Although not a common problem, the posttraumatic unstable distal radioulnar joint can be difficult to treat. This article focuses on the various methods for reconstruction of the unstable distal radioulnar joint. Attention focuses specifically on anatomic reconstruction of distal radioulnar ligaments. The authors' technique is presented in detail.

The indications and methods for operative treatment of phalangeal fractures are reviewed. Included are descriptions with examples of closed reduction and percutaneous pin fixation, percutaneous reduction and percutaneous fixation, open reduction and internal fixation, and static and dynamic external fixation. Also included are tips for facilitating phalangeal fixation and a description of potential complications.

Intra-articular fractures in the hand and wrist are common. Prompt treatment with adequate fixation to allow early motion provides the best chance for a successful outcome. Arthrodesis and arthroplasty are the two main options for treatment of residual pain, deformity, or instability in the hand. Management of late wrist symptoms may include proximal row carpectomy, limited wrist fusion, or complete wrist fusion.

The standard treatment for delayed union or nonunion of the scaphoid is operative management. Electrical stimulation has been employed in these clinical situations in patients unable or unwilling to undergo surgical intervention. Recent interest has also focused on the use of low-intensity ultrasound as an adjunct to healing in distal radius and tibial fractures. Results with the use of ultrasound for scaphoid fractures is encouraging. A review of the mechanisms of action, clinical results, and possible indications is presented for these two nonoperative modalities.

This report describes a case of isolated denervation of the extensor pollicis longus (EPL), which has not previously been reported. The patient was a healthy 13-year-old boy who sustained multiple lacerations to the left forearm secondary to a dog bite. No motor responses were evident in the EPL, but the remainder of the exam findings were normal. Electromyography confirmed complete isolated denervation of the EPL. This patient was managed nonoperatively and had no significant functional deficits at final follow-up.
