RESUMO

Despite highly variable efficacy, BCG (Bacillus Calmette-Guérin) is the only vaccine available to prevent the tuberculosis (TB). Genomic heterogeneity between attenuated BCG strains and virulent Mycobacterium tuberculosis might help to explain this vaccine's impaired capacity to induce long-term protection. Here, we investigate the lipid-related genes absent in attenuated BCG strains in order to correlate changes in both lipid metabolism and cell-wall lipid content to vaccine impairment. Whole genome sequences of M. tuberculosis H37Rv and the six most used BCG strains worldwide were aligned and the absent regions functionally categorized. Genomes of the BCG strains showed a total of 14 non-homologous lipid-related genes, including those belonging to mce3 operon, as well as the gene echaA1, which encodes an enoyl-CoA hydratase, and the genes encoding phospholipases PlcA, PlcB and PlcC. Taken together, the depletion of these M. tuberculosis H37Rv genomic regions were associated with marked alterations in lipid-related genes of BCG strains. Such alterations may indicate a dormant-like state and can be determining factors to the vaccine's inability to induce long-term protection. These lipids can be further evaluated as an adjuvant to boost the current BCG-based vaccine.

OBJECTIVE: To develop a registration standard with diagnoses, outcomes and nursing interventions for an Emergency Care Unit. METHOD: This is applied research of technological development developed in three steps: elaboration of diagnoses/outcomes and interventions statements following the International Classification for Nursing Practice; assessment of diagnosis/outcome relevance; organization of diagnosis/outcome and interventions statements according to health needs described in TIPESC. RESULTS: A total of 185 diagnoses were prepared, of which 124 (67%) were constant in the classification, and 61 had no correspondence. Of the 185 diagnoses, 143 (77%) were rated as relevant by 32 experienced emergency room nurses, and 495 nursing interventions were correlated to diagnoses/outcomes. CONCLUSION: It was possible to build a record standard for the Emergency Care Unit following standardized terminology, containing diagnostic statements/outcomes and relevant interventions for nursing practice assessed by nurses with practice in emergency.

OBJECTIVE: To analyze epidemiological surveillance actions for people with cancer based on the perspective of advocacy. METHOD: Qualitative study, Convergent Care Research type, combined with the framework of health advocacy. It was carried out in the Epidemiological Surveillance of the Health Department of a municipality in the southern region of Brazil. RESULTS: Eleven health service professionals participated in the study, from June 2020 to July 2021, making up 14 group meetings. Two aspects were discussed: (1) problems involving the management of the work process in the network services, directly influencing user assistance; and (2) issues related to the training management of the professional who works at these services, where the lack of knowledge regarding the legislation leads to serious consequences for the users. CONCLUSION: The advocacy strengthened health defense concepts and ideas, mobilizing actions related to cancer, acting as a bridge between the members of the group and the sectors holding power to change circumstances that prevent compliance with public policies and current legislation.

OBJECTIVES: To make a dialog about the nursing professionals' perception regarding how they cope with COVID-19 and the repercussions on their practice and personal life. METHODS: This is a qualitative study, typified as participatory action research, which was carried out using Paulo Freire's Research Itinerary linked to the steps of the Nursing Process. To that end, the following guiding question was launched: How is it for you to act as a nursing professional in the hospital area during the COVID-19 pandemic? RESULTS: Three syntheses emerged, which guided the discussion: The challenges of being a nursing professional in the pandemic. The learning and growth that the challenges of the pandemic have generated and Nursing as the protagonist of care. The Virtual Culture Circle was a space where, despite the limitations, provided a social interaction among the participants, with mutual exchange of experiences, with many reflections, besides expressions of feelings, experiences and learning obtained during the COVID-19 pandemic. CONCLUSIONS: The nurses perceived that, although this moment highlights and appreciates the profession, nursing is overloaded and exhausted by the COVID-19 pandemic, with repercussions on professional and personal life. The care for those who care needs to be planned and implemented in different scenarios, and the Nursing Process built based on theoretical and scientific knowledge guide the effective improvement of the quality of health care.

