RESUMO

INTRODUCTION: Studies regarding antibiotics administration during pregnancy and atopic dermatitis (AD) in children are only few. In this context, the objective of our study was to investigate the potential association between the timing of intrauterine exposure to antibiotics or prenatal antibiotic administration in general and AD occurrence in children. METHODS: This was a cross-sectional study in 1046 subjects. The exposure to antibiotics during pregnancy was initially evaluated using simple logistic regressions. Then, each period of antibiotics administration was adjusted with the other periods of antibiotics exposure (model 1) and with the other variables associated with AD in our database (model 2). RESULTS: In simple logistic regression analysis, the administration of antibiotics during pregnancy, as a whole period, presented a trend of association with AD (OR = 1.28, %CI: 0.99 - 1.65). When we analyzed antibiotic administration during each trimester of pregnancy, only antibiotherapy during the 3rd trimester was associated with AD (OR = 2.94, %CI: 1.21 - 7.12). After adjusting with all the other important risk factors associated with AD in the database, antibiotics administration during the 3rd trimester of pregnancy was still independently associated with AD (OR=2.64, %CI: 1.01 - 6.91). CONCLUSION: Antibiotic administration during the 3rd trimester of pregnancy was independently associated with AD in children.

RESUMO

PURPOSE: To evaluate the influence of Intraocular Lens (IOL) material and design on Posterior Capsule Opacification (PCO) and Neodymium-YAG (Nd-YAG) rates in eyes implanted with different Posterior Chamber Intraocular Lenses (PC IOLs) designs at the end of uncomplicated cataract surgeries. SETTING: Oculus Eye Clinic, Bucharest, Romania. DESIGN: Retrospective, observational study. METHODS: This study comprised 4805 eyes operated for cataract in 2012 and 2013 with a post-operative average follow up of 40 ± 6,15 months (27-54 months). The PCO and Nd-YAG rates were recorded and compared among different IOL materials and designs and among different pathology groups. RESULTS: From 4805 IOLs implanted, 2560 (53,27%) were hydrophilic and 2245 (45,73%) hydrophobic, 2937 (61%) were aspherical and 1868 (39%) spherical. We found statistical significant differences in the PCO and Nd-YAG rates between hydrophilic (18% and 14% respectively) and hydrophobic lenses (4% and 2% respectively) (p<0.0001). There were also statistically significant differences in the sub-group of hydrophilic aspheric IOLs, finding lower PCO and Nd:YAG rates with the C-loop haptics configuration (12,6% and 3,3% respectively) compared with the broad optic/ haptic junction (29,75% and 24,73% respectively) (p<0.001). No statistically significant differences on PCO and Nd:YAG rates were found for the different associated pathologies (p>0.05). CONCLUSIONS: Hydrophilic lenses showed statistically higher PCO and Nd:YAG rates than hydrophobic lenses. In contrast, the optic asphericity and the associated pathologies had no influence on the PCO and Nd:YAG rates. IOL design and material seem to be the main characteristics influencing PCO and Nd-YAG rates. ABBREVIATIONS: LECs = lens epithelial cells.

RESUMO

Calcinosis consists of abnormal calcium deposition in soft tissues, which appears often in patients with limited systemic sclerosis, being one of the criteria of CREST (calcinosis cutis, Raynaud phenomenon, esophageal dysfunction, sclerodactyly, and telangiectasia) syndrome. With a long evolution, the aim of the treatment is to control the symptoms and prevent complications. In this article, we present the challenging management of a profuse lesion of calcinosis in a patient with systemic sclerosis. We describe the case of a 52-year-old woman with systemic sclerosis and CREST syndrome who was admitted in our Department with multiple painful and disabling tumoral masses, situated in nearly all joints. The interscapular vertebral tumoral mass was excised and the defect was closed. Histopathological examination revealed cutaneous calcinosis, probably associated with CREST syndrome, a type of scleroderma. Postoperative results were favorable and no local complications were encountered. Six months follow-up revealed no evidence of recurrence. Despite the size and the invasion of the tumor in the muscle, complete resection was possible with an adequate reconstruction; the postoperative result being acceptable. With a lower response to medication, surgical treatment is considered the only option for treating symptomatic lesions of calcinosis in order to improve quality of life.
