RESUMO

Mortality from penetrating traumas involving the subclavian vessels can be as high as 60% in pre-hospital settings. Operating room mortality is in the range of 5-30%. This paper presents a case in which a strategy for damage control was employed for a patient with an injury to the origin of the left subclavian artery, using subclavian ligation, with no need for any other intervention, and maintaining viability of the left upper limb via collateral circulation. The authors also review surgical approaches and treatment strategies with a focus on damage control in subclavian vessel injuries.

Background: In the evaluation of men presenting for erectile dysfunction (ED), specific diagnostic tests, such as an intracavernous injection test (IIT) with Erection Hardness Score (EHS) assessment or penile Doppler ultrasound (PDU), may be necessary. Aim: The study sought to compare the prognostic value of PDU parameters with erection rigidity with EHS during IIT in predicting refractory ED after 5 years. Methods: Patients referred for ED were evaluated and had a PDU with at least 15 µg of intracavernous alprostadil and without any sexual stimulation. At 5 years of follow-up, current and past ED treatments were noted. Refractory ED was defined as having a penile prosthesis (PP) implanted, having failed nonsurgical treatments but having refused PP implantation, or having discontinuation of nonsurgical treatments due to loss of efficacy. Patients with hypogonadism and pelvic surgery were excluded. Receiver-operating characteristic curves were drawn and the area under the curve (AUC) was calculated. Outcomes: The outcome was the AUC for predicting refractory ED. Results: At 5 years, 69 men were still in follow-up with a mean age of 58.47 ± 10.39 years, and 13 (18.8%) were classified as having refractory ED. The AUC for the EHS, peak systolic velocity, end-diastolic flow, and resistive index to discriminate refractory ED were 0.820, 0.613, 0.730, and 0.714, respectively. Clinical Implications: EHS can be a good predictor of response to nonsurgical treatments in ED. Strengths and Limitations: This was a prospective study to compare IIT with PDU, and validated disease-specific questionnaires were used to assess both clinical efficacy and satisfaction. PDU was performed by a blinded third party. However, resulting from a single-center study, our sample size can be considered small, and the number of events observed was also low. Conclusion: Our data suggest that an abnormal EHS during an IIT is, at least, noninferior than an abnormal PDU in predicting those patients that will not respond to nonsurgical treatments and that will need a PP in long-term.

Although male hypogonadism (MH) is a prevalent comorbidity in patients presenting for erectile dysfunction (ED), its screening relies solely on total testosterone (TT). Ageing and other conditions can increase sex hormone-binding globulin (SHBG) and lower free testosterone (FT) causing symptomatic MH despite normal TT. The primary objective was to measure the prevalence of normal TT/low FT among patients presenting for ED. From January 2019 to December 2020, 408 patients referred for sexual dysfunction were screened; 180 men with a confirmed diagnosis of ED were included. MH was screened using TT, SHBG, albumin and LH. FT was calculated (cFT). Low TT, high SHBG and low cFT were defined as <345 ng/dL, >50 nmol/L and <6.5 ng/dL, respectively. Patients were divided into groups according to TT/cFT status and to age group. The frequency of normal TT/low cFT was 17.2%. From all 31 patients with normal TT/low cFT, only four (12.9%) had either hyperthyroidism, hepatic disease or HIV infection, while 23 (74.2%) were older than 60 years. Patients with normal TT/low cFT were older (65.57 ± 10.43 vs. 56.79 ± 10.63 yo, p = 0.001) and had higher SHBG (78.48 ± 40.14 vs. 52.35 ± 20.39 nmol/L, p = 0.014) than patients with normal TT/cFT. Patients over 60 years represented 48.9% of the sample, 52.5% had elevated SHBG and their frequency of normal TT/low cFT was 26.3%. Normal TT/low cFT is frequent and can be missed by current screening recommendations for MH in patients presenting for ED. Ageing seems to be the main culprit as elevated SHBG prevalence increases steeply after the sixth decade. TT cannot solely be relied on to exclude biochemical MH in patients presenting for ED, especially in patients over 60 years old. Current guidelines for MH screening in ED should be amended.