The objective of the experiment was to evaluate the effect of the addition of different concentrations of the antioxidants Coenzyme Q10 (CoQ10) and melatonin to equine semen freezing diluent, alone or in combination, during the cryopreservation process. Twenty ejaculates (n = 5 stallions) were divided in groups: Control (C, without the addition of antioxidants), melatonin 0.75 mM (MEL1), melatonin 1.5 mM (MEL2), CoQ10 40 µg/mL (Q1), CoQ10 200 µg/mL (Q2), and CoQ10 40 µg/mL+ 0.75 mM melatonin (Q1 +MEL1). Q1 and Q2 groups demonstrated intact plasma membrane and high mitochondrial membrane potential after 30 (M-30) and 60 (M-60) min of incubation compared with the control group (Q1: 64.8 % ± 9.9 %, Q2: 65.2 % ± 10.5 %, C: 55.1 % ± 10.0 %; M-30 and Q1: 63.3 % ± 10.4 %, Q2: 64.6 % ± 10.8 %, C: 53.1 ± 10.6 %; M-60; P < 0.05). Melatonin conferred greater membrane stability at all evaluated times compared with the control group (MEL1: 42.1 % ± 6.0 %; MEL2: 44.0 % ± 6.7 %, C: 35.9 % ± 5.9 %; M-0; MEL1: 40.8 % ± 5.6 %; MEL2: 42.6 % ± 7.2 %, C: 33.1 % ± 6.6 %; M-30 and MEL1: 37.5 % ± 7.4 %; MEL2: 39.1 % ± 7.2 %; C: 31.3 % ± 6.5 %; M-60; P < 0.05). The use of antioxidants alone or in combination resulted in lower levels of lipoperoxidation at all times evaluated compared with in the control group (P < 0.05). In conclusion, CoQ10 and melatonin were effective in the cryopreservation of equine semen by decreasing lipoperoxidation and promoting a higher percentage of spermatozoa with a high mitochondrial potential, total and progressive motility, and prevention of membrane lipid disorder.

INTRODUCTION: BCG is the only licensed vaccine against tuberculosis (TB) and, in Brazil, comprises part of the recommended vaccine schedule within the first month of life. Due to a local manufacturing shortage of BCG Moreau, BCG Russia was introduced in 2017 by the Brazilian Ministry of Health. OBJECTIVE: To evaluate differences in immune responses induced by BCG Moreau and BCG Russia in infants, in addition to scar formation. METHODS: The present case series involved 15 healthy infants who were vaccinated within the first seven days of life with one of two strains of BCG, then followed for 12 weeks or longer. Cytokine levels were measured before and after vaccination in whole blood culture supernatants previously stimulated in vitro with either BCG strain, heat-killed M. tuberculosis H37Rv or in the absence of stimulation. BCG scarring was also documented. RESULTS: Infants vaccinated with BCG Moreau exhibited increased background IL-2, IL-10 and IL-4 production, yet no differences were found in those vaccinated with BCG Russia. Although both strains induced higher levels of IL-2 and IFN-γ, elevated IL-6, TNF and IL-10 production was also seen in response to BCG Russia. In contrast, no specific responses were observed against heat-killed M. tuberculosis H37Rv, with the exception of increased IL-2 following BCG Moreau vaccination. Although documented in both groups, scarring was milder and less frequent following BCG Russia vaccination. CONCLUSIONS: Similar Th1 profiles were found following immunization with either type of BCG vaccine evaluated herein, with more pronounced cytokine production detected in response to the Russia strain. Overall, vaccination was well-tolerated and scarring evolved as expected for both BCG strains.

