Estudo comparativo entre as técnicas de imunofluorescência direta, isolamento viral em camundongos e isolamento viral em cultivo celular utilizadas no diagnóstico da raiva / Comparative study among direct fluorescent antibody, mouse inoculation test and rabies tissue culture infection test used in rabies diagnosis

O diagnóstico laboratorial da Raiva é de fundamental importância para avaliação dos casos suspeitos de infecção pelo vírus da raiva (RABV) e as técnicas utilizadas devem apresentar alta sensibilidade e especificidade, bem como rapidez na obtenção dos resultados. No diagnóstico da Raiva é recomendado a confirmação do resultado da Imunofluorescência direta (IFD) através do isolamento viral (IV). Como o isolamento viral em camundongos (IVC) está sendo substituído pelo isolamento viral em cultura de células (IVCC) por questões éticas, estudos são necessários para avaliação do emprego da técnica de IVCC na rotina de diagnóstico da Raiva do Instituto Pasteur (IP). O objetivo desse estudo foi avaliar o fluxo do diagnóstico da Raiva do Laboratório de Virologia do IP, por meio de análise estatística pelo índice de concordância, sensibilidade, especificidade e acurácia nos testes de IFD, IVCC e IVC. Para essa avaliação foram utilizados resultados obtidos na rotina de diagnóstico do Laboratório de Virologia do IP no período de 2008 a 2016 em 6.514 amostras de Sistema Nervoso Central (SNC) de diferentes espécies animais. A IFD foi a técnica que apresentou melhores resultados de sensibilidade (93,58%), especificidade (95,90%) e acurácia (95,67%). Para o IVCC os valores de sensibilidade, especificidade e acurácia foram inferiores a IFD sendo 70,42%, 86,16% e 84,62%, respectivamente. A concordância entre o IVCC e IFD foi moderada com índice de kappa k= 0,341. A sensibilidade, especificidade e acurácia do IVC foi de 89,58%, 100% e 98,97%, respectivamente. A concordância entre o IVC e IFD, teve um valor de k substancial (0,720). A IFD, considerada padrão ouro mostrou-se efetiva em todos os animais, com exceção de equinos. A partir destes resultados analisados foi possível inferir que a IFD apresentou resultados satisfatórios, porém o IVCC não apresentou resultados favoráveis para ser utilizado como uma técnica confirmatória, assim, faz-se necessário uma avaliação do fluxo estabelecido no diagnóstico do Laboratório de Virologia do IP.
The laboratory diagnosis of rabies is of fundamental importance for the evaluation of suspected cases of rabies virusinfection (RABV) and the techniques used must have high sensitivity and specificity, as well as speed in obtaining the results. In rabiesdiagnosis, confirmation of the result of the direct fluorescent antibody (FAT) through viral isolation (IV) is recommended. As mouse inoculation test (MIT) is being replaced by rabiestissuecultureinfection test (RTCIT) for ethical reasons, studies are needed to evaluate the use of the RTCIT technique in the routine diagnosis of rabies in the Pasteur Institute (IP). The objective of this study was to evaluate the flow of rabiesdiagnosis at the IP Virologylaboratory, by means of statistical analysis by the index of concordance, sensitivity, specificity and accuracy of the FAT, RTCIT and MIT tests. For this evaluation, results obtained in the diagnostics routine of the VirologyLaboratory of IP from 2008 to 2016 were used in 6,514 samples of Central Nervous System (CNS) from different animals. FAT was the technique that presented the best results of sensitivity (93.58%), specificity (95.90%) and accuracy (95.67%). For the RTCIT, the values of sensitivity, specificity and accuracy were lower than FAT with 70.42%, 86.16% and 84.62%, respectively. The concordance between the RTCIT and FAT was moderate with kappa quotient k= 0.341. The sensitivity, specificity and accuracy of the MIT was 89.58%, 100% and 98.97%, respectively. The concordance between the MIT and FAT, had a substantial k value (0.720). FAT, considered the gold standard, was effective in all animals, except horses. From these analyzed results it was possible to infer that the FAT presented satisfactory results, however the RTCIT did not present favorable results to be used as a confirmatory technique, thus, it is necessary to evaluate the flow established in the diagnosis of the VirologyLaboratory of IP.
