RESUMO

Pitch perception is important for understanding speech prosody, music perception, recognizing tones in tonal languages, and perceiving speech in noisy environments. The two principal pitch perception theories consider the place of maximum neural excitation along the auditory nerve and the temporal pattern of the auditory neurons' action potentials (spikes) as pitch cues. This paper describes a biophysical mechanism by which fine-structure temporal information can be extracted from the spikes generated at the auditory periphery. Deriving meaningful pitch-related information from spike times requires neural structures specialized in capturing synchronous or correlated activity from amongst neural events. The emergence of such pitch-processing neural mechanisms is described through a computational model of auditory processing. Simulation results show that a correlation-based, unsupervised, spike-based form of Hebbian learning can explain the development of neural structures required for recognizing the pitch of simple and complex tones, with or without the fundamental frequency. The temporal code is robust to variations in the spectral shape of the signal and thus can explain the phenomenon of pitch constancy.

This study systematically investigated the effects of frequency, level, and spectral envelope on pitch matching in twelve bimodal cochlear implant (CI) users. The participants were asked to vary the frequency and level of a pure or complex tone (adjustable sounds) presented in the non-implanted ear to match the pitch and loudness of different reference stimuli presented to the implanted ear. Three reference sounds were used: single electrode pulse trains, pure tones, and piano notes. The data showed a significant effect of the frequency and complexity of the reference sounds. No significant effect of the level of the reference sounds was found. The magnitude of effect of frequency was compressed in the implanted ear: on average a difference of seven semitones in the non-implanted ear induced the same pitch change as a difference of 19 to 24 semitones for a stimulus presented to the implanted ear. The spectral envelope of the adjustable sound presented to the non-implanted ear also had a significant effect. The matched frequencies were higher by an average of six semitones for the pure tone compared to a complex tone. Overall, the CI listeners might have matched the stimuli based on timbre characteristics such as brightness.

OBJECTIVES: To compare speech perception outcomes between bilateral implantation (cochlear implants [CIs]) and bimodal rehabilitation (one CI on one side plus one hearing aid [HA] on the other side) and to explore the clinical factors that may cause asymmetric performances in speech intelligibility between the two ears in case of bilateral implantation. DESIGN: Retrospective data from 2247 patients implanted since 2003 in 15 international centers were collected. Intelligibility scores, measured in quiet and in noise, were converted into percentile ranks to remove differences between centers. The influence of the listening mode among three independent groups, one CI alone (n = 1572), bimodal listening (CI/HA, n = 589), and bilateral CIs (CI/CI, n = 86), was compared in an analysis taking into account the influence of other factors such as duration of profound hearing loss, age, etiology, and duration of CI experience. No within-subject comparison (i.e., monitoring outcome modifications in CI/HA subjects becoming CI/CI) was possible from this dataset. Further analyses were conducted on the CI/CI subgroup to investigate a number of factors, such as implantation side, duration of hearing loss, amount of residual hearing, and use of HAs that may explain asymmetric performances of this subgroup. RESULTS: Intelligibility ranked scores in quiet and in noise were significantly greater with both CI/CI and CI/HA than with a CI-alone group, and improvement with CI/CI (+11% and +16% in quiet and in noise, respectively) was significantly better than with CI/HA (+6% and +9% in quiet and in noise, respectively). From the CI/HA group, only subjects with ranked preoperative aided speech scores >60% performed as well as CI/CI participants. Furthermore, CI/CI subjects displayed significantly lower preoperative aided speech scores on average compared with that displayed by CI/HA subjects. Routine clinical data available from the present database did not explain the asymmetrical results of bilateral implantation. CONCLUSIONS: This retrospective study, based on basic speech audiometry (no lateralization cues), indicates that, on average, a second CI is likely to provide slightly better postoperative speech outcome than an additional HA for people with very low preoperative performance. These results may be taken into consideration to refine surgical indications for CIs.

