailored efforts to bolster midwives job satisfaction, especially where resources are limited.

Genetic Counselling is essential for providing personalised information and support to patients with Rare Diseases (RD). Unlike most other developed countries, Spain does not recognize geneticists or genetic counsellors as healthcare professionals Thus, patients with RD face not only challenges associated with their own disease but also deal with lack of knowledge, uncertainty, and other psychosocial issues arising as a consequence of diagnostic delay. In this review, we highlight the importance of genetic counsellors in the field of RD as well as evaluate the current situation in which rare disease patients receive genetic services in Spain. We describe the main units and strategies at the national level assisting patients with RD and we conclude with a series of future perspectives and unmet needs that Spain should overcome to improve the management of patients with RD.

INTRODUCTION: Employer led apprenticeship trailblazer groups develop the occupational standard and end point assessment associated with a particular job or occupation. All degree apprenticeship programmes within England must align to these and for allied health professions, this is a new concept. Exploring stakeholder motivations for being part of the trailblazer groups is essential to inform subsequent curriculum design and implementation for this new model of education. METHODS: Eighteen semi-structured interviews were undertaken across the three radiography related (diagnostic, therapeutic and sonography) trailblazer groups using a pragmatic constructivist approach. RESULTS: Strong themes emerged around professional recognition, and conflicts between being 'educated' or 'trained', and between vocational and academic components of radiography training. Even within these pioneer groups there was a lack of understanding around degree apprenticeship programmes and their potential impact upon the workforce. Whilst the benefits on recruitment and retention of staff through widening participation were acknowledged, there were concerns around apprentice pay and mentorship. Evidence of professional protectionism was uncovered, balanced by professional pride and a strong desire for team working within and between institutions. CONCLUSION: The study highlighted an urgent need for further research and awareness raising for employers and practitioners, prior to implementation of radiography-related degree apprenticeships. IMPLICATIONS FOR PRACTICE: This is the first evaluation of stakeholder motivations related to the development of degree apprenticeships within the allied health professions. The results and recommendations for practise will inform the imminent implementation of degree apprenticeships for the three radiography related professions, facilitating a smooth transition to apprenticeships for clinical and education departments.

Nursing knowledge and innovations are disseminated primarily through peer-reviewed publications in scholarly journals. At the present time, there are approximately 250 journals in the profession of nursing, representing all specialties and an international, global focus. Of this group, 25 have been honored by induction into the Nursing Journal Hall of Fame, which was established by the International Academy of Nursing Editors in 2018. This article introduces the Hall of Fame and the journals that have been inducted to date. Hall of Fame journals have a sustained publication history of 50 years or more and represent excellence in nursing through the articles published and journal editorial leadership. Studying the history of this group of journals reveals trends within different specialties as well as the profession of nursing overall. For many of the journals, there were particular editors who were visionary and transformative. Knowing their stories is important for the historical record of nursing publication. Celebrating nursing journal history through the Hall of Fame and understanding the unique leadership role of nursing editors, past and present, is an important and fitting tribute to nursing knowledge during the Year of the Nurse and Nurse Midwife in 2020.

Objetivo: La investigación es una actividad accesoria para la mayoría de los profesionales sanitarios del Sistema Nacional de Salud. Una forma de reconocimiento de su valor es mediante las carreras profesionales. Fue nuestro objetivo comparar la valoración de la actividad investigadora entre las carreras profesionales de los servicios de salud de las distintas comunidades autónomas. Diseño: Revisión de los acuerdos, resoluciones y decretos que definen las carreras profesionales de cada comunidad recogidas de las webs corporativas de los servicios de salud y consejerías de sanidad. Sitio: Comisión de investigación e innovación en salud del Servicio Extremeño de Salud. Participantes: Miembros de la citada comisión. Métodos: Se realizó una comparación atendiendo si la actividad investigadora se valora de forma aislada de otros conceptos, si se considera para avanzar en distintos niveles, si para alguno es requisito y el porcentaje de puntuación asignada al bloque al que pertenece. Resultados: Casi todas las comunidades autónomas consideran la investigación como mérito en todos los niveles de carrera. La puntuación asignada al bloque que incorpora la investigación se sitúa entre 4 y 50%. Solo se considera de forma independiente en Canarias, Castilla-La Mancha, Castilla-León, Cataluña, Madrid y Murcia. Son requisito para subir de determinado nivel en Canarias y Castilla-León. Conclusiones: Existe una alta variabilidad en el reconocimiento de la actividad investigadora entre comunidades. Las carreras que mejor valoran la investigación serían las de Canarias y Castilla-León, que la consideran como actividad independiente y llega a ser un requisito. Con enfoques más negativos están aquellas en las que se pueden ascender, hasta el nivel más alto, sin realizar ninguna aportación en investigación.(AU)

