AIM: The aim of this retrospective study was to analyse the effectiveness of Andresen appliance in inducing an increased mandibular dimension and, above all, to test the "null" hypotesis that the patients' vertical skelettofacial morfology could influence this increase using the Ricketts' method of VERT. METHODS: Cephalometric records of 42 skeletal class II patients treated at the Orthodontic Department of Catania University were randomly selected. The sample was divided for vertical facial patterns, according to the facial classification method of Ricketts (VERT), into three groups: brachyfacial, mesiofacial and dolicofacial. Ten landmarks were located on the lateral cephalometric radiographs; growth in the horizontal-sagittal direction and in the vertical direction was evaluated using the measurements of total mandibular length (Co-Gn, Co-Pg, Ar-Pg, Ar-Gn), mandibular sagittal position (SNB, ANB) and mandibular ramus height (S-Go, Ar-Go, Co-Go). The intragroup comparisons were made using two tailed t-tests, while the intergroup comparisons were analyzed statistically using the Kruskal-Wallis test. RESULTS: The cephalometric values before (T0) and after (T1) treatment showed significant changes for the majority of the cephalometric variables in each group. Our findings demonstrated a statistically significant greater increase of mandibular dimension in the brachyfacial group, while similar results were found for mesiofacial and dolichofacial groups. CONCLUSION: The "null" hypothesis, that the skeletal pattern of subjects could influence the increase of mandibular dimension after Andresen treatment, had to be accepted: the brachyfacial patients showed a greater anterior mandibular displacement when compared to mesiofacial or dolichofacial group.

Two distinct alpha-(1-3)-fucosyltransferase activities have been identified in the colon carcinoma cell lines HT-29 and COLO-205. While both enzymatic activities exhibit similar affinities for a synthetic alpha-(1-3) acceptor and GDP-fucose, they differ with respect to divalent cation requirements, N-ethylmaleimide inhibition, and glycoprotein substrate specificity. The COLO-205 alpha-(1-3) activity exhibits maximal enzymatic activity in the presence of 20 mM Mn2+ but retains less than 10% activity in the absence of divalent cations. In contrast, the optimal Mn2+ concentration for the HT-29 enzyme is 1 mM, although this activity is relatively insensitive to divalent cation stimulation. In addition, the HT-29 alpha-(1-3)-fucosyltransferase activity is resistant to inhibition by 30 mM N-ethylmaleimide and relatively inactive toward the glycoprotein substrate fetuin as compared to its desialylated derivative, asialofetuin. The COLO-205 activity is inhibited approximately 90% by N-ethylmaleimide and is equally active with either glycoprotein acceptor. Although the alpha-(1-3) specific activities are similar in both cell lines, N-ethylmaleimide-sensitive alpha-(1-4) fucosyltransferase activity is 40-fold higher in COLO-205 as compared to HT-29, suggesting that the COLO-205 fucosyltransferase activity may be an alpha-(1-3/4) enzyme, while the HT-29 activity appears to be an alpha-(1-3) specific form. Further examination of a panel of cell lines, tumor biopsies, and xenografts, based on the effect of metal ions and N-ethylmaleimide, indicated that both enzyme activities are similarly expressed in human colon carcinoma tissue.

Transient transfection of simian COS cells with a recombinant plasmid encoding the human transforming growth factor TGF-beta 2 precursor protein results in the production of a latent, biologically inactive protein. Upon acidification, recombinant TGF-beta 2 exhibits full biological activity, including inhibition of mink lung epithelial cell growth, stimulation of anchorage-independent growth of murine embryonic fibroblasts, and competition for TGF-beta receptor binding. Further analysis of conditioned media with antiserum to either a pro- [amino acid (aa) residues 1-220] or mature [aa 297-414] peptide of the TGF-beta 2 precursor suggests that TGF-beta 2, similar to TGF-beta 1 production in Chinese hamster ovary cells [Gentry et al., Mol. Cell. Biol. 7 (1987) 3418-3427], is initially synthesized as a larger precursor protein which is proteolytically cleaved to yield the mature 112-aa transforming growth factor.