OBJECTIVE: To report a virtual Cultural Circle experience implemented with nurses to allow them to dialogue and promote health amidst the pandemic of the Coronavirus Disease 2019 (COVID-19). METHOD: Experience report of a virtual Cultural Circle based on Paulo Freire's framework. The stages were correlated to the oil lamp, the symbol of the nursing profession: the lamp body was considered the Thematic Investigation, the lamp support corresponded to Encoding and Decoding, and the light corresponded to Critical Unveiling. Fourteen nurses working in different health settings located in different Brazilian regions participated. RESULTS: Two themes emerged: adjusting my inner light and taking care of my family and work. FINAL CONSIDERATIONS: The nurses exchanged experiences and strengthened themselves to overcome the challenges imposed by the fight against the COVID-19, acquiring autonomy to take care of themselves and provide care to others.

OBJECTIVE: To reflect on the perception of nurses working in Primary Health Care about the development of the COVID-19 vaccination campaign. METHOD: This is a participatory action qualitative study, based on the methodological theoretical precepts by Paulo Freire, which consists of three interconnected phases: Thematic Research, Coding and Decoding, and Critical Unveiling. Three virtual Culture Circles were held between February and March 2021, with 11 nurses from Primary Health Care, residing in Santa Catarina, Brazil. RESULTS: Nurses defined two themes for discussion in the Culture Circle: 1. Matches in the COVID-19 vaccination campaign, representing the potentialities; 2. Mismatches in the COVID-19 vaccination campaign, in which the weaknesses were revealed. CONCLUSION: Nurses indicated potentialities such as teamwork, with hope, motivation, and joy in the vaccination campaign operationalization, through innovative and old actions. They also highlighted weaknesses such as lack of specific training and communication, difficulties with recording and application of the immunobiological, mentioning the anti-vaccine movement and the intense work, which requires adjustments to improve assistance to the population.

OBJECTIVE: to identify the main biosafety measures for preventing COVID-19 in healthcare professionals. METHODS: this is an integrative literature review, with studies published between January and July 2020, on the MEDLINE/PubMed, Scopus, Embase, Web of Science, LILACS, SciELo, Wiley Online Library, Cochrane CINAHL databases. The selection of studies followed the PRISMA recommendations. RESULTS: among the 2,208 publications identified, 12 studies comprised the sample, which enabled the analysis in four thematic categories: The importance of using recommendations about the use of personal protective equipment; The restructuring of new operational and clinical routines and flows in the practice of services; Monitoring professionals, especially testing; Conducting training. CONCLUSIONS: the phenomena involved are innumerable, covering operational management and the training of teams to deal with highly infectious pathogens and disease outbreaks.

We present a non-human primate mycobacterial growth inhibition assay (MGIA) using in vitro blood or cell co-culture with the aim of refining and expediting early tuberculosis vaccine testing. We have taken steps to optimise the assay using cryopreserved peripheral blood mononuclear cells, transfer it to end-user institutes, and assess technical and biological validity. Increasing cell concentration or mycobacterial input and co-culturing in static 48-well plates compared with rotating tubes improved intra-assay repeatability and sensitivity. Standardisation and harmonisation efforts resulted in high consistency agreements, with repeatability and intermediate precision <10% coefficient of variation (CV) and inter-site reproducibility <20% CV; although some systematic differences were observed. As proof-of-concept, we demonstrated ability to detect a BCG vaccine-induced improvement in growth inhibition in macaque samples, and a correlation between MGIA outcome and measures of protection from in vivo disease development following challenge with either intradermal BCG or aerosol/endobronchial Mycobacterium tuberculosis (M.tb) at a group and individual animal level.

