RESUMO

A technique for fabricating lingual rest seats and indirect incisal restorations using a digital workflow is described. After intraoral scanning, the incisal edge position of the restoration and lingual rest seats for a subsequent removable partial denture (RPD) were designed digitally. Adaptation was evaluated with trial restorations, and definitive restorations were printed from a 3-dimensional resin (Varseo Smile Crown Plus Bego; Wilcos). The restorations were cemented with a heated composite resin (Tetric N; Ivoclar AG) polymerized for 40 seconds. The RPD was fabricated and delivered following a conventional technique. This standard, rapid, cost-effective, and straightforward approach allows a controlled and standardized process to obtain lingual rest seats and incisal restorations simultaneously, providing support for a successful Kennedy Class I RPD.

OBJECTIVES: This randomized split-mouth clinical trial investigated the influence of implant macrogeometry on bone properties and peri-implant health parameters during the healing process. MATERIAL AND METHODS: Ninety-nine implants were placed bilaterally in posterior mandibles of 23 patients that received at least four dental implant macrogeometries: standard geometry, Integra (IN) and three geometries inducing "healing chamber": Duo (D), Compact (C), and Infra (IF). Insertion torque (IT) and implant stability quotient (ISQ) were measured. Peri-implant health were monitored by visible plaque index (VPI), peri-implant inflammation (PI), and presence of calculus (CC). Data were collected during 90 days. Data were assessed for normality using the asymmetry and kurtosis coefficients followed by the Shapiro-Wilk test. A one-way ANOVA was used to investigate differences in IT and linear bone dimensions between the macrogeometry groups. The repeated measurements ANOVA test or ANOVA-R was used for analysis of ISQ, VPI, and PI. Tukey-Kramer test or Student's t test was used for comparisons between the groups or within each macrogeometry. RESULTS: Macrogeometry did not significantly influence IT and ISQ values. The minimum ISQ was recorded after 7 days (71.95 ± 12.04, p = 0.0001). Intermediate ISQ was found after 14 days, when the ISQ reached values that are statistically identical to primary stability. The VPI showed significantly higher scores for the D (0.88 ± 1.03) and IN (0.72 ± 0.94) implants after 7 days. The PI was only influenced by the healing time significantly decreasing from 7 (1.07 ± 0.89) to 21 days (0.18 ± 0.18). CONCLUSION: Implant macrogeometry did not influence IT nor ISQ values. The relationship between IT and SS was more evident for the Duo implant, but only in the final stage of healing process. CLINICAL RELEVANCE: Show to the clinician that the macrogeometry and drilling protocols did not interfere in the clinical behavior of the implants during the healing process. However, the IT, primary and secondary stability, is quite dependent of the surgeon experience.

This randomized clinical trial evaluated the insertion torque (IT), primary, and secondary stability of dental implants with different surface treatments during the osseointegration period. Nineteen patients with bilateral partial edentulism in the posterior mandibular region were randomly allocated to two implant brand groups and received implants with different surface treatments in the opposite site of the arch: Osseotite and Nanotite or SLA and SLActive. During implant placement, the maximum IT was recorded using a surgical motor equipped with a graphical user interface. The implant stability quotient (ISQ) was assessed immediately after the IT, and was measured weekly via resonance frequency analysis during 3 months. The data were analyzed by a one-way ANOVA, the Bonferroni test, paired t tests and Pearson's correlation coefficient. The IT values were similar (p > 0.05) for all implant types ranging from 43.82 ± 6.50 to 46.84 ± 5.06. All implant types behaved similarly until the 28th day (p > 0.05). Between 35 and 56 days, Osseotite and SLActive showed lower ISQ values (p < 0.001) compared to Nanotite and SLA implants. After 56 days, only Osseotite maintained significantly lower ISQ values than the other implants (p < 0.05). After 91 days the ISQ values were significantly higher than the baseline for all four implant types (p < 0.001). The ISQ and IT values were significantly correlated at the baseline and at the final evaluation for Osseotite, Nanotite, and SLActive implants (p < 0.001). After 91 days, ISQ and IT values were only significantly correlated for the Osseotite implants (p < 0.05). All implants types exhibited acceptable primary and secondary stability.