Challenge of human cells with auranofin, 2,3,4,6-tetra-O-acetyl-1-thio-beta-D-glucopyranosato-S-triethylpho sphine gold(I) (Ridaura), a gold-containing compound approved by the FDA for the treatment of rheumatoid arthritis, induces the specific synthesis of a 32-kD stress protein (p32) [Caltabiano et al., Biochem. biophys. Res. Commun. 138, 1074 (1986)]. To establish a structure-activity relationship for this effect, a series of auranofin ligands, gold analogs, and other anti-arthritic agents were examined for their abilities to stimulate p32 synthesis. The results indicate that the gold atom is necessary for enhanced expression of p32. However, the structure of co-ordinated ligands also affected potency, and gold complexes bearing several phosphine or thiosugar groups exhibited the greatest activity. These data indicate that the distinct potencies of auranofin analogs probably reflect their membrane permeability and subsequent delivery of pharmacologically active concentrations of gold to the cytoplasmic compartment.

The acrofacial dysostoses (AFDs) are a heterogeneous group of disorders combining defects of craniofacial and limb development. The predominantly preaxial form is called Nager AFD, the predominantly postaxial form of AFD (POADS) is also known as the Genée-Wiedemann or Miller syndrome. The former appears to be about twice as common as the latter with well-documented autosomal dominant and recessive occurrences in both conditions. Only 1 AD occurrence of POADS is known, but 5 sets of sibs are suggestive of AR inheritance. Heterogeneity of apparently nonsyndromal AFD of both types is powerful support for the hypothesis that the AFDs are polytopic field defects arising during blastogenesis. Six other previously described forms of AFD include the AFD syndrome of Kelly et al. (AR), the Rodríguez or Madrid form of AFD (AR or XLR), the Reynolds or Idaho form of AFD (AD), the Arens or Tel Aviv type of AFD (AF?), the presumed AR AFD syndrome of Richieri-Costa et al., and the AD Patterson-Stevenson-Fontaine syndrome. Here we review the AFDs and report on a previously apparently undescribed autosomal or X-linked dominant form of AFD with mental retardation in a Sicilian mother and her 4 sons.

A prospective randomized clinical trial was conducted to test the effectiveness of long-term and short-term prophylaxis with cefuroxime in preventing morbidity after cesarean section. Sixty patients who required emergency cesarean section were randomly assigned to one of three treatment groups: a control group of 20 patients receiving no prophylactic antibiotics; a group of 20 patients receiving 24 hours of cefuroxime prophylaxis (0.75 gm 30 to 60 minutes before surgery and at 8 hours and 16 hours after surgery); and a group of 20 patients receiving five days of cefuroxime prophylaxis (0.75 gm three times a day, the first dose being given postoperatively). The short-term and long-term prophylaxes were equally effective in reducing morbidity, assessed by postoperative temperatures, presence or absence of endometritis, and duration of postoperative hospital stay.

The aim of the present study was to determine the effect of a calcium channel blocker on renal function, urinary endothelin excretion and endothelin receptor number in rats. Administration of cyclosporine resulted in a significant impairment of renal function when measured by either [14C]inulin or 24 h creatinine clearances. This nephrotoxicity was associated with statistically significant (P less than 0.05) increases in urinary endothelin excretion and renal endothelin receptor number. Treatment with nifedipine attenuated cyclosporine A-induced renal dysfunction and reduced urinary endothelin excretion. The data provide further evidence of a role for endothelin in cyclosporine A-induced nephrotoxicity.

The biosynthesis and modulation of the vasoconstrictor peptide endothelin was studied in the conditioned medium from cultured bovine pulmonary artery endothelial (BPAE) cells. Conditioned medium from cultured BPAE cells produced contraction of isolated rabbit aortic rings. Incubation of BPAE cells with the protease inhibitors TPCK or isatoic anhydride attenuated the extent of conditioned medium-induced contractions. Incubation of BPAE cells with thrombin produced an enhancement of conditioned medium-induced contraction by approximately 25%. Endothelin levels in conditioned medium were measured by RIA and incubation of BPAE cells with TPCK or isatoic anhydride significantly reduced endothelin levels, whereas incubation with thrombin or transforming growth factor beta-1 stimulated the levels of endothelin in the conditioned medium. These data indicate that endothelin may be modulated by certain protease inhibitors and by platelet and immune cell mediators and suggest a potential new mode of vascular tone regulation.