In 2022, an outbreak of monkeypox is being reported in non-endemic areas, with unusual clinical manifestations. The detailed clinical description of the first patient that received the diagnosis of monkeypox in Brazil is reported here, whose clinical manifestations can easily lead to misdiagnosis of sexually transmitted infections. A 41 years old male presented to an emergency room with a vesicular rash with eight days of evolution. He had traveled to Portugal and Spain and reported non-penetrative sexual involvement with three different male individuals. On the third day of symptoms, he sought medical care and received empirical treatment directed to sexually transmitted infections. As the symptoms did not improve, he sought medical attention at an infectious disease referral center presenting, on admission, an ulcerated penile lesion with central necrotic crusts, a disseminated pleomorphic skin rash and an oropharyngeal ulcer. The monkeypox diagnosis was suspected due to the characteristics of the lesions and the history of intimate contact with casual partners, and it was later confirmed by sequencing the almost complete monkeypox genome. The patient was hospitalized for pain control, which required opiate administration. He developed a secondary bacterial infection on the penile lesions, which were treated with oral antibiotics. He was discharged after 14 days, with lesions in process of re-epithelialization. Given the current outbreak, we must consider the possibility of monkeypox in patients with suggestive lesions, anywhere on the body (including the genitals), added to an epidemiological link or history of intimate contact with strangers or casual partners.

RESUMO

In this article, we report on the new endeavor of the European School of Oncology to offer collaborative educational activity to the Arab oncologists. We present the 4-year detailed programme held in different Middle East countries with the organization of master classes, advanced courses, or symposia as well as the fellowship programme and the press media events. In addition, we assess the evaluation questionnaires given to the participants during the courses.

In 2018, 22,000,000 Brazilian women experienced some type of violence (either physical or psychological), and 42% of these episodes occurred in the domestic environment. Therefore, government strategies have been developed to solve this problem. This study aimed to (a) carry out a survey of Brazilian government strategies for the protection of women after the creation of the Maria da Penha Law (Law No. 11.340, 2006) and (b) discuss these strategies from the concept of cultural design. Sixteen laws, five decrees, and two ordinances were found and analyzed. In a general way, the analysis suggests that they are not effective cultural plans because most do not show long-term results, and some of the laws that typify crimes and those that relate to the aggressor's behavior do not specify the target behaviors, so there is no explicit contingency. Despite this, such strategies are relevant, as they end up providing the protective context for women from a social, legal, and political point of view. Finally, it is expected that the discussions raised in the present work can help prepare interventions that favor socially beneficial cultural practices.

INTRODUCTION: In high-income countries, persons of high socioeconomic status (SES) have a lower cardiovascular risk. However, in middle and low-income countries, the results are controversial. OBJECTIVE: To evaluate the association between family income and cardiovascular risk factors in young adults. METHODS: A total of 2,063 individuals of a birth cohort initiated in 1978/79 in the city of Ribeirão Preto, Brazil, were evaluated at age of 23/25 years. Cardiovascular risk factors (hypertension, sedentary lifestyle, smoking, low high-density lipoprotein (HDL)-cholesterol, high low-density lipoprotein (LDL)-cholesterol, high fibrinogen, insulin resistance, diabetes, abdominal and total obesity, and metabolic syndrome) were evaluated according to family income. Income was assessed in multiples of the minimum wage. Simple Poisson regression models were used to estimate the prevalence ratios (PR) with robust estimation of the variance. RESULTS: High-income women showed lower prevalences of low HDL-cholesterol (PR = 0.47), total obesity (PR = 0.22), abdominal obesity (PR = 0.28), high blood pressure (PR = 0.28), insulin resistance (PR = 0.57), sedentary lifestyle (PR = 0.47), metabolic syndrome (PR = 0.24), and high caloric intake (PR = 0.71) (p < 0.05). High-income men showed lower prevalences of low HDL-cholesterol (PR = 0.73) and sedentarism (PR = 0.81) (p < 0.05). These results may be explained by the fact that high-income women pay more attention to healthy habits and those with the lowest family income are least likely to access health services resources and treatments. CONCLUSION: Women were in the final phase of the epidemiologic transition, whereas men were in the middle phase.

