RESUMO

Mediation analysis assesses whether an exposure directly produces changes in cognitive behavior or is influenced by intermediate "mediators". Electroencephalographic (EEG) spectral measurements have been previously used as effective mediators representing diverse aspects of brain function. However, it has been necessary to collapse EEG measures onto a single scalar using standard mediation methods. In this article, we overcome this limitation and examine EEG frequency-resolved functional connectivity measures as a mediator using the full EEG cross-spectral tensor (CST). Since CST samples do not exist in Euclidean space but in the Riemannian manifold of positive-definite tensors, we transform the problem, allowing for the use of classic multivariate statistics. Toward this end, we map the data from the original manifold space to the Euclidean tangent space, eliminating redundant information to conform to a "compressed CST." The resulting object is a matrix with rows corresponding to frequencies and columns to cross spectra between channels. We have developed a novel matrix mediation approach that leverages a nuclear norm regularization to determine the matrix-valued regression parameters. Furthermore, we introduced a global test for the overall CST mediation and a test to determine specific channels and frequencies driving the mediation. We validated the method through simulations and applied it to our well-studied 50+-year Barbados Nutrition Study dataset by comparing EEGs collected in school-age children (5-11 years) who were malnourished in the first year of life with those of healthy classmate controls. We hypothesized that the CST mediates the effect of malnutrition on cognitive performance. We can now explicitly pinpoint the frequencies (delta, theta, alpha, and beta bands) and regions (frontal, central, and occipital) in which functional connectivity was altered in previously malnourished children, an improvement to prior studies. Understanding the specific networks impacted by a history of postnatal malnutrition could pave the way for developing more targeted and personalized therapeutic interventions. Our methods offer a versatile framework applicable to mediation studies encompassing matrix and Hermitian 3D tensor mediators alongside scalar exposures and outcomes, facilitating comprehensive analyses across diverse research domains.

Protein Energy Malnutrition (PEM) has lifelong consequences on brain development and cognitive function. We studied the lifelong developmental trajectories of resting-state EEG source activity in 66 individuals with histories of Protein Energy Malnutrition (PEM) limited to the first year of life and in 83 matched classmate controls (CON) who are all participants of the 49 years longitudinal Barbados Nutrition Study (BNS). qEEGt source z-spectra measured deviation from normative values of EEG rhythmic activity sources at 5-11 years of age and 40 years later at 45-51 years of age. The PEM group showed qEEGt abnormalities in childhood, including a developmental delay in alpha rhythm maturation and an insufficient decrease in beta activity. These profiles may be correlated with accelerated cognitive decline.

This paper extends frequency domain quantitative electroencephalography (qEEG) methods pursuing higher sensitivity to detect Brain Developmental Disorders. Prior qEEG work lacked integration of cross-spectral information omitting important functional connectivity descriptors. Lack of geographical diversity precluded accounting for site-specific variance, increasing qEEG nuisance variance. We ameliorate these weaknesses. (i) Create lifespan Riemannian multinational qEEG norms for cross-spectral tensors. These norms result from the HarMNqEEG project fostered by the Global Brain Consortium. We calculate the norms with data from 9 countries, 12 devices, and 14 studies, including 1564 subjects. Instead of raw data, only anonymized metadata and EEG cross-spectral tensors were shared. After visual and automatic quality control, developmental equations for the mean and standard deviation of qEEG traditional and Riemannian DPs were calculated using additive mixed-effects models. We demonstrate qEEG "batch effects" and provide methods to calculate harmonized z-scores. (ii) We also show that harmonized Riemannian norms produce z-scores with increased diagnostic accuracy predicting brain dysfunction produced by malnutrition in the first year of life and detecting COVID induced brain dysfunction. (iii) We offer open code and data to calculate different individual z-scores from the HarMNqEEG dataset. These results contribute to developing bias-free, low-cost neuroimaging technologies applicable in various health settings.

