RESUMO

The present case describes an unusual lactation of a 15-month-old,unbred Holstein-Friesian heifer, which had four swollen, ampouleshaped udder quarters with milk secretion. Examination of the heifer using rectal palpation and transrectal ultrasonography revealed enlargement of the right ovary and partial replacement of original tissue by multiple cysts of variable size. Treatment of the assumed follicularcystic ovary disease was unsuccessful. At slaughter 8 months later, the ovaries were examined pathologically and a granulosa cell tumor on the right ovary was diagnosed. Udder development and lactation in cattle is regulated normally hormonally. Follicular and cystic changes and granulosa cell tumors may also display hormonal activity. Therefore, we assume one or both of these could have been the cause of the unusual lactation in this case. We thus advise careful examination of the inner reproductive tract when facing the symptom of unusual lactation in unbred heifers.

RESUMO

Personal experience with 132 galactographic examinations carried out on all types of breast secretion is reported, in 7 patients in the absence of secretion. Radiology showed 38 endoductal productive lesions which did not always reveal themselves with loss of blood from the nipple. Features of the ductal lesions in relation to the type of secretion are compared. No complications connected to the examination were ever encountered even in cases in which the contrast medium spread.

RESUMO

The authors report two cases of galactoceles with unusual radiographic patterns, different from those ordinarily described either (opacity or radiocrescent); that is an aspect with a clear superior crescent related to a level between milk and fat, included in a localized galactophoric dilatation.

RESUMO

OBJECTIVE: to evaluate techniques of massage and pumping in the treatment of postpartum breast engorgement through thermography. METHOD: the study was conducted in the Human Milk Bank of a hospital in Curitiba, Brazil. We randomly selected 16 lactating women with engorgement with the classification lobar, ampullary and glandular, moderate and intense. We compared the differential patterns of temperature, before and after the treatment by means of massage and pumping. RESULTS: we found a negative gradient of 0.3°C of temperature between the pre- and post-treatment in the experimental group. Breasts with intense engorgement were 0.7°C warmer when compared with moderate engorgement. CONCLUSION: massage and electromechanical pumping were superior to manual methods when evaluated by thermography. REBEC: U1111-1136-9027.

MÉTODO: a pesquisa foi realizada no Banco de Leite Humano de um hospital de Curitiba, Brasil. Selecionaram-se, aleatoriamente, 16 lactantes com ingurgitamento com classificação lobar, ampolar e glandular moderado e intenso. Compararam-se os padrões diferenciais de temperatura, antes e após o tratamento realizado, por meio de massagem e ordenha. RESULTADOS: constatou-se um gradiente negativo de 0,3ºC de temperatura entre o pré e o pós-tratamento no grupo experimental. MÉTODO: el estudio se realizó en el Banco de Leche Humana de un hospital en Curitiba, Brasil. Se seleccionaron al azar 16 mujeres en periodo de lactancia con congestión con clasificación lobar, ampular y glandular, moderada e intensa. Se compararon los patrones diferenciales de temperatura, antes y después del tratamiento por medio de masaje y bombeo. RESULTADOS: se encontró un degradado negativo de 0,3°C de temperatura entre el pre y post-tratamiento en el grupo experimental. Las mamas con intensa congestión eran 0,7°C más caliente en comparación a aquellos con congestión moderada. CONCLUSIÓN: el masaje y el bombeo electromecánico fueron superiores a los métodos manuales cuando se evaluaron por termografía. REBEC: U1111-1136-9027. .

PURPOSE: The aim of this study was to investigate and summarize the sonographic appearances of galactoceles. METHODS: We retrospectively reviewed the sonograms and clinical records of 10 patients with galactoceles who had been examined over a 5-year period to evaluate breast lumps that had occurred during lactation or the puerperium. RESULTS: Sonograms from all patients showed well-defined lesions with thin, echogenic walls. The internal appearances of the lesions included homogeneous contents with medium-level echoes in 6 patients and heterogeneous contents with fluid clefts and anechoic rims in 4 patients. Focal echogenic areas with distal shadowing were seen in 2 patients. Most of the lesions showed some distal acoustic enhancement, depending on the internal contents. The diagnosis of a galactocele was established by needle aspiration in 9 patients and by excision biopsy in 1 patient. Needle aspiration alone was therapeutic in 8 patients. CONCLUSIONS: Galactoceles can have a wide range of sonographic appearances and can mimic other lesions of the breast, both benign and malignant. The clues to the diagnosis are recent childbirth and lactation and the presence of a well-defined lesion with some distal acoustic enhancement. Needle aspiration of the lesion is both a diagnostic tool and an effective treatment in most patients with galactoceles.

RESUMO

During the investigation of ductographic findings in 200 patients with secretory disease of the breasts it was discovered that pathologically secreting lactiferous ducts were statistically predominantly localized in the retroareolar region and in the lower quadrates of the breasts. This prevalence does not correspond with the regions of the most voluminous portions of the mammary gland and with the most frequent localization of mammography manifestations of its fibroepithelial dysplasia.

Serum prolactin levels were determined in 25 women who underwent galactography on account of pathological nipple discharge. The test samples were obtained immediately before galactography as well as 5, 10, 15 and 30 minutes after. There was no significant change in serum prolactin levels following galactography. In 2 cases out of 25, the basal level of prolactin was well above normal while in 1 case it was below normal. Those three were cases of galactorrhea. The clinical aspect of pathological nipple discharge did not correlate with serum prolactin levels, and galactorrhea would appear to be compatible with serum prolactin levels below normal.

Xerogalactography represents the most important instrumental examination in the diagnosis of the secreting breast. The authors describe the radiological findings in the ductal ectasia, intraductal papillomas and papillomatosis and carcinomas in a personal experience concerning 52 patients with mammary secretion.

A retrospective clinicoradiological study of 80 patients with amenorrhoea and/or galactorrhoea showed: 1. Of 60 patients, in whom prolactin levels were estimated, 28 (47%) had hyperprolactinaemia (serum prolactin levels greater than 20 ng/ml). 2. Hyperprolactinaemia was present in 85% of patients with amenorrhoea and galactorrhoea, 35% of those with galactorrhoea only, and 12.5% of those with only amenorrhoea. 3. Pituitary adenomas were surgically removed from six patients; all but one had prolactin levels over 100 ng/ml. The level was 53 ng/ml in the sixth patient. 4. There is a considerable degree of observer variation in the interpretation of borderline sellar changes. 5. Minor abnormalities of the pituitary fossa were noted in 64% of patients with hyperprolactinaemia and 36% of those with normal prolactin levels. 6. The highest incidence of perfectly normal sellae was among patients with either amenorrhoea or galactorrhoea and normal serum prolactin.