INTRODUCTION: Aripiprazole, a dopamine D2 receptor partial agonist, has also partial agonist activity at serotonin (5-HT)1A receptors and antagonist activity at 5-HT2A receptors. METHODS: In this 8-week, multicenter, randomized, parallel-group, open-label, flexible-dose study, patients diagnosed with schizophrenia or schizoaffective disorder were randomized to aripiprazole 15-30 mg/day or haloperidol 10-15 mg/day. RESULTS: Patients treated with both aripiprazole and haloperidol improved from baseline in Positive and Negative Syndrome Scale total, positive, and negative scores as well as in Clinical Global Impressions scores (all P<.001). At the end of the study, the percentage of patients classified as responders--according to >or=40% reduction in the Positive and Negative Syndrome Scale negative subscale score--was significantly higher in the aripiprazole group (20%) than in the haloperidol group (0%) (P<.05). Additionally, a higher number of patients receiving haloperidol required more anticholinergic medications (P<.001) than aripiprazole-treated patients, whereas more aripiprazole (45.5%) than haloperidol-treated patients (12.9%) required benzodiazepines (P=.002). At endpoint, rates of preference of medication were higher in the aripiprazole group (63.2%) than in the haloperidol group (21.7%), as expressed by patients and caregivers (P=.001). CONCLUSION: Aripiprazole and haloperidol had similar efficacy in terms of reduction of overall psychopathology. Although aripiprazole has been demonstrated to be superior concerning negative symptoms and in terms of tolerability (extrapyramidal symptoms) and preferred by patients and caregivers than haloperidol, significantly more aripiprazole-treated patients required benzodiazepines.

É sabido que a cavidade oral é a porta de entrada para muitos microrganismos e que possui alguns gêneros de bactérias que quando se associam ou proliferam em excesso, com a falta de higienização correta, fatores ambientais e imunológicos podem causar danos em algumas partes dos dentes como no tecido pulpar e canais radiculares. Neste sentido, a endodontia tem sido capaz de tratar pessoas com problemas dentários em fases muito precoces como as medidas profiláticas até casos infeciosos que afetam a polpa dos dentes causando as lesões perirradiculares que são infecções causadas por microrganismos, Para tanto, é necessário que a odontologia ofereça transformações positivas por meio de técnicas que sejam mais previsíveis para os tratamentos. A literatura tem indicado o uso da laserterapia como técnica auxiliar para o tratamento endodôntico pelo seu potencial mínimo invasivo, preciso e eficaz em menor tempo de cura de patologias orais como as lesões perirradiculares. Este estudo pretendeu abordar a importância da laserterapia sobre as variáveis patológicas de pacientes com lesão perirradiculares. Para realiza-lo foi necessário buscar na literatura estudos que associem este tipo de tratamento e sua eficiência no tratamento desta patologia. Para realizar esta pesquisa foi necessário buscar dados na literatura que tratam sobre o tema para investigar o tempo indicado ao tratamento com a laserterapia bem como o estágio em que as lesões podem ser tratadas com auxílio deste método. Corroborando com o estudo apresentamos um caso clínico(AU)