UNLABELLED: Although the perception of music is generally poor in cochlear implant users, there are a few excellent performers. OBJECTIVE: The aim of this study was the assessment of different aspects of music perception in one exceptional cochlear implant user. DESIGN: The assessments included pitch direction discrimination, melody and timbre recognition, relative and absolute pitch judgment, and consonance rating of musical notes presented through the sound processor(s). STUDY SAMPLE: An adult cochlear implant user with musical background who lost her hearing postlingually, and five normally-hearing listeners with musical training participated in the study. RESULTS: The CI user discriminated pitch direction for sounds differing by one semitone and recognized melody with nearly 100% accuracy. Her results in timbre recognition were better than average published data for cochlear implant users. Her consonance rating, and relative and absolute pitch perception were comparable to normally-hearing listeners with musical training. CONCLUSION: The results in this study showed that excellent performance is possible on musical perception tasks including pitch perception using present day cochlear implant technologies. Factors that may explain this user's exceptional performance are short duration of deafness, pre- and post-deafness musical training, and perfect pitch abilities before the onset of deafness.

OBJECTIVES: The objectives of this report are to (1) describe the speech perception abilities of long-term pediatric cochlear implant (CI) recipients by comparing scores obtained at elementary school (CI-E, 8 to 9 yrs) with scores obtained at high school (CI-HS, 15 to 18 yrs); (2) evaluate speech perception abilities in demanding listening conditions (i.e., noise and lower intensity levels) at adolescence; and (3) examine the relation of speech perception scores to speech and language development over this longitudinal timeframe. DESIGN: All 112 teenagers were part of a previous nationwide study of 8- and 9-yr-olds (N = 181) who received a CI between 2 and 5 yrs of age. The test battery included (1) the Lexical Neighborhood Test (LNT; hard and easy word lists); (2) the Bamford Kowal Bench sentence test; (3) the Children's Auditory-Visual Enhancement Test; (4) the Test of Auditory Comprehension of Language at CI-E; (5) the Peabody Picture Vocabulary Test at CI-HS; and (6) the McGarr sentences (consonants correct) at CI-E and CI-HS. CI-HS speech perception was measured in both optimal and demanding listening conditions (i.e., background noise and low-intensity level). Speech perception scores were compared based on age at test, lexical difficulty of stimuli, listening environment (optimal and demanding), input mode (visual and auditory-visual), and language age. RESULTS: All group mean scores significantly increased with age across the two test sessions. Scores of adolescents significantly decreased in demanding listening conditions. The effect of lexical difficulty on the LNT scores, as evidenced by the difference in performance between easy versus hard lists, increased with age and decreased for adolescents in challenging listening conditions. Calculated curves for percent correct speech perception scores (LNT and Bamford Kowal Bench) and consonants correct on the McGarr sentences plotted against age-equivalent language scores on the Test of Auditory Comprehension of Language and Peabody Picture Vocabulary Test achieved asymptote at similar ages, around 10 to 11 yrs. CONCLUSIONS: On average, children receiving CIs between 2 and 5 yrs of age exhibited significant improvement on tests of speech perception, lipreading, speech production, and language skills measured between primary grades and adolescence. Evidence suggests that improvement in speech perception scores with age reflects increased spoken language level up to a language age of about 10 yrs. Speech perception performance significantly decreased with softer stimulus intensity level and with introduction of background noise. Upgrades to newer speech processing strategies and greater use of frequency-modulated systems may be beneficial for ameliorating performance under these demanding listening conditions.

The effects of four configurations of an environmental noise reduction (ENR) algorithm on preferences, speech understanding, and satisfaction were investigated. The gain reduction at 0 dB modulation depth was either 10 dB in all channels (ENR StrongFlat) or shaped from 2-10 dB across channels according to a speech importance function (ENR MildSII). This gain reduction was either invariant (ENR Constant) or varied with (ENR Variable) the noise level. Ten hearing-impaired participants blindly compared pairs of configurations in real-world situations and recorded their preferences. Sentence reception thresholds (SRTs) were measured in quiet and noise, and satisfaction was rated with speech in noise. Half of the participants preferred ENR MildSII and half preferred ENR StrongFlat. All preferred ENR Variable to ENR Constant. Overall, the preferred ENR configuration was preferred to ENR off in 90% of responses. No statistically significant effect on SRTs was found, but a clinically significant effect of up to 2 dB could not be ruled out from the available data. ENR significantly improved satisfaction for listening comfort, ease of speech understanding, and sound quality.

