RESUMO
We describe the case of a 32 year old man who developed such a stenosis after surgery for traumatic rupture of the corpus cavernosum. The aetiology of this rare complication is also given. The stenosis was well outlined by cystography and responded successfully to conservative treatment.

The authors present modified Denis's technique for hemostasis in 20 suprapubic prostatectomy. The technique is based on the separation between the bladder and the prostatic fossa with a temporary purse-string suture around the bladder neck. The vacuum of the prostatic fossa leads to the retraction and hemostatis. This technique is very effective on intra and post operative hemorrhage, so economy of irrigating fluid; then easier post operative care.

The authors report an usually uncommon case of psoas abscess after cystostomy for an acute urinary retention. Ultrasound and CT-Scan investigations suggested the diagnosis with confirmed by CT guided percutaneous drainage. On the basis of this observation, we discuss the mechanism and therapeutic aspects of this complication.

The authors reported two cases of impalpable testis in young mens. Laparoscopy is useful in both the diagnosis and the management of impalpable testes. Intraabdominal testicles can be removed laparoscopically if atrophic or can be partly devascularized by spermatic vessel clipping if apparently normal. A second-stage vasal-based orchidopexy can then be performed once adequate testicular reperfusion via the the defferential pedicle is believed to have occurred.

Retroperitoneal sarcoma is a rare solid tumor that accounts for less than 15% of soft tissue sarcoma. The authors conducted a retrospective study about 6 cases to evaluate the natural history of the tumors and the causes of treatment failure in an attempt to develop a rational approach to the management of the neoplasm. The histological grade and complete resection are the most important prognosis factors.

Spontaneous subcapsular haematoma (SCH) of the kidney is a rare condition, secondary to kidney tumors in more than 50% of cases. Diagnosis is suggested by sonography and confirmed by CT-scan. Renal angiography, performed in haemodynamically stable patients, shows the origin of bleeding and allows embolization. The authors report 6 cases of SCH of the kidney, one of which occurred after extra-corporeal shock-wave lithotripsy.

Ovarian vein syndrome corresponds to ureteral compression by a dilated ovarian vein. The pathogeny of this syndrome is discussed. It seems to be multifactorial, comprising an anatomical and hormonal factors. Ovarian vein syndrome is classically described on the right side. Patients present with lumbar pain or renal colic. Diagnosis is mainly urographic. The surgery constitutes radical treatment of the entity. However, this disorder can be successfully treated laparoscopically. Transcatheter embolisation provides excellent result which may replace or complement the traditional surgical approaches of this disease. On the basis of one case and other recent series, the authors review its different aspects, including some considerations on this rarely reorganized and poorly understood disease.

Spontaneous retroperitoneal hemorrhage is an uncommon affection, the diagnosis was recognized by sonography and CT scan, but the etiology remains unknown and exploration for diagnosis may become necessary. In the absence of an apparent etiology, patients with spontaneous renal bleeding should undergo radical nephrectomy, because of the extremely high incidence of small undetectable occult tumors. Three further cases were reported by the authors, who made a review of the literature.

The authors report two cases of renal multilocular cyst and review the cases reported in the literature, revealing the original features of this benign tumour which has an equal incidence in children and adults with a controversial acquired or malformative aetiopathogenesis, precise histological features, but a difficult preoperative and intraoperative diagnosis despite modern imaging techniques. In the great majority of cases, treatment consists in nephrectomy.

The authors report one case of indifferentiated sarcoma of the prostate revealed by phlebitis of the right inferior limb and lung's metastases in a man 41 years old. The tumor progress rapidly and infiltrate the bladder and the posterior urethra. The patient died five months later. They review the literature and study the clinical histological, therapeutic and evolutive aspects of this unusual tumor.

In this study, the case has been reported of a left renocolic fistula that was detected in a patient with hepatitis C and cirrhotic complications who was hospitalized for urogenital tuberculosis. It was decided to perform left nephrectomy and digestive suture, but surgery was delayed due to hemostatic abnormalities and massive cytolysis, and the patient died before surgical treatment, three days after the discovery of the fistula. The findings in the literature have been reviewed, and the common occurrence of this type of fistula has been underlined. They are the most frequently encountered type with of entero-urinary fistula, and account for 60% of documented cases. Their etiological and clinical characteristics of have been described in detail. In general, treatment consists of performing a nephrectomy and digestive suture.

Prostatic carcinoma metastasizing to the testis is an extremely rare occurrence. In this study, two new cases have been reported of unilateral testicular metastases from an adenocarcinoma of the prostate. Unlike primary tumors of the testis, this type of tumor develops later in life (between 50 and 60 years of age), and is in most instances unilateral. It is generally discovered accidentally during orchiectomy or autopsy. It is not always easy to establish a morphological diagnosis, as the macroscopic appearance of the tumor may be normal. However, microscopic investigation reveals the presence of neoplasms of the testis.

A case of myxoid paratesticular liposarcoma in a 68-year old patient is reported. The evolution was normal during the follow-up period (22 months). Scrotal liposarcoma is a rare tumour: less than 100 cases were reported in literature. The diagnosis is difficult, clinical and radiological signs are the same as those of any intrascrotal tumour. Orchidectomy is the usual treatment, prognosis depends on the histological features, it is usually better than that of other paratesticular sarcomas. It did not need any additional treatment.

OBJECTIVES: To report a technique of percutaneous endoscopic nephropexy, using a polyglactin suture passed through the kidney, in patients with nephroptosis. PATIENTS AND METHODS: Four women presenting with symptomatic right nephroptosis underwent a percutaneous endoscopic nephropexy. An upper-pole calyx was accessed percutaneously and a 24-F working sheath was placed. Another needle access was made through a lower-pole calyx and a #2 polyglactin suture was passed into the renal pelvis. It was then pulled out through the upper-pole tract using the nephroscope. A retroperitoneoscopy was performed and the tip of the nephroscope was used to cause nephrolysis. After inserting the nephrostomy tube the polyglactin suture was passed into the subcutaneous tissue and then tied without too much tension, to avoid cutting the parenchyma. RESULTS: The operative duration was 33 min and the hospital stay after surgery was 3.5 days. The nephrostomy catheter was removed 5 days after surgery. There were no complications, especially no haemorrhagic, infectious, lithiasic or thoracic complications. The four patients were relieved of their initial symptoms, with a mean follow-up of 28 months. Ultrasonography and/or intravenous urography showed the kidney at a higher location with the patient standing. CONCLUSIONS: This technique combines the nephrostomy tract used in percutaneous techniques with the suture and nephrolysis used in laparoscopic techniques. Moreover, this procedure seems to be safe, with satisfactory anatomical and clinical results and a lower morbidity. However, a larger series will be necessary to establish its long-term morbidity and success rate.
RESUMO
Renal hydatid cyst ranks third among all visceral localisations. It is most often latent due to the deep situation of the kidney. The authors report their experience with 22 cases of hydatid disease of the kidney, and place special emphasis on wedge resection of the cyst which is a simple and effective operation.

The association of an oncocytoma and of an angiomyolipoma within the same renal tumor is infrequent. The authors review the literature on the basis of two cases. Histogenesis and the clinicopathological analysis show that such an association is accidental. It makes up 0.5% of kidney tumors, and 1% of oncocytomas are associated with an angiomyolipoma. The risk to encounter a malignant tumor is of 25% in such cases.
