RESUMO

Necrotising fasciitis is a rare infection of the skin and underlying soft tissue. It primarily involves the extremities and rarely the breast. Primary necrotising fasciitis of the breast in a non-lactating, healthy female is rarer still. The authors present the case report of a patient presenting with primary necrotising fasciitis of the breast after sustaining a penetrating injury. The patient was managed successfully with serial debridement and negative pressure wound therapy (NPWT). To our knowledge only 19 such cases have been reported in the indexed literature so far. This is also the eighth case globally of primary necrotising fasciitis of the breast in a non-lactating female without any associated immunosuppression, which is the basis of reporting this case.

Background In the era of the coronavirus disease 2019 (COVID-19) pandemic, the use of full personal protective equipment (PPE) is advocated for patients undergoing emergency surgery in whom the infection status is unknown. This study aims to determine whether PPE has any influence on the rate of surgical site infection (SSI) in patients undergoing emergency exploratory laparotomy. Methodology Medical records of operated emergency cases in the general surgery department from 1st April 2020 to 24th February 2021 were studied. The surgeries done were divided into two groups: those done with full PPE (group A) and those done without full PPE (group B). The various parameters studied were the patient demography, presence of comorbidities, diagnosis, surgery done, class of surgery performed, the use of PPE, the post-operative presence, and type of SSI. Statistical analysis was done using SPSS software version 27.0 (IBM Corp., Armonk, NY). Chi-squared test was used to find the association of SSI with the use of PPE. Fisher's exact test was used to explore the association between SSI with various comorbidities, surgery performed, and the class of surgery performed. Results A total of 126 patients underwent emergency exploratory laparotomy during the study period. A total of 61 patients were in group A and 65 patients in group B. A significant association was noted between the use of full PPE and the development of SSI (p = 0.032). Diabetes mellitus, history of alcohol intake, and the class of surgery performed were found to be significantly associated with the development of SSI. Conclusion A significant association in the occurrence of SSI with the use of full PPE was observed.

Gallstone ileus is a rare complication of cholelithiasis seen in patients with a long history of cholelithiasis. It occurs more in the older age group and in the female gender. These patients have poor general condition and therefore selection of appropriate treatment is difficult. The clinician has to make a decision between immediate one-stage or two-stage closure of the cholecysto-intestinal fistula or waiting for natural closure. We have discussed the management of a rare cause of small bowel obstruction due to complication of untreated cholelithiasis.

Intestinal obstruction is one of the most important cause of acute abdomen. An internal herniation is an uncommon yet relevant clinical entity causing an acute intestinal obstruction that can occur after major bowel surgery. Here, we describe a case of acute intestinal obstruction caused by internal herniation in a patient with muscle-invasive urinary bladder carcinoma who underwent robot-assisted radical cystectomy with an ileal conduit. We also discuss the management of adjuvant chemotherapy-induced leukopenia.

Jejunal diverticulum is a very rare disease. Diagnosis of this condition is a challenge owing to non-specific complaints of the patient. Fifteen percent cases of jejunal diverticula present with acute abdomen. Approximately 77% of small bowel diverticular disease occur with multiple diverticula. Here we describe a case of complicated isolated jejunal diverticula presenting with perforation, which was successfully treated with resection of the involved segment with anastomosis.

<b>Introduction:</b> Gall bladder (GB) small cell carcinoma (SCC) comprises 0.5% of all gall bladder cancers. It carries a poor prognosis in view of its aggressive nature. <br><b>Case report:</b> We here report a case of small cell carcinoma of GB in a female who presented with obstructive jaundice. Examination revealed a hard lump in the right upper abdomen. Tumour markers showed raised CA 19-9. Staging CECT of the thorax and abdomen reported polypoidal enhancing wall thickening of the gall bladder with multiple metastatic deposits close to the pancreatic head encasing the main portal vein and common bile duct. Histopathology report was suggestive of small cell carcinoma, which was confirmed by immunohistochemistry. Patient was referred to the Oncology Department for palliative chemotherapy.

Tuberculosis (TB) is an infectious disease that can affect any organ system of the body. Abdominal TB can be gastrointestinal, lymph nodal, visceral or peritoneal. The gallbladder (GB) is rarely involved in abdominal TB as a primary organ. Extensive research literature on gallbladder TB is limited to case reports. There has been no review on this rare abdominal pathology. GB tuberculosis is a difficult diagnosis preoperatively. It is a rare differential among the more common gallbladder pathologies such as cholelithiasis, or a gallbladder malignancy. Typical histopathology of the resected specimen helps to establish this rare diagnosis. Subjecting every specimen to histopathological examination followed by medical treatment offers the chance of cure. Through this review, the authors attempt to provide an insight into this disease entity.