It is known that the oral cavity is the gateway to many microorganisms and that it has some genera of bacteria that when they associate or proliferate in excess, with the lack of correct hygiene, environmental and immunological factors can cause damage in some parts of the teeth such as in pulp tissue and root canals. In this sense, endodontics has been able to treat people with dental problems at very early stages such as prophylactic measures to infectious cases that affect the pulp of teeth causing perirradicular lesions that are infections caused by microorganisms, therefore, it is necessary that dentistry offer positive transformations through techniques that are more predictable for treatments. The literature has indicated the use of laser therapy as an auxiliary technique for endodontic treatment for its minimal invasive, precise and effective potential in shorter time of cure of oral pathologies such as perirradicular lesions. This study aimed to address the importance of laser therapy on the pathological variables of patients with perirradicular lesions. To accomplish this, it was necessary to seek studies in the literature that associate this type of treatment and its efficiency in the treatment of this pathology. To carry out this research it was necessary to seek data in the literature that deal with the subject to investigate the time indicated for treatment with laser therapy as well as the stage at which lesions can be treated with the aid of this method. Clinical descriptions of depression date back to antiquity. Scientific investigations of mood disorders started only 150 years ago. An historical overview of depression leads into today's knowledge that depressive disorders are common, serious, and sometimes life-threatening and that their effects are persistent and costly. Depressive disorders have a high prevalence, around 5% in the general population, with at least 20% of patients suffering with chronic conditions, such as cardiovascular disease and diabetes. Neuroimaging technology provides unprecedented opportunities for elucidating the neurobiological correlates of mood disorders. Neuroimaging studies of primary mood disorders have identified neurophysiologic abnormalities in the orbital and medial prefrontal cortex (PFC), the amygdala, and the related parts of the striatum and thalamus. There has been a revolution in our understanding of the pathogenesis of depression, with recent work demonstrating, among others, the critical impact of stress. There is an abundant evidence from family, twin, and adoption studies that genetic factors play an important role in the etiology of affective disorders. A variety of hormonal abnormalities, such as altered levels of cortisol, growth hormone, or thyroid hormones, indicate the existence of endoctrine disturbances. Despite the initial findings of immunosuppresion in depression, some studies have indicated that immune activation could also be present and might even play a role in the onset of depressive symptoms. Neurotrophic factors are among the growth factors that have been studied for their role in the adult nervous system. Despite advances in the pharmacotherapy of depression, only one third of patients respond favorably to antidepressant drugs. One third do not respond at all, and in clinical trials, at least one third respond to placebo. There is clearly an urgent need for novel antidepressants.

Este trabalho consiste em uma pesquisa realizada sobre a utilização de líquidos irrigantes por cirurgiões dentistas na cidade de Itabuna-BA. Para isto, delimitou-se a seguinte problemática: quais são as escolhas dos líquidos irrigantes nos tratamentos endodônticos com o intuito de promover uma total desinfecção dos canais radiculares? E como objetivo geral, elucidar sobre as escolhas dos cirurgiões dentistas localizados na cidade de Itabuna-Bahia em relação as substâncias utilizadas como coadjuvantes no tratamento endodôntico durante a etapa de irrigação. A pesquisa teve uma abordagem qualitativa e foi utilizado o instrumento questionário com cem dentistas da cidade selecionada. Este questionário consistia em oito perguntas, dentre elas qual irrigante utilizado e a porcentagem do mesmo. Com relação aos resultados, pode-se afirmar que no tratamento endodôntico, o hipoclorito de sódio continua sendo a solução irrigante de escolha entre os profissionais, diferentes concentrações de soluções de hipoclorito de sódio são empregadas durante o preparo biomecânico, por endodontistas e clínicas gerais que praticam a endodontia, não existindo uma unanimidade quanto a escolha, nenhum acordo existe em relação a sua concentração ideal(AU)


This work consists of a research carried out on the use of irrigating liquids by dental surgeons in the city of Itabuna-BA. For this, the following problem was defined: what are the choices of irrigating liquids in endodontic treatments in order to promote a total disinfection of the root canals? And as a general objective, elucidate about the choices of dentists located in the city of Itabuna-Bahia in relation to the substances used as adjuvants in endodontic treatment during the irrigation stage. The research had a qualitative approach and the questionnaire was used with 100 dentists in the selected city. This questionnaire consisted of eight questions, including which irrigant was used and the percentage of it. Regarding the results, it can be said that in endodontic treatment, sodium hypochlorite remains the irrigating solution of choice among professionals, different concentrations of sodium hypochlorite solutions are used during biomechanical preparation by endodontists and general practitioners who practice endodontics, there is no unanimity as to the choice, no agreement exists regarding their ideal concentration(AU)

Resumo Os traumas penetrantes de vasos subclávios atingem mortalidade de até 60% em um cenário pré-hospitalar. A mortalidade no intraoperatório varia de 5-30%. Apresenta-se um caso de estratégia de controle de danos para um paciente com lesão na origem da artéria subclávia esquerda, através de ligadura, sem necessidade de outra intervenção, mantendo a viabilidade do membro superior esquerdo por meio de circulação colateral. Os autores fazem uma revisão sobre vias de acesso e estratégias de tratamento com ênfase em controle de danos para lesões de vasos subclávios.