Heat shock protein (HSP) 27 is a member of the small HSP family that plays a part in the regulation of epithelial cell growth and differentiation, wound healing, apoptosis and cell protection against inflammatory cytotoxicity mediators. Thus the expression of HSP27 was investigated immunohistochemically in periapical granulomas with epithelial rests of Malassez and in radicular cysts. Anti-HSP27 mouse monoclonal antibody and peroxidase-labeled streptavidin-biotin standard technique were used to study the expression of HSP27. Proliferating epithelial cell rests, and islands of epithelium and epithelial lining of microcysts strongly reacted throughout all layers, whereas radicular cysts epithelial lining presented mainly a moderate suprabasal staining pattern. However both the proliferating epithelial cell rests and the radicular cysts shared an over-expression of HSP27 immunostaining intensity in coincidence with the presence of local infiltration of immune cells. HSP27 may play several roles in periapical lesions that include contributing to the migration of epithelial cell rests and an increased resistance both to necrotic and apoptotic cell deaths.

Vascular endothelial growth factor (VEGF), also known as vascular permeability factor (VPF), is a multifunctional cytokine. It is overexpressed in several conditions, which are characterized by vascular hyperpermeability and angiogenesis. In this investigation, we have evaluated the possibility that VEGF/VPF could be expressed in periapical lesions. We studied 17 periapical granulomas and 6 periapical cysts by immunohistochemistry. An immunopositive reaction for VEGF/VPF was observed in all 23 periapical lesions; however, the intensity of immunostaining by anti-VEGF antibody varied according to histopathological findings. In periapical granulomas without epithelium, almost all of the inflammatory cells were immunoreactive to anti-VEGF/VIP antibody. In periapical granulomas, which had rests of Malassez in them, some inflammatory cells were stained. On the other hand, epithelial cells always were stained by VEGF/VPF antibody, both in periapical lesions with epithelium and in radicular cysts. This study demonstrated that periapical lesions express VEGF/VPF, although with some differences in cell immunolabeling, which correlated to the lesions' stages of development. Initially, VEGF/VPF would assure angiogenesis and vascular hyperpermeability, resulting in accumulation of inflammatory cells, later it could be involved in cyst fluid accumulation. We hypothesize, therefore, that VEGF/VPF expression plays an important role in the pathogenesis of periapical granulomas and enlargement of radicular cysts by several mechanisms.

A high density of blood vessels is found in specimens of temporomandibular joint (TMJ) disc at any stage of internal derangement of the joint, but the factors responsible for angiogenesis in the disc have not been described. The purpose here was to investigate, in human TMJ discs, the expression of vascular endothelial growth factor (VEGF), a multifunctional cytokine that contributes to angiogenesis. Specimens, free of significant morphological alterations and with varying degrees of disc tissue degeneration/regeneration, were studied by immunohistochemistry for VEGF in order to correlate immunohistochemical with histopathological findings. In normal discs and discs with minor pathological changes, fibroblast-like cells, fibrochondrocytes and chondrocyte-like cells were either not or only weakly immunostained by VEGF antibody. In disc specimens from internal derangement of the TMJ with significant tissue degeneration/regeneration, VEGF was consistently expressed. In these specimens, immunoreaction products for VEGF were observed both in the disc and in the endothelial cells of newly formed vessels. This VEGF immunolocalization is consistent with the stimulation of angiogenesis and the morphogenesis and differentiation of chondrocytes. Therefore VEGF expression by disc chondrocyte-like cells might reflect the action of the cytokine as an inducer of angiogenesis and as an autocrine signal for cells of the chondrogenic lineage.

Past research on adults has found that the sensory appeal or taste of foods is a primary determiner of food consumption and how people think about food. The nomothetic nature of this research may have underestimated the impact of health considerations on food choice. This study compared 'nomothetic' and 'idiographic' modes of analysis in 1) determining the relative influence of palatability and perceived healthiness of foods, on preference for the food, and 2) assessing the relationship between palatability and evaluations of healthiness. Additionally, gender differences were examined in relation to within-person correlations between the concepts of preference, palatability and healthiness. Subjects (n = 139) rated 81 foods on preference, palatability and healthiness. Findings from both the idiographic and nomothetic analyses indicated that palatability rather than health considerations determined preferences in young adults. The within-person correlational analysis indicated a large number of persons, mostly female, who preferred unhealthy food. The sample was equally split in their evaluations of healthy food as palatable or not.

RESUMO

A case of squamous cell carcinoma arising in a dermoid cyst of the ovary of a 38 year-old woman is reported. Its frequency is nearly 1%; nevertheless it is opinion that the real frequency could be higher if the hystological examination of the cyst would be made more accurately. Clinical and pathological findings of this case are presented in this brief note. The importance to always make an intraoperatively frozen-section examination is emphasized. The patient is alive and well after six months after surgery.