BACKGROUND: Hearing aids amplify low-intensity sounds to make them audible while keeping high-intensity sounds at an acceptable loudness for listeners with impaired hearing. PURPOSE: The purpose of this analysis was to assess loudness and satisfaction at the same time using a combined loudness and satisfaction questionnaire to rate 18 everyday environmental sounds. RESEARCH DESIGN: Ten sets of data from four studies, covering three conditions, were analyzed. The three conditions were unaided, wide dynamic range compression (WDRC), and adaptive dynamic range optimization (ADRO). In total, there were 61 subjects giving over 3,000 pairs of ratings for loudness and satisfaction. RESULTS: The analysis found a strong relationship between loudness and satisfaction ratings for this set of listeners and conditions. The maximum satisfaction ratings corresponded to sounds with "comfortable" loudness ratings. Satisfaction was lowest for sounds that were "uncomfortably loud." Sounds that were very soft or inaudible also received low satisfaction ratings unless the sounds were expected to be soft, such as the sound of one's own breathing. CONCLUSIONS: Hearing aid fittings that place most sounds at a comfortable level are likely to be more satisfactory than hearing aid fittings that produce more sounds close to hearing thresholds or discomfort levels. Aided conditions gave higher loudness and satisfaction ratings than the unaided condition, and the ADRO hearing aids gave significantly higher satisfaction ratings than the WDRC hearing aids.

This article documented spoken language outcomes for preschool children with hearing loss and examined the relationships between language abilities and characteristics of children such as degree of hearing loss, cognitive abilities, age at entry to early intervention, and parent involvement in children's intervention programs. Participants were evaluated using a combination of the Child Development Inventory, the Peabody Picture Vocabulary Test, and the Preschool Clinical Evaluation of Language Fundamentals depending on their age at the time of assessment. Maternal education, cognitive ability, and family involvement were also measured. Over half of the children who participated in this study had poor language outcomes overall. No significant differences were found in language outcomes on any of the measures for children who were diagnosed early and those diagnosed later. Multiple regression analyses showed that family participation, degree of hearing loss, and cognitive ability significantly predicted language outcomes and together accounted for almost 60% of the variance in scores. This article highlights the importance of family participation in intervention programs to enable children to achieve optimal language outcomes. Further work may clarify the effects of early diagnosis on language outcomes for preschool children.

BACKGROUND: Sensorineural hearing loss is the most common sensory deficit among older adults. Some of the psychosocial consequences of this condition include difficulty in understanding speech, depression, and social isolation. Studies have shown that older adults with hearing loss show some age-related cognitive decline. Hearing aids have been proven as successful interventions to alleviate sensorineural hearing loss. In addition to hearing aid use, the positive effects of auditory training-formal listening activities designed to optimize speech perception-are now being documented among adults with hearing loss who use hearing aids, especially new hearing aid users. Auditory training has also been shown to produce prolonged cognitive performance improvements. However, there is still little evidence to support the benefits of simultaneous hearing aid use and individualized face-to-face auditory training on cognitive performance in adults with hearing loss. OBJECTIVE: This study will investigate whether using hearing aids for the first time will improve the impact of individualized face-to-face auditory training on cognition, depression, and social interaction for adults with sensorineural hearing loss. The rationale for this study is based on the hypothesis that, in adults with sensorineural hearing loss, using hearing aids for the first time in combination with individualized face-to-face auditory training will be more effective for improving cognition, depressive symptoms, and social interaction rather than auditory training on its own. METHODS: This is a crossover trial targeting 40 men and women between 50 and 90 years of age with either mild or moderate symmetric sensorineural hearing loss. Consented, willing participants will be recruited from either an independent living accommodation or via a community database to undergo a 6-month intensive face-to-face auditory training program (active control). Participants will be assigned in random order to receive hearing aid (intervention) for either the first 3 or last 3 months of the 6-month auditory training program. Each participant will be tested at baseline, 3, and 6 months using a neuropsychological battery of computer-based cognitive assessments, together with a depression symptom instrument and a social interaction measure. The primary outcome will be cognitive performance with regard to spatial working memory. Secondary outcome measures include other cognition performance measures, depressive symptoms, social interaction, and hearing satisfaction. RESULTS: Data analysis is currently under way and the first results are expected to be submitted for publication in June 2018. CONCLUSIONS: Results from the study will inform strategies for aural rehabilitation, hearing aid delivery, and future hearing loss intervention trials. Although the neural mechanisms underlying pitch perception are not yet fully understood, there is general agreement that place and temporal representations of pitch are both used by the auditory system. This paper describes a neural network model of pitch perception that integrates both codes of pitch and explores the contributions of, and the interactions between, the two representations in simulated pitch ranking trials in normal and cochlear implant hearing. The model can replicate various psychophysical observations including the perception of the missing fundamental pitch and sensitivity to pitch interval sizes. As a case study, the model was used to investigate the efficiency of pitch perception cues in a novel sound processing scheme, Stimulation based on Auditory Modelling (SAM), that aims to improve pitch perception in cochlear implant hearing. Results showed that enhancement of the pitch perception cues would lead to better pitch ranking scores in the integrated model only if the place and temporal pitch cues were consistent.