The main goals of the present study were to replicate and extend current knowledge related to paralimbic dysfunctions associated with psychopathy. The research evaluated the quantitative electroencephalography, current density (CD) source and synchronization likelihood analysis during the rest condition and structural magnetic resonance imaging images to compare volumetric and cortical thickness, in inmates recruited from two prisons located in Havana City. The Psychopathy Checklist-Revised (PCL-R) was used as a quantitative measure of psychopathy. This study showed most beta energy and less alpha activity in male psychopath offenders. Low-resolution electromagnetic tomography signified an increase of beta activity in psychopath offender groups within paralimbic regions. The superior temporal gyrus volume was associated with the F1 factor while the fusiform, anterior cingulate and associative occipital areas were primarily associated with the F2 factor of PCL-R scale. Cortical thickness in the left dorsal anterior cingulate cortex and the temporal pole was negatively associated with PCL-R total score.

Objective: This study compares the complementary information from semi-quantitative EEG (sqEEG) and spectral quantitative EEG (spectral-qEEG) to detect the life-long effects of early childhood malnutrition on the brain. Methods: Resting-state EEGs (N = 202) from the Barbados Nutrition Study (BNS) were used to examine the effects of protein-energy malnutrition (PEM) on childhood and middle adulthood outcomes. sqEEG analysis was performed on Grand Total EEG (GTE) protocol, and a single latent variable, the semi-quantitative Neurophysiological State (sqNPS) was extracted. A univariate linear mixed-effects (LME) model tested the dependence of sqNPS and nutritional group. sqEEG was compared with scores on the Montreal Cognitive Assessment (MoCA). Stable sparse classifiers (SSC) also measured the predictive power of sqEEG, spectral-qEEG, and a combination of both. Multivariate LME was applied to assess each EEG modality separately and combined under longitudinal settings. Results: The univariate LME showed highly significant differences between previously malnourished and control groups (p < 0.001); age (p = 0.01) was also significant, with no interaction between group and age detected. Childhood sqNPS (p = 0.02) and adulthood sqNPS (p = 0.003) predicted MoCA scores in adulthood. The SSC demonstrated that spectral-qEEG combined with sqEEG had the highest predictive power (mean AUC 0.92 ± 0.005). Finally, multivariate LME showed that the combined spectral-qEEG+sqEEG models had the highest log-likelihood (-479.7). Conclusion: This research has extended our prior work with spectral-qEEG and the long-term impact of early childhood malnutrition on the brain. Our findings showed that sqNPS was significantly linked to accelerated cognitive aging at 45-51 years of age. While sqNPS and spectral-qEEG produced comparable results, our study indicated that combining sqNPS and spectral-qEEG yielded better performance than either method alone, suggesting that a multimodal approach could be advantageous for future investigations. Significance: Based on our findings, a semi-quantitative approach utilizing GTE could be a valuable diagnostic tool for detecting the lasting impacts of childhood malnutrition. Notably, sqEEG has not been previously explored or reported as a biomarker for assessing the longitudinal effects of malnutrition. Furthermore, our observations suggest that sqEEG offers unique features and information not captured by spectral quantitative EEG analysis and could lead to its improvement.

Introduction. Functional brain differences related to sex in psychopathic behavior represent an important field of neuroscience research; there are few studies on this area, mainly in offender samples. Objective. The aim of this study was to investigate the presence of electrophysiological differences between male and female psychopath offenders; specifically, we wanted to assess whether the results in quantitative EEG, low-resolution electromagnetic tomography (LORETA), and changes in synchronous brain activity could be related to sex influence. Sample and Methods. The study included 31 male and 12 female psychopath offenders, according to the Hare Psychopathy Checklist-Revised criteria from 2 prisons located in Havana City. The EEG visual inspection characteristics and the use of frequency domain quantitative analysis techniques are described. Results. The resting EEG visual analyses revealed a high percentage of EEG abnormalities in both studied groups. Significant statistical differences between the mean parameters of cross spectral measures between psychopathic offender groups were found in the beta band at bilateral frontal derivation and centroparietal areas. LORETA showed differences especially in the paralimbic and parieto-occipital areas Synchronization likelihood revealed a significant group effect in the 26 to 30 Hz band. These results indicate that combining quantitative EEG, LORETA analysis, and synchronization likelihood may improve the neurofunctional differentiation between psychopath offenders of both sexes.