RESUMO

Epidermoid cysts are common cystic lesions in the skin, ovaries, and testicles, but their occurrence in the oral cavity is uncommon. They consist of cysts delimited by a fibrous capsule without cutaneous annexes and are lined by stratified squamous epithelium. The differential diagnosis includes ranula, dermoid cysts, and lingual thyroid. Despite their benign presentation, these cysts can cause functional limitations, requiring special clinical attention for extensive lesions located in regions that preserve vital structures. This paper aims to report a case of epidermoid cyst in patient with swallowing and breathing difficulty, highlighting the clinical and surgical planning.

Results: High-income women showed lower prevalences of low HDL-cholesterol (PR = 0.47), total obesity (PR = 0.22), abdominal obesity (PR = 0.28), high blood pressure (PR = 0.28), insulin resistance (PR = 0.57), sedentary lifestyle (PR = 0.47), metabolic syndrome (PR = 0.24), and high caloric intake (PR = 0.71) (p < 0.05). High-income men showed lower prevalences of low HDL-cholesterol (PR = 0.73) and sedentarism (PR = 0.81) (p < 0.05). These results may be explained by the fact that high-income women pay more attention to healthy habits and those with the lowest family income are least likely to access health services resources and treatments. Conclusion: Women were in the final phase of the epidemiologic transition, whereas men were in the middle phase.


RESUMO: Introdução: Em países de alta renda, indivíduos de situação socioeconômica elevada apresentam menor risco cardiovascular. Em países de média e baixa rendas, os resultados são controversos. Objetivo: Avaliar a associação entre renda familiar e fatores de risco cardiovascular em adultos jovens. Metodologia: Estudo seccional em que foram avaliados 2.063 indivíduos aos 23/25 anos de uma coorte de nascimento iniciada em 1978/79 na cidade de Ribeirão Preto, Brasil. Avaliaram-se fatores de risco cardiovascular (hipertensão arterial, sedentarismo, tabagismo, HDL - colesterol baixo, LDL - colesterol elevado, fibrinogênio alto, resistência insulínica, diabetes, obesidade abdominal e total e síndrome metabólica) de acordo com renda familiar. A renda foi avaliada em múltiplos do salário mínimo. As razões de prevalências (RP) foram estimadas em modelos de regressão de Poisson simples, com estimativa robusta da variância. Resultados: Mulheres de maior renda apresentaram menores prevalências de HDL - colesterol baixo (RP = 0,47), obesidade total (RP = 0,22) e abdominal (RP = 0,28), resistência insulínica (RP = 0,57), pressão arterial elevada (RP = 0,28), sedentarismo (RP = 0,47), síndrome metabólica (RP = 0,24) e alta ingestão calórica (RP = 0,71) (p < 0,05). Os homens de maior renda apresentaram menores prevalências de HDL - colesterol baixo (RP = 0,73) e sedentarismo (RP = 0,81) (p < 0,05). Pode ser que mulheres de alta renda prestem mais atenção aos hábitos saudáveis e aquelas com menor renda têm menor probabilidade de acessar recursos e tratamentos de serviços de saúde. Conclusão: As mulheres encontravam-se na fase final da transição epidemiológica, enquanto os homens, na fase intermediária.

Bariatric surgery may cause frequent vomiting episodes and gastroesophageal reflux, which promote the contact of gastric acids with the teeth leading to irreversible loss of tooth structure. The aim of this study was to assess prevalence of tooth wear in bariatric patients. One hundred and twenty-five patients were examined at a Public Hospital in São Luis, MA, Brazil, between July and October 2010, being patients who had already been submitted to the bariatric surgery at least 6 months previously (Bariatric group), morbidly obese patients who were on the waiting list for this surgery (Obese group) and patients who were waiting for ambulatory medical care in other sectors (Control group). The patients answered an investigative questionnaire and were clinically examined using the Basic Erosive Wear Examination (BEWE) scoring system, which allows the classification of the severity of noncarious dental lesions (NCDL) and evaluation of risk. All patients presented some degree of tooth wear at different levels. However, the presence of NCDL was associated with the group to which the patient belonged. The bariatric patients showed higher prevalence and a statistically significant level of risk with regard to NCDLs when compared with the other patients, followed by the obese and control groups. Reflux and vomiting did not seem to influence NCDL positively.