Purpose: Retinal prostheses provide vision to blind patients by eliciting phosphenes through electrical stimulation. This study explored whether character identification and image localization could be achieved through direct multiple-electrode stimulation with a suprachoroidal retinal prosthesis. Methods: Two of three retinitis pigmentosa patients implanted with a suprachoroidal electrode array were tested on three psychophysical tasks. Electrode patterns were stimulated to elicit perception of simple characters, following which percept localization was tested using either static or dynamic images. Eye tracking was used to assess the association between accuracy and eye movements. Results: In the character identification task, accuracy ranged from 2.7% to 93.3%, depending on the patient and character. In the static image localization task, accuracy decreased from near perfect to <20% with decreasing contrast (patient 1). Patient 2 scored up to 70% at 100% contrast. In the dynamic image localization task, patient 1 recognized the trajectory of the image up to speeds of 64 deg/s, whereas patient 2 scored just above chance. The degree of eye movement in both patients was related to accuracy and, to some extent, stimulus direction. Conclusions: The ability to identify characters and localize percepts demonstrates the capacity of the suprachoroidal device to provide meaningful information to blind patients. The variation in scores across all tasks highlights the importance of using spatial cues from phosphenes, which becomes more difficult at low contrast. The use of spatial information from multiple electrodes and eye-movement compensation is expected to improve performance outcomes during real-world prosthesis use in a camera-based system. Purpose: With a retinal prosthesis connected to a head-mounted camera, subjects can perform low vision tasks using a combination of electrode discrimination and head-directed localization. The objective of the present study was to investigate the contribution of retinotopic electrode discrimination (perception corresponding to the arrangement of the implanted electrodes with respect to their position beneath the retina) to visual performance for three recipients of a 24-channel suprachoroidal retinal implant. Proficiency in retinotopic discrimination may allow good performance with smaller head movements, and identification of this ability would be useful for targeted rehabilitation. Methods: Three participants with retinitis pigmentosa performed localization and grating acuity assessments using a suprachoroidal retinal prosthesis. We compared retinotopic and nonretinotopic electrode mapping and hypothesized that participants with measurable acuity in a normal retinotopic condition would be negatively impacted by the nonretinotopic condition. We also expected that participants without measurable acuity would preferentially use head movement over retinotopic information. Results: Only one participant was able to complete the grating acuity task. In the localization task, this participant exhibited significantly greater head movements and significantly lower localization scores when using the nonretinotopic electrode mapping. There was no significant difference in localization performance or head movement for the remaining two subjects when comparing retinotopic to nonretinotopic electrode mapping. Conclusions: Successful discrimination of retinotopic information is possible with a suprachoroidal retinal prosthesis. Head movement behavior during a localization task can be modified using a nonretinotopic mapping. Behavioral comparisons using retinotopic and nonretinotopic electrode mapping may be able to highlight deficiencies in retinotopic discrimination, with a view to address these deficiencies in a rehabilitation environment. Omnidirectional, supercardioid, and adaptive directional microphones (ADM) were evaluated in combination with the ADRO amplification scheme for eight participants with moderate sloping hearing losses. The ADM produced better speech perception scores than the other two microphones in all noise conditions. Participants performed the Hearing in Noise Test sentences at -4.5 dB SNR or better, which is similar to the level achievable with normal hearing. The Speech, Spatial and Qualities of Hearing Scale indicated no disadvantages of using the ADM relative to the omnidirectional microphone in real-life situations. The ADM was preferred over the omnidirectional microphone in 54% of situations, compared to 17% preferences for the omnidirectional microphone, and 29% no preference. The combination of the ADM to improve SNR, and ADRO to keep the signal output comfortable and audible provided near-normal hearing performance for people with moderate hearing loss. The ADM is the recommended microphone configuration for ADRO hearing aids.

