RESUMO

The AA. examine 75 cases of Crohn disease for studying the secretion of mucina in the different stages of disease. The result is that the acute stage is marked by a strong depletion of mucina; the interesting part of this work is that the study of sulpherized acid mucinae is an excellent standard of diagnosis in regard to the benign evolution of this disease.

RESUMO

With regard to vesica tumors, a remarkable difficulty for the interpretation of statistical data is often given by the different classifications that the different authors use. Therefore we have reported a comparative table always for being faithful to what we wished at the beginning of this work that has be useful to the doctor above all by a clinical point of view. In synthesis all the Authors think that radiotherapy is not the elective treatment for tumors in O or A stage, while it can be efficient in the therapy of vesica neoplasies in B1, B2 and C stages. The most Authors seem less favourable to preoperating irradiation of tumor above all for the unexact intraoperating valuation of "staging" that is inevitable after this treatment. Postoperating radiotherapy (5000-6000 R in 20-30 days) can be more useful in the cases of incomplete surgical exeresis although there is the problem of possible radiolesions of eventual uretero-intestinal anastomosis let alone of intestine. In the end urethra tumors are sensitive to radiotherapy for their superficiality and excellent tolerance of membrum to radiations that permits also the giving of 6000-7000 R in 4-6 days.

In the study of benign hepatic tumours, angiography of the coeliac tripod is a fundamental step in the diagnostic course, allowing them to be characterised in detail; within the framework of evaluation of the said expansive forms, particular attention has been directed to differential diagnosis in relation to those malignant lesions that can simulate a benign process.

RESUMO

The clinical case of a sixty-two year old woman suffering for about six years from rare episodes of cramplike pains in the right hypochondriac region, radiated to the homolateral scapula, is described. After diagnosis of biliary calculosis, the patient was admitted to hospital and her gallbladder removed. During the operation a number of venous ectasias of the portal system were evidenced at the hepatic hilus, and their biopsy led to a diagnosis of portal cavernoma. The possible part played by the cavernoma in producing the patient's clinical picture is discussed, with reference also to other cases of cavernoma of varying location. This is followed by a review of the literature, in which stress is placed on the different aetiopathogenetic interpretations of angiomas and in particular of portal cavernomas. The possible evolution of angiomas and present treatment trends are also mentioned.

RESUMO

In the field of malignant primitive hepatic tumours, the use of ultrasonic methods and C.A.T. has made a considerable contribution to diagnosis and topographical localisation of such tumours. In addition, hepatic biopsy supplies the histological diagnosis. However it is only by angiography that it is possible to evaluate the involvement of the lobes and segments, supplying the character of invasiveness or otherwise of the lesion through demonstration of the arterial peduncle, and thus giving the surgeon the essential elements for a therapeutic approach. Furthermore, in the case of a monofocal lesion, it is only angiography that can establish the usefulness or otherwise of surgical therapy (William M. - Richard P. J. - Philip C. G. - Robert C. Lim, 1979). The purpose of the work is to examine the angiographic readings, which allow: 1) a diagnosis of hepatic expansive formation to be made, with definition of its nature (bening or malignant); 2) the growth character of the lesion to be characterised through evaluation of the involvement of the vascular structures.