The only currently available approach to early efficacy testing of tuberculosis (TB) vaccine candidates is in vivo preclinical challenge models. These typically include mice, guinea pigs and non-human primates (NHPs), which must be exposed to virulent M.tb in a 'challenge' experiment following vaccination in order to evaluate protective efficacy. This procedure results in disease development and is classified as 'Moderate' in severity under EU legislation and UK ASPA licensure. Furthermore, experiments are relatively long and animals must be maintained in high containment level facilities, making them relatively costly. We describe an in vitro protocol for the direct mycobacterial growth inhibition assay (MGIA) for use in the macaque model of TB vaccine development with the aim of overcoming some of these limitations. Importantly, using an in vitro assay in place of in vivo M.tb challenge represents a significant refinement to the existing procedure for early vaccine efficacy testing. Peripheral blood mononuclear cell and autologous serum samples collected from vaccinated and unvaccinated control animals are co-cultured with mycobacteria in a 48-well plate format for 96 hours. Adherent monocytes are then lysed to release intracellular mycobacteria which is quantified using the BACTEC MGIT system and colony-forming units determined relative to an inoculum control and stock standard curve. We discuss related optimisation and characterisation experiments, and review evidence that the direct NHP MGIA provides a biologically relevant model of vaccine-induced protection. The potential end-users of the NHP MGIA are academic and industry organisations that conduct the assessment of TB vaccine candidates and associated protective immunity using the NHP model. This approach aims to provide a method for high-throughput down-selection of vaccine candidates going forward to in vivo efficacy testing, thus expediting the development of a more efficacious TB vaccine and offering potential refinement and reduction to the use of NHPs for this purpose.

Tuberculosis (TB) is a major global health problem and the only currently-licensed vaccine, BCG, is inadequate. Many TB vaccine candidates are designed to be given as a boost to BCG; an understanding of the BCG-induced immune response is therefore critical, and the opportunity to relate this to circumstances where BCG does confer protection may direct the design of more efficacious vaccines. While the T cell response to BCG vaccination has been well-characterized, there is a paucity of literature on the humoral response. We demonstrate BCG vaccine-mediated induction of specific antibodies in different human populations and macaque species which represent important preclinical models for TB vaccine development. We observe a strong correlation between antibody titers in serum versus plasma with modestly higher titers in serum. We also report for the first time the rapid and transient induction of antibody-secreting plasmablasts following BCG vaccination, together with a robust and durable memory B cell response in humans. Finally, we demonstrate a functional role for BCG vaccine-induced specific antibodies in opsonizing mycobacteria and enhancing macrophage phagocytosis in vitro, which may contribute to the BCG vaccine-mediated control of mycobacterial growth observed. Taken together, our findings indicate that the humoral immune response in the context of BCG vaccination merits further attention to determine whether TB vaccine candidates could benefit from the induction of humoral as well as cellular immunity.

OBJECTIVES: to analyze the philosophical, theoretical and methodological conceptions of the Theory of Praxic Intervention in Nursing in Collective Health by professors at a federal university, aiming at structuring a reference proposal for teaching nursing care. METHODS: Convergent Assistance Research, with 17 teachers, gathered in convergence groups for instrumentalization and production about the referential. RESULTS: the focus was primarily on issues considered preliminary to the theoretical and philosophical deepening of theory; and then a theoretical-methodological essay on historicity and dialectics was developed for teaching in nursing care. FINAL CONSIDERATIONS: when electing Theory of Praxic Intervention in Nursing in Collective Health, the potential of the referential to make operational, in the teaching of assistance, the assumptions of historicity and dialectics operational, founding new conceptions for care. This decision represents a paradigmatic challenge exposing weaknesses in the formation related to its Cartesian origin and its limitations regarding intrinsic philosophical complexity.

OBJECTIVES: To reflect on teaching experience in the application of the spiritual nursing care called permission for departing. METHODS: It is a methodological reflection and description of a subtle technology for spiritual nursing care called permission for departing. RESULTS: the permission for departing is a spiritual care that allows for an intentional therapeutic relationship of trust and safety among the professional, patients, and the family, enabling the expression of feelings, beliefs, and religious or spiritual rites that help in death and dying situations. FINAL CONSIDERATIONS: a concept structured by words and attitudes reinforcing what is positive was coined, aiming at a consciousness state of peace and the promotion of dignity in the death and dying process, as well as for time for the patients, their families and the team to experience contemplation and parting.