Purpose: Phosphenes are the fundamental building blocks for presenting meaningful visual information to the visually impaired using a bionic eye device. The aim of this study was to characterize the size, shape, and location of phosphenes elicited using a suprachoroidal retinal prosthesis. Methods: Three patients with profound vision loss due to retinitis pigmentosa were implanted with a suprachoroidal electrode array, which was used to deliver charge-balanced biphasic constant-current pulses at various rates, amplitudes, and durations to produce phosphenes. Tasks assessing phosphene appearance, location, overlap, and the patients' ability to recognize phosphenes were performed using a custom psychophysics setup. Results: Phosphenes were reliably elicited in all three patients, with marked differences in the reported appearances between patients and between electrodes. Phosphene shapes ranged from simple blobs to complex forms with multiple components in both space and time. Phosphene locations within the visual field generally corresponded to the retinotopic position of the stimulating electrodes. Overlap between phosphenes elicited from adjacent electrodes was observed with one patient, which reduced with increasing electrode separation. In a randomized recognition task, two patients correctly identified the electrode being stimulated for 57.2% and 23% of trials, respectively. Conclusions: Phosphenes of varying complexity were successfully elicited in all three patients, indicating that the suprachoroidal space is an efficacious site for electrically stimulating the retina. The recognition scores obtained with two patients suggest that a suprachoroidal implant can elicit phosphenes containing unique information. This information may be useful when combining phosphenes into more complex and meaningful images that provide functional vision.

RESUMO

Since the 1950s, vision researchers have been working towards the ambitious goal of restoring a functional level of vision to the blind via electrical stimulation of the visual pathways. Groups based in Australia, USA, Germany, France and Japan report progress in the translation of retinal visual prosthetics from the experimental to clinical domains, with two retinal visual prostheses having recently received regulatory approval for clinical use. Regulatory approval for cortical visual prostheses is yet to be obtained; however, several groups report plans to conduct clinical trials in the near future, building upon the seminal clinical studies of Brindley and Dobelle. In this review, we discuss the general principles of visual prostheses employing electrical stimulation of the visual pathways, focusing on the retina and visual cortex as the two most extensively studied stimulation sites. We also discuss the surgical and functional outcomes reported to date for retinal and cortical prostheses, concluding with a brief discussion of novel developments in this field and an outlook for the future.

Adaptive dynamic range optimization (ADRO) is an amplification strategy that uses digital signal processing techniques to improve the audibility, comfort, and intelligibility of sounds for people who use cochlear implants and/or hearing aids. The strategy uses statistical analysis to select the most information-rich section of the input dynamic range in multiple-frequency channels. Fuzzy logic rules control the gain in each frequency channel so that the selected section of the dynamic range is presented at an audible and comfortable level. The ADRO processing thus adaptively optimizes the dynamic range of the signal in multiple-frequency channels. Clinical studies show that ADRO can be fitted easily to all degrees of hearing loss for hearing aids and cochlear implants in a direct and intuitive manner, taking the preferences of the listener into account. The result is high acceptance by new and experienced hearing aid users and strong preferences for ADRO compared with alternative amplification strategies. The ADRO processing is particularly well suited to bimodal and hybrid stimulation which combine electric and acoustic stimulation in opposite ears or in the same ear, respectively.

