RESUMO

This cross-sectional study was planned to evaluate the prosthetic status and treatment needs using WHO (1997) format among prisoners of Haryana state, India. 1,393 subjects with age range of 35.26 ± 12.29 years were examined. Of the study subjects, 11 (0.8 %) were completely while 606 (43.5 %) were partially edentulous. Of these 617 subjects, 305 (49.4 %) were edentulous only in posterior region. Dental caries accounted for loss of teeth in 325 (52.7 %) subjects. 44 (7.1 %) subjects were wearing some prosthesis. Regarding dental arch wise prosthetic need, 335 (54.3 %) subjects needed prosthesis in maxillary arch while 482 (78.1 %) needed prosthesis in mandibular arch. With advancing age there was an increase in the number of complete dental prosthesis required. There was no statistically significant difference between length of imprisonment and prosthetic need, except for need of a combination of prosthesis. Only one-fourth of the prisons had a dentist. The prisoners were taken to a hospital outside the prison in case of health needs. Prosthetic needs of prisoners were high. The lack of dental infrastructure in prisons makes the provision for multi-visit conservative dental treatments very difficult leading to higher tooth mortality. People who migrate back and forth across the prisons and communities represent a public health opportunity that should be addressed.

RESUMO

INTRODUCTION: Limited treatment options exist for COVID-19 infections; thus, attempts from complementary and alternative systems (CAM) of medicine are being explored as possible therapeutic options. Ayurcov is a formulation made of ingredients mentioned in Ayurveda. These constituents have proven antiviral, detoxifying, immune-modulating, and bio-enhancing properties. The present study was carried out to evaluate the therapeutic effect and safety of Ayurcov in patients with various severity states of COVID-19 infections. METHODS: A randomized, single blinded, controlled trial was carried out in adults diagnosed with mild-to-moderate, and severe COVID-19 infections confirmed by real time reverse transcriptase polymerase chain reaction (rRTPCR) test. The interventional group received three doses of 'Ayurcov'. It is constituted of Haridra Churna (Curcuma longa), Go ark (Bos Indicus Distilled Urine), Sphatika (Alum), Sita (Rock Candy), Godugdham (Bos Indicus Milk) milk, Goghritam (Bos Indicus ghee) on Day 1, as an adjuvant to the standard of care, and the control group received only the standard of care. Key outcomes included: proportion of patients and time taken for symptom resolution, reduction in the rRT-PCR Ct values, safety, and functional status until 42 days after discharge. RESULTS: Ninety patients with mild-to-moderate and 30 patients with severe COVID-19 disease were recruited. It was observed that significantly more proportions of patients receiving Ayurcov had symptom relief much earlier than control group. Additionally, the interventional group showed significantly lower rRT-PCR Ct values. However, a shorter time of resolution of symptoms was observed with the interventional group in the mild to moderate category but not with those having severe symptoms. Similarly, a significantly better functional status was observed with interventional group on days 7 and 28 after discharge. Ayurcov was not observed with higher risks of any adverse/serious adverse events. CONCLUSIONS: Ayurcov as adjuvant with standard of care was associated with significantly earlier resolution of COVID-19 related symptoms than standard of care alone.

RESUMO

PURPOSE: To assess the prevalence of traumatic dental injuries (TDIs) among 12- to 15-year-old schoolchildren in the Ambala district of Haryana state, India. MATERIALS AND METHODS: A study was carried out on 963 schoolchildren of 15 government schools in the Ambala district. The examiner was calibrated and a pilot study was conducted. Maxillary as well as mandibular anterior teeth were taken into consideration to assess the traumatic dental injuries. The teeth involved, place of and reason for injury and other demographic details were recorded in a structured format. Data were analysed using SPSS version 13.0. RESULTS: Out of 963 subjects, 14.4% (139) had at least one tooth with TDI. Of these subjects, males and females accounted for 16.2% (77) and 12.7% (62), respectively. Permanent maxillary central incisors were the most commonly affected teeth. Enamel fracture was the most common (80% [111]) followed by enamel-dentin fracture (17.2% [24]). A higher number of children with incisal overjet greater than 3 mm had TDI than those with less than 3 mm, although this difference was not statistically significant. Subjects with Class III and Class II Div II malocclusion were more likely to have TDI, 16.2% and 17.7% respectively. Lip-closure incompetence was found to be more common in subjects having a TDI. Amongst the subjects having a TDI, only 83.4% (116) were aware of their injury, and falling was the reason most commonly reported. CONCLUSION: Traumatic dental injuries are highly prevalent among schoolchildren. Most of the TDIs involve only tooth enamel. Incompetent lip closure was significantly associated with TDIs and a fall was the major cause for TDIs in this age group.