OBJETIVES: to analyze the ingredients of the competence that the nurses use in the performance of their work in hemotherapy. METHODS: qualitative study with 22 nurses, accomplished through documentary study, observation and semi-structured interview, with resources of Atlas.ti software based on the foundations of Historical Materialism Dialectic and Ergology. Performed Content Analysis. RESULTS: the domain of specific knowledge of hemotherapy and the time of experience in the area, allied to the motivation of the worker and the ability to work in a team favor the competent action in the work activities. On the other hand, the lack of adequate work conditions, especially in relation to adequate materials, equipment and structure, impairs the work of the nurse in hemotherapy. FINAL CONSIDERATIONS: experience gained is critical to successful decision making. In addition, adequate working conditions, updating of knowledge and ability in teamwork favor a scenario of safe practices.

ABSTRACT Objective: To describe the process of implementing a cancer surveillance technical group based on the health advocacy framework. Method: Convergent Care Research addressing 11 representatives of the support and governance system of the healthcare network in a town in Santa Catarina, Brazil. Data were collected from June 2020 to July 2021 in online meetings called convergence groups. The analysis followed the apprehension, synthesis, theorization, and transference steps. Results: The motivations for creating the group included the town's cancer epidemiological context, the need to meet the annual health program, and the professionals' duties in this context. Based on the advocacy framework, the group associated the implementation process with concepts such as integrality, humanization, and professional practice in health. Along this path, strategies were acknowledged and supported the group, such as the establishment and appropriation of philosophical and theoretical bases, in addition to actions such as creating a statute, planning activities, developing instruments, and identifying priorities to implement tasks effectively. Conclusion: Knowledge was exchanged, and a process for providing integral and equitable healthcare in cancer surveillance was developed collectively. Hence, advocacy proved to be a theoretical pillar for the political action of the technical group's members, translating practice into patient rights advocacy.


RESUMEN Objetivo: describir el proceso de implementación de un grupo técnico de vigilancia del cáncer, fundamentado en el referencial de advocacy en salud. Métodos: Investigación Convergente Asistencial realizada con 11 profesionales, representantes de puntos de atención y del sistema de apoyo y gobernanza de la red de atención a la salud de un municipio de Santa Catarina, en Brasil. La recolección fue desarrollada de junio/2020 a julio/2021 en reuniones online denominadas grupos de convergencia. El análisis siguió las etapas de comprensión, síntesis, teorización y transferencia. Resultados: el grupo evidenció como motivaciones para su creación el escenario epidemiológico del cáncer en el municipio, la necesidad de atender la programación anual de salud, y los compromisos profesionales en este contexto. En su proceso de implementación, y a partir del referencial de advocacy en salud, el grupo lo asoció a otros conceptos como la integralidad, la humanización, y el ejercicio profesional en el área de la salud. En este recorrido, fueron reconocidas estrategias iniciales, como la definición y la apropiación de bases filosóficas y teóricas para anclar el grupo, así como estrategias de acciones desde la elaboración de un regimiento, planificación de actividades del grupo, construcción de instrumentos, e identificación de prioridades para implementación efectiva de los trabajos. Conclusión: hubo promoción e intercambio de conocimientos y, colectivamente, se estructuró un proceso para atención integral y ecuánime en la vigilancia del cáncer. Se confirmó el advocacy como pilar teórico para acción política de los profesionales en el grupo técnico, y sus prácticas se traducen en acciones de defesa de los derechos de los usuarios.


