RESUMO

Demographic aging of the population determines the need for the development of geriatric component in the work of medical services. In this regard, we have proved promising development of Gerontology and Geriatrics in Kazakhstan, the results of analysis of the existing system of care for the elderly persons and identify ways to improve towards rehabilitation help, long-term care for people over 80 years old, geriatric medical training and the creation of a scientific school of Gerontology and geriatrics.

Purpose of the investigation was to study group dynamics of seasonal changes in the autonomous nervous and cardiovascular functions in essentially healthy males occupationally dealing with stressful situations (field personnel and firefighters of the Alma Ata Emergency Management Department (Kazakhstan). The investigation was based at the Kazakh National Medical University (Alma Ata) and conducted in conjunction and parallel with the MARS-500 project. Methodology embodied the pre-nosology approach pointed at identification of states in-between the norm and pathology. Results of the year-long investigation demonstrated seasonal variations in the functioning of normal organism that could be associated with occupational factors and "arduous duty", as it follows from the EMD operations summary (autonomous balance shifting toward prevalence of the sympathetic control due to the spring and summer "arduous duties" and the parasympathetic control in the peaceful situation during wintertime). These observations testify to the importance of dynamic health monitoring of normal people occupationally exposed to chronic psycho-emotional strain for pre-nosology diagnosis and timely preventive intervention.

The article presents the results of studying electrophysiological changes in the myocardium of normal human subjects during 7-day dry immersion, and modification of the reaction to immersion by amlodipin and myostimulation. The dry immersion effects on the myocardium generally include a succession of electrophysiological shifts first in the ventricles and then in the left atrium due to, probably, blood pooling in the pulmonary vessels. Myostimulation and amlodipin affect the myocardium reaction visibly suggesting involvement of numerous components of the autonomous and central nervous systems. The positive effect of the pharmaceutical--alleviation of myocardium strain--is a result of amlodipin assistance to the coronary blood flow; however, it has a side-effect on orthostatic stability. The positive action of myostimulation is associated with unloading of the right heart and reflected in changed HD ECG parameters and growth of the "myocardium" index.

The paper presents the results of investigations into vegetative regulation of blood circulation and regulation modification by peroral amlodipine and myostimulation during 7-d dry immersion. It was shown that in immersion vegetative regulation readjusted towards predominance of the sympathetic mechanisms. Myostimulation and peroral amlodipine modified regulation substantially mobilizing high level suprasegmentary structures. It should be noted that amlodipine and myostimulation differ in mechanisms of their action on central regulation. Pharmaceutical intervention seems to have a more complex and varying effect on people including side-effects. Presumably, it was the cause of poor orthostatic tolerance of several test-subjects.

Asymmetric dimethyl hydrazine (ADH), a highly toxic propellant component, is known to cause metabolic disturbances in humans and laboratory animals. Focus of this investigation was placed on crystal-forming properties of blood serum, and types of plasma protein destructions in ADH-intoxicated rats. Object of investigation was blood plasma collected from rats exposed to 100 mg/kg of per oral ADH total (5 mg/kg per a day, 5 days a wk, 4 wks.). The crystal-forming ability of plasma was assessed with the teziographic technique. Severity of proteins destruction was determined by the content of carbonyl products of oxidative protein modification in plasma. Chronic exposure to low ADH doses caused oxidative destruction of proteins in blood plasma and, consequently, significant degradation of the plasma ability to form crystals reflected in an altered zonal phase structure and phase bursting on teziograms. Symmetry gave way to pathological structures, e.g. petals and loops. Amorphous areas grew in size, bursting lines thinned out. Concretions became less numerous. These data suggest a close relationship between the impairments in physical and chemical properties of blood plasma of experimental animals by highly toxic asymmetric dimethyl hydrazine.