Pesquisa qualitativa e descritiva cujo objeto de estudo é o reconhecimento e a valorização do trabalho docente de enfermagem no contexto econômico neoliberal. Objetivos: a) Descrever o processo de trabalho do docente de enfermagem a partir da inserção do modelo neoliberal; b) Analisar a percepção do docente de enfermagem acerca da valorização e do reconhecimento no trabalho a partir da introdução do modelo neoliberal; c) Discutir as repercussões no processo de trabalho e no processo saúde-doença de docentes de enfermagem mediante à percepção do reconhecimento e da valorização de suas atividades laborais.Salienta-se que o estudo foi aprovado pelo Comitê de Ética em pesquisa sob número de protocolo: 1.474.463.Os participantes desta pesquisa foram 27 docentes de universidades públicas federais do município do Rio de Janeiro. A coleta de dados ocorreu em junho de 2016 por meio de uma entrevista semiestruturada. Os dados da caracterização dos participantes foram analisados através de estatística simples. A técnica utilizada para o tratamento dos dados subjetivos foi a Análise Temática de Conteúdo, que fez emergir quatro categorias. I) Organização do trabalho docente: Aspectos subjetivos e objetivos da demanda laboral, em que se discutem as características dessa organização laboral e do processo de trabalho docente de enfermagem na atualidade. Elabora-se também uma análise acerca dos fatores facilitadores e dificultadores para a execução do trabalho docente. II) Reconfiguração do trabalho docente: antes e depois do neoliberalismo e da globalização, em que se discute as transformações do mundo do trabalho docente devido a inserção do modelo neoliberal, bem como as modificações no modo operatório desses profissionais proporcionadas por tal modelo. III) Reconhecimento do trabalho docente universitário: na contramão do contexto nacional, em que se analisa: i) a percepção do reconhecimento pelo docente, sendo mencionado que apesar da figura do professor ter perdido status, ainda confere prestígio social ser docente universitário. ii) Concepções, fatores e atitudes que influenciam no reconhecimento docente, na qual são discutidas algumas medidas que podem influenciar no reconhecimento deste coletivo profissional. IV) Novas configurações do mundo do trabalho docente e o processo saúde-doença dos trabalhadores, em que se analisa que, devido à configuração da organização do trabalho, esses docentes necessitam relegar a segundo plano o cuidado com a própria saúde, fato que repercute negativamente na saúde dos mesmos, sendo prevalentes doenças no âmbito físico como problemas osteomusculares e infecções devido à baixa imunidade, e da dimensão subjetiva como estresse, cansaço extremo e burnout. Concluiu-se que a elevada influência dos princípios neoliberais no trabalho docente de enfermagem repercute negativamente no processo de trabalho deste coletivo profissional, bem como na sua valorização e reconhecimento, deixando esses docentes vulneráveis ao adoecimento e ao sofrimento psicofísico.


Qualitative and descriptive research whose object of study is the recognition and appreciation of teaching nursing in the neoliberal economic context. Objectives: I) To describe the work process of the nursing faculty from the insertion of the neoliberal model; II) To analyze the perception of the nursing faculty about the appreciation and recognition they receive at work from the introduction of the neoliberal model; III) To discuss the repercussions in the work process and the health-disease process of teachers of nursing through the perception of recognition and appreciation of their labor activities. The study was approved by the Ethics in Research Committee under protocol number: 1474463. The participants in this study were 27 teachers of federal public universities in the city of Rio de Janeiro. Data collection took place in June 2016 through a semi-structured interview. The data from the characterization of the participants were analyzed by simple statistics. The technique used for the treatment of subjective data was content analysis, which sprouted four categories. I) Organization of teaching: subjective aspects and objectives of labor demand, which discusses the characteristics of work organization and teaching work process of nursing today. It also draws up an analysis of the facilitating and inhibiting factors for the implementation of teaching. II) Reconfiguration of the teaching job: before and after of neoliberalism and globalization, which discusses the transformations in the world of teaching due to the insertion of the neoliberal model, as well as the modifications in the method of operation of these professionals provided by such a model. III) Recognition of university teaching in the national context, examining: i) the perception of recognition for teaching, being mentioned that although the figure of the teacher has lost status, it still bestows social prestige for being a university lecturer. ii) conceptions, factors and attitudes that influence the teacher recognition, which discusses some measures that can influence the recognition of this professional group. IV) New settings of the teaching world and the health-illness of workers, which shows that, due to the configuration of work organization, these teachers need to relegate the care of their own health, a fact that has negative repercussions on their health and results in prevalent diseases in their physical realm such as osteomuscular problems and infections because of low immunity, and stress, extreme fatigue and burnout in the subjective dimension. It was concluded that the high influence of neoliberal principles in teaching nursing negatively affected the working process of this professional group, as well as their valuation and recognition, leaving these teachers vulnerable to illness and psychophysical suffering.

We implemented a questionnaire survey targeting mothers who are in child care and had participated in consultations regarding drugs and diseases. We examined the future roles of community pharmacists by exploring the mothers’ concerns and, anxieties about child cares and their backgrounds, and their expectations for profession of community pharmacist. Mothers have listed anxiety and concerns of child care about “dermatitis such as rash and atopic eczema”; “food allergies”; “infectious diseases such as measles, chicken pox, and mumps”; and “side effects of vaccination”. In addition, most of them indicated their own concerns and anxiety about “solutions to children’s illnesses.” Despite their anxieties and concerns, however, approximately 60% of the mothers have never consulted with community pharmacists. Among them, approximately a half of them indicated the following three reasons why they have never consulted with pharmacists: “I have nothing to talk about,”, “I do not know what I should talk about,”, and “I was not sure if it was alright to talk about my concerns.”. From these results, we concluded that community pharmacists in the future should improve their communication skills and inform their availability to consult about medicine and disease to local residents.