RESUMO Objetivo: descrever o processo de implementação de um grupo técnico de vigilância do câncer fundamentado no referencial de advocacy em saúde. Métodos: Pesquisa Convergente Assistencial realizada com 11 profissionais representantes de pontos de atenção e do sistema de apoio e governança da rede de atenção à saúde de um município de Santa Catarina, Brasil. A coleta foi desenvolvida de junho/2020 a julho/2021 em reuniões online denominadas grupos de convergência. A análise seguiu as etapas de apreensão, síntese, teorização e transferência. Resultados: o grupo evidenciou como motivações para sua criação o cenário epidemiológico do câncer no município, a necessidade de atender a programação anual de saúde, e os compromissos profissionais neste contexto. Em seu processo de implementação, e a partir do referencial do advocacy em saúde, o grupo o associou a outros conceitos como a integralidade, a humanização, e o exercício profissional na área da saúde. Neste percurso, foram reconhecidas estratégias iniciais como a definição e a apropriação de bases filosóficas e teóricas para ancorar o grupo, bem como estratégias de ações desde a elaboração de um regimento, planejamento de atividade do grupo, construção de instrumentos, e identificação de prioridades para implementação efetiva dos trabalhos. Conclusão: houve promoção e compartilhamentos de conhecimentos e, coletivamente, estruturou-se um processo para atenção integral e equânime na vigilância do câncer. Confirmou-se o advocacy como pilar teórico para ação política dos profissionais no grupo técnico, e suas práticas traduzem-se em ações de defesa dos direitos dos usuários.

RESUMO Objetivo: descrever a percepção de enfermeiros sobre o cuidado ao paciente com câncer na Atenção Primária à Saúde. Método: estudo qualitativo, desenvolvido em 26 Centros de Saúde da Família de um município do Oeste catarinense - Brasil, entre setembro e outubro de 2022. Participaram 33 enfermeiros, que responderam um questionário on-line. O texto foi interpretado com base nos preceitos conceituais da cultura organizacional da teoria administrativa de Chiavenatto. Questões objetivas foram analisadas pela frequência das respostas; para as dissertativas, utilizou-se o Discurso do Sujeito Coletivo. Resultados: os enfermeiros percebem-se despreparados e identificam ausência de protocolos e fluxos organizacionais. Considerações finais: na Atenção Primária à Saúde há a crença consolidada no cotidiano laboral e que se torna integrante da cultura organizacional dos serviços de saúde, que o paciente com câncer deve seguir para o cuidado especializado. Nota-se desconhecimento das políticas de saúde, o que implica o profissional na atenção ao paciente com câncer.


ABSTRACT Objective: To describe nurses' perceptions of cancer patient care in Primary Health Care. Method: A qualitative study was conducted in 26 Family Health Centers in a municipality in western Santa Catarina - Brazil, between September and October 2022. A total of 33 nurses took part and completed an online questionnaire. The text was interpreted based on the conceptual precepts of organizational culture from Chiavenatto's administrative theory. The frequency of responses analyzed objective questions; the Collective Subject Discourse was used for the essay questions. Results: Nurses perceive themselves as unprepared and identify a lack of protocols and organizational flows. Final considerations: In primary Health Care, there is a belief consolidated in everyday work and becomes part of the organizational culture of health services, that cancer patients should go on to specialist care. There is a lack of knowledge of health policies, which implicates professionals in caring for cancer patients.


RESUMEN Objetivo: describir las percepciones de los enfermeros sobre la atención al paciente oncológico en Atención Primaria. Método: estudio cualitativo realizado en 26 Centros de Salud de la Familia de un municipio del oeste de Santa Catarina - Brasil, entre septiembre y octubre de 2022. Participaron 33 enfermeros que cumplimentaron un cuestionario en línea. El texto se interpretó basándose en los preceptos conceptuales de la cultura organizativa de la teoría administrativa de Chiavenatto. Las preguntas objetivas se analizaron por la frecuencia de las respuestas; para las preguntas de ensayo, se utilizó el discurso del sujeto colectivo. Resultados: los enfermeros no se sienten preparados e identifican una falta de protocolos y flujos organizativos. Consideraciones finales: en Atención Primaria existe la creencia, consolidada en el trabajo diario y que se ha convertido en parte integrante de la cultura organizativa de los servicios sanitarios, de que los pacientes con cáncer deben pasar a la atención especializada. Existe un desconocimiento de las políticas sanitarias, que implica a los profesionales en la atención a los pacientes con cáncer.

