RESUMO

The growth processes in children depend on the proper functioning of some hormones and growth factors. Recently, a positive correlation between ghrelin and TSH (thyroid stimulating hormone) in patients with hyper- and hypothyroidism was proved. Moreover, in hypothyroid rats with high ghrelin concentration, growth hormone (GH) and insulin-like growth factor I (IGF-I) secretion was suppressed. We analyzed these relationships in euthyroid prepubertal children with idiopathic short stature (ISS). The analysis comprised concentration of ghrelin, GH in stimulating tests and during the night, as well as IGF-I, TSH, free thyroxine (FT4) and free triiodothyronine (FT3) in 85 children with ISS (36 girls, 49 boys) aged 9.65 ± 3.02 years (mean ± SD). A strong positive correlation between ghrelin and TSH was confirmed (r = +0.44, p < 0.05). A higher ghrelin but lower nocturnal GH and lower IGF-I were observed in children with higher normal TSH concentration than those in children with lower normal TSH. Interestingly, alterations of TSH level were without any impact on FT4 and FT3 concentrations. Summing up, in ISS prepubertal euthyroid children, ghrelin and TSH secretion are closely related. On the other hand, the higher the TSH, the lower the nocturnal GH and IGF-I levels. The contribution of the above findings in deterioration of growth processes requires further studies.

Background and Purpose: The Fugl-Meyer Assessment of Motor Performance and Sensory Assessment Scale (FMA) is the most commonly used and recommended outcome measure for the sensorimotor impairment of the upper and lower limbs in stroke patients. The aim of this study was to perform cross-cultural translation and adaptation of the scale into Polish and to evaluate the FMA's reliability of motor performance and sensation of the upper and lower limb sections among ischemic stroke patients. Methods: The Polish version of the FMA (FMA-PL) was developed using a forward-backward translation performed by a group of experts and then evaluated by a panel of judges according to international guidelines. The study involved 86 patients (F = 30, M = 56, i.e., 35%; the average age of patients was 64 ± 12 years, 36 with right-sided stroke and 50 with left-sided stroke). The FMA-PL was carried out twice by two experienced neurological physiotherapists with a 2 h gap between assessments (test-retest and inter-rater). The reliability of the outcome measure was defined by calculating the intraclass correlation coefficient (ICC). The standard error of measurement (SEM) and the minimum detectable change (MDC) were also calculated. The internal consistency of the test was determined by the Cronbach's alpha indicator. Results: Three domains were evaluated on the FMA-PL scale. From the whole test, results were obtained in the range of 12-124 points: 64 points for FMA-UE-PL 2, 34 points for FMA-LE-PL 4, and 24 points for FMA-S-PL 0. The ICC values were in the range of 0.99-1.00 for the total FMA-PL score and the results of each domain. The SEM and MDC for the entire FMA-PL calculated for test-retest measurements were 0.22 and 1.60, respectively. The SEM and MDC for the total FMA-PL score obtained during repeated measurements of the same investigator were 1.3 and 3.5 points, respectively. The Cronbach's alpha values calculated for the total FMA-PL, FMA-UE-PL, FMA-LE-PL, and FMA-S-PL items amounted to 0.938-0.939, 0.932-0.934, and 0.634-0.722, respectively. Conclusions: The Polish version of the FMA is a consistent and reliable outcome measure for the motor and sensory evaluation of the upper and lower limbs for patients in subacute and chronic stroke stages.

Introduction: Growth hormone (GH) and thyroid hormones are important for children growing. In some obese children a slightly elevated TSH concentration is observed. This may be an adaptive mechanism: stimulation of pro-TRH biosynthesis in the hypothalamus in response to elevated leptin. The increased TSH may also reflect the necessity of maintaining the resting energy expenditure or may be a result of inappropriate, low FT4 concentration. Thus, we evaluated serum TSH and FT4 concentrations in idiopathic short stature (ISS) children (non GH-deficient) and examined the effect of children's nutritional status and levels of selected adipocytokines on thyroid function, searching for the presence of various forms of subclinical hypothyroidism, which may be the cause of the slow growth rate. Methods: The study group included 115 children (50 girls and 65 boys) with ISS, aged (mean ± SD) 10.4 ± 3.34 years. In each child, lipids, TSH, FT4, IGF-1, maxGH during the stimulation tests, leptin, adiponectin and resistin concentrations were determined. Based on BMI SDS, 3 subgroups: slim (n=26), obese (n=21) and normal weight (n=68) were distinguished. Results: There was no correlation between leptin level and TSH, FT4 levels. The levels of leptin, total cholesterol and LDL-cholesterol in obese short children were significantly higher than in children from other subgroups. In turn, the levels of adiponectin, resistin, TSH and FT4 did not differ between subgroups. In 7% of children, an elevated TSH level was found (but less than 10 mIU/L), with a similar frequency across subgroups. The higher the leptin, the lower maxGH in clonidine stimulation test was recorded. Conclusions: It seems that in obese children with idiopathic short stature leptin does not increase TSH secretion. This may be related to a disruption of the effect of leptin on TSH production and could indicate wide ranging disturbances of hypothalamic signals, and consequently be the cause of inappropriate GH secretion.