This research was conducted to evaluate the efficacy of an online speech perception test (SPT) for the measurement of hearing and hearing aid fitting in comparison with conventional methods. Phase 1 was performed with 88 people to evaluate the SPT for the detection of significant hearing loss. The SPT had high sensitivity (94%) and high selectivity (98%). In Phase 2, phonetic stimulus-response matrices derived from the SPT results for 408 people were used to calculate "Infograms™." At every frequency, there was a highly significant correlation (p < 0.001) between hearing thresholds derived from the Infogram and conventional audiograms. In Phase 3, initial hearing aid fittings were derived from conventional audiograms and Infograms for two groups of hearing impaired people. Unaided and aided SPTs were used to measure the perceptual benefit of the aids for the two groups. The mean increases between unaided and aided SPT scores were 19.6%, and 22.2% (n = 517, 484; t = 2.2; p < 0.05) for hearing aids fitted using conventional audiograms and Infograms respectively. The research provided evidence that the SPT is a highly effective tool for the detection and measurement of hearing loss and hearing aid fitting. Use of the SPT reduces the costs and increases the effectiveness of hearing aid fitting, thereby enabling a sustainable teleaudiology business model.

To evaluate the efficacy of a suprachoroidal retinal prosthesis, a highly configurable external neurostimulator is required. In order to meet functional and safety specifications, it was necessary to develop a custom device. A system is presented which can deliver charge-balanced, constant-current biphasic pulses, with widely adjustable parameters, to arbitrary configurations of output electrodes. This system is shown to be effective in eliciting visual percepts in a patient with approximately 20 years of light perception vision only due to retinitis pigmentosa, using an electrode array implanted in the suprachoroidal space of the eye. The flexibility of the system also makes it suitable for use in a number of other emerging clinical neurostimulation applications, including epileptic seizure suppression and closed-loop deep brain stimulation. Open-set word and sentence speech-perception test scores are commonly used as a measure of hearing abilities in children and adults using cochlear implants and/or hearing aids. These tests are usually presented auditorily with a verbal response. In the case of children, scores are typically lower and more variable than for adults with hearing impairments using similar devices. It is difficult to interpret children's speech-perception scores without considering the effects of lexical knowledge and speech-production abilities on their responses. This study postulated a simple mathematical model to describe the effects of hearing, lexical knowledge, and speech production on the perception test scores for monosyllabic words by children with impaired hearing. Thirty-three primary-school children with impaired hearing, fitted with hearing aids and/or cochlear implants, were evaluated using speech-perception, reading-aloud, speech-production, and language measures. These various measures were incorporated in the mathematical model, which revealed that performance in an open-set word-perception test in the auditory-alone mode is strongly dependent on residual hearing levels, lexical knowledge, and speech-production abilities. Further applications of the model provided an estimate of the effect of each component on the overall speech-perception score for each child.

A new amplification strategy (ADRO), based on 64 independently operating channels, was compared with a nine-channel wide dynamic range compression strategy (WDRC). Open-platform in-the-ear hearing instruments were configured either with ADRO or the manufacturer's WDRC strategy. Twenty-two subjects with mild to moderate hearing loss took home the ADRO or WDRC hearing aids. After three weeks' acclimatization, the aids were evaluated using monosyllables in quiet at 50 to 65 dB SPL and sentences in eight-talker babble. The acclimatization and evaluation were repeated in the second phase of the balanced reverse-block blind experimental design. The ADRO program showed a statistically significant mean advantage of 7.85% word score (95% confidence interval 3.19% to 12.51%; p = 0.002) and 6.41% phoneme score for the monosyllables in quiet (95% confidence interval 2.03% to 10.79%; p = 0.006). A statistically significant advantage of 7.25% was also found for the ADRO program in background noise (95% confidence interval 1.95% to 12.55%; p = 0.010). The results are consistent with earlier data for listeners with moderate to severe hearing loss.