OBJECTIVE: To know the perception of expert professionals in infection control and prevention on the teaching of skills for the prevention and control of infections related to health care in undergraduate courses in Health Sciences. METHOD: We used the Delphi technique, developed in four sequential rounds. Thirty-one nurses and eight physicians participated in the study. Qualitative data were analyzed through content analysis; the quantitative ones, from the descriptive statistics. RESULTS: The importance of the courses has had teachers with expertise in infection control and prevention added to arguments about the development of the subject in the curricula by means of a specific subject or as a transversal theme. CONCLUSIONS: In order to cover the complexity of the elements that are interconnected for professional training, teaching must be based on pedagogical strategies that provoke reflection in students, encouraging them to develop critical thinking about their experiences.

ABSTRACT Objective: To analyze epidemiological surveillance actions for people with cancer based on the perspective of advocacy. Method: Qualitative study, Convergent Care Research type, combined with the framework of health advocacy. It was carried out in the Epidemiological Surveillance of the Health Department of a municipality in the southern region of Brazil. Results: Eleven health service professionals participated in the study, from June 2020 to July 2021, making up 14 group meetings. Two aspects were discussed: (1) problems involving the management of the work process in the network services, directly influencing user assistance; and (2) issues related to the training management of the professional who works at these services, where the lack of knowledge regarding the legislation leads to serious consequences for the users. Conclusion: The advocacy strengthened health defense concepts and ideas, mobilizing actions related to cancer, acting as a bridge between the members of the group and the sectors holding power to change circumstances that prevent compliance with public policies and current legislation.


RESUMEN Objetivo: Analizar las acciones de vigilancia epidemiológica de personas con cáncer desde una perspectiva de advocacy. Método: Estudio cualitativo, del tipo Investigación Convergente Asistencial, combinado con el referente de advocacy en salud. Realizado en la Vigilancia Epidemiológica del Departamento de Salud de un municipio de la región sur de Brasil. Resultados: Participaron 11 profesionales de los servicios de salud, entre junio de 2020 y julio de 2021, conformando 14 encuentros grupales. Se discutieron dos aspectos: (1) problemas relacionados con la gestión del proceso de trabajo en los servicios de red, que influyen directamente en la atención al usuario; y (2) problemas relacionados con la gestión de la formación del profesional que atiende estos servicios, donde el desconocimiento de la legislación trae graves consecuencias para los usuarios. Conclusión: La advocacy fortaleció conceptos y concepciones de defensa de la salud, movilizando acciones relacionadas con el cáncer, actuando como puente entre los integrantes del grupo y los sectores que detentan el poder para modificar circunstancias que impiden el cumplimiento de las políticas públicas y la legislación vigente.

Método: Estudo qualitativo, do tipo Pesquisa Convergente Assistencial, aliado ao referencial da advocacia em saúde. Realizado na Vigilância Epidemiológica da Secretaria de Saúde de um município na região Sul do Brasil. Resultados: Participaram 11 profissionais de serviços de saúde, entre junho de 2020 a julho de 2021, perfazendo 14 reuniões do grupo. Discutiram-se dois aspectos: (1) problemas envolvendo a gestão do processo de trabalho nos serviços da rede, que influenciam diretamente na assistência ao usuário; e (2) problemas relacionados à gestão da formação do profissional que atende nestes serviços, onde o desconhecimento relativo às legislações traz consequências graves para os usuários. Conclusão O advocacy fortaleceu conceitos e concepções de defesa à saúde, mobilizando ações relativas ao câncer, atuando como uma ponte entre os membros do grupo e os setores que detêm poder para modificar circunstâncias que impeçam o cumprimento das políticas públicas e da legislação vigente.