RESUMO

Our pelviscopic operative series from 1986 to June 1991 is reported. Nowadays our group performs 70% of female genital benign lesions by the pelviscopic technique. Infect laparotomy has been abandoned in case of benign ovarian cyst while the pelviscopic myomectomy rate is about 69%. Our results show that this kind of management is safe and also undeniable advantages such as morbidity reduction, shorter hospital stay, less social cost, quick return to work and renewing of the method make the indications more and more every day.

Australia's "sporting nation" image has been challenged by adolescents' decreasing involvement in active leisure pursuits. A significant number of adolescents experience leisure boredom and dissatisfaction, which have been implicated in drug use and delinquency. Researchers have largely ignored the multivariate nature of adolescent leisure experiences. This North Queensland, Australian adolescent leisure study explored the extent to which adolescent leisure experiences were mediated by individual and situational variables. Seventy-five Cairns and 65 Atherton high school students (66 males and 74 females) were surveyed from grades 8, 10, and 12 (ages 12 to 19), during normal class periods. The Self-Rating Scale (SPS) measure of self-esteem, Sensation-Seeking Scale Form II (SSS), Leisure Boredom Scale (LBS), and a time-use inventory yielded quantitative data. Urban adolescents reported less leisure satisfaction. Participation was highest for passive leisure and lowest for active leisure. Urban adolescents reported higher social leisure, while rural adolescents engaged in more passive leisure. For both Cairns and Atherton, the heaviest substance users were those who scored low on self-esteem and high on sensation-seeking. Atherton adolescents who scored low on self-esteem but high on sensation-seeking, reported the most crime involvement. Methodological issues and implications are discussed and suggestions made for future research.

This study compared professional and volunteer counsellors on a number of key variables in the progression to burnout, namely, stressors, strain, defensive coping, social support, Type A personality, and breakdown. Job stressors unique to each group were identified but there was overwhelming evidence for professionals and volunteers belonging to a homogeneous counselling population as evidenced by similarity across major constructs. Path analysis supported theoretical formulations on burnout by Maslach and Cherniss, and gave support to Lin, Dean, and Ensel's argument that social support may act as a buffer to burnout.

The period after the birth of a child may be characterized by occasions of physical exhaustion. Mothers who self-reported having experienced postnatal depression in the past year and physical exhaustion were examined.

The self-esteem of 69 women attending weight-loss centres was examined in relation to attitudes towards body and weight, actual weight, number of diets undergone and weight fluctuations. Rosenberg's Self-esteem Scale was used to measure self-esteem. Attitudes towards the body and weight were assessed using Freedman's Body-Love Questionnaire. The Body Mass Index was used as a weight-range classification for the women. Body image was assessed using the silhouette chart of Bell, Kirkpatrick, and Rinn. Women with stable weight had the highest self-esteem. The number of diets undertaken was associated with lowered self-esteem. High scores on evaluation of appearance were positively correlated with scores on self-esteem. No association was found between self-esteem and either eating restraint or significant others' attitudes. There was good agreement between the women's perceptions of their own bodies and independent raters' perceptions indicating a lack of body-image distortion for this non-eating-disordered group.

RESUMO

The action of a new antibiotic identified as CM 9164 ("Midecamycin") and provisionally called "Midecacin", has been experimented on patients admitted for dental conditions. The research was carried out on an open basis and erythromycin was used as a comparison. After a short review of the chemicophysical properties and toxicological and pharmacological features of Midecamycin, the purpose, materials and methods of the research, which was done on two groups of patients, are presented. The results demonstrated the therapeutics usefulness of the new antibiotic as its activity is certainly on a par if not greater than that of erythromycin whether considered "historically" or from the viewpoint of the comparison group. The tolerance of the antibiotic was clinically better than that of the other substances. The doses used were much lower than those required with erythromycin. On the basis of these results, the use of Midecamycin is therefore recommended in the treatment of Gram-positive infections in the dental field.

The pattern of experimental caries was examined in rats treated with magnesium fluosilicate associated with manganese and a cariogenic diet. A 33.34% fall in the incidence of caries was observed with respect to a control series, whereas Mg fluosilicate and manganese administered alone caused 30% and 20% reductions respectively. It was also noted that a slight toxic action, expressed as a 3 g fall in body weight, was found with the Mg fluosilicate dose employed; this effect was attenuated by simultaneous administration of manganese.
