A case of torsion of the spleen complicated by bowel occlusion occurred in a 3 1/2-yr-old boy is reviewed. Pathogenesis, symptoms and diagnostics are discussed. The rarity of the lesion in childhood is underlined together with the utility, in diagnostics, of plain films of the abdomen taken some hours one from the others.

BACKGROUND: In this retrospective investigation the symptoms, signs, and laboratory findings collected in 2 groups of patients with simple and complicated acute appendicitis, respectively, have been observed in order to give some indication for a correct diagnosis and surgical treatment. METHODS: A total of 103 consecutive patients affected by simple and complicated acute appendicitis submitted to surgical operation have been studied. RESULTS: Data collected show statistically significant differences between clinical presentation of simple and complicated acute appendicitis. CONCLUSIONS: The conclusion is draws in that anamnesis and clinical examination of the patients affected by acute appendicitis are the best indications for an exact diagnosis and to select patients who need an immediate operation.
RESUMO
The authors report a case of urachal fistula diagnosed after repeated omphalitis, this disease was associated with coronal hypospadias. Diagnosis was done thanks to a fistulography; subsequently, during the operation, to evidence the fistula, this was injected with methylene blue, this expedient showed that the fistula was longer than that disclosed from fistulography. To carry out a complete extirpation of the fistula the authors were bound to make a cystotomy. The course after operation was good and the patient discharged after a few days. After 5 years the patient is in perfect health. This case allowed some anatomic, clinical and semeiotic considerations.

The Authors report a case of peritonitis due to perforation of a single small bowel metastasis in a woman affected by right upper lobar lung carcinoma still with neck's lymphnodes and cerebellar metastasis. They underline the rarity of the finding and analyze its clinical, therapeutic, prognostic problems.

The Authors report their experience in the field of transcistic peroperating colangiography performed by the use of polythene cannulae and discuss the advantages of this procedure based on the results of more than 350 cases. The simplicity of the procedure, its quick employment and-most of all-its harmlessness in comparison to other techniques is underlined.

Authors experience in the Dixon's operation regarding the surgical treatment of cancer of the rectum and recto-sigmoid junction is reviewed. It consists in 224 cases of malignant lesions which were treated by Miles operation (106 cases--47%), Dixon's operation (49 cases--21%), palliation anterior resection (16 cases--7%), other operations (53 cases--24%). After a brief discussion on anatomo-pathological conditions and tecniques, the Authors consider their results: operatory mortality 4%; complications: total anastomosis dehiscence 1 case, partial anastomosis dehiscence 14%; local repetitions 10%; hepatic metastasis within 5 ys 40%; survival at 5 ys 46%. Functional results were always very good. The Authors conclude that, even considering the large rate (56%) of wide-spread cancers treated, Dixon's operation, at least in their experience, doesn't seem to satisfy those needs of radically requested by a correct surgical therapy, which only Miles' operation guarantees.

The authors review a total of 132 cases of acute neoplastic obstruction of the large gut seen in their clinical material over the last twenty years. In the light of the results obtained they discuss the indications and limitations of the various surgical strategies adopted (radical surgery, derivative surgery, and palliative resection); they conclude that radical procedures are best for the majority of cases.

The authors offer a critical review of eight cases of hemorrhagic colorectitis and Crohn's disease (regional ileitis) managed with medical treatments, and eleven like cases treated surgically, during the last ten years. After a discussion of the resources and limitations of medical therapy, the authors describe the various surgical strategies adopted in their cases; they define the possibility of obtaining final cures in Crohn's disease with extended ileocolectomy; and concerning ulcerative forms, they point out the greater efficacy of surgical procedures involving sacrifice of the rectum compared to those in which the terminal segment of the gut is used immediately or later for restoring continuity of the intestinal canal.
