RESUMO

As a result of digitalization in medicine wearable computing devices (wearables) are becoming increasingly more important. Wearables are small portable electronic devices with which the user can record data relevant to health, such as number of steps, activity profile, electrocardiogram (ECG), heart and breathing frequency or oxygen saturation. Initial studies on the use of wearables in patients with rheumatological diseases show the opening up of new possibilities for prevention, disease monitoring and treatment. This study provides the current data situation and the implementation of wearables in the discipline of rheumatology. Additionally, future potential fields of application as well as challenges and limits of the implementation of wearables are illustrated.

BACKGROUND: Freezing of gait (FOG) has been linked to increased numbers of steps taken while walking. We tested the hypothesis that an increased number of steps associated with FOG might predict the exacerbation of the severity of Parkinson's disease (PD). METHODS: We prospectively studied 26 patients. Clinical assessments were performed and balance was evaluated in 30 patients with Hoehn-Yahr stage III PD 6 years previously. Gait parameters were analyzed with the use of an originally designed, suddenly narrowed path. PD-related independent variables, balance investigation-related variables, and gait-independent-related variables were analyzed by multiple logistic regression analysis. RESULTS: The Hoehn-Yahr stage increased in 14 patients and was unchanged in 12 patients. The 36-item Short-Form Health Survey score (OR 1.079, p = 0.041, 95% CI 1.003-1.161) and the number of steps on the suddenly narrow path (OR 1.605, p = 0.047, 95% CI 1.006-2.56) were related to an increase in the Hoehn-Yahr stage. The number of steps was significantly higher on the suddenly narrowed path (11.3 ± 3.6) than on a straightly narrowed path (10.1 ± 3.2) at the time of final follow-up in the 26 patients (p < 0.001). CONCLUSIONS: An increased number of steps associated with FOG, which was elicited by the suddenly narrowed path, might be one predictor of an upgrade of stage in patients with Hoehn-Yahr stage III PD.

Step lap joints are kinds of lap structures, where butted laminations of each layer are consecutively offset in succeeding layers in the same direction. They are mainly designed this way to reduce the peel stresses at the edges of the overlap area observed in single lap joints. In their service, lap joints are often subjected to bending loads. However, the performance of a step lap joint under flexural loading has not been studied in the literature yet. For this purpose, 3D advanced finite-element (FE) models of the step lap joints were developed via ABAQUS-Standard. DP 460 and A2024-T3 aluminum alloy were used for the adhesive layer and adherends, respectively. The polymeric adhesive layer was modelled using cohesive zone elements with quadratic nominal stress criteria and power law interaction of the energies to characterize the damage initiation and damage evolution, respectively. A surface-to-surface contact method with a penalty algorithm and a hard contact model was used to characterize the contact between the adherends and the punch. Experimental data were used to validate the numerical model. The effects of the configuration of the step lap joint on its performance in terms of the maximum bending load and the amount of energy absorbed were analyzed in detail. A step lap joint with three steps (three-stepped lap joint) was found to show the best flexural performance, and when the overlap length at the upper and lower steps was increased, the amount of energy absorbed by the joint increased markedly.

UNLABELLED: Accurate and easy-to-use measurement tools are required to evaluate the effect of treatments on patient activity. Comfortable device placement and fixation are important for patient compliance. The aim of this study was the evaluation of the accuracy of an accelerometer at different placements and slow velocities. METHODS: A total of 43 healthy volunteers were included for a literature-based treadmill protocol using five accelerometer positions; a subset of 18 volunteers performed an extended treadmill protocol with velocities between 0.1 and 2.6m/s and finally stair climbing. RESULTS: An alternative accelerometer position at the anterolateral aspect of the middle shank did measure steps more accurately than at the manufacturer suggested position, especially during slow velocities. Participants preferred the alternative placement at the shank. The accuracy of different accelerometer positions was excellent at velocities between 1.0 and 2.2m/s. During slow velocities below 1.0m/s steps were recorded less accurately. Accepting an error of five percent, the accelerometer recorded steps accurately from 0.5m/s at the alternative placement and from 0.8m/s at the manufacturer suggested placement. Stair climbing was not recorded accurately by any accelerometer position. CONCLUSION: For measuring step number during slow velocities, the alternative position should be favoured. Stair climbing was not recorded accurately by any tested placement.