RESUMO

As more surgeries are moving out of the hospital setting, effective emergency response in freestanding ambulatory surgery centers requires organized preparedness. Rapid, consistent emergency response can be challenged by their rarity of occurrence, fast-paced environment, and relative lack of resources. Anesthesiologists who practice in these settings must be aware of the differences between the management of an anesthetic emergency in the hospital with virtually unlimited resources and staff, versus an ambulatory surgery center with limited resources and slightly different goal: stabilization and transfer of care. Regular simulation-based training schedules are effective for ambulatory surgery center preparedness for emergency response.

RESUMO

A synovial sarcoma (SS) is a rare form of cancer which usually occurs near the joints of the arm, neck, or leg, but has been documented in most human tissues and organs, including the brain, prostate, and heart. Primary pulmonary SS is an extremely rare tumor. We report a case of primary SS of lung who presented with severe chest pain and a large right lung mass with right-sided pleural effusion in computed tomography (CT) scan of thorax. The diagnosis was made on the basis of CT-guided core biopsy and immunohistochemistry. On immunohistochemistry, tumor cell expressed epithelial membrane antigen, bcl 2, Vimentin and smooth muscle actin and were immunonegative for S100 and cytokeratin. So, the final diagnosis was primary SS.

RESUMO

PURPOSE: This paper aims to explore prisoner dental health in Haryana, India. DESIGN/METHODOLOGY/APPROACH: The authors assessed the prevalence of dental caries and the treatment needs of prisoners in all 19 prisons in Haryana. The results were compared with the prison populations of other countries and the general population of Haryana. FINDINGS: The mean age of 1,393 subjects examined was 35.26±12.29 years. A large number of the subjects reported to be in need of dental treatment. The number of decayed teeth was found to be similar to the general population of Haryana but the number of filled teeth was quite low. The number of teeth missing and the need for tooth extraction was high. Social implications - Long-standing prisoner dental problems indicated a need for dental treatment in prisons. ORIGINALITY/VALUE: This is the first study of its kind covering all 19 prisons in Haryana, India. The results indicate that the government needs to further consider and address the oral health needs of prisoners.

RESUMO

OBJECTIVE: To determine the oral health status and treatment needs of subjects aged 60 years and above. BACKGROUND: The average life span in most parts of the world continues to increase. In the coming decades, dental practitioners will face the challenge of providing dental care for a growing number of elders who retain more natural teeth. MATERIALS AND METHODS: The study was conducted in 10 elders' homes of Ambala division on subjects 60 years or older. Modified WHO format (1997) was used. All the subjects present in the elders' home on the day of examination were included. One hundred and fifty-two subjects were examined. RESULTS: Around half (47.4 %) of the subjects had no functional teeth. Among the rest of the subjects, 26.9% subjects did not use any oral hygiene measure. 19.1% subjects reported wearing a denture. 36.8% subjects had not visited a dentist ever in their life. 28.7% subjects were suffering from pain. The mean number of decayed teeth per subject was 3.66. Maximum subjects had a Community Periodontal Index score of 2. CONCLUSION: Education regarding maintenance of oral hygiene and regular dental check up should be stressed for the elders. Dental care, especially prosthetic care, should be focused upon.
