RESUMO

Two cases have been observed in which the deep femoral artery originated at the medial surface of the femoral and surrounded medially the femoral vein before entering its distribution area. The literature is examined and stress laid on the rarity of the abnormality and its importance as a possible factor in phlebothrombosis.

RESUMO

Three cases of congenital a-v fistulas of the lung encountered in some 10,000 patients examined are reported. Emphasis is laid on the possibility that the disease may be misinterpreted for a long time and treated wrongly. It is considered that the most significant examination for diagnostic purposes is angiocardiopneumography because even in the rare cases in which it does not reveal the fistula, it shows up its existence by the early visualization of the left heart.

RESUMO

6 cases of suprarenal phaeochromocytoma were operated on at the Department of Special Surgical Pathology and Clinical Propaedeutics of the University of Turin between 1954 and 1975. The age of the patients was between 32 and 53, 5 cases being male and 1 female. Personal data are compared with larger series and mention is made of the importance of certain pharmacological tests and new radiological techniques for the diagnosis of tumour nature and localization. An atypical case in which exact clinical classification was only possible histologically a posteriori is discussed. Immediate and long-term results were good. Although not all patients presented normalised pressure values, the disappearance of hypertensive crises and normalization of glycaemia and urinary catecholamines were constant observations.

567 haemorrohoidectomies 83 of which total haemorrhoidectomies for piles with surgical indications, were performed with recourse to day-hospital admission. The authors examine the results of this experience and advise this therapy and this type of admission for pathologies with different cost/benefit ratio.

RESUMO

The paper describes the removal of a broad-based tubulo-villous adenoma from the middle rectum using a transanal endoscopic microsurgical technique. This procedure, which was introduced by Buess, Thess and Hutterer in 1983, represents an alternative to the classic transanal technique, compared to which it is more accurate, and to the trans-sacral and transperitoneal methods compared to which it is less invasive.

RESUMO

The authors examine the prophylaxis of infections caused by cholelithiasis in kidney and heart transplant candidates as a new indication for videolaparoscopic cholecystectomy (VLC). The study included 6 patients in dialysis for chronic renal insufficiency and one patient suffering from cyanogenic congenital cardiopathy with asymptomatic gallbladder calculosis. The results obtained show that there are no substantial differences compared to patients without associated pathologies and justifies the inclusion of "prophylactic" VLC in preparatory treatment protocols for kidney and heart transplant in patients suffering from cholelithiasis. The authors emphasise the necessary technical measures to prevent hemorrhage, intraoperative loss of CO2 and postoperative laparoceles.

RESUMO

The difficulties which arise when performing VLC in obese patients primarily concern the induction of pneumoperitoneum and the use of normal surgical instruments. This is due to the thickness of the abdominal wall and the scarce anchorage of the cutis and subcutis to the musculo-fascia level. Two technical variations of pneumoperitoneum are proposed in order to overcome the first problem, whereas with regard to the latter the authors suggest moving the trocar insertion points towards the site of the gallbladder.

RESUMO

The faecal loading associated with colonic obstruction is a fundamental deterrent to constructing a primary distal colonic anastomosis. The authors describe their preliminary experience of intraoperative colonic irrigation in the management of left sided large bowel emergencies. Five patients were operated using this technique (4 cancer obstruction and one sigma volvulus). They show good results performing primary anastomosis after emergency distal resection combined with complete colonic cleansing.

A case of paraganglioma situated between right renal vein, right renal artery and inferior vena cava is here with described. The report constitutes the rise to frame with precision the tumor according with WHO classification. Moreover criteria are to be discussed for nature and seat diagnosis, as well for the choice of the best way of access.