Objective: To verify the applicability of the Lasater Clinical Judgment Rubric (LCJR) instrument to rate nursing professionals' performance regarding the development of clinical judgment in the application of the Nursing Process (NP). Materials and methods: Descriptive survey using the LCJR instrument ­consisting of four phases and eleven dimensions­ to classify clinical judgment. Results: Thirty-four nurses from a public hospital in western Santa Catarina, Brazil, distributed in six sectors, participated in the study. Nurses were classified by performance levels in relation to the ability of clinical judgment in the application of the steps of the NP. It was observed that at the "beginner" level there is a higher prevalence of professionals related to the dimensions Search for information, Recognizing deviations from expected standards, and Technical skills, linked to the intensive care unit clinic. As for the "exemplary" level, none of the sectors scored on the analyzed dimensions. Moreover, it was found that a longer time of clinical experience and of systematic application of the steps of the NP fosters a better classification in almost all dimensions. Conclusions: Through this research, we verified the assertiveness of the use of the instrument to evaluate the clinical judgment of nurses regarding the application of the steps of the NP. Thus, the use of the LCJR is encouraged as a tool to evaluate the effectiveness of educational interventions performed to nurses and, consequently, stimulate clinical judgment.


Objetivo: validar la aplicabilidad del instrumento Lasater Clinical Judgment Rubric (LCJR) para clasificar el desempeño de los profesionales en enfermería con relación al desarrollo del juicio clínico en la aplicación del proceso de enfermería (PE). Materiales y método: investigación descriptiva que adoptó el instrumento LCJR, compuesto de cuatro fases y once dimensiones, para clasificar el juicio clínico. Resultados: el estudio contó con la participación de 34 profesionales en enfermería de un hospital público del oeste del estado de Santa Catarina (Brasil), distribuidos en seis grupos. Los participantes fueron clasificados por niveles de desempeño en relación con la capacidad de juicio clínico en la aplicación de los pasos del PE. Se evidenció que en el nivel "principiante" existe una mayor prevalencia de profesionales relacionados con las dimensiones: Búsqueda de información, Reconocimiento de desviaciones de los estándares esperados y Habilidades técnicas, vinculadas principalmente a la práctica en unidad de cuidados intensivos. En cuanto al nivel "ejemplar", ninguno de los sectores obtuvo puntajes en las dimensiones analizadas. Además, se identificó que un tiempo mayor tanto de experiencia clínica como de aplicación sistemática de los pasos de la PE conducen a una mejor clasificación en casi todas las dimensiones. Conclusiones: a través de esta investigación fue posible validar la idoneidad del uso del LCJR para evaluar el juicio clínico de los profesionales en enfermería en cuanto a la aplicación de las etapas del PE. Por ende, se recomienda el uso de esta herramienta para evaluar la efectividad de las intervenciones educativas con estos profesionales, estimulando con ello el juicio clínico.


Objetivo: verificar a aplicabilidade do instrumento Lasater Clinical Judgment Rubric (LCJR) para classificar o desempenho dos profissionais de enfermagem quanto ao desenvolvimento do julgamento clínico na aplicação do processo de enfermagem (PE). Materiais e método: trata-se de uma pesquisa descritiva que adotou o instrumento LCJR que é fundamentado em quatro fases e onze dimensões, a fim de classificar o julgamento clínico. Resultados: participaram do estudo 34 profissionais de enfermagem de um hospital público do oeste de Santa Catarina, Brasil, distribuídos em seis setores, os quais foram classificados por níveis de desempenho com relação à habilidade de julgamento clínico na aplicação das etapas do PE. Foi evidenciado que no nível "iniciante" há uma maior prevalência de profissionais relacionados às dimensões Busca de informações, Reconhecimento de desvios dos padrões esperados e Habilidades técnicas, vinculadas principalmente à unidade de terapia intensiva clínica. Quanto ao nível "exemplar", nenhum dos setores obteve pontuação nas dimensões analisadas. Ademais, verificou-se que o maior tempo de experiência clínica e o maior tempo de aplicação sistemática das etapas do PE repercutem em uma melhor classificação nos níveis, em quase todas as dimensões. Conclusões: a partir da pesquisa, verificou-se a assertividade quanto à utilização do instrumento LCJR para avaliar o julgamento clínico de enfermeiros com relação à aplicação das etapas do PE . Dessa forma, incentiva-se o uso dessa ferramenta para avaliar a efetividade de intervenções educativas realizadas com profissionais de enfermagem e assim estimular o julgamento clínico.