BACKGROUND: Companies affirm that tapered implants show adequate initial stability, while their installation in the lower arch is uncommon in clinical practice. PURPOSE: To compare the clinical outcomes of tapered and cylindrical implants and to study their effect on bone site characteristics and peri-implant health during healing. MATERIALS AND METHODS: The implant site dimensions were assessed by linear measurements using CBCT prior to the installation of 40 implants in the posterior mandible (20 tapered and 20 cylindrical). The bone type was registered during drilling via the surgeon's tactile perception, following the classification of Lekholm and Zarb. Primary stability (PS) was determined by the insertion torque (IT) and the implant stability quotient (ISQ). Secondary stability (SS) and the peri-implant health was monitored for 3 months through the visible plaque index (VPI), the peri-implant inflammation (PI), the probing depth index (PDI), and the gingival bleeding index (GBI). Significant differences were investigated with t-tests for independent samples, chi-square tests or Fisher's exact test. Pearson's correlation test was used to investigate the relationship between the bone site characteristics and PS (IT and ISQ), as well as the relationships between IT and ISQ for each implant type. RESULTS: Tapered and cylindrical implants showed no significant differences for any outcome variable (P > .05). A significant decrease in ISQ was observed after 7 days of healing (P = .0002), followed by a gradual increase beginning at 21 days (P = .0010) until the last follow-up time at 90 days (P = .0319). The cortical height was correlated with IT; while medullary bone dimensions were correlated with the PS as evidenced by the ISQ values. The insertion torque was significantly correlated with the PS only for the cylindrical dental implants. CONCLUSIONS: Tapered and cylindrical implants have similar biological behavior during the healing process. Bone site characteristics can influence insertion torque and implant stability.

Aim: This study characterized the implant surfaces available on the Brazilian market in terms of topography, chemical composition, and roughness. Methods: The following brands were selected according to their surfaces: Kopp (Ko), Signo Vinces (Sv), Neodent (Ne), Osseotite (Os) NanoTite (Nt), SIN (Si), Titanium Fix (Tf), conventional Straumann (Str), Active SLA (SLA). The morphological analysis and the alloy impurities and implant surface contaminants were analyzed by SEM-EDS. Surface roughness parameters and 3-D reconstructions were obtained by laser microscopy (20x). Two distinct areas were evaluated: i) the cervical portion (no surface treatment), and ii) the middle third (treated surface). Results: The characterization of the implant surfaces by SEM showed morphological differences between the thread geometries and surface morphology at 800x and 2000x magnification. The EDS elemental analysis showed a predominance of titanium (Ti) for all implants. The SLA surface showed only peaks of Ti while other implants brands showed traces of impurities and contaminants including Al, C, PR, F, Mg, Na, Ni, O, P, and SR. The implant surface roughness in the cervical portion did not exceed Ra 0.5­1.0 µm, constituting a minimally rough surface and obtaining acceptable standards for this region. Only Nt, Str, and SLA presented Ra above 2 µm in the middle third area showing a rough surface favorable for osseointegration. Conclusion: This study concluded that there is no established standard for morphology, chemical composition and implant surface roughness that allows a safe comparison between the available dental implant surfaces. National implant brands generally contain more impurities and surface contaminants than their international counterparts and were consequently more sensitive to the surface treatment techniques

RESUMO

Pacientes com atrofia óssea avançada dos maxilares ainda consistem em um desafio para a reabilitação oral. A literatura sugere várias opções de tratamento e cabe ao profissional escolher qual é a mais indicada conforme as condições psicossomáticas e financeiras do paciente. Este artigo relata e discute, embasado na literatura pertinente, o caso de uma paciente portadora das características ósseas acima, onde o tratamento escolhido foi a confecção de uma prótese total superior convencional e uma prótese fixa inferior suportada por implantes curtos com minipilares angulados devido à perda óssea vertical severa e ao prognatismo mandibular.


Patients with advanced bone jaw atrophy still consist of a challenge for oral rehabilitation. The literature suggests several treatment options, and clinicians must choose which is the most appropriate according to financial and psychological conditions. This article reports and discusses, based on relevant literature, the case of a patient with those characteristics, where the treatment of choice was the construction of a conventional denture and a lower fixed screw-retained, implant-supported prosthesis with short implants and mini-abutments due to the severe vertical bone loss and mandibular prognathism