We have identified an electroencephalographic (EEG) based statistical classifier that correctly distinguishes children with histories of Protein Energy Malnutrition (PEM) in the first year of life from healthy controls with 0.82% accuracy (area under the ROC curve). Our previous study achieved similar accuracy but was based on scalp quantitative EEG features that precluded anatomical interpretation. We have now employed BC-VARETA, a novel high-resolution EEG source imaging method with minimal leakage and maximal sparseness, which allowed us to identify a classifier in the source space. The EEGs were recorded in 1978 in a sample of 108 children who were 5-11 years old and were participants in the 45+ year longitudinal Barbados Nutrition Study. The PEM cohort experienced moderate-severe PEM limited to the first year of life and were age, handedness and gender-matched with healthy classmates who served as controls. In the current study, we utilized a machine learning approach based on the elastic net to create a stable sparse classifier. Interestingly, the classifier was driven predominantly by nutrition group differences in alpha activity in the lingual gyrus. This structure is part of the pathway associated with generating alpha rhythms that increase with normal maturation. Our findings indicate that the PEM group showed a significant decrease in alpha activity, suggestive of a delay in brain development. Childhood malnutrition is still a serious worldwide public health problem and its consequences are particularly severe when present during early life. Deficits during this critical period are permanent and predict impaired cognitive and behavioral functioning later in life. Our EEG source classifier may provide a functionally interpretable diagnostic technology to study the effects of early childhood malnutrition on the brain, and may have far-reaching applicability in low resource settings.

The goal of this study is to identify the quantitative electroencephalographic (qEEG) signature of early childhood malnutrition [protein-energy malnutrition (PEM)]. To this end, archival digital EEG recordings of 108 participants in the Barbados Nutrition Study (BNS) were recovered and cleaned of artifacts (46 children who suffered an episode of PEM limited to the first year of life) and 62 healthy controls). The participants of the still ongoing BNS were initially enrolled in 1973, and EEGs for both groups were recorded in 1977-1978 (at 5-11 years). Scalp and source EEG Z-spectra (to correct for age effects) were obtained by comparison with the normative Cuban Human Brain Mapping database. Differences between both groups in the z spectra (for all electrode locations and frequency bins) were assessed by t-tests with thresholds corrected for multiple comparisons by permutation tests. Four clusters of differences were found: (a) increased theta activity (3.91-5.86 Hz) in electrodes T4, O2, Pz and in the sources of the supplementary motor area (SMA); b) decreased alpha1 (8.59-8.98 Hz) in Fronto-central electrodes and sources of widespread bilateral prefrontal are; (c) increased alpha2 (11.33-12.50 Hz) in Temporo-parietal electrodes as well as in sources in Central-parietal areas of the right hemisphere; and (d) increased beta (13.67-18.36 Hz), in T4, T5 and P4 electrodes and decreased in the sources of bilateral occipital-temporal areas. Multivariate Item Response Theory of EEGs scored visually by experts revealed a neurophysiological latent variable which indicated excessive paroxysmal and focal abnormality activity in the PEM group. A robust biomarker construction procedure based on elastic-net regressions and 1000-cross-validations was used to: (i) select stable variables and (ii) calculate the area under ROC curves (AUC). Thus, qEEG differentiate between the two nutrition groups (PEM vs Control) performing as well as visual inspection of the EEG scored by experts (AUC = 0.83). Since PEM is a global public health problem with lifelong neurodevelopmental consequences, our finding of consistent differences between PEM and controls, both in qualitative and quantitative EEG analysis, suggest that this technology may be a source of scalable and affordable biomarkers for assessing the long-term brain impact of early PEM.