PURPOSE: Myofascial stiffness is the biomechanical property that can be considered as a modifiable risk factor injury of athletes. Moreover, the level of muscle stiffness can be crucial to assessment muscle adaptation to exercise. However, stiffness of flexor muscles in rock climbers has never been investigated. The purpose of this study was to evaluate the myofascial stiffness of the flexor digitorum superficialis (FDS) in the rock climbers. METHODS: The group of rock climbers (n = 16) has been compared to the non-climbers group (n = 16), matched in terms of anthropometric measures and an average level of physical fitness. Moreover, the impact of the one-time rock climbing training on FDS stiffness was assessed in the rock climbers group. The measurement of myotonometric myofascial stiffness of FDS muscles was performed, when the elbow was flexed and fully extended. In both positions, the measurements were taken with the hand open and closed. RESULTS: It has been shown that rock climber's FDS muscle stiffness before training was significantly higher (from 15 to 20%) compared to non-climbers group. Moreover, after climbing myofascial stiffness of FDS muscles increased significantly from 25 to 40%. CONCLUSIONS: Rock climbers have significantly higher stiffness of FDS muscles after rock climbing training. Moreover, the increased stiffness in the FDS differentiates rock climbers from the active and healthy control group. Results may indicate that decrease of muscle stiffness of FDS can be an important factor for the improvement of the climbing effectiveness and prevention of overload in the forearm flexor muscles and their recovery.

BACKGROUND: Diagnostics of thyroid disorders (TD) are frequently based on the measurements of thyroid stimulating hormone (TSH) concentration only. If TSH is outside the reference range, the diagnostic procedure used in patients with TD isintroduced. Observations indicate that in a considerable number of these patients, TD is not confirmed. The aim of the study was to assess the incidence of transient hyperthyrotropinemia in healthy children during acute infections of the respiratory system. PATIENTS AND METHODS: The study included consecutive children (49 boys and 45 girls), aged 2.2-17.3 years, who visited one General Practitioner (GP) due to respiratory tract infections. The tests: complete blood count (CBC), C-reactive protein (CRP), TSH and FT4 were run on the next day after the visit at the physician's (initial visit) and ≥2 weeks after recovery. RESULTS: Among these children, elevated TSH values were found in about 10% of patients, and they went back to normal values after recovery. A prospective analysis showed a reduction of TSH values in approx. 65% of all groups and TSH at the follow-up visit was significantly lower. CONCLUSIONS: Transient hyperthyrotropinemia was observed in about 10% of children with acute respiratory tract infection. This preliminary finding remains unexplained.

The aim of this study was to investigate perceived benefits for mental and physical health and barriers to horseback riding participation among professional and amateur athletes by gender. The empirical study of 2651 professional and amateur horseback riders was conducted during the last edition of Cavaliada competitions (held in Poznan in December 2019)-one of the biggest and most important horseback riding events in Europe. A diagnostic survey method was used in the study. In the questionnaire a division of benefits and barriers according to the EBBS (Exercise Benefits/Barriers Scale) was used. The results are presented by means of frequency distributions for individual items. The verification of hypotheses about the differences between the analyzed groups was conducted using the U-Mann Whitney test with a correction of tied ranks. For the compared groups the mean rank values were calculated. Research results showed that respondents rated the positive impact of equestrianism on mental health higher than on physical health. Among the barriers, the most frequently mentioned aspects were not related to the internal motivation of the respondents, but to external factors-money, time and distance of sports facilities. Men rated the social and psychological benefits higher, while women rated the positive impact of equestrianism on physicality. Professionals rated more highly a number of aspects related to positive effects on the body, while amateurs claimed that were more often not supported by loved ones. This is important research from the point of view of horseback riding promotion. Understanding the horseback riding benefits and barriers are needed, as such knowledge can be used to encourage horseback riding. Perceived benefits and barriers to horseback riding have so far been rarely studied by researchers.

Adolescent forms of idiopathic scoliosis are commonly encountered deformities of the thoracic and lumbar spine. They affect a significant number of adolescents, yet their cause is still unknown. The presented research is a cross-sectional analysis of 3933 volunteers (2131 girls and 1802 boys). The participants were primary school students aged 9 to 13 years old. This study determined a relationship between predictors such as: body mass, body height and body mass index (BMI) (independent variables) and angle of trunk rotation (ATR) value (dependent variable). Moreover, a stepwise multiple regression with backward selection was conducted to determine to what extent the dependent variable is explained by body mass, body height and BMI. In the group of 11,12,13-year-old girls, the analyzed results of multiple stepwise regression were statistically significant. Among the all studied predictors, it has been shown that body mass in the 11-year-old girls and body height in 12- and 13-year-old girls are major correlates of a 1-year ATR increase in proximal and main thoracic spine levels.