BACKGROUND: Few studies have investigated the impact of the psychopathic traits on the EEG of teenagers with conduct disorder (CD). To date, there is no other research studying low-resolution brain electromagnetic tomography (LORETA) technique using quantitative EEG (QEEG) analysis in adolescents with CD and psychopathic traits. OBJECTIVE: To find electrophysiological differences specifically related to the psychopathic traits. The current investigation compares the QEEG and the current source density measures between adolescents with CD and psychopathic traits and adolescents with CD without psychopathic traits. METHODS: The resting EEG activity and LORETA for the EEG fast spectral bands were evaluated in 42 teenagers with CD, 25 with and 17 without psychopathic traits according to the Antisocial Process Screening Device. All adolescents were assessed using the DSM-IV-TR criteria. The EEG visual inspection characteristics and the use of frequency domain quantitative analysis techniques (narrow band spectral parameters) are described. RESULTS: QEEG analysis showed a pattern of beta activity excess on the bilateral frontal-temporal regions and decreases of alpha band power on the left central-temporal and right frontal-central-temporal regions in the psychopathic traits group. Current source density calculated at 17.18 Hz showed an increase within fronto-temporo-striatal regions in the psychopathic relative to the nonpsychopathic traits group. CONCLUSIONS: These findings indicate that QEEG analysis and techniques of source localization may reveal differences in brain electrical activity among teenagers with CD and psychopathic traits, which was not obvious to visual inspection. Taken together, these results suggest that abnormalities in a fronto-temporo-striatal network play a relevant role in the neurobiological basis of psychopathic behavior.

UNLABELLED: Previous EEG studies attempted to examine violent behavior as homogeneous construct. Up to date, there is no other research studying Low-Resolution Brain Electromagnetic Tomography (LORETA) technique in psychopath offenders. OBJECTIVE: To find electrophysiological differences specifically related to the psychopathy construct and independent of the violent behavior. The current investigation compares the QEEG and the current source density measures of violent psychopath offenders to a non-psychopath violent group. METHODS: The resting EEG activity and LORETA for the EEG spectral fast bands were evaluated in 58 violent offenders, 31 with and 27 without psychopathy according to the Hare Psychopathy Checklist - Revised. All subjects were assessed using the DSM IV-R criteria. The EEG visual inspection characteristics and the use of frequency domain quantitative analysis techniques (Narrow band spectral parameters) are described. RESULTS: QEEG analysis showed a pattern of excess of beta activity on the left parieto-temporal regions and bilateral occipital areas and decrease of alpha band on the left centro-temporal and parieto-central derivations in the psychopath group. LORETA signified an increase of beta activity (17.18 Hz) in psychopath group relative to a non- psychopath group within fronto-temporo-limbic regions. CONCLUSIONS: These findings indicate that QEEG analysis and techniques of source localization may reveal differences in brain electrical activity among offenders with psychopathy, which was not obvious to visual inspection. Taken together, these results suggest that abnormalities in a fronto-temporo-limbic network play a relevant role in the neurobiological basis of psychopathy.

The presence of brain dysfunction in violent offenders has been frequently examined with inconsistent results. The aim of the study was to assess the EEG of 84 violent offenders by visual inspection and frequency-domain quantitative analysis in 84 violent prisoners. Low-resolution electromagnetic tomography (LORETA) was also employed for theta band of the EEG spectra. Antisocial personality disorder (ASPD) was present in 50 of the offenders and it was absent in the remaining 34. The prevalence of EEG abnormalities, by visual inspection, was similar for both the ASPD group (82%) and non-ASPD group (79%). The brain topography of these anomalies also did not differ between groups, in contrast to results of the EEG quantitative analysis (QEEG) and LORETA that showed remarkable regional differences between both groups. QEEG analysis showed a pattern of excess of theta-delta activities and decrease of alpha band on the right fronto-temporal and left temporo-parietal regions in the ASPD group. LORETA signified an increase of theta activity (5.08 Hz) in ASPD group relative to non-ASPD group within left temporal and parietal regions. Findings indicate that QEEG analysis and techniques of source localization may reveal differences in brain electrical activity among offenders with ASPD, which was not obvious to visual inspection.