The impact of sports events on the promotion of physical activity, healthy lifestyles and sports participation is debatable, and most of the literature is on mega-events. This begs the question if more evidence of this type of impact can be found for non-mega events. Research on sports legacy often refers to the tangible effects such as infrastructure that is left after the competition. However, the construction of new facilities does not automatically result in attracting participants. Despite the high expectations of events organizers in terms of their impacts on pro-health behavior of people, few studies provide empirical evidence that events encourage sport fans to become more physically active. The aim of this research was to examine whether a mass run promotes physical activity among spectators, and whether a mass run influences the willingness of spectators to start in half marathon in the future. A written paper-pencil survey was collected from 510 spectators during the 6th Poznan Half Marathon. The results show that observing a mass run event has a positive impact on the willingness to engage in regular physical activity as well as the willingness to take part in this type of sport in the future. Our work provides knowledge about the level of effectiveness in promoting active lifestyles among supporters depending on age, sex and place of residents. This work focuses on mass runs, which have been under-researched when it comes to impact on sport participation.

Despite the growing interest in extreme sports around the world, researchers have rarely investigated the complex factors that have led to a developed commitment to extreme sports in recent years. Precisely, the social identity of ultramarathoners remains a research niche. The aim of the article is to analyze the impact of a sports event on shaping social identity of ultramarathon runners on the example of Karkonosze Winter Ultramarathon (held in Poland). The qualitative method used in the article-interviews with runners-made it possible to examine the factors that create social identity, among which the motives for participation, sports subculture, and the authenticity of the experience play a key role. The first part of the article describes the theoretical aspects of social identity in sport. The second, empirical part presents the research results supplemented by the statements of the contestants. In this case, the subject of analysis is the motives for participation in a winter ultramarathon and their characteristics. Lastly, the article analyzes the subculture of ultramarathoners and the experience of contestants' authenticity. The investigated winter ultramarathon created the perfect space for creation, deepening and celebrating the social identity of ultramarathoners assessed as a value in itself. The article enriches the present knowledge about the motivation of ultramarathoners because, unlike the results of quantitative research, it presents in-depth responses of runners who were not always concerned by existing research questionnaires.

(1) Background: Idiopathic scoliosis is a deformity of the growing spine. It affects 2-3% of adolescents; yet its cause is still unknown. At the early stage of idiopathic scoliosis (IS), the signs are not very noticeable. That is why the primarily school-based screening for scoliosis is so important. (2) Methods: This was a cross-sectional analysis of 6850 respondents. Participants were elementary school students in the metropolitan area of Poland. The suspicion of IS was based on detection of three-dimensional deformity of the spine using scoliometer. (3) Results: Respondents were divided into two groups: Angle of trunk rotation (ATR) = 0-3º and ATR > 3º. Presented research using a referral criterion of 5º ATR showed that in the group of participants who had ATR > 3º the largest percentage of 5 degree values was recorded at the second and third measurement level of the spine (30.5%, 31.1%, respectively). Analyzing the differences between the two groups of girls (ATR = 0-3º, ATR > 3º), statistically significant differences were recorded between 9 and 11 years of age (p = 0.0388). Girls with ATR > 3º at all measuring levels are significantly slimmer than girls with ATR 0-3º; (4) Conclusions: Age; sex, and risk of developing angle of trunk rotation are very closely associated. The main thoracic (level 2) and thoraco-lumbar (level 3) level of measuring of the spine appears to be the most differentiating in the diagnosis of scoliosis. Girls with a lower degree of trunk deformity (4-6º trunk rotation), which can present mild scoliosis and those with a higher degree (7º trunk rotation) have lower body mass than girls within the norm.

INTRODUCTION: Iodine deficiency in pregnant women, even of a mild degree, may have adverse effects on both the mother and the foetus. Despite the obligatory model of functioning iodine prophylaxis in Poland, the iodine supply in women during pregnancy and physiological lactation is insufficient. Therefore, those groups should take additional iodine supplementation at a dose of 150-200 µg/day. The aim of this study was to examine the effectiveness of iodine prophylaxis in pregnant women in Poland. MATERIAL AND METHODS: The assessment of iodine supply, urine iodine concentration (UIC) in the spot urine sample, as well as levels of TSH, fT4, thyroid antibodies, and thyroid volume, was performed at one time point in 115 women (7 in the 1st trimester, 61 in the 2nd trimester, and 47 in the 3rd trimester). RESULTS: Only 45.2% of women were taking additional amounts of iodine at any time of pregnancy, and the median ioduria was 79.6 µg/L, which pointed to an insufficient supply of iodine. The percentage of women using iodine supplementation increased with the length of pregnancy, which indicates that the recommendations are implemented too late. In women who took iodine supplementation, ioduria was significantly higher than in those not applying iodine supplementation (median 129.4 µg/L vs. 73.0 µg/L; p < 0.001); however, this was still below recommended values. CONCLUSIONS: The effectiveness of iodine prophylaxis in pregnant women in Poland, evaluated on the basis of the analysis of randomly chosen sample, is not satisfactory in terms of compliance with the recommendations and, possibly, the quality of supplementation.