Introducción: El principal objetivo de las emociones consiste en garantizar la homeostasis, la supervivencia y el bienestar del organismo. Objetivo: Demostrar la utilidad de la aplicación de evaluaciones neurofisiológicas y neuropsicológicas a pacientes en quienes se pretenda demostrar el papel significativo de las emociones en la ejecución de determinadas conductas. Métodos: Se realizó entrevista psiquiátrica con fines forenses. Se registró la actividad eléctrica cerebral (EEG) en estado de vigilia y en condiciones basales, se calcularon los generadores de densidad de corriente en la banda theta y se realizó evaluación de reconocimiento de las emociones. Resultados: En la entrevista psiquiátrica se destaca que el miedo fue un elemento importante en el actuar impulsivo y poco previsor del acusado y se demostró una disminución sustancial de sus facultades para comprender el alcance de sus actos y dirigir su conducta en el periodo de tiempo de ocurrencia del delito que se le imputa. El EEG mostró alteraciones en regiones frontales y los generadores de densidad de corriente se localizaron en regiones frontotemporales y áreas asociativas occipitales. Conclusiones: Se recomienda asociar estos estudios con la evaluación psiquiátrica con fines forenses para acrecentar la objetividad de los diagnósticos formulados por peritos actuantes.


Introduction: The main objective of the emotions is to ensure the homeostasis, the survival and the well-being of the organism. Objective: To demonstrate the usefulness of performing neurophysiological and neuro-psychological assessments in patients, in order to demonstrate the significant role of the emotions in the execution of certain behaviours. Methods: A forensic psychiatric interview was conducted. EEG in vigil state was registered, the generators of current density to theta band were calculated, and the emotions recognition test was performed. Results: The results of the psychiatric interview demonstrated that fear was an important element in acting impulsively, and lack of foresight of the accused. A substantial decrease was demonstrated in the ability to understand the scope of the acts and the direction of the behaviour during the time the crime occurred. The EEG showed alterations in frontal regions, and the generators of current density were located in frontal-temporal regions and occipital associative areas. Conclusions: It is recommended to associate these studies with the forensic psychiatric assessment, in order to increase the objectivity of the diagnoses formulated by medical experts.

Los estudios electroencefalográficos en niños con Trastorno por déficit de atención con Hiperactividad (TDAH) reportan perfiles específicos en diferentes subtipos del trastorno. Este estudio investiga la presencia de alteraciones electroencefalográficas en un grupo de niños del subtipo hiperactivo/impulsivo y combinado. Se estudiaron 15 niños del subtipo hiperactivo/impulsivo y 15 del combinado del TDAH a los que se les realizó un electroencefalograma en vigilia. Los principales hallazgos al electroencefalograma fueron el incremento de la actividad lenta frontal y la presencia de descargas epileptiformes en ambos grupos. Se encontraron diferencias significativas entre los espectros de frecuencia de ambos grupos, por exceso en las bandas lentas y por defecto en las rápidas en el hiperactivo/impulsivo respecto al combinado. Estos resultados respaldan el modelo de un déficit de maduración del Sistema Nervioso Central como base del TDAH.


Electroencephalographic studies in children with Attention Deficit Hyperactivity Disorder (ADHD) report specific profiles in different subtypes. This study investigated the presence of Electroencephalogram abnormalities within a sample of children with the Hyperactive-Impulsive and combined subtypes of the disorder. Subjects consisted of 15 children of hyperactive-Impulsive and 15 children of combined subtypes. Electroencephalogram was recorded during an eyes closed resting condition. The principal findings were characterized by increased slow wave activity in frontal area and the visual analysis showed epileptiforms abnormalities in both groups. There was statistic difference in the spectrum of frequency in both subtypes characterized for more slow activity and less rapid activity in the hyperactive-Impulsive subtipe. These results support a model of ADHD resulting from a maturational lag in the Central Nervous System.
